Antifosfolipidový syndrom (APS) je autoimunitně podmíněné onemocnění s celou řadou potenciálně život ohrožujících manifestací. Vyskytuje se samostatně v primární podobě nebo jako sekundární doprovázející řadu chorobných stavů (autoimunitní, nádorová onemocnění atd.). Diagnostická kritéria zahrnují jak klinickou, tak laboratorní složku. Mezi klinické manifestace patří výskyt trombóz arteriálních či žilních a dále komplikace v těhotenství. Laboratorně musí být opakovaně prokázána přítomnost antifosfolipidových protilátek v minimálním odstupu 12 týdnů. Klinická manifestace APS může být velmi pestrá a často vyžaduje multidisciplinární přístup. Obávanou, ale vzácnou komplikací je tzv. katastrofický APS spojený s vysokou morbiditou a mortalitou. Včasná diagnóza spolu s terapií výrazně ovlivňuje prognózu pacientů s APS. APS occurs in a primary form with no associated conditions, or in a secondary form associated with various other illnesses (autoimmune, neoplastic and others). APS is diagnosed according to clinical and laboratory criteria. It is characterized by recurrent arterial or venous thrombosis and/or pregnancy‑related complications and laboratory findings of antiphospholipid antibodies in a minimum time interval of 12 weeks between individual laboratory tests. Clinical manifestations of APS can vary and a multidisciplinary approach is needed. Catastrophic APS is a very serious and life‑threatening condition associated with high mortality and morbidity, although it is rare. Sekundární prevence aterotrombotických příhod je doménou protidestičkové léčby, podle rizika jedním lékem či kombinací acetylsalicylové kyseliny s blokátory ADP receptorů. Význam kombinace duální protidestičkové léčby v kombinaci s xabany či s dabigatranem prověřovalo 6 klinických studií. Pouze jedna z nich (ATLAS ACS 2-TIMI 51) ukázala, že léčba malými dávkami rivaroxabanu (2krát 2,5 mg) může být přidána ke kombinaci kyseliny acetylsalicylové s klopidogrelem. Pro několikanásobně zvýšené riziko velkých krvácivých příhod se však čistý klinický přínos pohybuje jen kolem 0,5 % ročně. Duální léčba s kombinací kyseliny acetylsalicylové s prasugrelem či s tikagrelorem je přínosnější. V druhé části přehledu je rozebírán vyšší výskyt infarktu myokardu v kontrolovaných studiích při léčbě dabigatranem v porovnání s warfarinem. Vztah není dořešen, nicméně u nemocných s vyšším rizikem koronárních příhod je při indikaci antikoagulační léčby přímými antikoagulancii vhodnější volit ze skupiny xabanů (apixaban či rivaroxaban)., Secondary prevention of atherothrombotic events is the domain of antiplatelet therapy and according to present risk is used one drug strategy or combination of acetylsalicylic acid with ADP receptor blockers. The importance of the combination of dual antiplatelet therapy together with xabans or dabigatran was investigated in 6 clinical trials. Only one of them (ATLAS ACS 2-TIMI 51) indicated that treatment with small dose of rivaroxaban (2 × 2.5 mg) may be added to dual strategy of acetylsalicylic acid and clopidogrel. The risk of major bleeding event is increased and net clinical benefit is only about 0.5 % per year. Dual therapy with aspirin and prasugrel or tikagrelor is beneficial. In the second part of the review is discussed higher incidence of myocardial infarction in controlled group in the trial comparing treatment of dabigatran with warfarin. Literature data support that green tea and its major component epigallocatechin gallate (EGCG) have powerful antioxidant effects. Contrary, hepatotoxicity can be induced by high-dose EGCG. The timing of exposure to green tea in relation to administration of hepatotoxic agent plays an import role too. The aim of our work was a verification of antioxidative effect of EGCG on D-galactosamine-induced injury in primary culture of rat hepatocytes. Hepatocytes were incubated with EGCG at concentrations of 1.25-10 μM and toxic D-galactosamine (GalN) for 24 hrs. Alternatively, hepatocytes were pretreated with EGCG for 24 hrs, and then incubated with EGCG and GalN for further 24 hrs. Cytotoxicity was analysed by lactate dehydrogenase activity, functional capacity by albumin production. Oxidative stress was evaluated from a production of malondialdehyde and glutathione content in the cells. EGCG protected hepatocytes against GalN-induced cytotoxicity but preventive treatment of intact hepatocytes with EGCG was required to diminish the development of hepatocyte injury. Oxidative stress induced in our study seems to overcome the ability of hepatocytes to improve GSH depletion and albumin production. The aim of our study was to determine the effect of selected cytostatics on a human ovarian cancer cell line A2780 as a model system for ovarian cancer treatment. This cell line is considered cisplatin-sensitive. Panel of tested cytostatics included cisplatin, paclitaxel, carboplatin, gemcitabine, topotecan and etoposide. These cytostatics have a different mechanism of action. To evaluate cytotoxic potential of the tested compounds, the methods measuring various toxicological endpoints were employed including morphological studies, MTT assay, dynamic monitoring of cell proliferation with xCELLigence, cell cycle analysis, caspase 3 activity and expression of proteins involved in cell cycle regulation and cell death. Vzniku trombózy v arteriálním