Hemangiom je nejčastější primární benigní nádor páteře typu hamartomu, který nikdy nemalignizuje. V drtivé většině případů se jedná o asymptomatické solitární léze hrudní páteře jakožto náhodné nálezy radiografických vyšetření. Pouze v 1 % případů se hemangiom obratle projeví klinicky a agresivita této benigní léze pak spočívá v expanzivním chování nádorové tkáně či v důsledku patologické fraktury obratle. Klinickými příznaky potom jsou především dorzalgie, v pokročilých případech pak známky kořenové nebo míšní komprese. Radiodiagnostika je poměrně snadná, protože hemangiom obratle vykazuje zcela typické grafické příznaky, pro potvrzení diagnózy agresivního hemangiomu obratle jsou stanovena grafická kritéria. Spektrum léčebných modalit je velmi široké od konzervativní léčby přes hodně využívanou vertebroplastiku po radikální chirurgickou léčbu. Komplementárně jsou často využívány embolizační výkony, alkoholová sklerotizace a lokální radioterapie. Agresivní hemangiom obratle je poměrně velmi vzácná diagnóza. Proto zatím neexistují velké soubory či klinické studie, podle kterých by bylo možné stanovit optimální terapeutický postup., Hemangioma is the most common primary benign hamartoma type spine tumor. Malignant degeneration has never been reported. In the majority of cases, this is an asymptomatic solitary lesion of the thoracic spine found incidentally during a radiographic examination. Only 1% of cases manifests clinically and this benign lesion is considered aggressive because of the expansive nature of the tumor and because it may cause pathological fracture of the vertebrae. Clinical symptoms then include dorsalgia and either root or spinal neurological symptoms. Radiological diagnosis is relatively easy because vertebral hemangioma is associated with quite typical graphic signs; graphic criteria to confirm the diagnosis of aggressive vertebral hemangioma have been clearly defined. The range of treatment options is very wide, from conservative treatment through frequently used vertebroplasty to radical surgical treatment. Embolization procedures, alcohol sclerotherapy and local radiotherapy are often used as complementary procedures. Aggressive vertebral hemangioma is a relatively rare diagnosis, so far there are no large cohorts or clinical studies from which it would be possible to determine an optimal therapeutic approach. Zlatým standardem diagnostiky je stanovení specifických autoprotilátek a průkaz vilózní atrofie v biopsii z duodenální sliznice. Kauzální a ve většině případů efektivní léčbou je celoživotní striktní bezlepková dieta, která vede k restituci sliznice tenkého střeva a snížení rizika závažných komplikací., Coeliac disease is a common lifelong autoimmune disorder triggered by gluten consumption in genetically predisposed patients. Chronic inflammatory response damages the mucosa of the small intestine resulting in villous atrophy with chronic maldigestion and malabsorption. Untreated coeliac disease may present with a variety of intestinal and extraintestinal symptoms and complications, the most serious of which is a small intestine lymphoma. The diagnosis is usually established by specific serology markers and villous atrophy in duodenal biopsy. Syndróm polycystických ovárií (PCOS) je najčastejšou endokrinopatiou u žien fertilného veku a najčastejšou príčinou porúch menštruačného cyklu. Je charakterizovaný hyperandrogénnym stavom (klinickým alebo biochemickým) a ovariálnou dysfunkciou (anovuláciou alebo ultrasonografickým nálezom polycystických ovárií), čo sú zároveň kritériá pre jeho diagnózu podľa Androgen Excess and PCOS Society. Syndróm má mnohopočetné fenotypové prejavy, medzi ktoré patrí okrem uvedených charakteristík aj metabolický syndróm, predovšetkým obezita a inzulínová rezistencia. Diagnóza PCOS je v klinickej praxi pomerne náročná a stále platí, že je diagnózou per exclusionem, po vylúčení iných príčin hyperandrogénneho stavu a chronickej oligo-anovulácie. Vyžaduje si úzku spoluprácu gynekológa, endokrinológa a z hľadiska častých metabolických komplikácií aj internistu, diabetológa a prípadne aj kardiológa. Kľúčové slová: AES kritériá – diagnóza – diferenciálna diagnóza – syndróm polycystických ovárií, Polycystic ovary syndrome (PCOS) is the most frequent endocrinopathy among women of reproductive age and the most frequent cause of menstruation cycle disorders. It is marked by a hyperandrogenic state (clinical and/or biochemical) and ovulatory dysfunction (anovulation and/or ultrasonographic finding of polycystic ovaries), which are also criteria for its diagnosis according to Androgen Excess and PCOS Society. The syndrome has multiple phenotypic expressions, among them besides the above characteristics also a metabolic syndrome, primarily obesity and insulin resistance. Diagnosing of PCOS may be rather exacting in clinical practice and it remains to be a diagnosis per exclusionem, following elimination of other causes of hyperandrogenic state and chronic oligo-anovulation. It requires a close cooperation between a gynecologist and endocrinologist and with regard to frequent metabolic complications also with an internist, diabetologist and possibly cardiologist. Tauopatie jsou neurodegenerativní onemocnění, která se vyznačují