Imunostimulace polybakteriálním lyzátem (Urivac®) v prevenci recidivujících infekcí dolních močových cest. Infekce močových cest (IMC) zaujímají v evropských zemích i u nás dlouhodobě druhé místo ve výskytu infekčních onemocnění v populaci a představují tak významný socioekonomický problém. Spolu s respiračními infekty se rozhodující měrou podílejí na preskripci antimikrobiálních přípravků. Extenzivní antimikrobiální léčba je jednou z nejvýznamnějších příčin nárůstu bakteriální rezistence. Určitou možnost prevence recidiv infekcí dolních močových cest a snížení spotřeby antimikrobiálních preparátů představuje imunostimulace. Prospektivní multicentrická studie hodnotila klinický účinek, snášenlivost a přínos k léčbě recidivujících zánětů při podávání imunostimulačního přípravku Urivac®. Počet atak infekcí dolních močových cest se po imunostimulaci významně snížil jak v jejím průběhu, tak po jejím ukončení (p = 2,2x10-16). V průběhu imunostimulace bylo bez příznaků 85 % nemocných a při dalším sledování 70,8 %. Snížil se také počet aplikovaných antimikrobiálních přípravků. Subjektivní zlepšení přetrvávalo ve více než 57 % i po ukončení léčby. Mikrobiologická odpověď dosáhla 77 %, v odstupu od ukončení léčby se snižuje. U exacerbací byla uropatogenní flóra detekována v 61 %. Nežádoucí účinky u dvou nemocných vedly k předčasnému ukončení terapeutického režimu., In European countries, including the Czech Republic, urinary tract infections (UTIs) have long occupied the second place in the incidence of infectious diseases in the population, thus representing a major socioeconomic problem. Along with respiratory infections, they are the major contributors to the prescription of antimicrobial agents. Extensive antimicrobial therapy is one of the most important causes of increased bacterial resistance. Immunostimulation is a possible way of preventing recurrent lower urinary tract infections and reducing the consumption of antimicrobial agents. A prospective multicentre study evaluated the clinical effi cacy, tolerability, and benefi t to treatment of recurrent infections with the immunostimulatory drug Urivac®. The number of attacks of lower urinary tract infections following immunostimulation reduced signifi cantly both during its course and after its cessation (p = 2.2 x 10–16). During immunostimulation, 85 % of patients were symptom-free, and during subsequent follow-up it was 70.8 %. There was associated with a reduction in the number of the antimicrobial agents administered. Subjective improvement persisted in more than 57 % even after treatment cessation. Microbiological response reached 77 %; it decreases with increased time from treatment cessation. In the case of exacerbations, uropathogenic fl ora was detected in 61 %. Východisko: Množství intervenčních studií prokázalo, že pacienti s manifestní vaskulární chorobou profitují z agresivnějšího dávkování statinů. Položili jsme si otázku, jaké dávkování statinů je v praxi obvyklé a nakolik je v praxi splňována doporučená cílová hodnota LDL-cholesterolu. Metodika a výsledky: Analyzováno bylo celkem 948 pacientů o průměrném věku 64,5 let (SD ? 9,0) po akutním koronárním syndromu nebo koronární revaskularizaci (české soubory studií EUROASPIRE III a IV). Přestože statinem bylo v letech 2012/2013 léčeno více než 93 % pacientů, maximální dávku (atorvastatin 80 mg nebo ekvivalent) užívala pouze 2,4 % pacientů a oproti období 2006/2007 se zvýšila pouze preskripce středně dávkovaného statinu (atorvastatin 40 mg). Od let 2006/2007 sice došlo k mírnému zlepšení adherence k cílové hodnotě < 2,5 mmol/l (z 54 % na 65 %), nicméně aktuálně platnou cílovou hodnotu < 1,8 mmol/l dosahovalo stále jen < 25 % pacientů. Na základě individuálních hodnot LDL-cholesterolu lze odhadnout, že při maximální možné up-titraci dávky statinu by adherence k aktuální cílové hodnotě stoupla na asi 43 %. Závěr: Přestože převážná většina pacientů s manifestní ICHS je léčena statinem, užívané dávkování a zejména adherence k cílové hodnotě LDL-cholesterolu v praxi neodpovídá současným doporučeným terapeutickým standardům pro sekundární prevenci., Background: A number of clinical trials have shown that patients with overt atherovascular disease may benefit from more aggressive dosage of statins. We aimed to determined the usual dosage of statin in clinical practice and the adherence to recommended target concentration of LDL-cholesterol. Methods and results : We analyzed 948 patients with mean age 64.5 years (SD ? 