Článek podává stručný přehled o možnostech konzervativní léčby endometriózy. Na základě poznané patofyziologie nemoci představuje léčbu hormonální, a to nejen aktuálně používanou, jako je kombinovaná hormonální antikoncepce, progestiny nebo analoga gonadoliberinů, ale i tu, která prošla klinickými studiemi, ale z důvodu ekonomické náročnosti nebo výrazných nežádoucích účinků se dnes používá jen výjimečně. Článek zmiňuje i léčbu analgetickou a alternativní, která rozšiřuje terapeutické možnosti léčby endometriózy a pomáhá odstranit její hlavní symptom, a to bolest., The article gives a brief overview of the possibilities of conservative treatment of endometriosis. Cieľ: Príspevok má design prierezovej štúdie, s parciálnym cieľom testovať psychometrické vlastnosti nástroja kvality ţivota (Caregiver Quality of Life Index Cancer) opatrovateľov v starostlivosti o zomierajúcich z hľadiska formy poskytovanej starostlivosti. Metodika: Výskumný súbor tvorilo 263 neprofesionálnych opatrovateľov, poskytujúcich starostlivosť zomierajúcemu v podmienkach domácej, kombinovanej a paliatívnej/hospicovej starostlivosti v troch krajoch Slovenskej republiky (Prešovský, Ţilinský, Banskobystrický). Psychometrické vlastnosti nástroja pouţitého v našej štúdii boli testované na základe analýzy spoľahlivosti, korelačnej a faktorovej analýzy. Výsledky: Faktorovou analýzou sa v našom výskume podarilo potvrdiť 4-faktorovú štruktúru nástroja CQOLC (záťaţ/narušenie ţivotného štýlu; sociálna opora; pozitívna adaptácia; finančná situácia). Extrahované faktory na základe odpovedí respondentov v našom výskumnom súbore nie sú úplne identické s originálnou verziou škály CQOLC. Najlepšiu konštruktovú validitu a vnútornú konzistenciu škály CQOLC sme potvrdili u respondentov, ktorí poskytujú domácu starostlivosť zomierajúcemu príbuznému. Záver: Psychometrickou analýzou dotazníkov sme zistili, ţe reliabilita a konštruktová validita nami pouţitého nástroja je porovnateľná s originálnou verziou, napriek odchýlkam v lokalizácii niektorých poloţiek. Nástroj CQOLC je pouţiteľný na posúdenie kvality ţivota neprofesionálnych opatrovateľov v kontexte starostlivosti o zomierajúcich v domácom prostredí., Aim: The paper has the design of the cross-sectional study. The aim of the study was to test the psychometric properties of the quality of life (Caregiver Quality of Life Index of Cancer) the caregivers who provide care of the dying patients in terms of forms of care. Methods: The sample of our research consisted of 263 non-professional caregivers, providing home care of the dying, combined care and palliative care in three districts of the Slovak Republic (Prešovský, Ţilinský, Banskobystrický). Psychometric properties of the instrument used in our study were tested on the basis of the analysis of the reliability, of the correlation analysis and factor analysis. Results: Factor analysis of questionnaire CQOLC in our research confirmed the fourfactor structure of the instrument CQOLC (burden/lifestyle disruption; social support; a positive adaptation; the financial concerns). Extracted the factors on the basis of the answers of the respondents in our research are not completely identical with the original version CQOLC. The best construct validity and reliability of the CQOLC we have confirmed with the caregivers, who provide a home care of the dying patients. Conclusion: Psychometric analysis of the questionnaires we found that, the reliability and construct validity is comparable to the original version of CQOLC, despite variations in the location of some items. Cieľ: Cieľom výskumu bolo u detí s bronchiálnou astmou zistiť vplyv ochorenia a stupňa kontroly astmy na kvalitu života dieťaťa a kvalitu života jeho rodičov. Metodika: Na zber empirických údajov sme použili štandardizované dotazníky na kvalitu života – Paediatric Asthma Quality of Life Questionnaire (PAQLQ) a Paediatric Asthma Caregiver´s Quality of Life Questionnaire (PACQLQ) a na hodnotenie kontroly astmy – Asthma Control Questionnaire (ACQ). Výsledky boli analyzované pomocou metód deskriptívnej a induktívnej štatistiky. Výsledky: Vo vzorke detí (n = 72) sme zistili, že 27 (37,5 %) detí malo astmu pod kontrolou a 28 detí (38,9 %) bolo v pásme nekontrolovanej astmy. Celkové skóre ACQ (1,24 ±1,07) potvrdilo stredný stupeň kontroly astmy. Celkové skóre dotazníka PAQLQ u detí bolo 5,34 ±1,16, čo indikuje, že deti s astmou vyjadrujú dobrú kvalitu života. U detí ochorenie viac vplývalo na aktivity ako na emocionálne prežívanie a výskyt symptómov. Celkové skóre dotazníka PACQLQ u rodičov bolo 4,53 ±1,07, u rodičov nebol zaznamenaný rozdiel v doménach emocionálne prežívanie a aktivity. Výskum potvrdil, že stupeň kontroly astmy štatisticky významne ovplyvňuje kvalitu života dieťaťa aj jeho rodičov, pričom čím je vyššia kvalita života dieťaťa, tým je vyššia kvalita života rodičov. Záver: Cieľom komplexného manažmentu liečby je dosiahnuť čo najoptimálnejší stupeň kontroly astmy, a tým vytvoriť predpoklady k vyššej kvalite života dieťaťa a jeho rodičov. Využívaním posudzovacích nástrojov na hodnotenie kvality života je možné identifikovať závažný dopad choroby na každodenný život detí aj rodičov., Aim: The aim of the research was to find out the impact of the disease and the degree of asthma control on quality of life in children with bronchial asthma and quality of life in their parents. Methods: To collect empirical data we have used standardized questionnaires concerning quality of life – Paediatric Asthma Quality of Life Questionnaire (PAQLQ) and Paediatric Asthma Caregiver’s Quality of Life Questionnaire (PACQLQ) and for the assessment of asthma control we have used questionnaire Asthma Control Questionnaire (ACQ). The results were analysed using the methods of descriptive and inductive statistics. Results: In the sample of children studied (n = 72) we have found that 27 children (37.5%) have their disease under control and 28 children (38.9%) were in the range of uncontrolled asthma. Total ACQ score (1.24±1.07) confirmed medium degree of asthma control (partially controlled asthma). Total score of PAQLQ questionnaire in children (5.34±1.16) indicates that children with asthma represent good quality of life. Bronchial asthma in children had more significant influence on activities than on emotions and incidence of symptoms. Total score of PACQLQ questionnaire in parents was 4.53 ±1.07 and we have not registered differences between domains of emotions and activities. The research has proved that degree of asthma control has statistically significant influence on the quality of life of the child and his parents, whilst the higher the quality of life of the child, the higher the quality of life of parents. Conclusion: The aim of comprehensive management of asthma therapy is to achieve the most optimal level of asthma control and thus to establish conditions for higher quality of live of the child and his parents. Cieľ: Cieľom práce bolo zistiť, aká je kvalita života žien s karcinómom maternice vo vzťahu k veku a dosiahnutému vzdelaniu. Metodika: Súbor tvorilo 300 pacientiek, u ktorých bolo diagnostikované nádorové ochorenie maternice a bola u nich vykonaná hysterektómia. Výskum bol realizovaný prostredníctvom štandardizovaného dotazníka Quality of Life in Adult Cancer Survivors (QLASC). Výsledky: Ženy vo veku nad 50 rokov mali lepšie prežívanie kvality života ako ženy do 50 rokov. Priaznivejšie výsledky u pacientiek vo veku do 50 rokov sa preukázali len v doméne benefit z choroby. Vo vzťahu k vzdelaniu sme podľa priemerného skóre zistili v ôsmich doménach priaznivejšie výsledky u žien s vyšším vzdelaním. Zaujímavé bolo aj zistenie, že v doménach negatívne emócie a distres z opakovania mali ženy so stredoškolským vzdelaním bez maturity lepšie prežívanie faktorov kvality života než ženy so stredoškolským vzdelaním s maturitou. Záver: Na prežívanie kvality života žien s nádorovým ochorením maternice po hysterektómii vplýva biologický vek a stupeň dosiahnutého vzdelania., Aim: The aim