Úvod: Česká republika zaujímá v incidenci kolorektálního karcinomu (KRK) čelní místo ve světě. Radikální odstranění nádoru představuje nejúčinnější část v rámci současné multimodální terapie. Autoři prezentují průběžné výsledky roboticky asistované léčby u nemocných s karcinomem rekta (KR). Metoda: Vlastní soubor obsahuje 61 roboticky operovaných pacientů pro KR. U posledních 31 pacientů byla data získávána prospektivně. Analyzovány byly epidemiologické údaje, perioperační ukazatele, komplikace a onkologické parametry. Výsledky: Roboticky asistovanou operační léčbu pro karcinom rekta na našem pracovišti podstoupilo 61 nemocných: 34 mužů a 27 žen průměrného věku 62 let (33–80 let). Neoadjuvantní onkologická léčba byla indikována u 46 % pacientů. Průměrná krevní ztráta činila 187 ml, u tří nemocných byly podány transfuzní přípravky. Šest operací jsme byli nuceni konvertovat, u 16 pacientů jsme zaznamenali komplikace. Insuficience anastomózy se vyskytla u 10 % pacientů, u 4 nemocných si stav vyžádal operační léčbu. Žádný nemocný nezemřel. Rekurenci maligního onemocnění jsme diagnostikovali u 3 (5 %) pacientů. U 27 nemocných byla (od roku 2013) stanovena kvalita mezorektální excize (ME) a metrické stanovení cirkumferentního resekčního okraje – (y)pCRO. Ve dvou případech byla potvrzena pozitivita (y)pCRO a přibližně u čtvrtiny operantů nebyla ME kompletní. Závěr: Chirurgická léčba je v interdisciplinárním terapeutickém procesu u KR klíčová. Naše průběžné výsledky jsou srovnatelné se závěry některých literárně uveřejněných sdělení. Da Vinci systém je bezpečným manipulátorem v léčbě KR a operatérovi přináší nesporné výhody při preparaci ve stísněném pánevním prostoru. V současnosti chybí zhodnocení přínosu robotické operativy pro pacienta v oblasti břišní chirurgie z dlouhodobého hlediska a s dostatečným počtem operantů, resp. vysokým stupněm evidence based medicine (EBM). Vysoká pořizovací cena přístroje, jednotlivých instrumentů s vybavením a nesystémová úhrada představují významnou překážku v širším využití robotického systému v léčbě KR i jiných abdominálních malignit v ČR., Introduction: The incidence of colorectal cancer (CRC) in the Czech Republic is reported to be one of the highest on the global scale. Radical tumor removal has been observed to be the most effective part in the context of current multimodal therapy. The authors present their preliminary results of robotic assisted treatment of rectal carcinoma (RC). Method: The observed group includes 61 patients who underwent robotic assisted treatment for rectal cancer. The data were collected prospectively in the last 31 patients. Analyses were conducted on epidemiological data, perioperative outcomes, complications and oncological results. Results: Robotic assisted treatment of RC was performed in 61 patients: 34 men and 27 women, mean age of 62 years (33–80). Neoadjuvant oncological treatment was indicated in 46% of the patients. Average blood loss was 187 ml, transfusions were administered in three cases. Conversion to open procedure was performed 6 times, and 16 patients had postoperative complications. Anastomotic leak was observed in 10% of the patients, and 4 patients undewent surgical treatment. No patient died. Local recurrence of the cancer was diagnosed in 3 (5%) patients. The quality of mesorectal excision (ME) and the circumferential resection margin [(y)pCRM] have been determined in 27 patients since 2013. Positive (y)pCRM was recorded in two cases and incomplete ME was observed in 25.8% of the patients. Conclusion: Surgical treatment for RC is pivotal in multimodal therapy. Our preliminary results are similar to the conclusions in other published studies. The da Vinci robotic system is a safe manipulator in the treatment of RC and provides indisputable benefits to the surgeon when operating in the narrow pelvic space. However, the benefits of robotic treatment in abdominal surgery are yet to be evaluated in patients (with respect to long-term results, sufficient number of patients or a high EBM level of evidence). The high purchase price of the robotic device, individual instruments with equipment and non-systemic compensation constitute a significant hindrance that prevents wider use of the robotic system in the treatment of RC and other abdominal malignancies in the Czech