The aim of this study was to determine the prevalence of musculoskeletal problems among Czech dentists and to analyze the factors that affect these disorders. Information was gathered through questionnaire completed by 581 physicians. The questionnaire ascertained general information about physicians including their work habits and the characteristics of their work environment along with the occurrence of musculoskeletal problems as well as their intensity. In the past year the occurrence of at least mild difficulties associated with the locomotive system was reported by 96.9% of dentists surveyed (n = 557), with 66.3% of respondents (n = 381) reporting difficulties ofa moderate or major nature. Most respondents of both sexes indicated neck pain. A statistically significant correlation with the occurrence of musculoskeletal complaints of medium and major intensity was demonstrated for the following factors: sex, age, running a private practice, past injury or musculoskeletal diseases, and the perception of work as psychologically demanding. Musculoskeletal disorders in dentists in Czech Republic are relatively frequent and serious health problem. The causes of these diseases must be identified and appropriate preventive interventions undertaken that will contribute to a reduction in the incidence of these problems. and Z. Sustová, L. Cardiac surgery is inseparably linked to the activation of innate immunity cells recognizing danger signals of both endogenous and exogenous origin via pattern recognition receptors such as TLR receptors. Therefore, we followed by flow cytometry TLR2 and TLR4 expression on blood monocytes and granulocytes of patients who underwent coronary artery bypass grafting using beating heart surgery (off-pump, n = 34), with use of standard cardiopulmonary bypass (CPB), (on-pump, n = 30), and miniinvasive CPB (mini on-pump, n = 25), respectively, before, during surgery, and up to 7th postoperative day. TLR2 and TLR4 expression both on monocytes and granulocytes was significantly diminished already at the end of CPB being highly significantly decreased at the end of surgery in all patients' groups. TLR2 and TLR4 expression reached preoperative value at the 1st postoperative day being significantly higher at the 3rd postoperative day. Using intracellular staining we found the peak of TLR2 and TLR4 expression inside of monocytes and granulocytes at the first postoperative day in a subgroup of on-pump patients. In conclusion, TLR2 and TLR4 expression is significantly modulated in patients undergoing coronary artery bypass grafting as a part of adaptive homeostatic mechanisms induced by major surgery. The very surgical trauma is responsible for TLR2 and TLR4 modulation. Surprisingly, cardiopulmonary bypass itself was little contributing to the modulation of TLR2 and TLR4 expression. and J. The aim of this study is to evaluate the results of total hip arthroplasty in patients with Parkinson's disease during a period of five years, focusing on the assessment of the risks and benefits of surgery. During this period we performed total hip arthroplasty in 14 patients (15 hips) with Parkinson's disease. Patients were evaluated by subjective symptoms and objective findings, with a focus on the use of support while walking and walking distance, severity of Parkinson's disease before surgery and at the time of the last follow-up. During the postoperative period, the following parameters were assessed: length of ICU stay, mobilization, complications, the total duration of hospitalization and follow-up care after discharge. Of the 11 patients (12 hips) followed-up 1-5 years with an average of 3 years after operation 8 cases showed progression of neurological disability. 5 patients (6 hips) showed an increased dependence on the use of support when walking and reduced distance that the patient was able to walk. Subjectively, 10 hip joints were completely painless and 2 patients complained of only occasional mild pain in the operated hip. Complications that were encountered were urinary tract infection (5 patients), cognitive impairment (3 patients) and pressure ulcer (2 patients). We did not observe any infection or dislocation of the prosthesis. Three patients fell and fractured the femur and 3 patients in our cohort died during follow up. Implantation of total replacement is possible with judicious indication after careful evaluation of neurological finding in patients with minimal or mild functional impairment of the locomotor system. The pilot study deals with the clinical results of the regenerative therapeutical method in intraosseous periodontal defects done by surgical augmentation procedure using a material of a new generation prepared on the basis of a bioactive glass. A group of 10 individuals were treated properly for chronic periodontitis. Some of treated infraalveolar