Dysfunkce autonomního nervového systému patří k častým klinickým projevům roztroušené sklerózy (RS). Incidence autonomní dysfunkce (AD) u pacientů s RS se v různých studiích pohybuje mezi 16 a 80 % a jednoznačně se zvyšuje s délkou trvání RS a progredující disabilitou. Může se však projevit v kterékoli fázi demyelinizačního onemocnění a dokonce může být jeho prvním klinickým projevem. AD významně ovlivňuje kvalitu života pacientů s RS a přispívá k celkové disabilitě. Přesto postižení autonomního nervového systému patří mezi stále relativně poddiagnostikované symptomy tohoto onemocnění. Nejčastějším a nejzávažnějším typem AD u pacientů s RS je narušení funkce kardiovaskulárního a/nebo urogenitálního systému. Méně častá je dysfunkce gastrointestinální, sudomotorická či narušení pupilomotoriky. Autonomní (především kardiovaskulární) dysfunkce u pacientů s RS také úzce souvisí s rozvojem únavy. Cílem tohoto sdělení je poskytnout přehled jednotlivých typů AD u pacientů s RS a možností jejich diagnostiky., Autonomic nervous system dysfunction (AD) represents a frequent clinical presentation of multiple sclerosis (MS). According to published studies, the incidence of AD in MS patients ranges between 16 and 80% and gradually increases with the length of the demyelinating disease and with the progression of disability. However, AD can occur in any phase of MS and can even represent its first symptom in some patients. Autonomic dysfunction has a significant negative impact on the quality of life in MS patients and contributes to overall disability. Even so, autonomic nervous system dysfunction is frequently underdiagnosed in patients with multiple sclerosis. Cardiovascular and urogenital dysfunction are the most frequent types of AD in MS patients with the highest impact on their clinical status and the quality of life. Less frequently, gastrointestinal, sudomotor or pupilomotor dysfunction can be found. Autonomic (in particular cardiovascular) dysfunction in multiple sclerosis is closely related to fatigue, another frequent clinical symptom in MS patients. The aim of this paper is to sum up the spectrum of symptoms of autonomic dysfunction in multiple sclerosis and its diagnostic tools. Karcinom prsu v diseminovaném stadiu je nevyléčitelné onemocnění s definovanou dobou přežití, která v případě viscerálních metastáz dosahuje jen několika měsíců. U pacientek s viscerálním postižením, symptomatických a hormonálně nedependentních je základem léčby chemoterapie. U pacientek HER2 negativních (resp. triple negativních) má své místo antiangiogenní léčba bevacizumabem, která je schválena v I. linii léčby metastatického HER2 negativního karcinomu prsu v kombinaci s paclitaxelem, a to na základě významného prodloužení PFS ve studii III. fáze E2100 (1). Kazuistika: 53letá pacientka onemocněla triple negativním karcinomem prsu v roce 2008. Absolvovala neoadjuvantní chemoterapii, radikální mastektomii s exenterací axilly s pooperačním stagingem ypT1a ypN1. Následovala adjuvantní chemoterapie, ozáření. V květnu 2012 prokázány metastázy v plicích a v mediastinálních uzlinách dle CT. Provedena bronchoskopie a histologicky verifikovány metastázy duktálního karcinomu, triple negativního, dle FISH nebyla nalezena amplifikace genu HER2. Indikována ke kombinované léčbě paclitaxel + bevacizumab. Došlo ke kompletní remisi onemocnění. Léčba samotným bevacizumabem pokračuje dosud. Pacientka přežívá od zjištění generalizace dosud 43 měsíců! Kvalita života je výborná., Metastatic breast cancer is an incurable disease with a defined time of survival. This time is limited to only a few months when visceral metastases are present. Chemotherapy is the crucial treatment for patients with visceral involvement, symptomatic and hormone-independent disease. There is a role as well for an antiangiogenic therapy with bevacizumab in HER2-negative disease. Bevacizumab is approved for the treatment of metastatic HER2-negative breast cancer in combination with paclitaxel. This approval is based on significant improvement in PFS reported in phase III trial E2100. Case report: 53-years old patient, female, was diagnosed with triple-negative breast cancer in 2008. Chemotherapy was delivered in neoadjuvant setting, followed by radical mastectomy with dissection of axilla. Pathological staging was ypT1a ypN1. Adjuvant chemotherapy was applied as well as radiation therapy. In May 2012 the evidence of metastatic disease in lung and mediastinal lymph nodes was proved by CT scan. Triple negative breast cancer was verified from bronchology exam. FISH analysis of HER2 was also negative. Combination treatment with paclitaxel + bevacizumab was indicated. Jako Bouveretův syndrom je označována obstrukce v oblasti duodena