Cíl: Autoři prezentují výsledky operační léčby tumorů periferních nervů. Operační výkony byly provedeny na Neurochirurgické klinice LF MU a FN Brno v letech 2003–2013. Ve sdělení je prezentován současný přehled histologické klasifikace a její vliv na možnosti chirurgické léčby. Součástí práce je i kazuistika dokumentující operační postup u raritního objemného tumoru n. ischiadicus v hloubce hýžďové krajiny. Soubor a metodika: Operační revizi podstoupilo celkem 24 pacientů, z toho 13 mužů a 11 žen. Průměrný věk pacientů v souboru byl 46,5 roku (min. 22, max. 74 let). Výkony byly provedeny standardní mikrochirurgickou operační technikou, od roku 2007 s užitím peroperační elektrofyziologické monitorace. Retrospektivní analýzou je soubor definován dle postiženého nervu, rozsahu resekce a histologické klasifikace. Je zhodnocen výsledný pooperační neurologický stav. Výsledky: Nejčastěji postiženým periferním nervem byl n. tibialis. V souladu s literárními zdroji se histologicky nejčastěji jednalo o benigní schwannom, a to celkem ve 20 případech. Jednou šlo o neurofibrom, jednou o intraneurální ganglionovou cystu, jedenkrát byla histologie uzavřena jako raritní extraoseální Ewingův sarkom a u jednoho pacienta byl tumor způsoben chronickým zánětlivým procesem. Závěr: V našem souboru bylo dosaženo totální resekce u všech operovaných pacientů, kromě jednoho pacienta. U něj byla provedena pouze revize s otevřenou biopsií. U pacienta s maligním Ewingovým sarkomem byla nutná kompletní rozšířená resekce postižených nervů s příslušným totálním motorickým a senzitivním deficitem. Ve dvou dalších případech došlo po operaci ke zhoršení neurologického stavu, jednou se jednalo o lehkou parézu, jednou o trvalou hypestezii. V ostatních případech klinické obtíže prakticky vymizely., Aim: The authors present surgical treatment results of peripheral nerve tumors. All the surgical procedures were performed between 2003 and 2013 at the Department of Neurosurgery, The University Hospital Brno. The paper provides an overview of current histological classification and its impact on surgical treatment. The paper also includes a case report describing a surgical procedure used for a rare, bulky tumor of the sciatic nerve in the depth of the gluteal region. Methods: In total, 24 patients, 13 men and 11 women, underwent surgical revision. The mean age of the group was 46.5 years (min. 22, max. 74 years). All the surgical procedures were completed using standard microsurgical tools. Perioperative electrophysiological monitoring has been used since 2007. A retrospective analysis according to the localization of the affected nerve, extent of resection and histological classification of the tumor was performed and the final outcome in terms of neurological status was evaluated in the study group. Results: A tumor of the tibial nerve was the most frequent target of the surgery in our study group. Benign schwannoma was the most common tumor in our study group (20 cases) and this is in accordance with previously published data. Other tumors were classified as neurofibroma (one case) and intraneural ganglion cyst (one case). Histology confirmed the presence of a rare extraosseous Ewing’s sarcoma in one case. Finally, one patient suffered from chronic inflammation. Conclusion: In our study group, total resection was performed in all patients except one. Revision with open biopsy was performed in a patient with nerve inflammation. Complete extended resection of the affected nerves was necessary in a patient with malignant Ewing’s sarcoma. This resulted in total motor and sensitive deficit of the corresponding area. Neurological status has also worsened after the surgery in two other cases, one had light paresis and the other had permanent hypoesthesia. The clinical symptoms disappeared after surgery in all other cases. Podstatou hormonální léčby transsexuálních pacientů (TS) je podávání sexagenů: androgenů female-to-male transsexuálním pacientům (FtM) a estrogenů a antiandrogenů male-to-female transsexuálním pacientům (MtF). Doba hormonální léčby před provedením