periodontal lesions, both periodontal pockets and interradicular defects, persisted or recurred in posterior teeth. In order to eliminate them they were indicated for advanced periodontal surgery or regenerative surgical therapy stimulating healing processes of supportive tissues, mostly alveolar bone around treated teeth. Relevant clinical parameters, i. e. values of the pocket depth, gingival recession, and loss of attachment were evaluated before and after the periodontal surgery and compared with each other. Pozadie: Článok sa zaoberá vybranými aspektmi ústretovosti českého zdravotníckeho systému z pohľadu slovenských imigrantov so zameraním na dostupnosť zdravotnej starostlivosti a komunikáciu medzi pacientom a lekárom. Metódy: Kvalitatívna štúdia bola realizovaná prostredníctvom rozhovorov u 20 Slovákov žijúcich v Českej republike. Výsledky: Najväčšie prekážky pri čerpaní zdravotnej starostlivosti vidia respondenti v nedostatočnom počte registračných miest u lekárov primárnej starostlivosti. Naopak respondenti boli spokojní s prístupom českých lekárov. Závery: Pravdepodobným dôvodom ťažkostí slovenských imigrantov pri registrácii u lekárov primárnej starostlivosti je pracovná preťaženosť lekárov., Background: The article deals with selected aspects of health system responsiveness from the perspective of Slovak immigrants in the Czech Republic. It focuses on access to health care and communication between patient and physician. Methods: A qualitative study was conducted through interviews with 20 Slovaks living in the Czech Republic. Results: The biggest obstacles to health care receiving consist, according to respondents, in inadequate number of registration places at primary care physicians. However, respondents were satisfied with physician`s approach in the Czech Republic. Neexistuje jedna obecná defi nice seniorské křehkosti. Někteří autoři ji popisují jako symptom, jiní jako komplexní syndrom, nebo jako nahromadění funkčních defi citů u seniora. Dle odborných publikací se křehkost vyskytuje až u 25 procent seniorů (Holmerová, 2007, s. 100). Rozvoj seniorské křehkosti může být mimo jiné podmíněn multimorbiditou, nežádoucími účinky léků, pohybovou inaktivitou, malnutricí, osamělostí či depresí (Kalvach, 2008, s. 122). Cílem práce bylo vyhledání a analýza odborných zahraničních článků týkající se křehkosti seniorů a následně získané poznatky využít v diagnostice NANDA International. Pro získání relevantních dat byly využity elektronické databáze Pubmed, Medline, ProQuest a Springer a vyhledávač Google. Byla využita specifi cká vyhledávací strategie. Na základě diagnostického algoritmu dle NANDA International byly identifi kovány diagnostické prvky a stanoveny ošetřovatelské diagnózy. Ošetřovatelská diagnostika dle NANDA International se jeví jako vhodná při poskytování ošetřovatelské péče u křehkých seniorů., There is no general definition of frailty in the elderly. Some authors describe it as a symptom, others as a complex syndrome, or as an accumulation of functional defi cits in older adults. According to professional publications, frailty occurs in up to 25 percent of the elderly (Holmerová, 2007, p. 100). Developing of frailty in older adults may be subject to multimorbidity, undesired eff ects of medication, physical inactivity, malnutrition, loneliness or depression (Kalvach, 2008, p. 122). The aim of the article is to search for and analyse foreign scientifi c articles related to frailty in the elderly and to use gained knowledge in NANDA International diagnostics. To obtain the relevant data, electronic databases Pubmed, Medline, ProQuest, Springer and Google were used. Published articles concerning frailty in the elderly were found by a specifi c search strategy. Diagnostic elements were identifi ed and nursing diagnoses were determined according to the diagnostic algorithm by NANDA International. Melanocytární léze vykazují značnou morfologickou heterogenitu. Jejich diagnostika je náročná i přes současné využití moderních histopatologických a imunohistochemických metod. Významným přínosem v diagnostice melanocytárních lézí je zavedení molekulárně genetické metody fluorescenční in situ hybridizace s použitím čtyřbarevné sondy značící geny CCND1, RREB1, MYB a centromeru chromozomu 6. V letech 2008–2012 bylo v naší laboratoři úspěšně metodou fluorescenční in situ hybridizace vyšetřeno 160 vzorků různých typů melanocytárních lézí, 44 névů a 116 melanomů. Změny asociované s diagnózou melanomu byly prokázány u 116 vzorků (72,5 %), normální nález u 41 vzorků (25,6 %), ve dvou případech bylo rozhodnutí na hranici hodnotitelnosti metody (1,2 %) a v jednom případě se jednalo o vyšetření recidivy melanomu v jizvě, který byl svým nálezem atypický (monozomie chromozomu 6) (0,6 %). Na základě dosažených výsledků byla stanovena senzitivita metody 96,6 % a specifita metody 91%. Tyto