Akutní poškození ledvin je obecně definováno jako náhlé zhoršení funkce ledvin podmíněné poklesem glomerulární filtrace a je diagnostikováno hromaděním konečných produktů metabolizmu dusíku, sníženým výdejem moči nebo obojím. Akutní poškození ledvin je důsledkem řady onemocnění, které mohou akutně poškodit funkce ledvin. Jde o častou komplikaci hospitalizace a je velmi časté i u kriticky nemocných pacientů jako důsledek jiných závažných onemocnění. S včasnou diagnózou mohou pomoci nové diagnostické možnosti včetně renálních biomarkerů. Žádná specifická terapie akutního poškození, která by zmírnila tíži poškození anebo urychlila reparaci funkce ledvin, dosud neexistuje. Léčba nadále zůstává symptomatická. K náhradě funkce ledvin pomocí dialýzy přistupujeme v případě těžkého renálního poškození spojeného s nárůstem cirkulujícího volumu, biochemickými změnami a uremickými komplikacemi. Obvykle dojde k reparaci renálních funkcí a pacient není závislý na dialyzační léčbě. Pacienti po prodělaném akutním poškození ledvin mají zvýšené riziko chronického onemocnění ledvin. Obecně je akutní poškození ledvin spojeno se závažnými a dalekosáhlými důsledky nejen pro nemocného (nutnost dialýzy, zvýšení morbidity a mortality), ale i s významnými ekonomickými dopady pro zdravotní systém., Acute kidney injury is generally defined as a condition characterised by the rapid loss of the kidney's excretory function with a decline of the glomerular filtration rate and is typically diagnosed by the accumulation of the end products of nitrogen metabolism or decreased urine output, or both. It is the clinical manifestation of a number of disorders that can affect the kidney acutely. Acute kidney injury is common in hospital patients and very common in critically ill patients, and is often secondary to extrarenal causes. New diagnostic techniques including renal biomarkers may help with early diagnosis. No specific therapies have emerged that can attenuate acute kidney injury or expedite recovery; thus, treatment is supportive. Patients are given renal replacement therapy if acute kidney injury is severe and biochemical or volume‑related, or if uraemic‑toxaemia‑related complications are of concern. Patients typically recover to dialysis independence. However, evidence suggests that patients who have had acute kidney injury are at increased risk of subsequent chronic kidney disease. K diagnostice chronické choroby ledvin je v současnosti doporučováno stanovení odhadu glomerulární filtrace (eGFR). Příslušná doporučení vznikla z iniciativy nefrologů v rámci činnosti Národního edukačního programu pro ledvinové choroby NKDEP USA. K stanovení eGFR se doporučuje používat výpočtu podle vztahu MDRD (Modification of Diet in Renal Disease). Překážkou dosažení spolehlivých a srovnatelných hodnot eGFR je nedostatečná srovnatelnost výsledků různých metod měření sérového kreatininu. Klíčovým problémem je vysoká a u různých měřicích systémů různá hodnota pozitivního bias u hodnot kreatininu v rozmezí referenčního intervalu a hodnot mírně zvýšených nad jeho horním referenčním limitem (do 133 μmol/l). Tento fakt byl opakovaně potvrzen velkými a sofistikovanými mezilaboratorními studiemi národními i mezinárodními. Důsledkem této skutečnosti je velká závislost hodnot eGFR na použitých metodách měření sérového kreatininu. Návrh řešení spočívá v rekalibraci diagnostických kitů pomocí referenční metody ID-MS a ve zlepšení výsledků analýz v oblastech nízkých a lehce zvýšených koncentrací kreatininu. Předpokládá se nezbytnost spolupráce klinických laboratoří, referenčních pracovišť, organizátorů EHK a v neposlední řadě také výrobců IVD. Cílem procesu je dosažení přesnosti měření ≤ 5 % a nejistoty ≤ 10 % pro koncentrace blízké 88 μmol/l. Dalším cílem je dosáhnout obecného používání formule MDRD. Tato změna musí být provázena revizí příslušných referenčních intervalů. Pro organizátory programů EHK vyplývá z doporučení NKDEP aspoň občasné používání nativních kontrolních materiálů o koncentracích do 133 μmol/l s cílovými hodnotami, certifikovanými referenční metodou ID-MS., Calculation of the estimate glomerular filtration rate (eGFR) by using the MDRD (Modification of Diet Renal Disease) equation is recently recommended for diagnosis of chronic kidney disease. National Kidney Disease Education Program summarizes these recommendations. The lack of comparability among different methods for creatinine measurement is main problem for effective implementation the MDRD formula into clinical laboratories. Significant and different bias in serum creatinine measurements was several times confirmed by large interlaboratory comparison studies. The largest bias values are typical for the crucial concentrations for eGFR calculation. It means for concentrations lower than 133 μmol/l. Recalibration of all routine methods of creatinine measurement by means of ID-MS reference method is proposed as solution for this problem Cooperation