Tato práce pojednává o některých klíčových aspektech diagnostických kritérií pro MPN podle klasifikace Světové zdravotnické organizace (WHO) z roku 2008, přičemž většina poznámek se omezuje na onemocnění polycythaemia vera (PV). Při analýze hlavních kritérií poznamenáváme, že existují pacienti s PV, jejichž erytrocytóza může být dokumentována pouze měřením objemu erytrocytární masy a nikoli měřením koncentrace hemoglobinu nebo hematokritu. Probíráme reprodukovatelnost histopatologie pro diagnostiku různých stadií MPN, existenci normálních hladin EPO u pacientů s PV, specifitu růstu endogenních erytroidních kolonií (EEC) pro diagnózu PV a naše současné znalosti týkající se cytogenetiky a molekulární biologie PV a jiných MPN a jejich patofyziologie. Rovněž analyzujeme různé prognostické ukazatele morbidity (trombóza) a mortality u PV – vysokou hladinu hematokritu, leukocytózu a procentuální zastoupení buněk s mutací JAK2V617F. V případě esenciální trombocytemie (ET) komentujeme rozlišení pravé ET a “časné PMF”. V závěru stručně shrnujeme obecně přijímaná doporučení European LeukemiaNet (ELN) pro léčbu Ph-negativní MPN na základě nové stratifikace rizika, na důkazech založené riziko kancerogeneze u hydroxyurey a racionální doporučení pro výběr konkrétních léků., In this paper, we discuss some critical parts of the World Health Organization (WHO) 2008 criteria for classification of MPNs classification, with most of the remarks limited to polycythemia vera (PV). When analyzing the major criteria, we note that there are PV patients whose erythrocytosis can only be documented by red cell mass measurements and not by hemoglobin concentration or hematocrit measurements. We discuss the reproducibility of histopathology for the diagnosis of different stages of MPNs, the existence of normal EPO levels in PV patients, the specificity of Endogenous erythroid colony (EEC) growth for PV diagnosis and our current knowledge of the cytogenetics and molecular biology of PV and other MPNs and their pathophysiology. We also analyze various prognostic morbidity (thrombosis) and mortality markers in PV – high hematocrit level, leukocytosis and percentage of cells with the JAK2V617F mutation. In essential thrombocythemia (ET), we comment on the distinction of true ET and “early PMF”. Cíl. Práce je zaměřena na možnosti provedení CT angiografie břišní aorty a dolních končetin na nové generaci dvouzdrojového CT přístroje využívající skenování s vysokým pitch faktorem a ohledem na redukci radiační zátěže a množství podávané kontrastní látky. Metoda. V rámci prospektivní studie bylo vyšetřeno 29 osob s klinickými známkami či již známým postižením tepen dolních končetin. Vyšetření byla provedena na přístroji Somatom Definition FLASH (Siemens, Německo), použita kolimace 128 x 0,6 mm, rychlá rotace rentgenky (0,28 s/otáčku) a vyšším fázovým posunem (pitch faktor 1,55). Kontrastní látka v množství 60-70 ml byla aplikována rychlostí 5 ml/s, načasování akvizice bylo provedeno na základě monitorace testovacího bolusu (10 ml) v popliteálních tepnách. Hodnocení kvality zobrazení bylo provedeno na základě vizuálního posouzení a stanovení denzity v jednotlivých částech tepenného systému. Výsledky. Ve všech případech bylo dosaženo vysoké kvality zobrazení aorty, pánevních, femorálních i popliteálních tepen na základě vizuálního hodnocení i stanovení denzity (aorta - 456,3 HU, třísla - 507,3 HU, podkolenní - 554,3). V případě distální části bérců a nohy došlo u tří osob k suboptimální náplni vlivem rychlejší akvizice vůči postupu bolusu kontrastní látky. Pouze v jednom případě byla distální oblast bérců nehodnotitelná. Průměrná úroveň denzity v úrovni kotníků byla 367,7 HU. Průměrná radiační dávka byla nízká -CTDIvol 2,40, efektivní dávka 1,79 mSv. Závěr. První výsledky naší prospektivní studie prokázaly možnost provedení CT angiografie dolních končetiny pomocí "high-pitch" módu. Přes velmi nízkou hodnotu radiační dávky i aplikovaného množství kontrastní látky bylo dosaženo vysoké diagnostické kvality v oblasti aorty, pánve stehen a proximálních částí bérců. Tento způsob vyšetření je zvlášť vhodný pro osoby s vysokým rizikem kontrastní nefropatie, eventuálně pro mladé osoby v produktivním věku., Aim. Our work was focused on the feasibility of CT angiography of abdominal aorta and lower limbs using new generation of dual-source CT with a high pitch factor, concerning on the reduction of radiation exposure and the amount of administered contrast agent. Methods. In the prospective study