výsledky ukazují, že fluorescenční in situ hybridizace je vhodnou metodou pro zajištění přesné a rychlé diagnostiky nejednoznačných melanocytárních lézí. Právě rychlost a správnost diagnózy je u vysoce maligních melanomů zásadní a má neoddiskutovatelný vliv na prognózu a úspěšnost léčby pacienta., Melanocytic lesions exhibit significant morphological heterogenity, making their diagnosis difficult, although both sophisticated histopathological and immunohistochemical methods are used. The introduction of molecular genetic method flourescence in situ hybridization (FISH) has contributed significantly to the diagnosing of melanocytic lesions. Between 2008 and 2012, we studied a total of 160 samples of different types of melanocytic lesions, including 44 nevi and 116 melanomas, using a four-color probe mix for CCND1, RREB1 and MYB genes and for the centromere of chromosome 6. A positive result was found in 116 melenoma samples (72.5 %) and a negative result in 41 samples (25.6 %). A borderline value was found in two samples (1.2 %), and an atypical finding (monosomy of chromosome 6) was seen in a melanoma recurrence in one case (0.6 %). Sensitivity and specificity were 96.6 % and 91 %, respectively. These results show that FISH is a suitable and relatively low-cost method for accurate, rapid diagnosis of ambiguous melanocytic lesions. Cíl: Přesnost screeningových škál pro vyhledávání pacientů s rizikem vzniku dekubitů byla u nás dosud zkoumána jen okrajově. Cílem bylo navázat na pilotáž zabývající se inter-rater reliabilitou (shodou mezi posuzovateli) Škály Bradenové (ŠB) a zjistit shodu mezi dvěma posuzovateli u většího vzorku respondentů. Metodika: Dva posuzovatelé nezávisle na sobě hodnotili 60 pacientů hospitalizovaných s neurologickým onemocněním pomocí ŠB. Celková skóre (max. 23 bodů) byla dichotomizována (riziko versus absence rizika) dle doporučeného cut-off skóre (18 bodů). Inter-rater reliabilita byla vyjádřena pomocí koeficientu kappa (κ), a to pro dichotomizovaná celková skóre i pro skóre v jednotlivých položkách škály. Výsledky: Koeficient κ byl pro dichotomizovaná celková skóre 0,564 (95% konfidenční interval [KI] = 0,182; 0,946). V jednotlivých položkách škály byl κ následující: „smyslové vnímání“ 0,557 (95% KI = 0,306; 0,808); „vlhkost“ 0,454 (95% KI = 0,142; 0,766); „aktivita“ 0,613 (95% KI = 0,425; 0,801); „mobilita“ 0,091 (95% KI = – 0,127; 0,309); „výživa“ 0,283 (95% KI = 0,095; 0,471); a „tření a střih“ 0,554 (95% KI = 0,240; 0,868). Závěr: Inter-rater reliabilita dichotomizovaných celkových skóre byla střední úrovně (κ = 0,564). V položce „aktivita“ lze shodu mezi posuzovateli považovat za poměrně dobrou (κ = 0,613); v ostatních položkách však byla nižší. Jsou prezentovány faktory, které mohly tyto výsledky ovlivnit., Aim: The accuracy of the screening scales for identification of patients who are at risk of developing pressure sores has, so far, received only marginal attention in the Czech Republic. The aim was to extend a pilot study that focused on inter-rater reliability of the Braden Scale and to determine inter-rater reliability for a larger patient sample. Methods: Two raters performed independent assessments of 60 patients hospitalized with a neurological disease, using the Braden Scale. Total scores (max. 23 points) were dichotomized (at risk versus not at risk) using the recommended cut-off score (18 points). Using the kappa (κ) coefficient, inter-rater reliability was expressed for the dichotomized total scores as well as for scores obtained for each category of the scale. Results: For the dichotomized total results, the κ coefficient was 0.564 (95% confidence interval [CI] = 0.182; 0.946). For the individual categories, the κ coefficient was as follows: “sensory perception” 0.557 (95% CI = 0.306; 0.808); „moisture“ 0.454 (95% CI = 0.142; 0.766); „activity“ 0.613 (95% CI = 0.425; 0.801); „mobility“ 0.091 (95% CI = – 0.127; 0.309); „nutrition“ 0.283 (95% CI = 0.095; 0.471); and „friction and shear“ 0.554 (95% CI = 0.240; 0.868). Conclusion: For the dichotomized total scores, moderate inter-rater reliability (κ = 0.564) was obtained. Cíle studie: 1. Seznámení biochemické obce s využitím mikrodialýzy v neurochirurgii, resp. v neurointenzivní péči. 2. Před - stavení hypotézy o kreatinfosfátu coby alternativním zdroji energie pro mozkovou tkáň. Typ studie : Přehledná práce Materiál a metody: Sběr vzorků extracelulární tekutiny umožňuje ve standardním protokolu vyšetřit metabolity glukózové - ho metabolismu s cílem včasného záchytu rozvíjející se ischemie mozku. Mikrodialýzu využíváme převážně u pacientů po kraniocerebrálním poranění či subarachnoidálním krvácení z prasklé mozkové výdutě. Nicméně v odebraných vzorcích je možno vyšetřit rozsáhlou škálu dalších metabolitů a biologicky aktivních látek a rozšířit tak naše poznání o procesech pro - bíhajících v