Republic., and D. Úvod: Odběr ledvin z dárce s nezvratnou zástavou oběhu (DCD – donor after circulatory death) je důležitou součástí transplantačních programů v celosvětovém měřítku. V České republice byla první transplantace ledvin z DCD úspěšně provedena v roce 2002. Autoři diskutují vlastní a literární zkušenosti s tímto programem. Snahou je zvýšit potenciál odběru ledvin z DCD v České republice. Metoda: Od roku 2002 do roku 2015 bylo v plzeňském Transplantačním centru odebráno celkem 44 ledvin z DCD. Technikou byl „in situ“ odběr pomocí „double baloon triple lumen“ katetru při zachování 5–10 min. „no-touch“ intervalu. Metoda hypotermické pulzatilní perfuze byla použita k testování viability odebraných ledvin. Transplantováno bylo celkem 28 ledvin příjemcům o průměrném věku 51,1 roku (26–73 let). Dle maastrichtských kritérií bylo 16 (57,1 %) ledvin ve skupině II, 8 (28,6 %) ve skupině III a 4 (14,3 %) ve skupině IV. Výsledky: 30denní pooperační mortalita byla 0 a morbidita 10,7 tj. 14,3 % (N=4). Primární afunkce ledviny byla přítomna u 2 (7,1 %), opožděný nástup funkce pak u 5 (17,9 %) nemocných. Jeden rok, 5 a 10 let po transplantaci žije 100 %; resp. 86,4 % a 76,7% nemocných a ve stejných intervalech je funkčních 92,9; 69,6 a 61,9 % transplantovaných ledvin. Dlouhodobé výsledky jsou plně srovnatelné s transplantacemi ledvin od dárců se smrtí mozku. Závěr: DCD jsou významným zdrojem ledvin pro transplantace. Transplantace ledvin z DCD je metodou logisticky, ekonomicky a personálně náročnou s velmi dobrými dlouhodobými výsledky., Introduction: Kidney procurement from donors after circulatory death (DCD) is an important part of worldwide transplantation programmes. The first kidney transplantation from DCD was successfully performed in the Czech Republic in 2002. Method: Forty four kidneys from DCD were procured in the Transplant Centre of Pilsen between 2002 and 2015. We used the technique of “in situ“ procurement with the double balloon triple lumen catheter and 5−10 minutes of the no-touch interval. The method of pulsatile hypothermic perfusion was used to test the viability of the kidneys. Twenty eight recipients with mean age 51.1 (26−73) years were transplanted. Sixteen (57.1%) kidneys were from the 2nd, 8 (28.6%) from the 3rd and 4 (14.3%) from the 4th category according to the Maastricht criteria. Results: 30-day mortality and morbidity rates were 0 and 10.7% i. e.14.3% respectively (N=4). Primary non-function was presented in 2 (7.1%), and delayed graft function in 5 (17.9%) cases. One, five and ten years of recipient and graft survival rates were 100%, 86.4% and 76.7%; and 92.9%, 69.6% and 61.9%, respectively. The long-term results are fully comparable with kidneys transplanted from donors after brain death. Conclusion: DCD are an important source for kidney transplantation. BACKGROUND: Human immunodeficiency virus (HIV) continues to be a serious health issue and one of the world most devastating epidemics. An estimated 1.5 million people died from AIDS-related illnesses in 2013, and an estimated 37 million people with AIDS have died worldwide since the epidemic has begun. HIV infection is known for its oral manifestations which causes discomfort and pain for infected individuals. The objective of this study was to document oral conditions of HIV positive patients and the pattern and frequency of oral and dental lesions. METHODS: All patients with confirmed HIV infection who were treated at the Department of Dentistry, University Hospital in Hradec Králové, were examined. RESULTS: During the study period, 29 HIV positive patients were examined and treated--19 men, 10 women, with mean age of 32.9 years (range 22-58 years). 