Sometimes intraorbital foreign bodies lead to unexpected findings. A 16-year old boy was referred due to ocular surface trauma. A conjunctival laceration was detected at the level of the left caruncle with associated left exotropia, reduced adduction as well as a preretinal hemorrhage along the nasal periphery of the fundus. A blow-out fracture of the medial orbital wall was suspected and a CT scan of the orbits was scheduled which revealed the presence of a large intraorbital foreign body. The removal of the intraorbital foreign body (which proved to be a bullet) and precautionary laser photocoagulation along the nasal periphery of the left eye were performed. Variant branching pattern of the cords of brachial plexus coupled with erroneous communications has been an area of concern for surgeons opting to explore this region. Anaesthetic blocks and surgical approaches are the highlights of these interventions, where a keen familiarization of the anatomy of this region is mandatory. The present case description reports a unilateral variant branching pattern of the posterior cord coexistent with a neural communication between lateral and medial cords in an adult male cadaver. This intercordal neural communication between lateral and medial cords was oriented obliquely and measured 2.2 cm in length. Furthermore, the posterior cord revealed a variant branching pattern. It branched into three upper subscapular nerves and a common trunk for the thoracodorsal and lower subscapular nerves. The lowest of the three upper subscapular nerves gave a communicating twig to the thoracodorsal nerve. Úraz podtlakem v bazénu je skupina specifických raritních poranĕní s velkým rizikem fatálních důsledků. Častĕjší obĕtí s častĕji vážnějšími důsledky jsou zejména malé dĕti. Při tomto druhu poranĕní dochází ve vĕtší či menší míře k tonutí postiženého. Případ 19-letého pacienta zachyceného v oblasti hýždí nezajištěným otvorem sací drenáže zajímavĕ ilustruje nĕkteré zvláštnosti této problematiky. Preventivní opatření jsou v současnosti již známá, problém bývá vĕtšinou s jejich důslednou aplikací. K fatálním příčinám patří kromĕ utonutí, hypovolemický šok z náhlé redistribuce tekutin třetího prostoru a eviscerace s devastací vnitřních orgánů. Lokalizované často bizardní a velké otoky a sufúze lze v naprosté vĕtšinĕ případů řešit konzervativnĕ. Pro prevenci je klíčové rozšíření informace o existenci těchto poranĕní do obecného povědomí. Pro jejich správný management je pak důležité prohlubování povědomí a sdílení zkušeností s řešením těchto raritních poranĕní v odborné veřejnosti., Swimming pool suction injuries are unique and rare with a substantive risk of fatal consequences. Little children under the age of 8 are the most frequent victims with serious injuries. Drownings of different seriousness are also a usual part of accidents. The case of a 19 year old man trapped in the gluteal area by a unsecured suction drainage hole illustrates the uniqueness of this problem in an interesting way. Prophylactic arrangements are well known but the problem is with their strict application. Fatal causes excluding drowning include hypovolemic shock from the sudden redistribution of intersticial fluid and blood and also the evisceration of the bowel and other abdominal organs. Localised often bizarre and large swellings and sufusions can be treated nonoperatively in the vast majority of cases. For the prevention of these injuries it is important to inform the public and increase their awareness to these injuries. „Upside-down stomach syndrome“ je sporadicky se vyskytující typ velké paraezofageální hiátové hernie vyžadující při známkách uskřinutí neodkladné chirurgické řešení. Autoři článku prezentují kazuistiku 53letého nemocného s volvulem žaludku při „upside-down stomach syndromu“. Chirurgický výkon byl komplikován poraněním abdominální části jícnu, které bylo úspěšně léčeno mimo jiné použitím samoexpandibilního metalického stentu. Přes závažný klinický pooperační průběh s nutností revize dutiny břišní, zavedení jícnového stentu, provedení torakotomie pro absces mediastina a sekundárního hojení laparotomie byl pacient po padesáti dvou dnech propuštěn do domácího léčení v uspokojivém stavu, plně realimentován a rehabilitován, se zhojenou perforací jícnu i laparotomií., Upside-down stomach syndrome is a rare type of a large paraoesophageal hiatal hernia, which requires an immediate surgical treatment in case of incarceration. The authors present a case report of a 53-year-old male patient with gastric volvulus related to the upside-down stomach syndrome. Surgical treatment was complicated by an injury to distal oesophagus, which was successfully treated using a self-expandable metallic stent among other methods. Despite the complicated postoperative course with a necessity of reoperation, insertion of an oesophageal stent, thoracotomy