mozku. Výsledky: Shrnujeme poznatky o mikrodialýze v neurointenzivní péči neurochirurgických pacientů, možnosti vlivu na aktuální terapii stejně jako význam prediktivní. V naší experimentální práci se zaměřujeme na metabolismus kreatinfosfátu, který dle našich prvních pozorování může být klíčovou energetickou alternativou kyslíkového metabolismu, jak demonstrujeme na dvou kazuistikách. Závěr: Mikrodialýza nabízí možnost přímé, průběžné monitorace metabolismu mozku v běžné klinické praxi. Standardní diagnostiku lze rozšířit o spektrum dalších metabolitů a biologicky aktivních látek. Design: Review article Methods: Collection of samples of the extracellular fluid enables examination of metabolites of the glucose oxidative me - tabolism with the aim of detecting early signs of brain ischemia. Microdialysis is used predominantly in patients with brain injury and subarachnoid hemorrhage after aneurysm rupture. Nonetheless, the samples may be examined for a wide scale of metabolites and biological active substances and extend our knowledge of brain metabolism. Results: We summarize the current knowledge on the use of microdialysis in neurointensive care of neurosurgical patients, its influence on the therapy and its predictive meaning. In the experimental work we focused on the metabolism of creatine phosphate, which, according to our first observations, can represent alternative energy to glucose oxidative metabolism, as demonstrated on 2 case reports. Conclusions: Microdialysis provides direct and continuous online monitoring of brain metabolism in everyday clinical prac - tice. Cíl: Cílem výzkumu bylo zjistit vztah reminiscenčních aktivit (RA) a depresivity u seniorů žijících v institucionální péči. Sledována byla míra depresivity u seniorů starších 65 let. Byl sledován rozdíl mezi skupinou experimentální a kontrolní po 50 - ti seniorech. Metodika: Pro sběr dat a vymezení depresivity respondentů byl použit screeningový nástroj Škála deprese pro geriatrické pacienty (GDS). Studiem zdravotní dokumentace byla sbírána data, jako je věk, výsledky kognitivního testu MMSE a data z oblasti psychofarmakologické medikace, jelikož je to významný faktor ovlivňující míru depresivity. Hodnocení depresivity proběhlo ve dvou měřeních. U experimentální skupiny proběhla měření před a po absolvování strukturovaných RA. U kontrolní skupiny byla měření provedena ve stejném časovém období, jako ve skupině experimentální, ale již bez RA. Výsledky: Prevalence depresivity v celém souboru respondentů byla 43 %. Depresivita v experimentální skupině byla prokázána u 20 % respondentů. Ve stejném časovém období u skupiny kontrolní byla zjištěna depresivita u 23 % respondentů. Po absolvování RA klesla depresivita u experimentální skupiny o 4 %. Ve zkoumaném souboru bylo statisticky potvrzeno, že RA snižují míru depresivity u seniorů v institucionální péči. Přítomnost depresivní symptomatologie po absolvování RA u experimentální skupiny byla nižší než u skupiny kontrolní. Závěr: Deprese je vážným a život ohrožujícím onemocněním, které se podílí na snížení kvality života seniorů a jejich rodin. Správná diagnostika a léčba deprese vede ke zlepšení kvality života seniorů a jejich rodin. RA jsou významnou psychosociální nefarmakologickou intervencí, která může výrazně přispět ke zlepšení kvality života., Aim: The aim of this research was to determine the relationship between reminiscence activities (RAs) and depression in seniors living in institutional care. The rate of depression was studied in seniors over 65 years of age. Differences between experimental and control groups, each comprising 50 individuals, were assessed. Methods: The Geriatric Depression Scale was used for data collection and identification of depression in the participants. Medical records were used to ascertain their age, Mini Mental State Examination results and psychopharmacological drug use, a significant factor affecting the rate of depression. Depression was evaluated by two measurements. In the experimental group, the measurements were carried out before and after completion of structured RAs. In the control group, the measurements were carried out at the same times but no RAs were provided. Results: The overall prevalence of depression in all participants was 43%. In the experimental and control groups, the rates were 20% and 23%, respectively. After completion of RAs, the rate decreased by 4% in the experimental group. RAs were confirmed to lower the rates of depression in seniors in institutional care. Depressive symptoms were less frequent after RAs in the experimental group than in the controls. Conclusion: Depression is a serious and lifethreatening disease contributing to impaired quality of life of both seniors and their families. Correct diagnosis and treatment of depression lead to improved quality of life of seniors and their families.