operačního zákroku se změnou pohlaví (gender reassignment surgery) musí trvat nejméně 1 rok. Potlačuje původní pohlaví a indukuje alespoň částečně změny nového pohlaví. Hormonální léčba pokračuje i po provedení chirurgického zákroku během dalšího života. Hormonální léčba u MtF TS zahrnuje užití estrogenů a antiandrogenů. U velmi mladých jedinců obou uvedených skupin se mohou podávat blokační gonadoliberinová analoga. Při léčbě u FtM TS se podává samotný testosteron (p.o. a/nebo injekční). Autoři popisují své vlastní zkušenosti s hormonální léčbou 282 osob s TS (163 FtM a 119 MtF). Při hormonální léčbě došlo ke statisticky významnému zvýšení hmotnosti v obou skupinách. Hladina celkového cholesterolu se zvýšila u FtM. U léčené skupiny MtF došlo ke zvýšení hladiny prolaktinu z 350,1 na 570,5 mU/l bez klinického významu. Celková průměrná doba podávání hormonální terapie byla 6,73 roku u FtM a 4,64 roku u MtF, a tak hodnocení celkové bezpečnosti léčby není možné. Výskyt jakékoliv endokrinopatie byl na začátku sledování nalezen u 35 osob (12,4 %): prostá struma, autoimunitní tyreoiditida, hypotyreóza, hypertyreóza, gynekomastie, diabetes mellitus 1. typu, kongenitální adrenální hyperplazie (CAH), Klinefelterův syndrom a afunkční adenom hypofýzy. Tak jako u jiných méně obvyklých medicínských stavů je vhodné, aby diagnostika a léčba probíhala v centrech, která mají s touto problematikou zkušenosti., Hormonal therapy in transsexual patients (TS) includes sexagens administration: androgens in female-to-male transsexual patients (FtM) and oestrogens and antiandrogens in male-to-female transsexual patients (MtF). Duration of hormonal therapy should continue at least 1 year before gender reassignment surgery. Hormonal therapy supresses former gender and induces partially new gender changes. Hormonal therapy continues subsequently after surgery during life. Hormonal therapy in MtF TS includes oestrogens and antiandrogens application. In very young persons in both groups blocking gonadoliberin analogues can be used. In FtM TS testosterone onself is given (orally and/or parenterally). Authors describe their own experiences with hormonal treatment in 282 TS (163 FtM and 119 MtF). During hormonal therapy statistically significant weight increasing was found in both groupes. Total cholesterol increased in FtM. In MtF during hormonal therapy average prolactin level increased from 350.1 to 570.5 mU/l without clinical significance. Total average hormonal therapy duration was 6.73 years in FtM and 4.64 years in MtF and so overall therapy safety assessment is not possible. Any endocrinopathy occurence in the beginning of surveillance was found in 35 persons (12.4 %): simple goiter, autoimmune thyreoiditis, hypothyroidism, hyperthyroidism, gynecomastia, DM type 1, congenital adrenal hyperplasia (CAH), Klinefelter syndrome and nonfunctional pituitary adenoma. Úvod: Výskyt dobře diferencovaného nízkorizikového karcinomu štítné žlázy zaznamenal v posledních 3 desetiletích celosvětový nárůst. Léčba karcinomu štítné žlázy se týká vhodného chirurgického výkonu a použití adjuvantní terapie radiojodem u vybraných pacientů. Pro stanovení vhodné léčby je rozhodující hodnocení prognostických faktorů a stratifikace rizika. Přežívání pacientů s nízkorizikovým karcinomem štítné žlázy je vynikající. Vhodnou volbou léčebného postupu je u většiny pacientů dosaženo úplného vyléčení, riziko relapsů stoupá s velikostí primárního nádoru, při nálezu rizikových faktorů a u mužů. Metodika a výsledky: Do sledovaného souboru bylo zařazeno celkem 1 980 pacientů, u kterých byly v letech 2003–2012 diagnostikovány nádory T1a a T1b. Jednalo se o 1 675 žen (84,6 %), průměrný věk 45,22 let, a 305 mužů (15,4 %), průměrný věk 50,0 let. Převážnou část souboru představovaly karcinomy papilární (1 868; 94,4 %), menší část pak karcinomy folikulární (112; 5,6 %). Pacienti byli rozděleni do 4 skupin podle velikosti nádoru. U pacientů byly hodnoceny rizikové faktory: unifokalita bez dalších rizikových faktorů, multifokalita – více ložisek nádoru ve štítné žláze, metastázy ve