Zavedení nových léčebných postupů do léčby AML a další zlepšení samotných transplantačních postupů může v budoucnu potenciálně zvýšit možnost zařazení transplantace do léčby pacientů starších 60 let s AML., Acute myeloid leukemia (AML) is disease of older age with the median of age at diagnosis 69 years. Unfortunately results of intensive treatment of patients older than 60 years with AML, thus patients with high-risk of AML development, are unsatisfactory. The reasons for this unsatisfactory results are higher risks of treatment-related mortality among older patients treated with intensive chemotherapy and biological characteristics of AML in older age with higher risk of AML resistance to intensive treatment and higher risk of AML relapse. According to published data reduced-intensity allogeneic transplantation as part of consolidation treatment and advancement in supportive care after transplantation can improve outcome of patients older than 60 years with AML. The limitation of this strategy is a fact that majority of older patients cannot undergo allogeneic transplantation due to unsatisfactory control of AML or due to diminished performance status which does not alow transplant procedure. New approaches to AML treatment and future improvements in transplant procedure could potentially increase the role of allogeneic transplantation in treatment of patients older than 60 years with AML., Michal Karas, and Lit.: 22
Akutní lymfoblastická leukemie dospělých s pozitivitou filadelfského chromozomu a fúzního genu BCR/ABL je agresivní hematoonkologické onemocnění, typické pro pacienty vyššího věku. Léčba spočívá v podávání chemoterapie a inhibitoru tyrozinkinázy. V případě relapsu je prognóza onemocnění extrémně nepříznivá., Philadelphia chromosome positive adult acute lymphoblastic leukemia with BCR/ABL fusion gene is an aggressive hematological malignancy most common in the elderly. Both chemotherapy and a tyrosine kinase inhibitor are engaged in the treatment. The best outcome can eventually be achieved using allogeneic hematopoietic stem cell transplantation. However, only a minority of patients can undergo such intensive therapy. Disease relapse is associated with an extremely dismal prognosis., František Folber, Eva Létalová, Michael Doubek, and Lit.: 23
Avaskulární nekróza hlavice kosti stehenní u dospělých patří mezi onemocnění, u nichž včasná diagnostika a léčba s cílem uchránit kloub podstatně zlepšuje prognózu nemocných. Spojení scintigrafie s CT v hybridním přístroji vnáší do funkčního zobrazení anatomickou přesnost a CT obraz oceníme zejména v pozdějších stadiích, kdy se objevují morfologické změny na subchondrální kosti., Avascular necrosis of the femoral head of adults is one of the diseases whose early diagnosis and treatment with the aim to protect the joint substantially improves the prognosis of patients. The disadvantage of both conventional radiographs and the native CT is a low sensitivity to early stages of the disease. Functional image through the bone scintigraphy brings into the diagnostic process sensitivity for the initial stages of ischemia when the radiographic image is still normal. The combination of both methods enables to localize anatomically the altered metabolic activity of osteoblasts, to assess the morphological and metabolic image of the bone reparative changes at the same time, to diagnose secondary complications and to find out whether a persistent metabolic activity in residual changes after an AVN remains. This article will present a survey of the basic pathogenetic events that occur during the development of the disease and detect the pathophysiological substance of scintigraphic and of radiological findings on CT and X-ray images. It describes the technique of the SPECT/CT examination following immediately the whole-body bone scintigraphy as well and tries to put an objective view of the possibilities, diagnostic contribution and limits of this method. Although nowadays the nuclear magnetic resonance imaging (MRI) for the diagnosis of aseptic necrosis is more and more often used, bone scintigraphy, especially in a three-phase modification, remains a highly sensitive method of detecting early stages of the disease. The combination of the scintigraphy and CT in a hybrid apparatus ensures the anatomical precision into functional imaging and a CT scan is useful above all in later stages when the morphological changes in the subchondral bone appear., Ladislav Zadražil, Petr Libus, and Lit.: 16
Znalost těchto prognostických faktorů může napomoct ke správné volbě systémové terapie renálního karcinomu., Treatment selection is difficult in metastatic renal cell cancer. It is based mainly on tumour histology and biologic behaviour, patients characteristics such as comorbidities and performance status, and expected treatment toxicity. Several recently published studies have identified prognostic factors for patients treated with targeted agents including sunitinib, sorafenib, pazopanib, bevacizumab, temsirolimus, and everolimus. These prognostic factors should be considered when selecting the treatment strategy., Tomáš Büchler, and Lit.: 19
Vytvoření standardů založených pouze na gestačním stáří, které často nejsme schopni přesně stanovit, by neodpovídalo medicínské i etické složitosti situace a bránilo v individuálním rozhodování., Determining the threshold of viability is the initial point in considering medical treatment. Viability depends - aside from the level of maturity of the fetus and its health condition – on the advancement of medical technology. Its quality is also based upon given socio-economical and cultural factors. The threshold of viability in regard of the possibility to be born alive and survive lies between the 22-24 weeks. An expert agreement on providing active care is based upon the probability of survival and survival without serious disability. Resuscitation and intensive care from the 26 weeks of gestation is usually mandatory in developed countries because infants born at this week have a very high chance of survival without severe disabilities and with good quality of life. The 22-25 week of gestation is known as the grey zone, during which the results of care are uncertain and unpredictable in advance. However, they depend not only on gestational age but on birth weight, sex, multiple gestation, place of delivery, and induction of lung maturity with corticosteroids. They are influenced by the caregivers approach and attitude. Surviving the first week changes the prediction significantly. It is essential to precede our decision-making according to a comprehensive concept of viability. Creating standards based entirely on gestational age which we cannot determine accurately would not meet the medical and ethical complexity of the situation and would prevent us from individual deliberation., Blanka Zlatohlávková, and Literatura 31