were examined 29 patients with clinical signs, or known disability of lower limbs arteries. The examinations were performed on the Somatom Definition Flash (Siemens, Germany) using collimation 128 x 0.6 mm, high tube rotation speed (0.28 s/rotation) and a higher pitch factor of 1.55).Amount of contrast agent (60 to 70 ml) was applied at 5 ml/s, the timing of acquisition was based on results of the test bolus monitoring (10 ml) in the popliteal arteries. Evaluation of image quality was based on visual assessment and determination of density in different parts of the arterial system. Results. In all cases were achieved a high display quality of the aorta, pelvic, femoral and popliteal arteries by visual assessment and also determination of luminal density (aorta - 456.3 HU, groin - 507.3 HU, knee - 554.3). In the region of the distal leg and ankle occurred in 3 people in suboptimal quality due to overrun of bolus. Only in one case, the distal region leg was uninterpretable. The average level density at ankle level was 367.7 HU. The average radiation exposure was low - CTDIvol 2.40,1.79 mSv effective dose. Conclusion. The preliminary results of our prospective study showed the possibility of performing CT angiography of the lower limbs in "high-pitch" mode. Despite the very low radiation dose and the amount of applied contrast agent was achieved a high diagnostic quality of the aorta, pelvic, femoral, popliteal and proximal crural arteries. Myeloproliferativní neoplázie (MPN) představují skupinu klonálních onemocnění hematopoetické kmenové buňky s fenotypickými změnami, které jsou důsledkem zvýšené myeloidní proliferace se zachovanou maturací a akumulací myeloidních buněk v periferní krvi: leukocytózou, trombocytózou a zvýšeným počtem červených krvinek nebo různou kombinací proliferace jednotlivých buněčných linií. Cytogenetické analýzy odhalily opakující se chromozomové změny u 5–45 % nemocných, ale specifická chromozomová změna zatím nebyla nalezena. K častým chromozomovým změnám, které u MPN nacházíme, patří především ztráty genetického materiálu v podobě delecí dlouhých ramen chromozomu 20 (20q-), dlouhých ramen chromozomu 13 (13q-) a delece krátkých ramen chromozomu 12 (12p-). Zmnožení genetického materiálu se týká především trisomií chromozomů 8 (+8) a 9 (+9) a parciálních duplikací dlouhých ramen chromozomu 1 (1q+). K dalším pozorovaným změnám patří balancované translokace, například chromozomu 8 s různými chromozomovými partnery. Takový nález je označován jako syndrom 8p11 (EMS – eight myeloproliferative syndrome) a vždy zahrnuje gen FGFR1. Cytogenetika doplněná molekulárně cytogenetickými a genetickými metodami doplňuje diagnostiku MPN, ale má význam i pro stanovení prognózy. Určení přestavby FIP1L1/ PDGFRA u nemocných s MPN s eosinofílií umožňuje zahájit efektivní cílenou léčbu imatinibem. Přesné určení genetických změn umožňuje porozumět molekulární etiopatogenezi MPN a přispívá ke zpřesnění klasifikace, prognózy a personalizace léčby nemocných., Myeloproliferative neoplasias (MPNs) are a group of hematopoietic stem cell clonal diseases with phenotypic changes resulting from increased myeloid proliferation with preserved maturation and accumulation of myeloid cells in peripheral blood: leukocytosis, thrombocytosis and increased red blood cell count or their various combinations. Cytogenetic analyses revealed recurrent chromosomal aberrations in 5–45 % of patients, but so far, a specific chromosomal change has not been found. Frequent chromosomal aberrations observed in MNPs mainly include losses of genetic material such as deletions of the long arms of chromosome 20 (20q-), long arms of chromosome 13 (13q-) and deletions of the short arms of chromosome 12 (12p-). Gains of genetic material are particularly in the form of trisomies of chromosomes 8 (+8) and 9 (+9) and partial duplications of the long arms of chromosome 1 (1q+). Other observed changes are balanced translocations, such as those of chromosome 8 with various chromosomal partners. These findings are referred to as the 8p11 myeloproliferative syndrome (EMS), always involving the FGFR1 gene. Cytogenetic analyses supplemented with molecular cytogenetic and genetic methods play supplementary role in the diagnosis of MPNs but are also contributory prognostic determinants. Recognition of the FIP1L1/PDGFRA rearrangement in patients with NPM with eosinophilia enables initiation of targeted therapy with imatinib, significantly improving their prognosis. Cíl studie: Zhodnocení významu