9.0) after acute coronary syndrome and/or coronary revascularization (Czech samples of EUROASPIRE III and IV). In spite that more than 93 % of patients were in 2012/2013 treated with statin, only 2.4 % with the highest dose (atorvastatin 80 mg or equivalent). On the other hand, medium-dosed statin (atorvastatin 40 mg) was more often prescribed, in comparison to 2006/2007. We observed mild improvement in adherence to former LDL-cholesterol target < 2.5 mmol/l (from 54 % to 65 %), but the recent target < 1.8 mmol/l was reached only in less than one quarter of patients in 2012/2013. It can be approximate (using individual LDL-cholesterol values), that after maximal possible up-titration of statin, the adherence to recent LDL-cholesterol target may improve up to 43 %. Úvod: Rozsah nádoru a kvalita jeho chirurgického odstranění patří k hlavním prognostickým faktorům v léčbě karcinomu rekta. Počet pooperačních lokálních recidiv závisí na úplnosti odstranění mezorekta – TME (total mesorectal excision) – a dosažení bezpečných resekčních okrajů bez nádoru. Provedení a kvalita operace záleží na předoperační diagnostice a detekci rizikových míst na rektu a mezorektu, na chirurgické schopnosti výkon správně provést a na patologickém vyšetření, hodnotícím úplnost odstranění nádoru v parametrech resekčních okrajů včetně cirkumferentního (CRO) a kompletnost mezorektální excize. Cílem práce bylo zavést a standardizovat metodu hodnocení kvality chirurgického výkonu − mezorektální excize – v léčbě karcinomu rekta hodnocením resekčních okrajů včetně CRO a kompletnosti excize. Metody: Studie měla dvě části. V první, retrospektivní části multicentrické studie byly v sestavě 288 pacientů analyzovány jednotlivé dílčí parametry diagnostiky, operací a histologického vyšetření karcinomu rekta, identifikována kritická místa a připraven jednotný protokol sledování. Ve druhé, prospektivní části studie byla v sestavě 600 pacientů tato data sledována parametricky a byla hodnocena kvalita TME ve vztahu k onkologickému výsledku léčby. Výsledky: V předoperační diagnostice se zvýšil podíl pacientů, kterým byl proveden restaging po neoadjuvantní léčbě z 60,0 % na 81,7 %. Podíl resekátů, u nichž nebyla hodnocena kvalita excize mezorekta, se snížil z 52,9 % v retrospektivní části na 22,8 % v části prospektivní. Podíl kompletních excizí mezorekta se zvýšil z 22,6 % na 26,0 %, podíl téměř kompletních excizí se zvýšil z 10,1 % v retrospektivní části na 24,0 % v prospektivní části studie. Závěr: Zavedení parametrického sledování do rutinní praxe zvýšilo kvalitu předléčebné a předoperační diagnostiky, vyšetření resekátu patologem a vlivem zpětnovazebné informace také kvalitu chirurgického výkonu., Introduction: Tumour size and the quality of its complete surgical removal are the main prognostic factors in rectal cancer treatment. The number of postoperative local recurrences depends on whether the mesorectum has been completely removed – total mesorectal excision (TME) – and whether tumour-free resection margins have been achieved. The surgery itself and its quality depend on the accuracy of preoperative diagnosis and detection of risk areas in the rectum and mesorectum, on the surgeon’s skills, and finally on pathological assessment evaluating whether complete tumour excision has been accomplished including circumferential margins of the tumour, and whether mesorectal excision is complete. The aim of our study was to implement and standardize a new method of evaluation of the quality of the surgical procedure – TME – in rectal cancer treatment using an assessment of its circumferential margins (CRO) and completeness of the excision. Methods: The study consisted of two parts. The first, multi-centre retrospective phase with 288 patients analysed individual partial parameters of the diagnosis, operations and histological examinations of the rectal cancer. Critical points were identified and a unified follow-up protocol was prepared. In the second, prospective part of this study 