hromaděním abnormně modifikovaných forem proteinu zvaného tau především v kůře frontálních, temporálních a parietálních laloků, v bazálních gangliích a v mezencefalu. Molekulárně biologicky a biochemicky dobře definovaná skupina chorob má však dosti heterogenní klinické obrazy a je podstatou několika různých neurodegenerativních onemocnění tvořících frontotemporální lobární degenerace. Společným klinickým znakem těchto entit je ve větší či menší míře vyjádřený syndrom frontotemporální demence. Klinické jednotky, jež se řadí mezi klasické frontotemporální demence, však nejsou vždy podmíněny jen tauopatiemi, mnohdy je jejich etiopatogenetická podstata jiná. Účelem tohoto stručného přehledu je utřídění současné úrovně poznání vztahů mezi skupinou tauopatií a frontotemporálních lobárních degenerací. Článek obsahuje i návrh praktického postupu k upřesnění klinické diagnózy jednotlivých nozologických jednotek v rámci tauopatií., Tauopathies are neurodegenerative disorders characterized by accumulation of abnormally modified forms of the tau-protein, predominantly in frontal, temporal and parietal cortical regions, basal ganglia and in the midbrain. Tauopathies are well defined from the molecular biological and biochemical point of view; clinical symptoms may be, however, heterogeneous. Common signs of frontotemporal lobar degenerations include a more or less prominent syndrome of frontotemporal dementia. On the other hand, the clinical picture of frontotemporal dementia may not be caused by a tauopathy only; in many cases the underlying etiopathogenic cause is different. The aim of our review is to classify the relationship between tauopathies and frontotemporal lobar degenerations. This review contains a proposal of a practical approach to refining clinical diagnosis of the different tauopathies. Dráždivý tračník (IBS) spolu s funkční dyspepsií jsou nejčastějšími funkčními poruchami trávicího traktu. Předmětem článku je souhrnný pohled na etiologii a patogenezi této choroby, diagnostiku a léčbu. Dráždivý tračník je charakterizován zejména bolestmi břicha a změnou tvaru a frekvence stolice. Je doporučováno definovat a klasifikovat jej podle „římských kritérií III“. Současný výzkum je zaměřen zejména na studium viscerální hypersenzitivity a na teorii tzv. minimálního zánětu a neuroimunní interakce. Etiologe a patogeneze jsou však stále neobjasněné a v klinické praxi zůstává farmakologická léčba dráždivého tračníku na úrovni léčby symptomatické, tj. zmírnění nebo odstranění bolesti, průjmu nebo zácpy, a v podávaní farmak s antianxiózním, event. antidepresivním účinkem. Lékem první volby by dnes měl být sulprid, antagonista D1 a D3 receptorů, který je u nemocných s IBS předepisován v dávce třikrát denně 50–100 mg, a sice pro anxiolytický a bolest potlačující účinek. V poslední části článku jsou diskutovány vedlejší účinky léků IBS na kardiovaskulární systém, Irritable bowel syndrome (IBS) and functional dyspepsia are the most common functional gut disorders. The main topic of the article is a summary review of the aetiology and pathogenesis of this disease as well as its diagnostic procedures and treatment. Irritable bowel syndrome is characterized by abdomen pain with altered stool frequency or consistency. It is recommended that it be defined and classified according to the Rome III classification. The symptoms are induced by physiological aberrations (like dysmotility and visceral hypersensitivity) or by the mechanisms of the initial inflammation and neuro‑immune interaction. This is also the focus of contemporary research. The aetiology and pathogenesis, however, remain unclear and the therapy of the day remains at the level of decreasing the main symptoms: abdominal pain, constipation, diarrhoea and psychological factors. Sulpiride, an antagonist of the D1 and D3 receptors, at a dose of 50 or 100 mg 3 times daily should be a first–choice treatment of IBS due to its anxiolytic and analgesic effects. Familial Mediterranean Fever is an autosomal recessive inherited disease with a course of autoinflammation, which is characterized by the episodes of fever and serositis. It affects the populations from Mediterranean basin. Genetic mutation of the disease is on MEFV gene located on short arm of Chromosome 16. The disease is diagnosed based on clinical evaluation. Amyloidosis is the most important complication. The only agent that decreases the development of amyloidosis and the frequency and severity of the episodes is colchicine, which has been used for about 40 years. Úvod: Správné zhodnocení jaterních pseudolézí pomocí multidetektorového CT či MR je velmi důležité z důvodu odlišení těchto benigních změn od primárních či sekundárních malignit v jaterním parenchymu. Metoda: Autoři provedli systémový přehled literatury v databázi PubMed s vyhledáváním slov játra, pseudoléze v angličtině a češtině od roku 2000 do roku 2014. Výsledky: Autoři předkládají přehled literatury. Dále autoři retrospektivně zhodnotili soubor pacientů léčených pro jaterní onemocnění ve FN Ostrava, kdy se setkali s touto abnormalitou celkem v 7 případech. U 3 pacientů byla provedena diagnostická laparoskopie s vizuální kontrolou léze, s upřesněním diagnostiky pomocí peroperační ultrasonografie (IOUS) a s