for a mediastinal abscess and secondary healing of the laparotomy, the patient was discharged in a good condition with healed oesophageal perforation and laparotomy after 52 days., and L. Bio-degradable stents are be made of different synthetic polymers (like polylactide or polyglycolide) or their co-polymers (polydioxanone). They can be used for treating benign stenoses of the small and large intestine, particularly in Crohn’s disease. Endoscopic introduction of bio-degradable stents into small and large intestinal stenoses is feasible and relatively simple. Initial results are encouraging and the complication rate is low. However, there are still some difficulties that need to be overcome. The rate of early stent migration is still rather high (up to one third of patients). This might be solved by changes in the shape or rigidity of the stents as well as by further improvement in the design. The pilot study deals with the clinical results of the regenerative therapeutical method in intraosseous periodontal defects done by surgical augmentation procedure using a material of a new generation prepared on the basis of a bioactive glass. A group of 10 individuals were treated properly for chronic periodontitis. Some of treated infraalveolar periodontal lesions, both periodontal pockets and interradicular defects, persisted or recurred in posterior teeth. In order to eliminate them they were indicated for advanced periodontal surgery or regenerative surgical therapy stimulating healing processes of supportive tissues, mostly alveolar bone around treated teeth. Relevant clinical parameters, i. e. values of the pocket depth, gingival recession, and loss of attachment were evaluated before and after the periodontal surgery and compared with each other. Epilepsia je chronické ochorenie, ktoré je často sprevádzané psychiatrickou komorbiditou. Najčastejšou z nich je depresia, ktorá je sprevádzaná subsyndromálnymi, atypickými a chronifikovanými príznakmi depresie. Incidencia depresie je u pacientov s epilepsiou vyššia ako u iných neurologických alebo chronických ochorení. Psychopatológia spojená s epilepsiou obťažuje pacienta častokrát viac ako samotné ochorenie. Cieľom práce je prezentácia a analýza kazuistiky 37-ročného muža s diagnózou epilepsie so sprievodnou psychiatrickou symptomatikou, ktorá znížila kvalitu jeho života oveľa viac ako samotné neurologické ochorenie. V kazuistike popisujeme klinické symptómy neuropsychiatrického ochorenia – epilepsie ako aj životné udalosti, ktoré sú u každého pacienta jedinečné a majú závažný vplyv na priebeh a liečbu epilepsie., Epilepsy is a chronic disease that is often accompanied by psychiatric comorbidity. The most frequent of them is depression accompanied by subsyndromic, chronic and atypical symptoms of depression. The incidence of depression in patients with epilepsy is higher than in other neurological or chronic diseases. Psychopathology associated with epilepsy often bothers the patient more than the disease itself. The aim of this work is to present and analyze a case report of a 37‑year‑old man diagnosed with epilepsy and associated psychiatric symptomatology that reduced his quality of life much more than the neurological disease itself. In this case report, we describe clinical symptoms of neuropsychiatric disease – epilepsy as well as life events that are unique to each patient and have a significant impact on the course and treatment of epilepsy. Úvod: Důležitou součástí diagnostického algoritmu neuropatické bolesti jsou jednoduché dotazníkové nástroje založené na přítomnosti typických popisných charakteristik (tzv. deskriptorů). V českém jazykovém prostředí však není k dispozici validovaná verze žádného z mezinárodně akceptovaných dotazníků. Cíl: Cílem práce bylo vytvoření české jazykové verze jednoho z nejpoužívanějších dotazníků zaměřených na diagnostiku neuropatické bolesti a charakteristiku jednotlivých jejích subtypů, a to tzv. NPSI (Neuropathic Pain Symptom Inventory) a její validace. Soubory a metodika: Dotazník byl přeložen metodou „forward‑backward translation“. Pro potřeby validace byla touto českou jazykovou verzí následně vyšetřena skupina pacientů s neuropatickou bolestí periferního (n = 66) nebo centrálního (n = 30) původu (v rámci polyneuropatie či roztroušené sklerózy) a skupina nemocných s bolestí nociceptivní v důsledku pokročilé artrózy kyčelního nebo kolenního kloubu (n = 70). Za účelem hodnocení opakovatelnosti testu byl u části pacientů (n = 19) dotazník administrován dvakrát s odstupem přibližně tří hod. Výsledky: Srovnání jednotlivých hodnocených položek, dílčích škál i celkového NPSI skóre pomocí ROC analýzy prokázalo vysokou schopnost diskriminace mezi neuropatickou a nociceptivní bolestí (p < 0,001), ale nízkou schopnost odlišení neuropatické bolesti centrálního a periferního původu (p > 0,05). Opakovaná administrace