společného cytologického vyšetření likvoru a energetických poměrů v likvorovém kompartmentu pro detekci a určení charakteru zánětlivého procesu v centrálním nervovém systému. Materiál a metody: Vyšetřili jsme 437 vzorků likvoru pacientů se serózními záněty centrálního nervového systému (170), s purulentními záněty centrálního nervového systému (121), se záněty s oxidačním vzplanutím makrofágů v centrálním nervovém systému (25) a pacientů bez postižení centrálního nervového systému (121). Pro statistické hodnocení energetických poměrů v likvorovém kompartmentu jsme použili Fisherův F-test a Studentův t-test a pro společné hodnocení cytologických obrazů likvoru a energetických poměrů v likvorovém kompartmentu jsme použili výpočtu specificit, senzitivit a diagnostických účinností. Výsledky: Mezi uvedenými skupinami pacientů jsme zjistili statisticky vysoce významné rozdíly (p < 0,001) rozptylů i průměrů hodnot koeficientu energetické bilance. Při společném hodnocení cytologických obrazů CSF a energetických poměrů v likvorovém kompartmentu jsme dospěli k velmi dobrým výsledkům ve skupině pacientů se serózními záněty CNS infekční etiologie (specificita = 100,0 %, senzitivita = 97,1 % a diagnostická účinnost = 98,9 %), ve skupině pacientů s purulentními záněty CNS bakteriální etiologie (specificita = 100,0 %, senzitivita = 96,7 % a diagnostická účinnost = 99,1 %), ve skupině pacientů s oxidačním vzplanutím makrofágů v CNS infekční etiologie (specificita = 98,6 %, senzitivita = 100,0 % a diagnostická účinnost = 98,6 %) i ve skupině pacientů s oxidačním vzplanutím makrofágů v CNS nádorové etiologie (specificita = 98,8 %, senzitivita = 100,0 % a diagnostická účinnost = 98,9 %). Závěr: V našem souboru vyšetřených pacientů vedlo společné hodnocení cytologických obrazů likvoru a energetických poměrů v likvorovém kompartmentu ke spolehlivé diferenciaci postižení centrálního nervového systému na serózní záněty, na purulentní záněty a na záněty s oxidačním vzplanutím makrofágů. Následně byly tímto způsobem umožněny efektivnější specifikace příslušných patogenních agens a v některých případech odhalení nádorové infiltrace mening., Objective: Evaluate the significance of combined cytology and energy balance in the cerebrospinal fluid compartment for the detection and characterization of the inflammatory processes in the central nervous system (CNS). Design: Retrospective study. Material and Methods: We examined 437 cerebrospinal fluid (CSF) specimens in patients with serous inflammations of infectious etiology in the CNS (170), purulent inflammations bacterial etiology in the CNS (121), inflammations with oxidative burst of macrophages of infectious and tumorous etiology in the CNS (25) and in patients without CNS impairment (121). We used the Fisher’s F-test and the Student’s t-test for the statistical evaluation of the energy balance in the cerebrospinal fluid compartment. We subsequently counted the specificities, sensitivities and diagnostic efficiencies for the combined evaluation of the cytological picture of the CSF and energy balance in the CSF compartment. Results: We found highly statistically significant differences (p<0.001) between the variations and the mean values of coefficients of energy balance in all our groups of patients. We achieved very good results in combined evaluations of cytological pictures and the energy balance in the CSF compartment in the group of patients with serous inflammations in the CNS of infectious etiology (specificity = 100.0 %, sensitivity = 97.1 % and diagnostic efficiency = 98.9 %), in the group of patients with purulent inflammation in the CNS bacterial etiology (specificity = 100.0 %, sensitivity = 96.7 % and diagnostic efficiency = 99.1 %), in the group of patients with oxidative burst of macrophages in the CNS infectious etiology (specificity = 98.6 %, sensitivity = 100.0 % and diagnostic efficiency = 98.6 %) and also in the group of patients with oxidative burst of macrophages in the CNS of tumorous etiology (specificity = 98.8 %, sensitivity = 100.0 % and diagnostic efficiency = 98.9 %). Conclusion: Combined evaluation of the cytological pictures and energy balance in the CSF compartment led to reliable differentiation of the CNS impairment into the serous inflammations, the purulent inflammations and the inflammations with oxidative burst of macrophages in our group of patients. Úvod: Byly srovnány dvě různé metody izoelektrické fokusace v agarosovém gelu pro detekci oligoklonálních IgG pásů v likvoru a