To study efficiency of the combined treatment of advanced gastric cancer with inclusion Trastuzumab. Material: We present the intermediate analysis of the use of target therapy with Trastuzumab in patients with a HER2positive gastric cancer. Up to 01.10.2009y 118 patients have been tested for HER2 expression, and in 24 gastric cancer patients it is revealed HER2positive status of tumor. It is lead chemotherapy to all patients by the PLF regimen and Herceptin in doze 6 mg/kg once in 3 weeks (6 cycles). In control group in 26 patients it is lead only chemotherapy by the PLF regimen once in 3 weeks without addition Trastuzumab (6 cycles). Results: At the moment of the analysis of preliminary data, the median remission duration in compared groups has made 8.3 months, and 5.2 months, accordingly. Jako light chain escape fenomén (LCE) je označována změna biologického chování nádoru u pacientů s mnohočetným myelomem (MM). Při diagnóze jsou v séru přítomny kompletní monoklonální imunoglobulin (mIg) a monoklonální volné lehké řetězce (mFLC), při relapsu nebo progresi onemocnění dochází k prudkému vzestupu koncentrace pouze mFLC, obvykle při stabilní nebo klesající koncentraci mIg, společně s těžkým orgánovým postižením. Pro časnou diagnostiku LCE u nemocných s MM je doporučováno sériové vyšetřování FLC. LCE byl popsán v literatuře zatím pouze u pacientů po hematologické terapii. V tomto sdělení prezentujeme kazuistiku pacienta vstupně hodnoceného hematologem jako doutnající myelom, bez nutnosti hematologické terapie. Domníváme se, že vývoj nálezu odpovídal kritériím LCE., The light chain escape phenomenon is characterized as a change in tumor biological movement in patient with multiple myeloma. Complete monoclonal immunoglobulin molecules (mIg) together with monoclonal free light chains (mFLC) are present in diagnosis, only mFLC great increase is observed in relapse or disease progression together with heavy organ impairment whether mIg concentration is usually stable or decreasing. Serum FLC monitoring is recommended for early diagnostics LCE phenomenon in patients with MM. To this time LCE phenomenon was described only in patients after hematological therapy. Stárnutí populace je považováno na celém světě za hlavní faktor, který je v pozadí růstu nákladů na zdravotní péči. V odborné literatuře se však poslední dobou množí důkazy, které naznačují, že stárnutí populace má jen malý vliv na růst těchto nákladů. Autoři textu předkládají rešerši literatury, analyzují náklady na zdravotnictví v České republice a upozorňují na oblasti zdravotní péče, které mají největší vliv na růst výdajů na zdravotnictví v ČR. V závěru konstatují, že v ČR je vliv stárnutí obyvatelstva na výdaje ve zdravotnictví relativně malý a podobá se situaci popsané v zahraniční literatuře. Vyslovují názor, že by centrálním mechanismem způsobujícím rychlý růst výdajů v ČR mohla být nabídkou indukovaná poptávka., Population aging is purported to be a major factor behind the rise of the health care cost worldwide. The increasing body of evidence available in the literature on the subject of aging population and its influence on health care cost suggests that demographic changes play a relatively minor role in driving up the health care bill. First, we review some available evidence. Second, we analyze the situation in the Czech Republic using financial and utilization data and demographic projections. Analýza dat ze druhé vlny šetření ESS vede k těmto zjištěním: (1) V evropských státech hodnotí zdravotnictví zpravidla příznivěji muži než ženy. Tento rozdíl je patrný především ve státech, které nemají komunistickou minulost. (2) Věk vykazuje s hodnocením zdravotnictví křivku ve tvaru otevřeného J: nejpříznivěji hodnotí zdravotnictví staří lidé, nejméně příznivě lidé ve středním věku. (3) V postkomunistických státech platí, že čím jsou jedinci – zejména ženy – vzdělanější, tím nepříznivěji hodnotí zdravotnictví. V ostatních státech je tento vztah mírně kladný. (4) Ve všech státech hodnotí respondenti zdravotnictví tím příznivěji, čím jsou celkově šťastnější a spokojenější se svými životy. (5) Tato souvislost je tím slabší, čím vyšší je kvalita života na úrovni států. (6) V postkomunistických státech hodnotí respondenti v průměru zdravotnictví méně příznivě než ve státech ostatních. V těchto státech je současně nižší