Projekt PROFICIO zahrnuje 20 klinických studií hodnotících účinek evolokumabu na výskyt kardiovaskulárních onemocnění včetně jeho bezpečnostního profilu a tolerance. Většina zařazených studií sleduje průměrné procentuální snížení koncentrací LDL-cholesterolu za 10 a 12 týdnů podávání evolokumabu ve srovnání se vstupními hodnotami. První výsledky byly prezentovány na přelomu srpna a září 2015 na kongresu Evropské kardiologické společnosti (ESC) v Londýně. Do této subanalýzy bylo zařazeno 3 146 pacientů, kteří absolvovali jednu z vybraných studií 3. fáze klinického zkoušení a kterým byl evolokumab podáván v dávce 140 mg s.c. 1krát za 2 týdny nebo 420 mg s.c. 1krát za 4 týdny. Hodnoty LDL-cholesterolu se po evolokumabu snížily v jednotlivých studiích o 56,5–74,9 % ve srovnání s placebem a o 36,9–44,9 % oproti ezetimibu. Výskyt nežádoucích účinků se nelišil od skupiny užívající placebo. Most of the included studies follow the average proportional decrease of LDL-cholesterol concentrations over 10 and 12 weeks of administering evolocumab as compared to the input values. The first results were announced at the congress of the European Society of Cardiology (ESC) in London at the end of August and beginning of September 2015. Poranění míchy není v současném stavu poznání v klinické praxi stále léčitelné. Standardní péče se tak soustředí na včasnou dekompresi míchy, stabilizaci poraněné páteře a prevenci sekundárního míšního poranění. Na druhou stranu se v experimentální oblasti rozvíjí velké množství nových terapeutických postupů. Některé z nich prokázaly částečný funkční a morfologický efekt u laboratorních zvířat. V posledních 20 letech pak byly tyto experimentální postupy aplikovány i v klinických studiích, zatím však bez jednoznačných výsledků. V této práci poskytujeme souhrn současného stavu klinické a především experimentální terapie míšního poranění., Spinal cord injuries still remain incurable in current clinical practice. Standard therapy focuses on early decompression and stabilization of the injured spine together with prevention of a secondary injury. On the other hand, several experimental therapies are being developed in laboratories. Some of these showed partial functional and morphological effect in laboratory animals. Over the last 20 years, some of the experimental therapies have been applied in clinical studies but did not provide unambiguous results. In the present paper, we provide an overview of current clinical and experimental therapies of spinal cord injuries. Rastúci vplyv expozície environmentálnemu hluku a jeho vplyv na zdravie, patrí k významným problémom súčasnej doby. Týka sa to predovšetkým obyvateľstva, žijúceho v mestských aglomeráciách s vysokou hladinou environmentálneho hluku. Príčinou tohto nežiaduceho javu je najmä narastajúca doprava a priemysel. Hladiny hluku narastajú nielen v niektorých typických pracovných prevádzkach, ale obťažujú ľudí už aj pri rekreácii, oddychu, či spoločenských aktivitách. Tento negatívny faktor životného prostredia sa odlišuje od iných škodlivín tým, že sa jeho hladiny stále zvyšujú a pôsobí na človeka neustále, teda aj v čase určenom na relaxáciu ako napríklad pri spánku. Z verejno-zdravotníckeho hľadiska je významné, že rušivé pôsobenie environmentálneho hluku sa prejavuje neskôr rastúcim rizikom chronických ochorení, ktoré majú priamu súvislosť s poruchami spánku a iných funkcií vo vegetatívnej, endokrinnej a regulačnej sfére. Cieľom tejto prehľadovej práce je zosumarizovať aktuálne poznatky o škodlivých sluchových a nesluchových účinkoch environmentálneho hluku, ktoré bude možné využiť pri kvantifikácií rizika ako aj pri plánovaní preventívnych opatrení., The growing impact of environmental noise exposure on health is a major problems of our times. It particularly concerns the urban population living in areas with high levels of environmental noise. Increased levels of traffic and industrial activity are the major causative factors of this phenomenon. Increased noise levels cause interpersonal problems not only in occcupational settings, but during recreation, leisure, and social activities. This negative environmental factor differs from others in specifically affecting sleep and relentlessly increasing. In terms of public health, the disturbing effects of environmental noise manifest in increased risk of chronic diseases dircetly associated with sleep disorders and other vegetative, endocrine and regulatory functions. Ovarian cancer is the fifth most common malignancy in the world's female population and with the highest lethality index among gynecological tumors. The prognosis of metastatic disease is usually poor, especially in platinum-resistant cases. There are several options for the treatment of metastatic disease resistant to platinum derivates (e.g. paclitaxel, topotecan and pegylated