Angiofibromas are rare vascular tumors which originate predominantly in the nasopharynx and occur typically in male adolescents. Extranasopharyngeal sites such as nasal cavity and paranasal sinuses are less frequent. This review article was undertaken to evaluate the incidence, clinical features and management of extranasopharyngeal angiofibromas originating exclusivelly from nasal cavity structures. Our focus of interest was to evaluate the significance of immunohistochemical analysis in diagnosis of such extremely rare neoplasms. In the PubMed and Google Search, we found only 39 cases of nasal angifibroma, 27 males and 12 females from 1980 to 2012. The most prevalent site of origin was nasal septum, followed by inferior and middle turbinate. The commonest symptoms were nasal obstruction and epistaxis. Nasal angiofibromas are clinically distinct from nasopharyneal angiofibromas and can therefore be misdiagnosed. The differential diagnosis includes other vascular lesions, such as lobular capillary hemangioma and sinonasal-type hemangiopericytoma. Although immunohistochemistry is not necessary for differentiation between angiofibroma and capillary hemangioma, that diagnostic procedure may be helpful in distinction from sinonasal hemangiopericytoma. As an ilustration for immunohistochemical analysis, we presented a case of an elderly woman with tumor arising from the middle turbinate, diagnosed as angiofibroma. The staining was positive for CD34, CD31, factor VIII, vimentin and smooth muscle alpha-actin, and negative for desmin. and A. Perić, J. Sotirović, S. Cerović, L. Zivić
Tento článek prezentuje kazuistiku pacientky se spontánním hematomem jícnu řešeným konzervativně a diskuzi o dalších možnostech průběhu a léčby onemocnění., Intramural hematoma of the oesophagus is a rare diagnosis presenting with retrosternal pain, dysphagia and haematemesis. It is caused by barotrauma or direct injury to the esophagus, or it may occur spontaneously without apparent cause, which is more common in patients taking antiplatelet or anticoagulant medications. In most cases, treatment is conservative. Rarely, endoscopic, endovascular or surgical treatment is effective. This article presents a case report of a female patient with spontaneous haematoma of the oesophagus solved conservatively and discusses further possibilities of the course and treatment of the disease., and M. Autoři prezentují případ intramuskulárně lokalizovaného hemangiomu v podpaží u tříletého dítěte s náhle vzniklou poruchou hybnosti levé horní končetiny. Hlavním cílem práce je upozornit na podobnost různých měkkotkáňových lézí v zobrazovacích metodách a nezbytnost histologického vyšetření ke stanovení správné diagnózy., The authors present a case of intramuscular hemangioma localized in the armpit of a three-years old child with sudden onset movement disorders left upper extremity. The discussion is the contribution of imaging in the differentiation from other soft tissue lesions, particularly sarcomas. The main goal is to draw attention to the similarity of different soft tissue lesions in imaging methods and the need for histological examination to establish the correct diagnosis., Červinková I, Skotáková J, Pavlovský Z., and Literatura
Včasná diagnóza a následně zahájená léčba, která se zásadně liší od terapie karcinomu prsu, velmi často vede k remisi onemocnění., Incidence of primary lymphoma of the breast is less than 0.5% of all malignant breast lesions. Therefore it belongs in the group of rare malignant diseases, frequency of less than 5% of all malignant tumors. Lymphoma could be found as primary extranodal lymphoma or secondary to systemic involvment. The first clinical sign is usually a palpable mass or nodule. Its appearance on mammography, sonography and also magnetic resonance is nonspecific, usually a well circumscribed, hypervascularised lesion with fast growth tendency. Although breast lymphoma is rare it has to be considered in differencial diagnosis. Early diagnosis and treatment, which is radically different to treatment of breast carcinoma, very often leads to remision., Eva Němcová, Andrea Janíková, Jiřina Little, Eva Jandáková, Hana Čučková, Karel Dvořák, and Literatura