nebo venózním cévním systému předchází složitá souhra faktorů vnějšího prostředí a faktorů genetických. Rozsáhlé genomové asociační studie se ukazují jako velice přínosné v identifikaci lokusů spojených s kardiovaskulárními onemocněními a souvisejícími rizikovými faktory. Ještě zbývá mnoho práce v identifikaci určujících genů a jejich kauzálních variant, stejně jako v definici mechanizmů, které ovlivňují vývoj a progresi onemocnění. Z výsledků studií lokusů spojených s dříve identifikovanými onemocněními se očekává zlepšení našich znalostí o patofyziologii kardiovaskulárních onemocnění a možnost identifikovat nové cíle pro léčbu. V oblasti aterotrombózy a trombofilie jsou významné výsledky z asociačních studií zaměřených na oblast ischemické choroby srdeční, ischemické cévní mozkové příhody a žilního tromboembolizmu. The genomewide association approach has proved successful in identifying loci associated with cardiovascular disease and related risk factors. However, much work remains to identifyning the culprit genes and causal variants as well as the mechanisms whereby they influence disease development and progression. In-depth studies of previously identified disease-associated loci are expected to improve our understanding of the pathophysiology of cardiovascular disease and identify novel targets for treatment. In the field of atherothrombosis and thrombophilia are significant results from association studies focused on the area of coronary artery disease, ischemic stroke, venous thromboembolism. INTRODUCTION: The issue of resistance to antiplatelet therapy has raised many questions in the area of neurovascular diseases. The first objective of this work was to determine the prevalence of aspirin resistance in neurovascular patients with clinical non-responsiveness to aspirin treatment and a high-risk of atherothrombotic complications using two interpretable and independent methods (aggregation and PFA 100). The second objective was to find the correlation between both assays and to evaluate the results in groups at risk for various cerebrovascular diseases. MATERIAL AND METHODS: Laboratory tests of aspirin resistance were performed in 79 patients with clinical non-responsiveness to aspirin treatment suffering from neurovascular diseases. Patients were divided into the two groups: expected low risk for aspirin resistance due to the first manifestation of a neurovascular disease (n = 34) and expected high risk due to the second clinical manifestation of a neurovascular disease (n = 45). RESULTS: The prevalence of aspirin resistance in both groups combined as determined by the PFA-100 and CPG techniques were 50.6% and 17.7%, respectively. No correlation was found between the two techniques. CONCLUSIONS: No significant prevalence of aspirin resistance was demonstrated by either method despite the heterogeneous pathophysiological mechanisms. Cíl: Převod Testu paměti pro záměry (Memory for Intentions (Screening) Test; MIST, 2010) do české verze zahrnoval standardizaci pomůcek, převod administrace a skórování, validační studii na české populaci, aby MIST bylo možné použít na české populaci. Úvod: Konstrukt prospektivní paměti (PP), tj. schopnosti si zapamatovat a realizovat záměry, je nezbytný pro uchování soběstačnosti, vykonávání aktivit denního života. V posledních letech hraje čím dál významnější roli pro diagnostiku poruch paměti. Standardizovaná a validovaná měřítka PP však v české diagnostice chybí. Metodika: Český převod MIST jsme administrovali 30 zdravým osobám. Výsledky: Celkový skór PP v testu MIST koreloval na střední úrovni s měřítky z neuropsychologické baterie zejména s retencí v paměti, mentální flexibilitou, odolností k interferenci a premorbidní inteligenční úrovní (všechna rho = 0,37–0,42, všechna p < 0,05). Reliabilita testu MIST ve smyslu vnitřní konzistence byla u základních osmi subškál nedostatečná (α = 0,50) i ve smyslu reliability obou polovin testu (split‑half reliabilita = 0,56), avšak reliabilita šesti subškál tříděných dle typu (oddálení, vodítka a modality odpovědi) byla vysoká (α = 0,88; split‑half = 0,95). Závěr: Výsledky validační studie naznačují, že český převod MIST je srovnatelný s verzí originální. Studie zpřístupňuje standardní vyšetření PP a umožňuje měření PP na klinických populacích. We included standardized testing material, translation of administration and scoring, and assessment of normative data for the MIST in the Czech population. Introduction: Prospective memory (PM), i.e., the ability to remember and implement intentions after a delay, is essential as a subsystem of episodic memory for the maintenance of independence and execution of activities of daily living. PM assessment thus plays an important role in the diagnosis of episodic memory disorders. However, there are currently no standardized and validated PM tools in Czech language. Methods: The Czech version of MIST was administered to 30 healthy persons. Results: The MIST Summary score correlated at a medium level with a range of neuropsychological measures including memory retention, mental flexibility, and resistance to interference (all rho = 0.37–0.42; all p < 0.05). The reliability of MIST in terms of internal consistency was insufficient when analyzing the eight individual MIST trials (α = 0.50), as was split‑half reliability (split‑half