of clinical laboratories, EQA providers and IVD manufacturers is necessary. Reaching the precision ≤ 5% and total error (measurement uncertainty) ≤ 10% at creatinine concentration about 88 μmol/l is recommended as the aim of process. Úvod: Furosemid je kličkové diuretikum používané při stavech retence tekutin. Nutnost podání vysokých dávek je důsledkem jeho snížené účinnosti způsobené nižší dosaženou koncentrací na místě účinku v lumen ledvinného tubulu a vlivem adaptačních mechanizmů. Vysoké dávky jsou spojovány s rozvojem iontové dysbalance, přímé toxicity a intravaskulární fluktuací objemu. Možnosti ovlivnění účinnosti a bezpečnosti furosemidu jsou v literatuře rozporuplně hodnoceny v souvislosti se způsobem jeho podávání (intermitentně vs kontinuálně). Cíl: Cílem této práce je analyzovat dostupná literární data týkající se zhodnocení účinnosti a bezpečnosti intermitentních vs kontinuálních dávkových režimů. Metodika: Systematická literární rešerše v databázi PubMed od roku 1990 do roku 2013 s použitím klíčových slov – furosemid, kličkové diuretikum, bolusové podání, kontinuální infuze, účinnost, bezpečnost, srdeční selhání, intenzivní péče, ICU. Závěr: Farmakokinetické a farmakodynamické znalosti furosemidu vytvářejí určitý teoretický předpoklad pro preferenci kontinuálních infuzí před intermitentními bolusy. Zhodnocení dostupných literárních dat však zatím v klinické praxi neprokázalo jednoznačné výhody jednoho způsobu podávání oproti druhému. The need for high doses is due to its reduced efficacy caused by lower concentration of furosemide achieved at the site of action in the renal tubule lumen and adaptation mechanisms. High doses have been associated with the development of ionic dysbalance, direct toxicity and intravascular volume fluctuations. The way of furosemide administration (intermitent versus continuously) to influence efficacy and safety is contradictory evaluated in EBM. Aim: The aim of this study is to analyze the available data for evaluation of the efficacy and safety of intermittent versus continuous dose regimens. Methods: A systematic search on PubMed from 1990 to 2013 using the keywords – furosemide, loop diuretic, bolus, continuous infusion, efficacy, safety, heart failure, ICU, critical care. Conclusion: The pharmacokinetic and pharmacodynamic knowledge of furosemide create a theoretical assumption for the preference of continuous infusions before intermittent boluses. Assessement of available studies, however, yet in clinical practice did not proof the advantage of one over the other route of administration. Background Patent ductus arteriosus (PDA) is common in very premature infants. Pharmacological closure of PDA with indomethacin, a prostaglandin inhibitor, has remained the mainstay of treatment in premature infants over the last three decades. Intravenous ibuprofen was recently shown to be as effective and to have fewer adverse reaction in preterm infants. If equally effective, then oral ibuprofen for PDA closure would have several important advantages over the intravenous route. This study was designed to assess the efficacy and safety of oral ibuprofen and intravenous ibuprofen for the early pharmacological treatment of PDA in LBW preterm infants with respiratory distress syndrome. Methods A randomized, single-blinded, controlled study was performed on premature neonates at the neonatal care unit of the University Hospital for Obstetrics and Gynecology”Koco Gliozheni”, Tirana, Albania, from January 2010 to December 2012. The study enrolled 68 preterm infants with gestational age between 28-32 weeks, birth weight ≤ 2000 g, postnatal age 48-96 h, and had echocardiographically confirmed significant PDA. The preterm infants received either intravenous or oral ibuprofen randomly as an initial dose of 10 mg/kg, followed by 5 mg/kg at 24 and 48 h. After the first dose of treatment in both groups, echocardiographic evaluation was performed, to determine the need for a second or third dose. The rate of ductal closure, adverse effects, complications, and the patient’s clinical course were recorded. Results All patients were born after 28 until 32 weeks’ gestation. 36 patients were treated with oral ibuprofen and 32 with intravenous ibuprofen in this period. After the first course of the treatment, the PDA closed in 30 (83.3%) of the patients assigned to the oral ibuprofen group versus 23 (71.8%) of those enrolled in the intravenous ibuprofen group (p = 0.355). There was no difference between treatment groups in demographics or baseline renal function. In the evaluation of renal tolerance, none of the patients had oliguria. There were no significant differences with respect to complications during the stay. Conclusions In low birth weight infants, the rate of early ductal closure with oral ibuprofen is at least as good as with the intravenous route. Serum or plasma creatinine level determination has been serving for decades as