průměrná kvalita života. Při kontrole kvality života na úrovni států se rozdíl v hodnocení zdravotnictví mezi postkomunistickými a ostatními státy výrazně sníží. (7) V naprosté většině evropských států hodnotí pravicově orientovaní jedinci zdravotnictví příznivěji než jedinci orientovaní levicově. Tento vztah je silnější ve státech, které nemají komunistickou minulost. Nelze ho vysvětlit rozdíly v kvalitě života v různých státech., The analysis of data from the second wave of the ESS (European Social Survey) leads to these findings: (1) Men in European states evaluate the health systems in their countries more favorably than women. (2) The relationship between age and evaluation of health system has the form of an open J: Old people evaluate the health system the best, people of middle age the worst. (3) The more educated are people in the post-communist states (esp. women), the less favorably they evaluate the health systems. In the other European states, the relationship is mildly positive. (4) The better the subjective well-being of individuals, the better their evaluation of the health system. (5) This relationship is the weaker, the better is the quality of life on the state-level. (6) Respondents in the post-communist states evaluate health systems less favorably than respondents in the other European countries. At the same time, the quality of life in the post-communist countries is worse than in the other countries. When statistically controlling for the difference between the quality of life in these two categories of countries, the difference in evaluations of the health systems in these two categories of countries is remarkably reduced. (7) In the majority of European states the more are the respondents politically to the right, the more favorably they evaluate the health system in their country. This relationship is stronger in non-postcommunist than in postcommunist states. Radikální resekce je jedinou metodou léčby, která u nemocných s karcinomem pankreatu dává reálnou šanci na dlouhodobé přežití a je potencionálně kurabilní. Pětileté přežití po R0 resekci s adjuvantní chemoterapií je uváděno v rozmezí 11–28 %. Česká Republika se ve výskytu karcinomu pankreatu nachází na 6. místě v Evropě. 70–80 % nemocných je v době stanovení diagnózy neresekabilních. K radikální resekci indikujeme nemocné se stagingem T1-3, N0-1, M0. Radikální resekce je doplněna standardní lymfadenektomií, resekce portomezenterického žilního segmentu je indikována v rámci R0 resekce. Při nálezu přesahujícím tento staging je indikována u nemocných v celkově dobrém stavu paliativní resekce či v případě ikteru biliodigestivní anastomóza, v ostatních případech endoskopické zavedení stentu. Rozšířené resekce neprokázaly signifikantní benefit pro pacienta. V současné době peroperační mortalita nepřesahuje 3–5 %, morbidita dosahuje až 60 %. Nejzávažnější je únik pankreatické šťávy. Adjuvantní chemoterapie signifikantně potencuje přínos R0 resekce. Paliativní chemoterapie prodlužuje signifikantně přežívání u pokročilých stadií ve srovnání se symptomatickou terapií. Možnosti neoadjuvantrní léčby jsou limitovány. Léčbu nemocných s karcinomem pankreatu je třeba koncentrovat na specializovaná centra, rozhodnutí o léčbě stanoví multioborový tým., Radical resection is only method of treatment giving the chance for long-term survival in the patients with pancreatic cancer with 5-year survival 11–28 %. Czech Republic is in the incidence of pancreatic cancer in the 6.place in Europe. 70–80 % patients are irresectable in the moment of diagnosis. Patients with staging of T1-3, N0-1, M0 are indicated for radical resection with standard lymphadenectomy, resection of portomesenteric segment is indicated as a part of R0 resection. In the case of advanced carcinoma over that staging in patients in good condition palliative resection or biliodigestive anastomosis is indicated, in other cases endoscopic stenting. Extended resections did not prove the benefit for patients. Current perioperative mortality is not over 3–5 %, morbidity reach till 60 %. Pancreatic leak is the most serious complication. Adjuvant therapy significant prolongs survival after R0 resection. Palliative chemotherapy prolongs survival in advanced carcinoma to compare to symptomatic therapy. The possibilities of neoadjuvant therapy are very limited.