spádových lymfatických uzlinách, vzdálené metastázy, případně kombinace více rizikových faktorů. Skupina A: V souboru 678 sledovaných nemocných s papilárním a folikulárním mikrokarcinomem do 5 mm bylo při histologickém vstupním nálezu zachyceno jedno ložisko (unifokální typ karcinomu) u 566 nemocných, multifokalita byla nalezena u 112 pacientů, ve 24 případech byly prokázány lokální uzlinové metastázy a v 1 případě byl nález plicních vzdálených metastáz. Skupina B: V této skupině bylo 576 sledovaných pacientů s papilárními a folikulárními mikrokarcinomy velikosti 5–10 mm. Při histologickém vstupním nálezu bylo zachyceno jedno ložisko karcinomu u 434 nemocných, multifokalita u 142 pacientů, v 53 případech lokální uzlinové metastázy a v 1 případě kostní metastázy. Skupina C: V této skupině bylo 467 sledovaných pacientů s papilárními a folikulárními mikrokarcinomy velikosti 10–15 mm. Při histologickém vstupním nálezu byl zachycen unifokální typ karcinomu u 344 nemocných, multifokalita u 123 pacientů, v 45 případech lokální metastázy a ve 3 případech plicní metastázy. Skupina D: V této skupině bylo 259 sledovaných pacientů s karcinomem velikosti 16–20 mm. Při vstupním nálezu bylo zachyceno jedno ložisko karcinomu u 188 nemocných, multifokalita u 71 pacientů, v 24 případech byly prokázány lokální metastázy a u 2 pacientů byl nález vzdálených plicních metastáz. U pacientů, u kterých byly nalezeny rizikové faktory, byla indikována léčba radiojodem. Jednalo se o 744 pacientů. V této skupině pacientů došlo po roce a více k relapsu u 74 pacientů (9,94 %). U 1 236 pacientů, u kterých nebyl podán radiojod, došlo k relapsu u 49 pacientů (3,96 %). Závěr: Na základě naší analýzy je nezbytné stratifikovat riziko relapsu podle rizikových faktorů. V případě vynechání léčby radiojodem u nemocných s nízkorizikovým karcinomem bez potvrzení rizikových faktorů je nezbytné i tyto pacienty vést v dispenzárním programu a pravidelně je vyšetřovat klinicky, laboratorně a sonograficky. Je důležité rozlišovat pacienty s rizikovými faktory, které se mohou podílet na recidivě onemocnění. Jen tak zabráníme tomu, aby na jedné straně docházelo k nadměrné léčbě nemocných s nízkorizikovým karcinomem štítné žlázy a ke zvyšování vynaložených nákladů na zdravotní péči a aby na druhé straně nedošlo ke snížení úrovně péče provázené nárůstem relapsů., Introduction: The incidence of well-differentiated low-risk thyroid cancer have increased globally over the last three decades. Thyroid cancer treatment relates to a suitable surgical procedure and the use of adjuvant radioiodine therapy in selected patients. Evaluation of prognostic factors and risk stratification are critical for determining appropriate treatment. Survival of patients with low-risk thyroid cancer is excellent. Appropriate choice of medical treatment resulted in full recovery in most patients. Relapse risk increases with the size of the primary tumor, along with the findings of the risk factors in men. Methods and results: Our study included a total of 1 980 patients in whom were diagnosed T1a and T1b tumors between the years 2003 to 2012. The population included 1 675 women (84.6 %) of average age of 45.22 years and 305 men (15.4 %) of average age of 50.0 years. The bulk of the file represented papillary carcinomas (1 868; 94.4 %), and smaller group of follicular carcinomas (112; 5.6 %). Patients were divided into four groups according to tumor size. Patients were evaluated according to risk factors: unifocality no other risk factors, multifocality – more bearings in thyroid tumor, metastases in regional lymph nodes, distant metastases or combination of risk factors. Group A: In the monitored set of 678 patients with papillary and follicular microcarcinoma up to 5 mm, during histological input, the findings revealed one bearing (unifocal type of cancer) in 566 patients. Multifocality was found in 112 patients, local nodal metastasis were demonstrated in 24 cases and pulmonary metastasis was discovered in 1 case. Group B: In this group there were 576 study patients with papillary and follicular