600 patients were monitored parametrically focusing on the quality of the TME and its effect on the oncological treatment results. Results: The proportion of patients with restaging following neoadjuvant therapy increased from 60.0% to 81.7% based on preoperative diagnosis. The number of specimens missing an assessment of the mesorectal excision quality decreased from 52.9% in the retrospective part of to the study to 22.8% in the prospective part. The proportion of actually complete TMEs rose from 22.6% to 26.0%, and that of nearly complete TMEs from 10.1% to 24.0%. Conclusion: The introduction of parametric monitoring into routine clinical practice improved the quality of pre-treatment and preoperative diagnosis, examination of the tissue specimen, and consequently improved quality of the surgical procedure was achieved., and J. Úvod: Observační, prospektivní a neintervenční projekt SOLOSTAR měl za cíl získat informace z klinické praxe v České republice o spokojenosti pacientů s používáním předplněného inzulinového pera SoloStar. Metodika: 1 805 pacientů s diabetem 1. nebo 2. typu bylo po zahájení používání pera SoloStar sledováno 3–4 měsíce. Spokojenost pacientů s perem SoloStar a s jeho jednotlivými parametry byla posuzována ve skupině všech pacientů a v podskupinách rozdělených dle demografických parametrů včetně zdravotního omezení. Pacienti také porovnávali pero SoloStar s pery používanými před vstupem do projektu. Výsledky: 98,3 % pacientů ohodnotilo celkovou spokojenost s perem SoloStar jako „výbornou“ a „dobrou“ (nejlepší a druhé nejlepší hodnocení na 5stupňové škále). Na celkovou spokojenost s perem SoloStar nemělo statisticky významný vliv žádné z demografických kritérií, a to ani zrakový handicap či omezení manuální zručnosti. Parametry pera SoloStar byly u více než 90 % pacientů hodnoceny jako „výborné“ a „dobré“. Lépe byly některé parametry hodnoceny zejména pacienty bez předchozí zkušenosti s aplikací inzulinu. Pacienti, kteří používali jiný typ aplikátoru před zařazením do projektu, byli více spokojeni s perem SoloStar než s předchozím aplikátorem a většina z nich ohodnotila používání pera SoloStar (93,1 %) a aplikaci inzulinu perem SoloStar (83,5 %) v porovnání s původním perem jako „mnohem jednodušší“ a „jednodušší“ (nejlepší a druhé nejlepší hodnocení na 5stupňové škále). Závěr: Používání pera SoloStar je v běžné klinické praxi v České republice spojeno s vysokou spokojeností pacientů, a to včetně pacientů se zdravotním omezením. Pacienti navíc pero SoloStar upřednostňovali před inzulinovými pery používanými před vstupem do projektu. Methodology: 1 805 patients suffering from type 1 or 2 diabetes who began using the pen SoloStar were observed for 3 to 4 months. Satisfaction of patients with SoloStar and with its particular features was evaluated in the group of all patients and in subgroups defined by demographic parameters including the type of handicap. Patients also compared SoloStar with the pens used before the entry into the survey. Results: 98.3 % of patients rated overall satisfaction with the pen SoloStar as “excellent” and “good” (the highest and second highest rating on a 5-point scale). Demographic criteria, including visual handicap and manual dexterity handicap, did not have any statistically significant effect on the rating of the overall satisfaction with SoloStar. Features of SoloStar were rated as “excellent” and “good” by more than 90% of patients. Higher rating of some of the features of SoloStar was awarded mostly by patients without prior experience with insulin application. Patients who used other insulin applicators before entering into the survey were more satisfied with the pen SoloStar than with the previously used applicator and most of them rated the use of SoloStar (93.1 %) and insulin application with SoloStar (83.5 %) as “much easier” and “easier” (the highest and second highest rating on a 5-point scale) in comparison with the previously used pen. Conclusion: The use of the pen SoloStar in a clinical practice setting in the Czech Republic is associated with high levels of satisfaction of patients, including handicapped patients. In addition, patients preferred SoloStar over insulin pens used before the entry into the survey.