excizí části ložiska pro následné histologické vyšetření. Histologický nález u všech 3 pacientů odpovídal steatotickému postižení jater. Další 2 pacienti byli operováni pro jiná ložiska metastazujícího kolorektálního karcinomu a revize suspektní léze byla provedena během operačního výkonu otevřenou cestou. Kromě vizuální kontroly a peroperačního IOUS byla provedena excize k histologickému vyšetření, které prokázalo známky velkokapénkové i malokapénkové steatózy a nepřítomnost maligních změn. Poslední dva pacienti jsou dále sledováni v hepatologické poradně a jsou v rámci dispenzarizace kontrolováni v 6měsíčních intervalech. Závěr: Autoři prezentují vlastní zkušenosti získané v rámci mezioborové spolupráce na multidisciplinárních konferencích a připojují literární údaje o této neobvyklé problematice. Správným vyhodnocením nálezů této variety zejména u onkologických pacientů je možno zabránit zbytečným biopsiím, duplicitním vyšetřením a diagnostickým laparoskopiím či explorativním laparotomiím., Introduction: Accurate detection of hepatic pseudolesions using multi-detector CT and MRI examinations is crucial for the differentiation of benign alterations from primary and secondary malignant lesions in hepatic parenchyma. Method: The authors conducted a systematic literature review in PubMed. “Liver“ and “pseudolesion“ were used as keywords in English and Czech, and papers/articles published from 2000 to 2014 were retrieved. Results: The authors presented a literature review. In addition, the authors performed a retrospective evaluation of a group of patients treated for liver disease at University Hospital Ostrava where this anomaly was encountered in 7 cases. In 3 of the patients, diagnostic laparoscopy was done, with visual examination of the lesion accompanied by intraoperative ultrasound exam (IOUS) and partial excision, to establish the diagnosis. Subsequent histological assessment of the specimens confirmed the diagnosis of a steatotic lesion in each of these 3 patients. Additional 2 of the 7 patients underwent liver surgery for concurrent metastatic lesions of colorectal cancer and an open-access revision of the suspected lesions was performed. Visual inspection and intraoperative ultrasound (IOUS) was followed by excisional biopsy. The histology revealed macro- and micro-vesicular steatosis and excluded malignant changes. The last 2 patients still continue to be followed–up regularly on a 6-month routine check-up basis at our hepatology unit. Conclusion: The authors presented their own experience gained through inter-disciplinary cooperation at Multidisciplinary conferences. A literature overview of this unusual subject is also included. Particularly in oncologic patients, correct interpretation of these pseudolesions may help to avoid unnecessary biopsies, further imaging examinations and diagnostic laparoscopies and/or explorative laparotomies., and P. Hepatorenální syndrom je život ohrožující komplikace jaterních chorob. Jedná se o funkční postižení ledvin, které se vyskytuje jak u akutních (typ I), tak i chronických (typ II) onemocnění jater a je spojeno s vysokou mortalitou. Léčba má omezené možnosti, ale podávání vazopresorů (terlipresin), albuminu a portosystémový shunt dokážou prognózu zlepšit. Transplantace jater je léčbou nejefektivnější. Autoři prezentují aktuální data o této problematice včetně doporučení pro léčebnou praxi., Hepatorenal syndrome (HRS) is a life-treating complication of liver diseases. This functional kidney impairment is classified into acute (type I) and chronic (type II) types and is connected with high mortality. Treatment options are limited, but administration of vasoconstrictors (terlipressin), albumin and portosystemic shunt may improve their prognosis. Liver transplantation is the most effective method for these patients. Angiofibromas are rare vascular tumors which originate predominantly in the nasopharynx and occur typically in male adolescents. Extranasopharyngeal sites such as nasal cavity and paranasal sinuses are less frequent. This review article was undertaken to evaluate the incidence, clinical features and management of extranasopharyngeal angiofibromas originating exclusivelly from nasal cavity structures. Our focus of interest was to evaluate the significance of immunohistochemical analysis in diagnosis of such extremely rare neoplasms. In the PubMed and Google Search, we found only 39 cases of nasal angifibroma, 27 males and 12 females from 1980 to 2012. The most prevalent site of origin was nasal septum, followed by inferior and middle turbinate. The commonest symptoms were nasal obstruction and epistaxis. Nasal angiofibromas are clinically distinct from nasopharyneal angiofibromas and can therefore be misdiagnosed. The differential diagnosis includes other vascular lesions, such as lobular capillary hemangioma and sinonasal-type hemangiopericytoma. Although immunohistochemistry is not necessary for differentiation between angiofibroma and capillary hemangioma, that diagnostic procedure may be helpful in distinction from sinonasal hemangiopericytoma. As an ilustration for immunohistochemical analysis, we presented a case of an elderly woman with tumor arising from the middle turbinate, diagnosed as angiofibroma.