microcarcinoma size of 5–10 mm. Histological findings were captured input one bearing carcinoma in 434 patients, 142 patients with multifocality, in 53 cases of local nodal metastasis, and 1 case of bone metastases. Group C: In this group there were 467 study patients with papillary and follicular microcarcinoma size 10–15 mm. The histological initial finding captured unifocal type of cancer in 344 patients, multifocality in 123 patients, in 45 cases local metastases and in 3 cases of pulmonary metastases. Group D: 259 patients were monitored in this group with breast size 16–20 mm. At the initial finding was captured one bearing cancer in 188 patients, multifocality in 71 patients, in 24 cases evidence of local metastases and 2 patients had a case of distant lung metastases. In patients in whom risk factors were found, radioiodine treatment was indicated. This included 744 patients. In this group of patients after a year or more, relapse was observed in 74 patients (9.94 %). In 1 236 patients who did not undergo radioiodine treatment, there was a relapse in 49 patients (3.96 %). Conclusion: Based on our analysis, it is necessary to stratify the risk of relapse according to risk factors. In case of missed radioiodine therapy in patients with low-risk cancer without confirmed risk factors, it is also necessary to have regular clinical, laboratory and ultrasound examination. It is important to distinguish patients with risk factors that may contribute to disease recurrence. Only in this way, on one hand we prevent excessive treatment of patients with low-risk thyroid cancer which leads to increased cost of health care, and on the other hand prevent reduced level of care for patients with an increase in relapses. OBJECTIVES: The main aim of the study was to investigate the physiological and social needs of patients hospitalized with schizophrenia to uncover potential issues in these areas. METHODS: The relevant self-evaluating CANSAS questionnaire for physiological and social needs was used by nurses in a cohort of hospitalized schizophrenic patients undergoing rehabilitation before discharge from the mental hospital. RESULTS: Two hundred and forty-four patients (women N = 115) aged 18-58 years were involved in the study. Intimate relations, financial matters, treatment of psychotic symptoms, and sexual life were among the most pressing physiological and social needs in our study subjects. CONCLUSION: The results of our study should stimulate psychiatric nurses in their effort not only to detect but also address the problems of schizophrenic patients concerning unfulfilled needs. and J. Cíl: Cílem této práce je popis klinického obrazu pacientů s neuromyelitis optica a poruch jejího širšího spektra (NMO Spectrum Disorders; NMO SD) v České republice, včetně srovnání pacientů se začátkem onemocnění před 50. rokem života a po něm v kontextu výskytu longitudinálně extenzivní myelitis (LETM) a poškození zrakového nervu. Dalším cílem je hodnocení likvorových nálezů a séropozitivity protilátek proti akvaporinu‑4 (AQP4-IgG). Soubor a metodika: Byla provedena systematická retrospektivní analýza klinických a paraklinických nálezů 32 pacientů s NMO SD. Tloušťka nervových vláken sítnice (RNFL) každého oka byla měřena metodou optické koherentní tomografie. Tíže neurologického deficitu byla vyjádřena pomocí EDSS skóre. Výsledky: NMO SD častěji postihují ženy (8 : 1). Medián věku počátku onemocnění je 36 let. Medián délky trvání onemocnění je 10,5 roku. Medián EDSS skóre je 4,5 (rozmezí 1,0–8,5) a délky onemocnění 10,5 let (rozmezí 0,5–24). U 31 z 32 pacientů nacházíme AQP4-IgG v séru a jejich titr pozitivně koreluje s EDSS skóre. RNFL oka pravého (medián 63 ?m) a oka levého (medián 58 ?m) je sníženo pod dolní hranici normy (97 ?m) u 57 %, resp. 64 % pacientů. RNFL koreluje s délkou choroby. Myelitida jako první manifestace onemocnění je častější u pacientů starších 50 let (p < 0,05). Ve srovnání s remisí nacházíme v likvoru během relapsů vyšší koncentraci bílkoviny a počet elementů (p < 0,001; pro oba parametry). Závěr: LETM je typicky