liposomal doxorubicin), all of which are considered equipotent. Pegylated liposomal doxorubicin (PLD) is a liposomal form of the anthracycline antibiotic doxorubicin. It is characterized by more convenient pharmacokinetics and a different toxicity profile. Cardiotoxicity, the major adverse effect of conventional doxorubicin, is reduced in PLD as well as hematotoxicity, alopecia, nausea and vomiting. Skin toxicity and mucositis, however, emerge as serious issues since they represent dose and schedule-limiting toxicities. The pharmacokinetics of PLD (prolonged biological half-life and preferential distribution into tumor tissue) provide new possibilities to address these toxicity issues. The extracorporeal elimination of circulating liposomes after PLD saturation in the tumor tissue represents a novel and potent strategy to diminish drug toxicity. Gastrointestinální stromální tumory (GISTy) představují většinu mezenchymálních nádorů zažívacího traktu. Extragastrointestinální tumory (EGISTy) jsou nádory, které se v histologickém a imunohistochemickém obraze GISTům podobají, ale nacházejí se mimo trávicí trubici, nemají žádnou spojitost se stěnou žaludku či střeva. V naší kazuistice představujeme případ 68letého pacienta s EGISTem vycházejím z mezenteria. U pacientov s meduloblastómom sú extraneurálne metastázy známym, ale veľmi zriedkavým javom. Prezentujeme prípad 11ročného chlapca s meduloblastómom lokalizovaným vo vermis cerebella, s včasnou disemináciou v spinálnom kanáli, ktorý podstúpil chirurgickú extirpáciu, rádioterapiu kraniospinálnej osi a vysokodávkovú chemoterapiu. Po 3 mesiacoch od ukončenia liečby došlo k recidíve nádoru s metastázou vo frontálnom laloku. Chirurgickú extirpáciu nasledovala rádioterapia, plánovaná bola II. línia chemoterapie. Po ukončení rádioterapie sa objavila hmatná masa na krku vľavo. Histológia potvrdila metastázu nezrelého embryonálneho nádoru, najskôr anaplastického meduloblastómu, v diferenciálnej diagnostike bol zvažovaný tiež neuroblastóm. Na restaging ochorenia boli indikované PET/CT s 18F-FDG a scintigrafia s 123I-MIBG. Nález scintigrafie s 123I-MIBG bol negatívny, pri PET/CT s 18F-FDG boli identifikované mnohopočetné metastázy v kostnej dreni, lymfatických uzlinách a paranazálnych dutinách. Pri iniciálnom stagingu, gradingu a hodnotení prognózy meduloblastómu zohráva najvýznamnejšiu úlohu MRI vyšetrenie. The adhesive material used to bond orthodontic brackets to teeth should neither fail during the treatment period, resulting in treatment delays, untoward expenses or patient inconvenience nor should it damage the enamel on debonding at the end of the treatment. Although the effectiveness of a bonding system and any unfavorable effects on the enamel may be studied by conducting in-vivo studies, it is nearly impossible to independently analyze different variables that influence a specific bonding system in the oral environment. In-vitro studies, on the other hand, may utilize more standardized protocols for testing different bonding systems and materials available. Thus, the present review focused attention on in-vitro studies and made an attempt to discuss material-related, teeth-related (fluorotic vs non-fluorotic teeth) and other miscellaneous factors that influences the shear bond strength of orthodontic brackets. Within the limitations of this review, using conventional acid-etch technique, ceramic brackets and bonding to non-fluorotic teeth was reported to have a positive influence on the shear bond strength of orthodontic brackets, but higher shear bond strength found on using ceramic brackets can be dangerous for the enamel. and W. Předkládáme popis případu nemocného s familiární formou amyloidové polyneuropatie. Mladý nemocný s postupně progredujícími projevy léze periferních nervů byl zprvu diagnostikován a léčen pro chronickou zánětlivou demyelinizační polyneuropatii. Pro následné komplikace bylo provedeno endoskopické vyšetření zažívacího traktu s bioptickým průkazem amyloidových depozit. Následné komplexní vyšetření potvrdilo hereditární formu transtyretinové amyloidové polyneuropatie na podkladě raritní mutace v genu pro transtyretin na pozici Val50Ala. Anamnesticky, matka nemocného byla léčena pro amyloidózu s neuropatickými projevy. Léčba pacienta preparátem tafamidis meglumine vedla k zabránění další progrese onemocnění a následně byla provedena ortotopní transplantace jater. V současnosti dochází ke klinickému zlepšování stavu nemocného s postupnou regresí motorických a senzitivních neuropatických projevů., We present a case of a patient with familial amyloid polyneuropathy. A young patient with gradually progressing symptoms of peripheral neuropathy was initially diagnosed and treated for chronic inflammatory demyelinating polyneuropathy. Subsequent complications resulted in endoscopic examination