of the paper was to ascertain the quality of life of women with uterine cancer in relation to their age and educational level. Methods: The sample consisted of 300 patients who were diagnosed with uterine cancer and underwent hysterectomy. The research was using a standardized questionnaire - Quality of Life in Adult Cancer Survivors (QLASC). Results: Women aged over 50 years reported better quality of life than women aged under 50 years. More favorable results recorded in patients under 50 years of age were only shown in the domain of Benefit of disease. In relation to educational attainment, more favorable results were recorded in women with higher education based on the average scores in eight domains. Interesting was the finding that in the domains Negative emotions and Distress of recurrence, women with lower education were experiencing better quality of life than women with higher education. Článek Lidské embryo v perspektivě bioetiky se podrobně věnuje současné bioetické diskuzi o morálním statusu lidského embrya. Nejdříve si klade otázku, zda je lidské embryo od okamžiku oplození individuálním jedincem lidského druhu a kriticky hodnotí některé námitky proti individualitě lidského embrya. Poté se zaměřuje na problematiku kritérií personální identity a jejich relevanci pro bioetickou diskuzi. Kriticky se vyrovnává rovněž s tzv. konstitucionalizmem. Metafyzickou reflexi uzavírá obhajoba animalismu. V poslední části článku se hodnotí etický relativismus, utilitarismus a předkládá se nárys etické teorie Johna M. Finnise. Závěr práce tvoří teze, že lidské embryo je od okamžiku svého vzniku lidskou osobou obdařenou inherentními, nezcizitelnými lidskými právy., The article The Human Embryo from a Bioethical Perspective devotes in detail to the contemporary bioethical discussion on the moral status of the human embryo. First, it rises an issue whether a human embryo is an individual being of a human kind since the moment of fertilization and it critically assesses some objections against individuality of the human embryo. Following that it focuses on criteria of personal identity and its relevance for bioethical discussion. It critically deals with the so called constitutionalism. Metaphysical reflection is concluded by a defense of animalism. The last part of the article evaluates ethical relativism, utilitarianism and brings in an outline of an ethical theory of John M. The article presents results of a study of the relationship 'locus of control health and subjective wellbeing. In a heterogeneous sample (N = 239) in the stage of youth (1930 years) shall be established: locus of control determines 11.56% of the changes in the assessment of health, changes in health contribute to 12.82% of the variation in assessment of happiness and 6.10% in the measurement of life satisfaction. Young people with an internal locus of control are healthier, happier and more satisfied with their lives than those with an external locus of control. In a study of cause effect relationship is established that internal locus of control determines a healthier lifestyle. Inadequate health care, more severe risk factors (stress, unhealthy diet, inadequate sleep, smoking) among young people with an external locus of control, outline prospects for greater vulnerability of this group of the population of the so called "Diseases of modern society." Introduction and Objectives: One of the characteristic changes of tumor formation is accumulation of genetic disorders in mitochondrial and nuclear genome. Mitochondrial disorders, from its side, are responsible for failure of metabolism, apoptosis, cell growth, formation of reactive oxygen species, etc. Overprpoduction of reactive oxygen species (ROS) significantly impacts the respiration chain enzymes and entirely the antioxidant system of mitochondria. Finally this may become a favorable condition for normal cells transformation. The purpose of the presented work was to study the mitochondrial defects and to establish their role