první klinickou manifestací NMO u pacientů starších 50 let a je spojena s těžkým neurologickým deficitem již v úvodu onemocnění., Aim: The aim of this study was to describe the clinical picture of patients with NMO spectrum disorders (NMO SD) in the Czech Republic, including comparison of patients with disease onset before and after 50 years of age in the context of longitudinally extensive transverse myelitis (LETM) and optic nerve involvement. Another objective was to evaluate cerebrospinal fluid samples and aquaporin-4 antibody (AQP4-IgG) seropositivity. Methods: A systematic retrospective analysis of clinical and paraclinical findings in 32 patients with NMO SD was performed. The retinal nerve fiber layer (RNFL) thickness in each eye was measured with optical coherence tomography. Severity of neurological deficit was expressed by the EDSS score. Results: NMO SD are more frequent in women (8 : 1). Median age of disease onset was 36 years. Median disease duration was 10.5 years. Median EDSS score was 4.5 (range 1.0–8.5) and median disease duration 10.5 years (range 0.5–24). AQP4-IgG were found in the serum of 31 out of the 32 patients and their titer positively correlated with the EDSS score. The RNFL thickness of the right eye (median 63 ?m) and the left eye (median 58 ?m) was below the lower limit of normal (97 ?m) in 57% and 64% of patients, respectively. The RNFL thickness correlated with the disease duration. Myelitis as the first manifestation of the disease was more frequent in patients older than 50 years (p < 0.05). Compared to remission, patients with a relapse had increased protein levels and element count in the cerebrospinal fluid (p < 0.001 for both parameters). Conclusion: LETM is typically the first clinical manifestation of NMO patients older than 50 years and is associated with severe neurological deficit at the early stages of the disease. Úvod: Tři jednonukleotidové polymorfismy (SNP) v NOD2/CARD15 genu (3020insC, R702W, G908R) jsou pro Crohnovu chorobu významným rizikovým faktorem. Jejich diagnostické a terapeutické využití v klinické praxi je však doposud limitované. Cílem studie byla analýza možné asociace těchto polymorfismů s rizikem reoperací a posouzenísouvislosti s fenotypem onemocnění. Metody: U 76 pacientů s Crohnovou chorobou a minimální délkou trvání nemoci 5 let byly testovány tři sekvenční varianty NOD2/CARD15 genu (R702W, G908R, 3020insC). Anamnestická a klinická data, včetně chirurgické léčby byly retrospektivně získány ze zdravotnické dokumentace a prospektivně doplňovány podle protokolu studie. Analyzovány byly souvislosti mezi vybranými genovými polymorfismy, nutností reoperací a fenotypem nemoci. Výsledky: U 24 pacientů (32 %) se našla alespoň jedna sledovaná varianta v NOD2/CARD15 genu. 25 pacientů (33 %) bylo reoperováno, 51 (67 %) představovalo kontrolní skupinu. Trend vyšší četnosti reoperací ve skupině s genovou mutací nedosáhl statistické významnosti (p=0,2688). Dva ze čtyř homozygotů (50 %) nesoucích variantu 3020insC byli opakovaně operováni. Neprokázali jsme signifikantní rozdíly mezi soubory s wild formou a mutovanou formou NOD2/8CARD15 genu s ohledem na věk v době diagnózy (p=0,4356), formu onemocnění (p=0,6610) a lokalizaci (p=0,4747) podle Montrealské klasifikace. Závěr: Polymorfismy v NOD2/CARD genu signifikantně neovlivnily četnost reoperací, homozygoti nesoucí mutaci 3020insC genu však představovali vysoce rizikovou skupinu. Fenotyp onemocnění signifikantně nesouvisel s přítomností vyšetřovaných sekvenčních variant., Introduction: Three NOD2/CARD15 gene variants (3020insC, R702W, G908R) have been identified as genetic risk factors for Crohn’s disease patients. However the diagnostic and therapeutic relevance for clinical practice remains limited. The aim of this study was to evaluate the association between these variants, the risk of reoperation and disease phenotype. Methods: In 76 Crohn’s disease patients (41 female, 35 male) with a minimum 5 year follow-up, three polymorphisms of the NOD2/CARD15 gene (R702W, G908R, 3020insC) were tested. Detailed clinical and medical history including surgical procedures and reoperations were obtained by reviewing the medical charts and completed prospectively. Association between the need for reoperation, disease phenotypes and gene variants were analyzed. Results: 24 patients (32%) showed at least one NOD2/CARD15 mutation. 