séru: komerční metoda s imunofixací (Sebia) a „home-made“ metoda s použitím přístroje Multiphor II následovaná afinitním imunoblottingem. Byla posouzena shoda mezi metodami i hodnotícími, pokud jde o přítomnost intrathékální syntézy IgG, zařazení nálezu do jednoho z pěti typů podle mezinárodní klasifikace, počet intrathékálně syntezovaných IgG pásů a počet pásů v likvoru a v séru. Výsledky: Pro posouzení shody jsme použili statistiku kappa. Pokud jde o přítomnost intrathékální syntézy IgG, byla shoda velmi dobrá (kappa 0,870 až 1,000 mezi metodami a 0,947 a 0,920 mezi hodnotícími, což představuje 0 až 5 neshodně hodnocených nálezů z celkového počtu 114). V zařazení nálezu do jednoho z pěti typů podle mezinárodní klasifikace byla shoda méně vyjádřená (kappa 0,389 až 0,596 mezi metodami); při použití „home-made“ metody byla shoda mezi hodnotícími horší (kappa 0,478) než v případě komerční imunofixační metody (kappa 0,791). Shoda v počtu intrathékálně syntezovaných IgG pásů byla jen mírná, shoda v počtu oligoklonálních IgG pásů v likvoru a v séru byla slabá. Závěry: Oběma metodami lze spolehlivě detekovat oligoklonální IgG a metody lze považovat za rovnocenné. Pro klasifikaci typu nálezu však byla shoda mezi hodnotícími lepší při použití komerční imunofixační metody. Poměrně špatná reprodukovatelnost počítání oligoklonálních IgG pásů by měla být známa klinickým lékařům, protože porovnávání počtu pásů s sebou přináší potenciální riziko zavádějících interpretací., Background: We compared two different agarose isoelectric focusing methods for detection of oligoclonal IgG bands in cerebrospinal fluid and serum: commercial method with immunofixation (Sebia) and home-made method using Multiphor II apparatus followed by affinity immunoblotting. Interobserver agreement for both methods was tested concerning the presence of intrathecal IgG synthesis, the detailed isoelectric focusing pattern type, the number of CSF-restricted oligoclonal IgG bands, and the number of oligoclonal IgG bands in CSF and in serum. Findings: Using kappa statistics for evaluation of agreement, we found there was very good agreement concerning the presence of intrathecal IgG synthesis (kappa 0.870 to 1.000 between methods, and 0.947 and 0.920 between observers, respectively, representing 0 to 5 out of 114 samples classified differently). The agreement was less pronounced when international consensus classification of isoelectric focusing patterns into 5 different types was taken into account (kappa 0.389 to 0.596 between methods); using home-made method, the interobserver agreement regarding pattern type was worse (kappa 0.478) than using commercial Sebia method (kappa 0.791). There was moderate agreement on the number of CSFrestricted oligoclonal IgG bands, and mostly poor agreement on the number of oligoclonal IgG bands in CSF and serum. Conclusions: Both methods were capable to detect oligoclonal IgG reliably, and neither method could be evaluated as superior to the other. However, better interobserver agreement regarding to the pattern type was obtained using commercial Sebia immunofixation method. Centrum pro tyreoidální onkologii a endokrinní orbitopatie Nádory štítné žlázy vycházejí z folikulárního epitelu štítné žlázy, jsou hormonálně dependentní, vyskytují se 5x častěji u žen než u mužů ve všech věkových skupinách, obvykle dobře vychytávají jod. Klinicky se může projevovat jako lokálně rostoucí uzel ve štítné žláze, nádorová infiltrace spádových krčních lymfatických uzlin, vzdálené plicní a kostní metastázy. Diagnostika se opírá o sonografické vyšetření, tenkojehlou aspirační biopisii, hladinu Tg. Léčba je interdisciplinární. Totální tyreoidektomie (TTE), následné doléčení radiojodem v hypotyreóze, supresní léčba tyreoidálními hormony (TSH < 0,1 mIU/l), doživotní dispenzarizace. Purpose: The aim of this phase III trial was to compare the efficacy and safety of bevacizumab alone with those of bevacizumab and capecitabine plus oxaliplatin (XELOX) as maintenance treatment following induction chemotherapy with XELOX plus bevacizumab in the first-line treatment of patients with metastatic colorectal cancer (mCRC). Patients and Methods: Patients were randomly assigned to receive six cycles of bevacizumab, capecitabine, and oxaliplatin every 3 weeks followed by XELOX plus bevacizumab or bevacizumab alone until progression. The primary endpoint was the progression-free survival (PFS) interval; secondary endpoints were the overall survival (OS) time, objective response rate (RR), time