of the digestive tract. Biopsy showed evidence of amyloid deposits. Subsequent comprehensive examination confirmed hereditary form of amyloid polyneuropathy on the basis of a rare mutation in the transthyretin gene in Val50Ala position. Family history revealed that the mother of the patient was treated for amyloidosis with neuropathic symptoms. Treatment with tafamidis meglumine prevented further disease progression, and was followed by orthotopic liver transplantation. At present, clinical condition of the patient gradually improves with regression of motor and sensory neuropathic symptoms. Úvod: Terapie ran kontrolovaným podtlakem (NPWT) je efektivní metodou lokální terapie širokého spektra nehojících se ran. V současnosti je využívána především u hospitalizovaných pacientů, pro systematické použití v ambulantním režimu nejsou v České republice dostupná klinická data. Metody: V prospektivní nerandomizované studii autor porovnává klinickou a nákladovou efektivitu léčby ulcerací na dolní končetině u diabetiků kontrolovaným podtlakem (NPWT) ambulantně versus za hospitalizace. Cílem sledování je stanovit účinnost NPWT v ambulantním režimu na základě hodnocení vývoje spodiny rány, kvantifikovat předpokládanou redukci celkových nákladů na terapii a porovnat efektivitu obou režimů navzájem. Do sledování bylo zařazeno 36 hospitalizovaných (NPWT systémy RENASYS GO nebo PICO) a 28 ambulantních pacientů (PICO). NPWT byla ukončena v momentě, kdy byla spodina rány zcela pokryta granulační tkání nebo při vzniku vážných komplikací léčby. Hodnotili jsme velikost ulcerace, charakter její spodiny, počet NPWT výměn, její celkovou délku, intenzitu bolesti, přítomnost komplikací a náklady na terapii. Výsledky: Sledování prokázalo porovnatelnou klinickou účinnost NPWT v ambulantním režimu (n=28) versus za hospitalizace (n=36, statisticky nesignifikantní rozdíly v hojení ulcerací verifikované na základě vývoje spodiny rány a redukce plochy ulcerací v čase), stejné riziko komplikací spojených s NPWT (statisticky nesignifikantní rozdíly ve funkčnosti obou technologií a ve výskytu komplikací vzniklých v průběhu sledování) i obdobné dlouhodobé výsledky (statisticky nesignifikantní rozdíly v počtu amputací). Celkové náklady na ambulantní terapii ran byly statisticky signifikantně nižší (16 700 Kč versus 36 000 Kč, p 0,001), stejně tak průměrné náklady na 1 den podtlakové terapie (817 Kč versus 3400 Kč, p 0,001). Závěr: Analýza klinických výsledků jednoznačně prokázala, že ambulantní podtlaková terapie ulcerací syndromu diabetické nohy (metoda PICO) má stejnou efektivitu jako podtlaková terapie za hospitalizace. V ambulantním provozu je NPWT levnější, převazy jsou vnímány méně bolestivě a NPWT je pacienty lépe snášena., Introduction: The negative pressure wound therapy (NPWT) is an effective local treatment method of many non-healing wounds. NPWT is routinely used for inpatient treatment in the Czech Republic; however, no clinical data is available for systematic outpatient treatment. Methods: The prospective non-randomized study was used to compare the clinical impact and cost-effectiveness of diabetic leg-ulcer and foot-ulcer negative pressure treatment in outpatient and inpatient settings. The aim of the study was to assess the effectiveness of outpatient NPWT on the basis of wound bed evaluation, to detect and quantify pre-expected reductions in the total costs of the therapy, and to compare the efficacy of both treatment options. Enrolled subjects included inpatients (n=36, NPWT systems RENASYS GO or PICO) and outpatients (n=28, PICO system). NPWT was concluded when the wound bed had been totally covered by granulation tissue, or when serious health problems occurred. We monitored the ulcer area, wound bed trait, number of NPWT dressing changes, length of NPWT, intensity of pain (VAS scale), presence of complications, and treatment costs. Results: We found comparable clinical efficacy of outpatient (n=28)/inpatient (n=36; statistically non-significant differences in healing of ulcers verified based on wound bed development and reduction of ulcer areas in time) NPWT, the same risk of complications associated with NPWT (statistically non-significant differences in the function of both technologies and in complications developed during follow-up) as well as similar long-term results (statistically non-significant differences in the count of amputations). The total costs for outpatient wound treatment were statistically significantly lower (EUR 600 versus EUR 1300, p=0.001), and so were the average one-day-NPWT costs (EUR 30 versus EUR 120, p=0.001). Conclusion: We demonstrated that outpatient diabetic-foot-ulcer NPWT provided the same clinical efficiency as inpatient NPWT, while outpatient NPWT was less expensive, less painful and better tolerated than inpatient NPWT., and J.