25 patients (33%) required reoperation, 51 (67%) represented the control group. The expected trend that patients with NOD2/CARD15 variants have a higher frequency of reoperations was not confirmed to a level of statistical significance (p=0.2688). Two of the four patients (50%) with the 3020insC variant required further surgery. We did not confirm any association between NOD2/CARD15 mutations and age at diagnosis (p=0.4356), behavior (p=0.6610), or localization (p=0.4747) according to the Montreal classification. Conclusion: NOD2/CARD15 polymorphisms did not significantly affect the reoperation rate. Homozygosity for the 3020insC variant in the NOD2/CARD15 gene is associated with a high risk of reoperation. Cíl: Vyšetření čichu Novým testem libosti pachů (New Test of Odor Pleasantness; NTOP) umožňuje hodnocení chemosenzorické percepce v klinické praxi. Výhodou je možné použití i u pacientů s kognitivním deficitem a u dětských pacientů. Cílem studie bylo stanovit normální (fyziologické) hodnoty NTOP u zdravé populace. Soubor a metodika: Vyšetřeno bylo 236 zdravých osob ve věkovém rozmezí od 16 do 93 let. Nový test libosti pachů obsahuje 29 výrazně hédonicky zabarvených pachových látek a tři látky s výraznou trigeminovou složkou. Vyšetřovaný měl po přičichnutí označit ze čtyř možností nejpřiléhavější pro danou pachovou látku: voní, neutrální, zapáchá a výrazně zapáchá (nebo dráždí). Na základě nejčetnějších odpovědí osob kontrolní skupiny ve věkovém rozmezí 16–35 let bylo nastaveno bodové hodnocení. Výsledky NTOP byly porovnány se standardizovanými testy subjektivní olfaktometrie OMT (Odourized Markers Test) a Sniffin’ Sticks (část identifikace). Výsledky: V Novém testu libosti pachů byl medián bodového zisku kontrolní skupiny 20 bodů, ve skupině 36–55 let 19,5 bodu a ve skupině > 56 let 17 bodů. Za nejnižší ještě normální bodový zisk byl stanoven 10. percentil kontrolní skupiny, který byl 16 bodů. Hodnota mediánu v souboru klesá s věkem. Statisticky významný pokles je patrný především po 56. roce věku. Závěry: Nový test libosti pachů vykazuje obdobnou charakteristiku jako standardně užívané testy subjektivní olfaktometrie. Současně však test přináší jinou informaci o čichových schopnostech osob. Normální hodnoty zdravé populace jsou klíčové pro posouzení čichu Novým testem libosti pachů v klinické praxi., Objective: Olfactory test of odor pleasantness may prove to be additions to comprehensive chemosensory test batteries used in clinical settings. The goal of our work was to establish normative data of the New Test of Odor Pleasantness (NTOP). Methods: A total of 236 healthy participants aged 16 to 93 years were included in the study. NTOP is based on classification of 29 odorants and three chemicals with trigeminal component. The participants were asked to categorize the hedonic tone of the odorant into four classes: pleasant, neutral, unpleasant, and fetid (very unpleasant). The normative data were determined based on results of persons of 16 to 35 years old. The NTOP was compared with two standard tests – Odourized Markers Test (OMT) and Sniffin’ Sticks (identification). Results: Median of the NTOP was of 20 in group of age 16–35 years, 19.5 in group of age 36–55 years and 17 in group of > 56. Tenth percentile of the age group 16–35 was 16 points, which is considered as lowest normal points scored. The median numbers of points decreases with age. There is statistically significant decrease in the olfactory capability with increasing age (> 56 years old). Conclusion: NTOP has a similar characteristics as the standard tests of subjective (psychophysical) olfactometry. At the same time the test give us information different from other olfactory tests. The goal of study was to determine the normal value the NTOP of