to response, duration of response, and safety. Results: The intent-to-treat population comprised 480 patients (XELOX plus bevacizumab, n = 239; bevacizumab, n = 241); there were no significant differences in baseline characteristics. The median follow-up was 29.0 months (range, 0–53.2 months). There were no statistically significant differences in the median PFS or OS times or in the RR between the two arms. The most common grade 3 or 4 toxicities in the XELOX plus bevacizumab versus bevacizumab arms were diarrhea, hand–foot syndrome, and neuropathy. Conclusion: Although the noninferiority of bevacizumab versus XELOX plus bevacizumab cannot be confirmed, we can reliably exclude a median PFS detriment >3 weeks. Nádory štítné žlázy jsou nejčastějšími neoplaziemi endokrinního systému. Nejvíce zastoupeným typem nádorů štítné žlázy je papilární karcinom, u kterého jsou nalézány bodové mutace genů BRAF a RAS a RET/PTC přeskupení. Nejčastějšími molekulárně-genetickými změnami u folikulárního karcinomu jsou mutace RAS a PAX8-PPARγ přeskupení. Bodové mutace v RET proto-onkogenu jsou klíčové pro vývoj medulárního karcinomu štítné žlázy, kde se u familiární formy vyskytují zárodečné mutace a u sporadické formy mutace somatické. U nejagresivnějšího anaplastického karcinomu štítné žlázy jsou detekovány mutace BRAF a RAS a dále mutace v genech TP53 a CTNNB1. Díky zjištění genetických příčin vzniku nádorů štítné žlázy začaly být tyto geny a jejich mutace zkoumány jako nadějné terapeutické cíle. Mnoho látek ukázalo protinádorový efekt v preklinických studiích. The major type of thyroid tumor is papillary thyroid carcinoma, in which point mutations in the BRAF and RAS genes and RET/PTC rearrangements are found. The most frequent molecular genetic changes in follicular thyroid carcinoma are mutations in the RAS genes and PAX8-PPARγ rearrangemets. Point mutations in the RET proto-oncogene are crucial for development of medullary thyroid carcinoma, where germ-line and somatic mutations occur in familial and sporadic form, respectively. Mutations in the BRAF and RAS genes and moreover mutations in the TP53 and CTNNB1 genes are also detected in the most aggressive anaplastic thyroid carcinoma. Thanks to the finding of genetic causes of thyroid cancer, these genes and their mutations begin to be investigated as promising therapeutic targets. Many compounds show anti-tumor effect in preclinical studies. Expozice směsím látek mohou indukovat biologické interakce, které výrazně ovlivňují výslednou odpověď organismu. Česká verze databáze "miXie", prezentovaná v této publikaci, zvyšuje spolehlivost odhadu aditivního a interakčního potenciálu dané směsi látek (aditivity, supra-aditivity, infra-aditivity) a může usnadňovat rozhodovací procesy v hygieně práce, průmyslové toxikologii a pracovním lékařství. Strukturu databázové aplikace tvoří celkem 647 HTML dokumentů, shrnujících dostupné hygienicko-toxikologické údaje o každé látce. Údaje zahrnují cílové orgány, účinky na cílové orgány, mechanismy těchto účinků a toxikokinetické charakteristiky. Další součástí aplikace je 32 HTML dokumentů, charakterizujících třídy podobných biologických (toxických) účinků a 209 PDF dokumentů, charakterizujících binární interakce. Spolehlivost výstupů databáze je limitována stávající úrovní našich znalostí o problematice biologických interakcí. Databáze je koncipována jako otevřený systém, umožňující provádění změn podle úrovně aktuálních poznatků., Exposure to mixtures of substances can induce biological interactions that significantly affect the final response of the organism. The Czech version of the database "miXie", presented in this publication, increases the reliability of an estimate of the additive and interaction potential of any mixture of substances (additivity, supra-additivity, infra-additivity), and can facilitate decision-making processes in the field of occupational hygiene, industrial toxicology and occupational medicine. The structure of the database application consists of 647 HTML documents, summarizing the hygienic-toxicological data on each substance. The data include target organs, effects on target organs, mechanisms of adverse effects and toxicokinetic characteristics. Another part for application consists of 32 HTML documents, describing classes of similar biological (toxic) effects and 209 PDF documents describing binary interactions. The reliability of the outputs from the database is limited to the current level of our knowledge on biological interactions.