72.41% patients received ART. In total, all patients underwent 186 visits. The most frequent treatments were associated with teeth and periodontal lesions (71.80%), oral mucosal lesions were diagnosed and treated only in 3.96% cases. BACKGROUND: The goal of this prospective study was to determine the frequency of micrometastases in patients with squamous cell carcinoma (SCC) of the oral cavity, pharynx and larynx in whom elective neck dissection was indicated (cN0). PATIENTS AND METHODS: A total of 12 patients (10 males and 2 females) were enrolled in the study. The age ranged 42-73 years (median 62 years). Elective neck dissection was performed in all patients (8 ipsilateral, 4 bilateral) and a total of 256 lymph nodes were removed and sent for microscopic examination. RESULTS: The presence of tumor cells in cervical lymph nodes was found in 5/12 (42%) patients. Micrometastases of SCC were found in two patients and isolated tumor cells (ITC) in two other patients. In the remaining one patient with oropharyngeal SCC, a micrometastasis of papillary thyroid carcinoma (PTC) was detected. Positive lymph nodes were localized in level II in three patients with SCC of larynx, hypopharynx and tongue base, respectively, in level I in one patient with SCC of oral tongue and in level III in one patient with PTC. CONCLUSION: Our results indicate that SCC of head and neck has a high potential for creating micrometastases which frequency is higher compared to clinically detected macrometastases. AIM: The purpose of this study was to develop a revised version of the Brief Bedside Dysphagia Screening Test for determining penetration/aspiration risk in patients prone to dysphagia. The priority was to achieve high sensitivity and negative predictive value. METHODS: The study screeners conducted bedside assessment of the swallowing function in 157 patients with a neurological (mainly stroke) or an ear, nose, and throat diagnosis (mainly head and neck cancer). The results were compared with a gold standard, flexible endoscopic examination of swallowing. RESULTS: For the neurological subgroup (N = 106), eight statistically significant bedside assessment items were combined into the Brief Bedside Dysphagia Screening Test-Revised (BBDST-R). Cut-off score 1 produced the highest sensitivity (95.5%; 95% confidence interval CI [CI]: 84.9-98.7%) and negative predictive value (88.9%; 95% CI 67.2-96.9%). Úvod: Biliární ileus představuje 1−4 % mechanických obstrukcí gastrointestinálního traktu, ale u pacientů nad 65 let již 25 % nestrangulačních mechanických ileů tenkého střeva. Objemný kámen žlučníku obvykle vytvoří tlakovou nekrózu, protlačí se vzniklou biliodigestivní píštělí a obturuje gastrointestinální trakt. Kazuistiky: Mezi daty 1. 1. 2000 a 31. 1. 2015 bylo na Chirurgické klinice FN v Plzni operováno 35 pacientů pro poruchy pasáže s nálezem biliárního ileu. Průměrný věk pacientů v době operace byl 79 let (medián 78 let, min. 58 let, max. 92 let). Aerobilie na rentgenovém nativním snímku břicha byla přítomna z 27 pořízených snímků v 7 případech (26 %). Předoperačně byla provedena dvakrát kolonoskopie a devětkrát ezofagogastroduodenoskopie. Z operačních výkonů byla 21x provedena extrakce kamene z enterotomie, třikrát extrakce z gastrotomie, sedmkrát resekce tenkých kliček, jedenkrát Hartmannova resekce, jedenkrát resekce ileoascendentoanastomózy a jedenkrát extrakce konkrementu s následnou duodenojejunální anastomózou a s cholecystektomií v jedné době. Jedenkrát byla provedena ileotransverzoanastomóza. Pooperační 30denní mortalita byla 14,3 %. Pooperační morbidita dle klasifikace Clavien-Dindo činila u I. stupně 22,8 %, u II. stupně 5,7 %, u III. stupně 11,5 % a u IV. stupně 11,5 %. K recidivě biliárního ileu došlo u 8,8 % pacientů. Primární ošetření píštěle a cholecystektomie byla provedena v jednom případě. Závěr: Rozhodnutí o ošetření cholecystoenterální píštěle a cholecystektomii v první době bude vždy záležet na celkovém stavu pacienta a místním nálezu v oblasti žlučníku. Při ponechání píštěle je nutné počítat s rizikem recidivy ileu. Při operaci je nutné též zrevidovat celé střevo k vyloučení přítomnosti dalších konkrementů a snížit tak riziko časné recidivy. However, the rate of non-strangulated small bowel obstructions reaches as much as 25% in patients over 65 years of age. Usually, a pressure necrosis is created by a large gallstone that passes through the developed biliodigestive fistula, subsequently obturating the gastrointestinal tract. Case reports: 35 patients underwent a surgical procedure at the Department of Surgery, Teaching Hospital Pilsen for biliary ileus from January 1, 2000 to January 31, 2015. Mean age was 79 years (median 78 years; min. 58 years; max. 92 years). Aerobilia was visible in abdominal X-ray scans in 7 cases (26%) of 27 acquired images. Preoperatively, colonoscopy was done 2 times and esophagogastroduodenoscopy 9 times. Regarding surgical