healthy persons in Czech population. Práce obsahuje údaje o obsahu jódu v moči především studentů Jihočeské univerzity v Českých Budějovicích. Do pokusu bylo zařazeno celkem 42 studentů a 21 zaměstnanců. Empirická data byla získána v rámci řešení grantového projektu GAJU 011/2013/Z. Obsah jódu ve vzorcích byl stanoven laboratorně spektrofotometricky po alkalickém spalování vzorku podle Sandell-Kolthoffa. Cílem práce bylo posoudit zásobení studentů a zaměstnanců Zemědělské fakulty Jihočeské univerzity v Českých Budějovicích (JU v ČB) jódem na základě vyšetření jodurie. Průměrný obsah jódu v moči u všech osob (n=63) byl 230,40±169,90 µg.l-1. U mužů byl průměrný obsah jódu v moči 244,59±190,46 µg.l-1, u žen (n=42) 223,18±157,82 µg.l-1. Hodnota mediánu se pohybovala u mužů i žen okolo hodnoty 150 µg.l-1. U 89,4 % vyšetřených odpovídala jodurie dostačujícímu, popřípadě nadbytečnému příjmu jódu, u 10,60 % pak různému stupni jodopenie. V zájmu prevence nežádoucího dopadu nutričního nedostatku nebo nadbytku jódu je doporučena pravidelná kontrola jodurie i u studentů vysokých škol., The aim includes data of the iodine content in urine, mainly students of the University of South Bohemia in the České Budějovice. The experiment included a total of 42 students and 21 employees. Empirical data were obtained within the grant project GAJU 011/2013/Z. The content of iodine in the samples was determined on the basis of alkaline combustion by a spectrophotometric method according to Sandell-Kolthoff. The aim of the study was to assess the supply of students and staff Faculty of Agriculture, University of South Bohemia in the České Budějovice (JU v ČB ) iodine on urinary iodine examination. The average content in the urine of all (n=63) was 230.40±169.90 µg.l-1. Average iodine content in urine was 244.59±190.46 µg.l-1 for men (n=21), 223.18±157.82 µg.l-1 for women (n=42). The median value for women and men was approximately 150 µg.l-1. For 89.4% of the examined urinary iodine corresponded to sufficient, or excessive iodine intake, 10.6% corresponded to varying degrees jodopenie. Úvod, cíle: Poruchy ejakulace jsou po míšním poranění velmi časté. U nemocných s přáním vlastního potomka je většinou nutný asistovaný odběr spermií. Penilní vibrostimulace (PVS) je neinvazivní metoda k získání ejakulátu u pacientů s lézí nad úrovní T10. Cílem práce je zhodnocení efektivity a bezpečnosti PVS, vyhodnocení kvality a dalšího využití ejakulátu. Metodika: Od 2010 do 2014 jsme provedli PVS u 20 pacientů. Poranění krční (C) míchy bylo u 13 pacientů, hrudní (T) míchy u sedmi pacientů. Průměrný věk byl 30,8 let. Průměrná doba od úrazu do PVS byla 64 měsíců. První PVS mělo sedm nemocných déle než 3,5 roku a 13 do 3,5 roku od úrazu. K PVS jsme užili přístroj FERTI CARE Multicept. Výsledky: Ejakulace jsme dosáhli u 11 pacientů (55 %), z toho devět mělo poranění C míchy a dva T míchy. U nemocných se začátkem PVS do 3,5 roku od poranění byla úspěšnost 77 % a nad 3,5 roku 14 %, p = 0,027. Při srovnání ejakulátu z druhé PVS došlo ke zvýšení celkového počtu spermií (p = 0,03), koncentrace spermií (p = 0,025) a počtu progresivně pohyblivých spermií (p = 0,027) oproti první PVS. Získaný ejakulát je v 72 % pouze kryoprezervován k pozdějšímu využití. U dvou pacientů byl ejakulát použit k fertilizaci. Jeden nemocný měl azoospermii. Autonomní dysreflexie se vyskytla v průběhu PVS u sedmi pacientů, příznaky odezněly do 3 min. Závěr: PVS je bezpečná metoda asistovaného odběru ejakulátu. Při opakování PVS jsme zjistili zlepšení kvality ejakulátu. U dvou pacientů byl ejakulát úspěšně použit k fertilizaci., Objectives: Following spinal cord injury (SCI), ejaculatory dysfunctions are frequent. In men who wish to father children, assisted sperm retrieval is often necessary. Penile vibratory stimulation (PVS) is a non-invasive method for sperm retrieval in patients with a lesion above T10. The aim of our research was to evaluate the effectiveness and safety of PVS, semen quality and further utilization of the ejaculate. Methods: Between 2010 and 2014, we performed PVS in 20 