impaktovaným žlučovým kamenem, který vycestuje cholecystoduodenální či cholecystogastrickou píštělí. Onemocnění je vzácné, vyskytuje se zejména u starších žen s četnými komorbiditami a vysokým operačním rizikem. K diagnóze se zpravidla dospěje endoskopicky nebo pomocí zobrazovacích metod. Pokud stav nelze řešit endoskopicky, je indikováno operační řešení. Autoři prezentují případ Bouveretova syndromu u 79leté pacientky, u níž se zdařilo zdokumentovat všechny fáze vyšetřovacího a léčebného procesu. Součástí sdělení je přehled literatury týkající se diagnostiky a terapie tohoto vzácného syndromu., Bouveret syndrome is a gastric outlet obstruction caused by impaction of a gallstone that passes through a cholecystoduodenal or cholecystogastric fistula. It is a rare disease, most common in elderly women with multiple comorbidities and high surgical risk. The diagnosis can be made either radiologically or endoscopically. Endoscopic extraction is the preferred therapeutic option. Surgical intervention is indicated when endoscopic methods fail. We describe a case of Bouveret syndrome in a 79 years old woman. Východisko a cíl: Prognóza inoperabilního karcinomu jícnu je velmi závažná, léčba těchto pacientů je paliativní. Cílem sdělení je retrospektivní hodnocení intraluminální brachyterapie vysokým dávkovým příkonem nádorové stenózy inoperabilního karcinomu jícnu. Pacienti a metody: Od února 1996 do června 2011 bylo intraluminální brachyterapií léčeno 41 pacientů s karcinomem jícnu, z toho v 19 případech se jednalo o dlaždicobuněčný karcinom a ve 22 případech o adenokarcinom. U všech pacientů byla před zahájením brachyterapie přítomna dysfagie. Výsledky: U většiny pacientů došlo po brachyterapii k úlevě polykacích obtíží. Medián doby přežití činil 396 dní (95 % CI: 270–492 dní). Nebyla pozorována mechanická komplikace zavedení brachyterapeutického aplikátoru. Závěr: Intraluminální brachyterapie je účinná a bezpečná paliativní metoda léčby dysfagie způsobené nádorovou stenózou jícnu. The aim of this study is to retrospectively evaluate the effectiveness of intraluminal high dose rate brachytherapy in the palliative treatment of tumorous esophageal stenoses. Diabetes mellitus je významným rizikovým faktorem pro rozvoj ischemické choroby srdeční. Její prevalence se u diabetiků udává v širokém rozmezí 10–55 %. Po přítomnosti ischemie myokardu je nutno u diabetiků aktivně pátrat, neboť čekání na rozvoj symptomů je u těchto pacientů spojeno s nálezem již pokročilé koronární aterosklerózy s horšími výsledky koronárních intervencí chirurgické léčby. Podle výsledků studie BARDOT se zdá, že vhodným nástrojem pro stratifikaci rizika kardiálních příhod u diabetiků by mohla být zátěžová scintigrafie myokardu. Toto vyšetření dokáže velmi dobře odhalit přítomnost ischemie myokardu, zhodnotit její rozsah i lokalizaci a odlišit, zda jde o reverzibilní ischemii. Tyto údaje jsou velmi důležité pro určení správné strategie léčby. U diabetiků často nalézáme pokročilou koronární aterosklerózu, která je vhodnější k chirurgické léčbě. Nicméně v době moderních stentů je možno i diabetiky léčit intervenčně. Každý́ diabetik by však měl mít důsledně intervenovány všechny ovlivnitelné rizikové faktory ischemické choroby srdeční a měl by být léčen tak zvanou optimální farmakoterapií. Jedná se o medikaci s prokázaným vlivem na prognózu pacientů, ovlivnění progrese koronární aterosklerózy a zmírnění ischemie myokardu., Diabetes mellitus is a major risk factor for the development of ischemic heart disease. Its prevalence in diabetic patients is reported to range broadly between 10–55 %. It is necessary to actively search for the presence of myocardial ischemia in patients with diabetes, since waiting for the development of symptoms is associated with the finding of already advanced coronary atherosclerosis in these patients, with less satisfactory outcomes of coronary interventions and surgery. Results of the BARDOT study seem to indicate that the appropriate tool for stratification of the risk of cardiac events in diabetics could be stress myocardial scintigraphy. This test is successful in detecting the presence of myocardial ischemia, assessing its size and location and identifying whether reversible ischemia is involved. Such data is very important with regard to choosing the right treatment strategy. We often find advanced coronary atherosclerosis in diabetic patients for which surgical treatment is more appropriate. Nonetheless in the era of modern stents also the patients with diabetes can be treated with intervention. All the influenceable risk factors for ischemic heart disease should be consistently treated by intervention in each diabetic patient, who should be given optimum pharmacotherapy.