procedures, enterolithotomy was done in 21 cases, extraction of the gallstone from gastrotomy was done 3 times, small bowel resection 7 times, Hartmann resection 1 time, resection of ileoascendentoanastomosis 1 time, and extraction of the gallstone from duodenotomy with duodenojejunoanastomosis and cholecystectomy was done in 1 case. Ileotransversoanastomosis was performed in 1 case. Postoperative 30-day mortality was 14.3%. Postoperative morbidity according Clavien-Dindo was 22.8% for grade 1, 5.7% for grade 2, 11.5% for grade 3, and 11.5% for grade 4. A recurrence was observed in 8.8%. Primary treatment of the fistula and cholecystectomy were done in 1 case. Conclusion: The indication of primary biliodigestive fistula treatment always depends on the general condition of the patient and on the local finding in the gallbladder area. Residual fistula increases the risk of reccurence, but primary treatment is associated with higher mortality. Dna je zánětlivé, metabolicky podmíněné onemocnění indukované krystaly. Prevalence dny stále vzrůstá. V klinické praxi je často špatně diagnostikována a terapie především akutního záchvatu není adekvátní. V první části publikace jsou rozebírány diagnostické možnosti dnavé artritidy. Především je zdůrazněna výhodnost analýzy synoviálního výpotku a přímého průkazu krystalů v polarizačním mikroskopu. Při nedostupnosti materiálu na krystalografickou analýzu je nutné použít kombinaci klinických kritérií tak, jak např. uvádí ve svých doporučeních Evropská liga proti revmatizmu (EULAR). V druhé části je rozebírána terapie dny, která je rozdělena na období asymptomatické hyperurikemie, akutního dnavého záchvatu, interkritického období a chronické tofózní dny. Asymptomatickou hyperurikemii léčíme pouze při opakovaně vyšších urikemiích než 540 ?mol/l nebo při přítomnosti dalších rizikových faktorů a komorbidit. Pro léčbu akutního dnavého záchvatu je důležitější včasné zahájení léčby než výběr preparátu. Alternativou jsou NSA, kolchicin nebo glukokortikoidy. Nově registrovaným lékem pro léčbu refrakterního akutního zánětu je inhibitor IL1 kanakinumab. Při léčbě hyperurikemie se uplatňují režimová opatření, dietní opatření, abstinence a hypourikemická léčba. K dispozici jsou inhibitory xantinoxidázy alopurinol a febuxostat, který je vhodnější u pacientů se středně závažnou renální insuficiencí. Novým lékem pro léčbu těžké refrakterní tofózní dny je peglotikáza. In clinical practice it is frequently wrongly diagnosed and the therapy of acute attacks in particular is not adequate. The first part of the publication discusses diagnostic possibilities of gouty arthritis. First of all the advantage of the analysis of synovial exudate and of direct evidence of crystals in the polarization microscope is emphasized. If the material for crystallographic analysis is not available, it is necessary to use a combination of clinical criteria as specified e.g. in the recommendations of the European League Against Rheumatism (EULAR). The second part focuses on the therapy of gout which is divided into the periods of asymptomatic hyperuricemia, acute gouty attack, intercritical and chronic tophaceous gout. Asymptomatic hyperuricemia is only treated when uricemia greater than 540 ?mol/l occur repeatedly, or when other risk factors and comorbidities are present. In the treatment of acute gouty attack its early start is more important than the choice of a preparation. Alternatives are NSA, colchicine or glucocorticoids. A newly registered medicine for the treatment of refractory acute inflammation is the IL-1 inhibitor canakinumab. The treatment of hyperuricemia involves regimen and diet measures, abstinence and hypouricemic therapy. Available are the xanthine oxidase inhibitors, allopurinol and febuxostat; the latter is better suited for patients with moderate renal insufficiency. Tauopatie jsou neurodegenerativní onemocnění, která se vyznačují hromaděním abnormně modifikovaných forem proteinu zvaného tau především v kůře frontálních, temporálních a parietálních laloků, v bazálních gangliích a v mezencefalu. Molekulárně biologicky a biochemicky dobře definovaná skupina chorob má však dosti heterogenní klinické obrazy a je podstatou několika různých neurodegenerativních onemocnění tvořících frontotemporální lobární degenerace. Společným klinickým znakem těchto