patients. An injury of the cervical (C) and thoracic (T) spinal cord was present in 13 and seven patients, respectively. The mean age of patients was 30.8 years. The mean time from the injury to PVS was 64 months. Seven patients had the first PVS more than 3.5 years after the injury and 13 patients less than 3.5 years. FERTI CARE Multicept was used for PVS. Results: Ejaculation was achieved in 11 patients (55%) – in nine with C and in two with T SCI. Success rate of PVS in patients less than 3.5 years after the injury was 77%, compared to 14% in patients more than 3.5 years after the injury, p = 0.027. Total sperm count (p = 0.03), sperm concentration (p = 0.025) and the number of sperm with progressive motility (p = 0.027) increased in the semen from the second PVS. Obtained ejaculate is mostly (72%) cryopreserved only for further use. The ejaculate was used for fertilisation in two patients. One patient had azoospermia. Autonomic dysreflexia during PVS occurred in seven patients, symptoms resolved within 3 mins. Conclusion: PVS is a safe method for sperm retrieval. Semen quality improved with repeated PVS. The ejaculate was successfully used for fertilisation in two patients. Spinální trauma je silným negativním inzultem postihujícím všechny orgánové soustavy a má komplexní dopad. Sexuální dysfunkce jsou výrazným elementem snižujícím kvalitu života. Význam sexuálních dysfunkcí v celém spektru trvalých následků po poranění míchy vyzdvihuje zejména věk, ve kterém nejčastěji ke spinální lézi dochází. Jsou limitujícím faktorem při plánování rodiny a rodičovství. Bez lékařské asistence nejsou mnozí muži schopni mít biologického potomka. Práce mapuje rozsah sexuálních dysfunkcí (kromě erektilních) u mužů po poranění míchy, jako jsou změny v oblasti libida, ejakulace, orgazmu, senzitivity genitálu, a to obecně a v závislosti na rozsahu míšní léze. Soubor tvořilo 93 pacientů po spinálním traumatu dispenzarizovaných v urologické ambulanci FN u sv. Anny v Brně. Byla použita anonymní dotazníková metoda s dotazníkem vlastní konstrukce. Vyhodnocením dat bylo konstatováno, že trauma míchy ovlivňuje celé spektrum sexuálních funkcí. Hyposenzibilita genitálu, anejakulace, anorgazmie a snížení libida jsou běžným symptomem. Byla prokázána statisticky významná závislost rozsahu poranění míchy a míry sexuálních dysfunkcí v oblastech ejakulace, orgazmu a senzitivity genitálu. Signifikantní závislost mezi snížením sexuální apetence a rozsahem spinální léze nebyla v našem souboru vyjádřena., Spinal trauma is a strong negative insult affecting all organ systems which has a complex impact. Sexual dysfunction is a significant element reducing the quality of life. The importance of sexual dysfunction in the entire spectrum of permanent consequences after spinal cord injury highlights in particular the age at which the spinal lesion occurs most often. It represents a limiting factor for family planning and parenting. A lot of men are not able to have a biological child without medical assistance. This study monitors the entire range of sexual dysfunctions (with the exception of erectile dysfunction) in men after spinal cord injury, such as changes in libido, ejaculation, orgasm, genital sensitivity - bothgenerally and specifically depending on the extent of spinal cord lesions. The sample group consisted of 93 patients after spinal trauma monitored at the Urological Clinic of the St. Anna University Hospital in Brno. We used an anonymous questionnaire method using non-standardized questionnaire of our own design. By evaluating the data it has been concluded that spinal cord trauma affects the entire spectrum of sexual functions. Reduced genital sensitivity, anejaculation, anorgasmia and decreased libido are the common symptoms. A statistically significant correlation between the extent of spinal cord injury and the degree of sexual dysfunction in the areas of ejaculation, orgasm and genital sensitivity has been shown. Significant correlation between the reduction of sexual desire and the extent of the spinal lesion was not expressed in our group.
