Diastematomyelie je vzácná vrozená vada patřící mezi spinální dysrafizmy a obvykle je asociována se syndromem míšního ukotvení. Diastematomyelie je tvořena dvěma polovinami rozštěpené míchy s vlastními durálními vaky, které jsou rozděleny osteokartilaginózním septem. Tato vada bývá diagnostikována především v dětském věku. U dospělých je vzácná a dominujícím příznakem je bolest. Autoři prezentují kazuistiku ženy ve věku 44 let s diastematomyelií, která si stěžovala na chronické bolesti zad a progredující slabost dolních končetin. U pacientky jsme provedli laminektomii, deliberaci míchy a plastiku durálního vaku. Po operaci pacientka zůstala bez neurologického deficitu a udávala výraznou úlevu od bolestí., Diastematomyelia is a rare congenital defect, a type of spinal dysraphisms, often associated with the tethered cord syndrome. Diastematomyelia is formed by the two halves of the split spinal cord that each has own dural sac separated by a rigid or fibrotic septum. This defect is mainly diagnosed in children. Pain is the dominant symptom in adult patients. The authors present a case of a woman aged 44 years with diastematomyelia who complained of chronic back pain and progressive weakness in her legs. In this patient, laminectomy was performed, the spinal cord released and the dural sac restored. After the surgery, the patient had no neurological deficits and reported significant pain relief. Cílem práce je ukázat v průběhu 18letého sledování kognitivní, emoční a behaviorální změny u pacientky s roztroušenou sklerózou mozkomíšní (RS). Předkládaná kazuistika dokumentuje maladaptaci na diagnózu RS a dlouhodobé maladaptivní zvládání, prolínání a vliv afektivních složek na inhibici kognitivních schopností a psychického stavu, vliv stresu na vznik „pseudoatak“ RS, psychosociální problematiku a kvalitu života. Diagnóza RS měla negativní vliv na pacientku a rodinu. Nepředvídatelný průběh ovlivňoval potíže v adaptaci a v plánování podpory. Vliv stresu souvisejícího s onemocněním a zároveň vedlejší negativní efekt léčby vedli k mnoha psychosociálním potížím, které snížily kvalitu života pacientky a měly negativní vliv na její spolupráci při léčbě a rehabilitaci. Součástí komprehensivní diagnostiky i léčby RS byla opakovaná neuropsychologická vyšetření a v závěru sledování, ve fázi chronicko‑progresivní RS, i kognitivně‑behaviorální terapie cílená na snížení zátěže způsobené chronicitou nemoci a zlepšení kvality života., The aim of the paper is to show cognitive, emotional and behavioural changes in a multiple sclerosis (MS) patient over eighteen years of observation. Case study documents maladaptation to MS diagnosis and prolonged maladaptive coping, diffusion and influence of affective components on inhibition of cognitive abilities and mental health status, the contribution of stress to development of „pseudo-relapses“ of MS, psychosocial problems and quality of life. The diagnosis of MS was devastating for the patient herself and her family. Its unpredictable course complicated adjustment and created difficulties in planning for support. Effects of disease-related stress as well as a negative side-effect of treatment led to many psychosocial problems that impaired the patient’s quality of life and affected compliance with treatment and rehabilitation. Repeated neuropsychological assessment was a part of a comprehensive diagnostic process and treatment of MS and, at the end of observation, in chronic-progressive phase of MS, was complemented with cognitive-behavioural therapy aimed at reducing burden of disease and improving the quality of life., and A. Úvod: Léčba intratékálním baklofenem (ITB) kontinuálně pomocí pumpových systémů je důležitou léčebnou modalitou u nemocných s těžkou spasticitou, která již nezabírá na perorální léky nebo když se objeví výrazné nežádoucí účinky. Tato metoda je relativně velmi bezpečná, přesto se ale mohou vyskytnout komplikace spojené s touto léčbou. Nejčastěji se objevují komplikace spojené s implantací systému. Jde zejména o problémy s katétrem. Rotace pumpy je extrémně vzácnou komplikací a její výskyt je méně než 0,3 %. Kazuistika: U 74leté ženy s těžkou spasticitou po chronickém míšním poranění ve výši Th9 byla úspěšné provedena implantace baklofenové pumpy (Medtronic, Synchromed II). Pacienka měla bezprostřední úlevu se snížením spasticity již na dávce baklofenu 100 ?g/den. Na první ambulantní kontrole se opakovaně nezdařilo doplnit rezervoár pumpy. Bylo vysloveno podezření na její rotaci. Pacientka byla okamžitě odeslána na rentgenologické vyšetření, kde bylo potvrzeno přetočení pumpy o 180 stupňů. Tentýž den byla pacienta v lokální anestezii operována, pumpa byla otočena a důkladně zafixována stehy. Vzhledem k rychlé revizi se u pacientky nerozvinul syndrom z náhlého odnětí baklofenu při prázdném rezervoáru pumpy. Závěr: Rotace pumpy je vzácná komplikace léčby ITB. Je nutné okamžité vyřešení tohoto problému, neboť