testu potvrdila vysokou opakovatelnost testu (ICC, Intraclass Correlation Coefficient = 0,86 pro sumární skóre). Pomocí clusterové analýzy bylo identifikováno šest odlišných symptomových profilů. Závěr: Vytvořená česká verze dotazníku NPSI je schopna spolehlivě diskriminovat neuropatickou a nociceptivní bolest a identifikovat odlišné symptomové profily bolesti při akceptovatelné opakovatelnosti testu. Dotazník lze tedy doporučit jako vhodný a snadno použitelný nástroj v diagnostickém algoritmu neuropatické bolesti, využitelný i pro longitudinální sledování., Introduction: Simple questionnaires based on so called “neuropathic pain descriptors” are the most frequently used screening tools in the diagnosis of neuropathic pain, especially by non‑specialists. However, no validated Czech version of any of the internationally accepted nueropathic pain questionnaires is available. Aim: The aim of this study was to develop and validate a Czech version of the Neuropathic Pain Symptom Inventory (NPSI), a questionnaire that allows the user to characterize subtypes of neuropathic pain, is sensitive to change, and has discriminatory power. Patients and methods: The NPSI was translated into the NPSIcz using the forward‑backward translation method and validated in a group of patients suffering from neuropathic pain of peripheral (n = 66) or central origin (n = 30) due to polyneuropathy or multiple sclerosis, and in a group of individuals suffering from nociceptive pain (due to severe osteoarthritis of the knee or hip joint; n = 70). The NPSIcz was repeated within three hours in 19 individuals to determine test‑retest reliability. Results: The NPSIcz showed good diagnostic validity in discriminating between patients with neuropathic and nociceptive pain (p < 0.001) but low ability to discriminate between patients with central and peripheral neuropathic pain (p > 0.05). Repeated administration confirmed acceptable test‑retest reliability (intra‑class correlation coefficient of 0.86 for the NPSIcz sum score). Cluster analysis identified six different pain profiles. Conclusions: The NPSIcz is able to discriminate between neuropathic and nociceptive pain, to identify different pain profiles, and has acceptable test‑retest reliability. It can, therefore, be recommended as an easy‑to‑administer questionnaire suitable for the diagnosis of neuropathic pain, including long‑term follow up. Cíl: Cílem bylo určit a zobrazit typický rozsah plicních laloků a jeho krajní varianty. Metodika: Z 250 HRCT vyšetření hrudníku dospělých pacientů bez nálezu výraznější patologie jsme zjistili varianty rozsahu plicních laloků a následně jsme typické polohy a jejich krajní varianty promítli na prosté snímky. Výsledky: Na levé straně byla hranice mezi plicními laloky nejvýše dorzálně nejčastěji u čtvrtého žebra (45 %), s rozsahem od druhého po osmé žebro. Laterodorzálně bylo dolní rozhraní nejčastěji v úrovni šestého žebra (52 %), s rozsahem od čtvrtého po osmé žebro. Vpravo byla hranice mezi horním a dolním plicním lalokem nejčastěji nejvýše u pátého žebra (49 %), s rozsahem od čtvrtého po osmé žebro. Laterodorzálně byla dolní hranice nejčastěji v úrovni šestého žebra (50 %), s rozsahem od čtvrtého po osmé žebro. Vedlejší interlobium probíhalo nejčastěji v úrovni ventrálního konce čtvrtého žebra. Závěr: Průmět plicních laloků na snímcích se mezi jednotlivými osobami poměrně významně liší. I při kombinaci dvou projekcí jsou v obou plicích velké oblasti, kde se mohou překrývat průměty laloků. Vzhledem k velké variabilitě polohy interlobií snímky často neumožňují spolehlivou lokalizaci plicních lézí do laloků., Aim: To determine and depict typical extend of lung lobes and its border variants. Methods: HRCT examinations of the lung in 250 adult patients without major pathology were assessed to find out variants of lung lobes localizations. Afterwards we projected typical localizations and their extreme variants on the chest X-rays. Results: On the left side, the most frequent uppermost localization of the border between lung lobes was posteriorly at the fourth rib (45%), range from the second to the eighth rib. Posterolaterally the inferior border was most frequently at the sixth rib (52%), range from the fourth to the eighth rib. On the right side, the most frequent uppermost localization of the border between upper and lower lung lobes was posteriorly at the fifth rib (49%), range from the fourth to the eighth rib. Posterolaterally the inferior border was most frequently at the sixth rib (50%), range from the fourth to the eighth rib. Minor fissure was most frequently located at the level of anterior part of the fourth rib. Conclusion: The projection of lung lobes on the x-rays is considerably different among individuals. Even with two projections there remain extensive areas in both lungs, where the demarcations of lobes overlay each other.