the most commonly used marker of renal function. However, creatinine levels are affected by a number of other factors not related to renal function, particularly age, gender, muscle mass, starvation, wasting diseases, post surgical states, and activity or exertion. Alternatives for the assessment of renal function or glomerular fi ltration rate (GFR), through measurement of the clearance of exogenous substances like, inulin, 51Cr-ethylendiaminotetraacetate (51Cr-EDTA), 99mTc-diethylenetriaminepentaacetic acid or iohexol are more accurate, but too complex, laborious, and not suitable for routine clinical use. Therefore, serum creatinine measurement yet remains the most common method to estimate renal function in routine clinical practice. In the presented case study, nearly 50% decline in serum creatinine level as measured by enzymatic colorimetry method according to the manufacture’s recommendation is described. Relatively rapid serum creatinine concentration decline from 67 µmol/L baseline value to 30 µmol/L within 4 days in a CF patient post bilateral lung transplantation, who, was on courses of essential immunosuppressive, antimycotic, and antibacterial drugs including an aminoglycoside (amikacin) was observed. The change in serum creatinine level within four consecutive days after bilateral lung transplantation (mean difference, -49.25 % µmol/L; with 95% confi dence interval, -29 to -37 µmol/L) was signifi cant. Herein, the event is not essentially refl ecting better renal function, since amikacin trough level was unacceptably high (20 mg/L) at the same time, rather explaining probably low production of creatinine as a result of underlying pathology in cystic fi brosis. Patient related explanations and existing methodological challenges have also been discussed., Hladina sérového nebo plazmatického kreatininu slouží již desítky let jako nejčastější používaný ukazatel renální funkce. Přesto je koncentrace sérového kreatininu ovlivněna jinými, s funkcí ledvin nesouvisejícími faktory (věk, pohlaví, svalová hmota, stav výživy, nemoci poškozující organismus, pooperační stavy, tělesná aktivita nebo vyčerpávající fyzický výkon). Alternativy pro zjištění renální funkce/gromerulární fi ltrace (GFR) pomocí podání exogenních látek (inulin, 51Cr-ethylendiaminotetraacetát (51Cr-EDTA), 99mTc-diethylenetriaminepentaacetát nebo iohexol) jsou přesnější, ale zároveň velmi složité, pracné a nehodí se pro rutinní praxi. Měření sérového kreatininu proto stále zůstává běžnou metodou k odhadu renální funkce v rutinní klinické praxi. V tomto sdělení popisujeme cca 50% pokles sérového kreatininu měřeného enzymatickou metodou s kolorimetrickou detekcí podle návodu výrobce. Relativně rychlý pokles sérového kreatinu z 67 µmol/l na 30 µmol/l v průběhu 4 dnů byl pozorován u pacientky s cystickou fi brózou po oboustranné transplantaci plic, s náležitou imunosupresivní, antimykotickou i antibakteriální léčbou včetně aminoglykosidů (amikacin). Změny v hladině sérového kreatininu během 4 dnů (průměrný rozdíl 49, 25 %, při 95% intervalu spolehlivosti, -29 až -37 µmol/l) jsou významné. Tento jev přesto nemusel být odrazem lepší clearance vzhledem k nepřijatelně vysoké „trough“ hladině amikacinu a nejspíše poukazuje na možné snížení produkce kreatininu v souvislosti s cystickou fi brózou. Rychle progredující glomerulonefritidy (RPGN) jsou nejzávažnější formou nefritického syndromu. Bez terapie dochází k ireverzibilní ztrátě funkce ledvin v řádu dnů až týdnů. RPGN se obvykle manifestuje proteinurií pod 3 g/d, hematurií s dysmorfními erytrocyty a často erytrocytárními válci. RPGN se může vyskytovat samostatně nebo jako součást systémového onemocnění. Specifickým nálezem v renální biopsii je postižení více než 70 % glomerulů srpky. Nejčastějšími stavy projevujícími se jako RPGN jsou ANCA‑asociované vaskulitidy, antirenální nefritida, méně často se vyskytují imunokomplexové glomerulonefritidy. Základem terapie jsou glukokortikoidy, cyklofosfamid a novější imunosupresiva, nejnověji i biologická léčba (rituximab), někdy v kombinaci s plazmaferézou., Rapidly progressive glomerulonephritides (RPGN) are the most severe form of the nephritic syndromes. In this case, glomerular injury is so acute that irreversible renal failure develops over the course of a few days to weeks. RPGN usually presents as proteinuria < 3g/day, hematuria with dysmorphic red blood cells and/or red cell casts, with or without signs of systemic vasculitis. A specific finding upon kidney biopsy is a crescent formation in more than 70% of glomeruli. The most common diseases presenting as RPGN are ANCA‑associated vasculitides, Goodpasture’s disease (anti‑renal nephritis), and the much less common immunocomplex glomerulonephritides. Treatment includes glucocorticoids, cyclophosphamide and novel therapies including targeted treatment (rituximab), sometimes in combination with plasma exchange., and Zakiyanov O., Vachek J., Tesař V.