reliability = 0.56). In contrast, there was a high degree of reliability between six subscales classified by type (delay, cue and mode of response; α = 0.88, split‑half = 0.95). Conclusion: The reliability and validity of the Czech version of MIST is comparable to the original English version. Psoriasis patients are often susceptible to cardiovascular diseases (CVD), including atherosclerosis. Traditional markers (biochemical and inflammatory) and diagnostic tools could detect occlusive but not subclinical atherosclerosis. Carotid intima-media thickness (CIMT), has recently been recognised as a non invasive diagnostic tool for identification of premature atherosclerosis. Therefore we evaluated 80 psoriasis patients and 80 age sex matched healthy controls for serum leptin levels and apolipoprotein B/apolipoprotein A-I ratio (apoB/apoA-I ratio) in relation with CIMT of carotid artery. Carotid intima-media thickness and carotid plaques were simultaneously measured by carotid sonography. Serum concentration of leptin and apolipoprotein were measured using enzyme-linked immuno sorbent assay (ELISA) and nephelometry respectively. Raised CIMT correlated to age of onset of the disease, serum leptin and apoB/apoA-I ratio in psoriasis patients. Taking into account, values that were above the 75 percentile of the three markers (leptin, apoB/apoA-I ratio and CIMT) the odds ratio was 4.26 (2.06-8.80 CI). Leptin and apoB/apoA-I ratio showed significant cumulative association with CIMT. Results of predictive analysis supports measurement of CIMT along with estimation of serum leptin and apoB/apoA-I ratio for prediction of premature atherosclerosis in psoriasis patients. and K. Stenóza cervikálního kanálu může vést ke kompresi krční míchy a představuje nejvýznamnější mechanický faktor v patofyziologii cervikální spondylogenní myelopatie. Míšní tkáň je však vůči kompresi poměrně odolná a k rozvoji manifestní myelopatie dochází až při vyšším stupni komprese a v kombinaci s dalšími patofyziologickými faktory, zejm. dynamickým faktorem a zevním traumatem. Asymptomatická spondylogenní komprese krční míchy (ASCCC) je velmi častým nálezem zvláště v populaci vyššího věku nad 50 let. Objektivizace a kvantifikace cervikální stenózy a komprese krční míchy jsou však nejednotné a nejsou jasně stanoveny prediktory progrese do manifestní myelopatie a s tím související indikace případné preventivní chirurgické dekomprese. Přehled obsahuje nejčastěji používané metody stanovení cervikální stenózy a cervikální míšní komprese pomocí zobrazovacích metod. Nativní radiogram a výpočetní tomografie mají význam v objektivizaci cervikální stenózy, zatímco dominantní metodou objektivizace míšní komprese je magnetická rezonance (MR). Z parametrů, které nejvíce korelují s klinickou manifestací komprese, jsou uváděny příčná plocha míchy v místě komprese a přítomnost intenzitních změn míšního signálu. Z novějších zobrazovacích modalit je nejcitlivější MR zobrazení tenzorů difúze. Z prognostických faktorů rozvoje manifestní myelopatie u ASCCC jsou obecně akceptovány přítomnost manifestní radikulopatie a abnormita motorických a senzomotorických evokovaných potenciálů. Význam zobrazovacích metod v predikci rozvoje manifestní myelopatie a benefit provedení preventivní dekomprese ve skupině nemocných s asymptomatickou kompresí a vyšším rizikem rozvoje manifestní myelopatie bude nutné ověřit dalšími studiemi., Cervical spinal stenosis may lead to cervical cord compression and represents the most important mechanical factor in pathophysiology of cervical spondylotic myelopathy. Medullar tissue is, however, rather resistant to compression and development of symptomatic myelopathy occurs only when higher degree stenosis is present and in combination with other pathophysiological factors, mainly dynamic compression and trauma. Asymptomatic spondylotic cervical cord compression (ASCCC) is a quite frequent finding in older population above the age of fifty. However, reliability of the methods used to verify and quantify cervical stenosis and cervical cord compression is low and clear predictors of development of symptomatic myelopathy and related indication of potential preventive surgical decompression in ASCCC have not been determined yet. The overview discusses the most frequently used methods to establish cervical spinal stenosis and cervical cord compression using imaging methods. Radiogram and especially computed tomography are important for verification of cervical spinal stenosis, while magnetic resonance imaging (MRI) is a preferable method to detect cervical cord compression. The cross‑sectional spinal cord area and T2 MRI spinal cord hyperintensity are among the parameters considered to be the most closely correlated with clinical manifestation of spinal cord compression. Among newly introduced imaging modalities, MRI diffusion tensor imaging seems to be the most promising one. The presence of symptomatic radiculopathy and abnormality of motor and somatosensory evoked potentials are among generally accepted predictors of symptomatic myelopathy. The importance of imaging methods as predictors of symptomatic myelopathy development as well as benefits of preventive surgical decompression in ASCCC individuals with high risk of developing symptomatic myelopathy is to be established in future studies.