V této práci popisujeme náhodný nález kraniocervikální pneumatizace u pacienta s úrazem na kole a následnými polykacími problémy a potížemi s mluvením. Pro vznik kraniocervikální pneumatizace u tohoto pacienta se jeví klíčové opakované rýmy a záněty paranazálních dutin a rovněž záliba v potápění. Klinické potíže, trvající krátkodobě, jsme si vysvětlili současným nálezem pneumorrhachis., Pneumatization of the occipital bone and cervical vertebrae is observed infrequently and its incidental discovery by radiological imaging methods may be confusing. This condition is most commonly attributed to dysfunction of the Eustachian tube and frequent repetition of the Valsalva manoeuvre. In this paper, we describe a rare case of craniocervical pneumatization accompanied by difficulties in swallowing and speaking in a patient after a bicycle accident. Laryngeal electromyography is used in the evaluation of vocal cord paralysis to confirm the diagnosis, to guide the diagnostic work-up for etiology, to provide prognostic information and to help choose the correct treatment for the patient. Cardiovocal syndrome is characterised by vocal cord paralysis due to a cardiovascular disease. A wide spectrum of conditions can result in this syndrome. Here we present a case of cardiovocal syndrome in association with primary pulmonary hypertension. Laryngeal electromyography was used to guide the work-up of differential diagnosis and also for further intervention with respect to vocal cord paralysis in this patient. and S. Akbulut, R. Inan, M. Low-grade myofibroblastic sarcoma (LGMS) is a very rare, atypical myofibroblastic tumor with fibromatosis-like features with predilection mostly in head and neck region. LGMS occurs primarily in adult patients with a slight male predominance. Only few cases of LGMS affecting the larynx have been reported in literature to this date. We describe a case of low-grade myofibroblastic sarcoma of the larynx in a 40-year-old male patient. The clinicopathological characteristics, immunohistochemical findings and treatment are discussed. and P. Kordač, D. H. Nikolov, K. Smatanová, D. Kalfeřt
Popisujeme případ onemocnění 25letého muže s nespecifickými příznaky a hepatomegalií, u kterého jsme diagnózu stanovili na základě výsledků zobrazovacích metod a laboratorních vyšetření. Vzhledem k inoperabilitě je pacient v současnosti dispenzarizován infektologem, dlouhodobě léčen albendazolem a je klinicky stabilní, zařazen v čekacím listu k transplantaci jater., Alveolar echinococcosis is a rare parasitic disease, especially of liver, caused by larval stage of tapeworm Echinococcus multilocularis. At the end of the last century France, Germany, Austria and Switzerland were the most often regions with this disease, these days is this infection diagnosed also in our territory. We describe the case of the disease of the twenty-five years old male with nonspecific signs and hepatomegaly, who was diagnosed on the basis of imaging and laboratory sampling. Due to inoperability the patient is now in infectologist follow-up on a long-term treatment with albendazole. He is clinically stable, included in waiting list for liver transplantation., and Tomáš Kupka, Peter Baľa, Lubomíra Hozáková, Jaroslav Havelka, Martina Bojková, Bohdana Břegová, Arnošt Martínek, Petr Dítě
A vice versa, navzdory příznakům pro neuroborreliózu typickým, včetně přisátí klíštěte a údaji o časné lokalizované formě borreliózy – erythema migrans, patří u dětí do diferenciální diagnostiky i nádory CNS., Tick‑borne neuroinfections are rare among children with oncologic diagnosis. Nevertheless, the diagnostic procedure may be more complicated due to suspicion of the relapse of the primary illness. Despite typical symptoms of neuroborreliosis, including tick bite and early localized borreliosis – erythema migrans, tumours of central nervous system have to be taken into account for the differential diagnosis in childhood., and L. Neurologické postižení je poměrně vzácná komplikace hepatitidy E. Uvedená kazuistika popisuje 52letého imunokompetentního muže s aseptickou meningitidou a mnohočetným postižením periferních nervů na horních končetinách typu mononeuropatia multiplex, u kterého byla sérologickým vyšetřením prokázána akutní virová hepatitida E. Navíc byl virus hepatitidy E zjištěn polymerázovou řetězovou reakcí v séru i likvoru (genotyp 3f). Infekci virem hepatitidy E je vhodné zvažovat u pacientů se současným neurologickým a jaterním postižením., Neurological complications of acute hepatitis E are relatively rare. The case report describes mononeuropatia multiplex with aseptic meningitis in a 52‑year‑old immunocompetent man. Viral hepatitis E was confirmed serologically and by polymerase chain reaction from the blood and cerebrospinal fluid (genotype 3f). Symptomatic treatment was given. Residual neurological deficit persisted six months after the onset of the disease. Diagnostic of hepatitis E should be considered in patients with concurrent neurological symptoms and liver cytolysis.