in prostate cancer development. Results: Experimental results demonstrate significant increase of the activity of mitochondrial succinate dehydrogenaze (complex II) of the malignant epithelial cells of prostate, and slight changes in cytochrome oxydase (complex IV) activity. Also significant activation of the antioxidant system (glutathione-dependant system) of mitochondria in prostate malignant epithelial cells was revealed. Hysteroskopia je efektívna diagnostická a terapeutická metóda v intrauterinnej patológií, ktorá má zo všetkých dostupných diagnostických metodík najvyššiu senzitivitu a špecificitu. Hysteroskopia postupne nahrádza diagnostickú kyretáž, ktorá v porovnaní s hysteroskopiou neposkytuje adekvátnu diagnostiku organického nálezu v dutine maternice, prípadne riešenie intrauterinnej patológie diagnostickou kyretážou je skôr náhodné., Hysteroscopy is an effective diagnostic and therapeutic methods of intrauterine pathology that has all of the available diagnostic methodologies highest sensitivity and specificity. Cieľ: Cieľom práce bolo zistiť výskyt komplikácií v závislosti od veku a pohlavia pacientov, od dĺžky zavedenia stimulačnej elektródy, od zvolenej cesty prístupu zavedenia stimulácie u pacientov hospitalizovaných na Oddelení arytmií a koronárnej jednotky v Univerzitnej nemocnici Martin. Metodika: Pre zber potrebných údajov sme použili obsahovú analýzu zdravotnej dokumentácie, nemocničný informačný systém, pozorovanie na základe vypracovaného záznamového hárku. Údaje sme analyzovali pomocou deskriptívnej a induktívnej štatistiky. V sledovanom súbore bolo 38 pacientov od 58 rokov do 93 rokov. Pacienti boli akútne prijatí na Jednotku intenzívnej starostlivosti za účelom realizácie dočasnej kardiostimulácie, ktorá bola užívaná v priemere 5,28 ± 2,33 dní. Výsledky: Na základe spracovania údajov sme u 17 pacientov zistili výskyt komplikácií, pričom najčastejšou komplikáciou bola lokálna infekcia. Štatistickou analýzou sme zaznamenali významné vzťahy medzi dĺžkou zavedenia stimulačnej elektródy a výskytom lokálnej infekcie, kde nám vznikla silná signifikantná korelácia r(S) = 0,518 na hladine významnosti p = 0,001 a rovnako pri výskyte hematómu sme zaznamenali koreláciu r(S) = 0,329 na hladine významnosti p = 0,023. Záver: Analýzou získaných údajov sme identifikovali najčastejšie vyskytujúce sa komplikácie v spojitosti s dočasnou kardiostimuláciou. Najčastejšie sme zaznamenali výskyt lokálnej infekcie, ktorý súvisí s následnou ošetrovateľskou starostlivosťou, ale aj s významnými prediktormi, ktoré môžu do značnej miery ovplyvniť ich vývoj., Aim: The aim of our study was to determinate the incidence of complications depending on the age and gender of patients, duration of implantation of stimulation electrode, of selected access point of implementation stimulation in patients hospitalized at the Arrhythmia unit and Coronary care unit in University hospital in Martin. Methods: For the collection of required data we have used content analysis of medical records, hospital information system, surveillance based on own designed sheet. The data were analysed using descriptive and inductive statistic. The sample consisted of 38 patients in the range from 58 to 93 years of age. Patients were acutely admitted to ICU with the aim of realization of temporary cardiac pacing, which lasted 5.28 ± 2.33 days in average. Results: On the basis of processed data, we found incidence of complications in 17 patients, where the most frequent complication was local infection. Using statistical analysis we found a significant relationship between duration of implementation of stimulation electrode and incidence of local infection, which resulted in strong significant correlation (r(S) = 0.518, p = 0.001) and similarly, we noted correlation in the incidence of haematoma (r(S) = 0.329, p = 0.023). Conclusion: Using analysis of the obtained data we have identified the most frequently occurring complications in conjunction with the temporary cardiac pacing.