hrozí rozvoj abstinenčního syndromu z náhlého odnětí baklofenu. Léčbu ITB je třeba provádět v centrech se specializovanou péčí o tyto pacienty. Despite generally favorable outcomes and relative safety of this method, complications associated with ITB can be observed. The most common problems include surgery‑related complications resulting from system implantation, including catheter failure. Pump rotation is a rare complication with incidence less than 0.3%. Case report: Seventy‑four‑years‑old female suffered from spasticity after chronic spinal cord injury at T9 level. e underwent successful implantation of ITB pump (Medtronic, Synchromed II) with prompt spasticity relief on baclofen dose of 100 ?g/day. During the first outpatient appointment, it was difficult to puncture and fill the reservoir with baclofen. Plain x‑ray revealed rotation of the pump with reservoir in downright position. On the same day, the patient underwent emergency surgical revision with correction of the pump position and refill of the reservoir. No baclofen withdrawal syndrome or other complications occurred and the patient recovered well. Conclusion: Pump rotation is a very unusual and rare complication of ITB treatment. Still's disease is an inflammatory disorder of unknown etiology. First-line therapy is based on corticosteroids and non-steroidal anti-inflammatory drugs (NSAIDs) but the frequency of relapses and corticosteroid-induced adverse events are limiting factors. The efficacy of intravenous tocilizumab (TCZ) has been shown at a dose of 8 mg/kg but the corticosteroid-sparing effect of intravenous low-dose TCZ followed by subcutaneous (SC) injection in the course of the disease has been poorly investigated. We report the case of a 28-year old Caucasian woman presenting a relapse of Still's disease eleven months after diagnosis under treatment with 6 mg of methylprednisolone. TCZ at a dose of 4 mg/kg every 2 weeks was combined with 32 mg of methylprednisolone, followed by 162 mg SC every 3 weeks. Evolution was rapidly favourable with a decrease in corticosteroid doses. The present article describes two unrelated cases of progressive myoclonic epilepsy (PME) of the Lafora's disease and Unverricht-Lundborg types who were treated with topiramate (TPM) as add-on therapy for their myoclonus. After the initiation of topiramate therapy both cases responded with marked decrease in myoclonic seizure frequency and improvement of quality of life. Autor v článku popisuje šest případů emfyzematózní pyelonefritidy. Průměrný věk nemocných byl 63,5 roku, onemocnění se vyskytovalo častěji u mužů a ani v jednom případě nebyl příčinou konkrement. Dvě třetiny nemocných byli diabetici a nejčastěji byla prokázána jako etiologické agens Escherichia coli a v jednom případě Klebsiella pneumonie. Autor uvádí způsoby léčby jednotlivých případů onemocnění. Pouze v jednom případě vedla emfyzematózní pyelonefritida ke smrti pacienta., The author of this communication describes six cases of emphysematous pyelonephritis. The mean age of patients was 63.5, with the disease seen most frequently in men. None of the cases were associated with stone disease. Two thirds of patients had diabetes mellitus. Escherichia coli was confi rmed in the urine most frequently, and in one case, Klebsiella pneumoniae was identifi ed. The author presents the methods of treatment used in each case. INTRODUCTION: The splenium of the corpus callosum has a specific structure of blood supply with a tendency towards blood-brain barrier breakdown, intramyelinic edema, and damage due to hypoxia or toxins. Signs and symptoms of reversible syndrome of the splenium of the corpus callosum typically include disorientation, confusion, impaired consciousness, and epileptic seizures. CASE REPORT: A previously healthy 32-year-old man suffered from weakness, headache, and fever. Subsequently, he developed apathy, ataxia, and inability to walk, and therefore was admitted to the hospital. Cerebrospinal fluid showed protein elevation (0.9 g/l) and pleocytosis (232/1 ul). A brain MRI showed hyperintense lesions in the middle of the corpus callosum. The patient was treated with antibiotics, and subsequently, in combination with steroids. Two months later, the hyperintense lesions in the splenium and the basal ganglia had disappeared. Almost seven months since his hospitalization in the Department of Neurology, the patient has returned to his previous employment. He now does not exhibit any mental changes, an optic edema and urological problems have improved. In addition, he is now actively engaged in sports. CONCLUSION: We have described a case of a 32-year-old man with confusion, ataxia, and inability to stand and walk. The man developed a febrile meningeal syndrome and a hyperintense lesion of the splenium, which lasted for two months. Neurological changes, optic nerve edema, and urinary retention have resolved over the course of seven months.