entit je ve větší či menší míře vyjádřený syndrom frontotemporální demence. Klinické jednotky, jež se řadí mezi klasické frontotemporální demence, však nejsou vždy podmíněny jen tauopatiemi, mnohdy je jejich etiopatogenetická podstata jiná. Účelem tohoto stručného přehledu je utřídění současné úrovně poznání vztahů mezi skupinou tauopatií a frontotemporálních lobárních degenerací. Článek obsahuje i návrh praktického postupu k upřesnění klinické diagnózy jednotlivých nozologických jednotek v rámci tauopatií., Tauopathies are neurodegenerative disorders characterized by accumulation of abnormally modified forms of the tau-protein, predominantly in frontal, temporal and parietal cortical regions, basal ganglia and in the midbrain. Tauopathies are well defined from the molecular biological and biochemical point of view; clinical symptoms may be, however, heterogeneous. Common signs of frontotemporal lobar degenerations include a more or less prominent syndrome of frontotemporal dementia. On the other hand, the clinical picture of frontotemporal dementia may not be caused by a tauopathy only; in many cases the underlying etiopathogenic cause is different. The aim of our review is to classify the relationship between tauopathies and frontotemporal lobar degenerations. This review contains a proposal of a practical approach to refining clinical diagnosis of the different tauopathies. Huntingtonova nemoc (HD) je neurodegenerativní porucha způsobená elongací CAG repetic v genu kódující protein huntingtin (Htt). U pacientů jsou v postižených tkáních přítomny vedle monomerní formy hlavně N‑koncové fragmenty, oligomery a polymery mutovaného huntingtinu (mtHtt), oproti tomu samotná monomerní forma mtHtt je exprimována v podstatě ve všech buňkách. Nejvíce postižené tkáně jsou bazální ganglia a mozková kůra. V této studii jsme analyzovali přítomnost N‑koncových fragmentů a oligomerů Htt v různých tkáních 24- a 36měsíčních transgenních (TgHD) miniprasat exprimujících N‑koncovou část lidského mutovaného huntingtinu a jejich zdravých sourozenců. Zjistili jsme, že mozková kůra a varlata na rozdíl od svalu a srdce TgHD miniprasat obsahují kromě monomerní formy i N‑koncové fragmenty a oligomerní smíry. Ve svalech z 36 měsíčních TgHD miniprasat však již začíná mírná fragmentace. Tato zjištění napodobují časnou progresi onemocnění u lidí, a proto miniprase poskytuje slibný model pro terapeutické testování HD. Redakční rada potvrzuje, že rukopis práce splnil ICMJE kritéria pro publikace zasílané do biomedicínských časopisů., Huntington’s disease (HD) is a neurodegenerative disorder caused by the the elongation of CAG triplet repeat in the gene encoding the huntingtin protein (Htt). In patients, in addition to the monomeric form of huntingtin, N‑terminal fragments, oligomers, and polymers are present mostly in the affected tissues, even though the mutated huntingtin (mtHtt) is expressed basically in all cells. The most affected tissues are basal ganglia and cerebral cortex. In this study we analyzed the presence of N‑terminal fragments and oligomers of Htt in different tissues of 24 and 36 months old experimental animals. This was done in our large animal model of HD, which uses transgenic (TgHD) minipigs expressing N‑terminal part of human mtHtt. Among all the tissues tested, we found cortex and testes to contain N‑terminal fragments as well as oligomeric smears in TgHD minipigs compared to wild type siblings. On the other hand, we did not detect any fragments or oligomers in muscle and heart of TgHD minipigs, only starting fragmentation in muscles of 36 months old animals. In this paper a longitudinal study is presented of age-dependent speech acoustic characteristics from the utterances of Czech actresses, acquired from movie databases recorded in the years from 1985 to 2014. The age dependence of phonation is analyzed using the parameters fundame ntal frequency, jitter, and shimmer. For assessment of articulation, the formant-based voice area index of vocals and the length of the burst from apalatal stop consonant isused. For prosody, no characteristics were found that were applicable to the analysis of film archives. The results of age dependencies described by the phonetic categories are consistent with the assumptions and conclusions presented in the literature.
