We measured plasma concentrations, adipose tissue and placental mRNA expression of hepatokines fetuin A, fetuin B and fibroblast growth factor 21 (FGF21) in 12 healthy pregnant women (P group), 12 pregnant women with gestational diabetes (GDM) and 10 healthy non-pregnant women (N group) to explore their potential role in the etiopathogenesis of GDM. GDM and P group had comparable BMI, C-reactive protein (CRP) and glycated hemoglobin levels while IL-10 and TNF-α levels were higher in GDM group. Fetuin A and fetuin B levels were higher in pregnancy as compared to N group and decreased after delivery with no apparent influence of GDM. In contrast, the pattern of changes of circulating FGF21 levels differed between GDM and P group. Fetuin A concentrations positively correlated with CRP, TNF-α mRNA expression in adipose tissue and IL-6 mRNA expression in placenta. Fetuin B positively correlated with CRP. FGF21 levels correlated positively with IFN-γ mRNA in adipose tissue and inversely with IL-8 mRNA in the placenta. Taken together, fetuin A and fetuin B levels were increased during pregnancy regardless of the presence of GDM. Smoking during pregnancy presents health risks for both the mother and her child. In this study we followed changes in the production of steroid hormones in pregnant smokers. We focused on changes in steroidogenesis in the blood of mothers in their 37th week of pregnancy and in mixed cord blood from their newborns. The study included 88 healthy women with physiological pregnancies (17 active smokers and 71 nonsmokers). We separately analyzed hormonal changes associated with smoking according to the sex of newborns. In women with male fetuses, we found higher levels of serum cortisone, dehydroepiandrosterone (DHEA), 7α-OH-DHEA, 17-OH pregnenolone, testosterone, and androstenedione in smokers at the 37th week compared to non-smokers. In women with female fetuses, we found lower serum levels of 7β-OH-DHEA and higher androstenedione in smokers at the 37th week. We found significantly higher levels of testosterone in newborn males of smokers and higher levels of 7α-OH-DHEA in female newborns of smokers. Smoking during pregnancy induces changes in the production of steroids in both the mother and her child. Cíl práce: Upozornit na možnosti laparoskopického chirurgického řešení náhlé příhody břišní v graviditě. Typ práce: Kazuistika. Vlastní pozorování: Formou kazuistiky prezentujeme případ těhotné ve 23. týdnu gravidity s náhlou příhodou břišní neznámé etiologie a její laparoskopické řešení. Závěr: Laparoskopie v rukou zkušeného operatéra se jeví jako velmi bezpečná metoda v případech nejasné bolesti podbříšku, a to i v graviditě v závislosti na velikosti dělohy, tedy stáří gravidity. V tomto případě byla příčinou torze fibromu tuby, která byla řešena salpingektomií., The aim of the study: Draw attention to possibilities of LPSC surgical solutions of acute abdominal evens in pregnancy. Type of paper: Case study. Own observation: We are presenting a case of a pregnant woman at 23 weeks who suffered an acute abdominal event of unknown etiology and its laparoscopic solution. Conclusion: Laparoscopy in the hands of an experienced surgeon appears as a very safe method in cases of lower abdominal pain of unclear origin also in pregnancy, depending on the size of the uterus, i.e. the stage of pregnancy. Chronická neinfekční onemocnění představují závažný problém soudobé společnosti. Jejich etiologie má komplexní charakter. Jsou výsledkem v čase se vyvíjející interakce genetických a vnějších faktorů, jejichž podstatná část je vázána na životní styl a chování jedince. Genetická složka může být životním stylem modifikována, do jisté míry však sama chování a životní styl determinuje. Celoživotní vliv prostředí na genom je z velké části zprostředkován epigeneticky. Epigenetické mechanismy řídí chování genů, jsou podkladem fenotypové plasticity. Expozice faktorům modifikujícím epigenom v prenatálním a časně postnatálním období formuje vnímavost ke vzniku onemocnění, v dalším životě pak modifikuje manifestaci genetické vlohy a potenciálně participuje na patogenezi. Prenatálně indukované změny epigenomu mohou být předány na další generace potomků. Významným induktorem těchto alterací jsou nutriční faktory a kouření. V souvislosti s environmentální modifikací genové exprese je studována zejména DNA metylace, post-translační modifikace histonů a regulační aktivita nekódující RNA. Potenciální reverzibilita epigenetických procesů otevírá nové možnosti pro prevenci komplexních chorob. Poznatky o interakci genů, chování a prostředí potvrzují opodstatněnost primárně preventivních opatření a mohou přispět ke změně postojů ke zdraví., Non-infectious diseases represent a serious health problem in current society. Their etiology is complex as they result from a time-evolving interaction of genetic and environmental factors, a significant part of which are based on individual behaviour and lifestyle. Lifestyle can modify the impact of genetic load; nonetheless, behaviour and lifestyle are themselves determined by genes to a certain extent. Lifelong environmental influences on the genome are largely mediated epigenetically. Epigenetic mechanisms modify the activity of genes and form a base of the phenotype plasticity. Exposure to epigenetic modificators prenatally and in the early post-natal stage form susceptibility to diseases, modifiying manifestation of a genetic predisposition later in life and potentially participating in pathogenesis. Prenatally induced changes of the epigenome can be passed on to subsequent generations of offspring. Nutritional factors and smoking are major inducers of these epigenetic alterations. The most studied epigenetic mechanisms in relation to environmentally induced modification of gene expression are DNA methylation, post-translational histone modifications, and non-coding RNA regulatory activity. The potential reversibility of epigenetic processes opens up new possibilities for the prevention of complex diseases. The aim of our study was to determine the significance of lymphocyte-platelet adhesion (LPA), interleukins, transforming included 139 pregnant women aged between 17 and 27 years (21.3±4.22 years). GH was diagnosed in 119 women after 20 weeks of pregnancy. 20 patients (control group) were with physiological course of pregnancy. The distribution of patients by groups was carried out according to the level of blood pressure (BP) in accordance with ICD-10 (Geneva, WHO, 2002). The survey was conducted at the moment of detection pregnancy from 7 to 10 weeks and in dynamics of I, II and III trimesters of gestation. In the dynamics of gestation, were studied the number of desquamated endothelial cells circulating in the systemic circulation (CECs), nitrates levels, the adhesion of platelets by estimation their ability to form co-aggregates with lymphocytes by determining the percentage of lymphocytes aggregates with thrombocytes (lymphocyte-platelet plugs), at risk of hypertensive disorders, especially after 20-22 weeks of gestation and later, lymphocytes ability to platelets adhesion is rose, the concentrations of pro-inflammatory cytokines and NO level are increased. Práce prezentuje prenatální diagnostiku, možnosti terapie a následnou péči u plodů a novorozenců s kongenitálním chylothoraxem na podkladě zhodnocení 14 případů na naší klinice v letech 1993 až 2012. Povšechný hydrops plodu byl popsán ve 4 případech (29 %), v 10 případech (71 %) byla provedena odlehčovací amniodrenáž a ve 3 případech (21 %) prenatální odlehčovací punkce hrudníku. Spontánně bylo porozeno 11 plodů (78 %), hrudní drenáž byla nezbytná v průměru 9 dnů po porodu. Mortalita dosáhla v souboru 29 % (4 případy). V článku popisujeme vyšetřovací algoritmy, terapeutické prenatální i postnatální postupy a je zdůrazněna nutnost mezioborové spolupráce., The work presents prenatal diagnosis, treatment and care options in fetuses and neonates with congenital chylothorax based on a detailed evaluation of 14 cases recorded in our department between years 1993 and 2012. Generalized fetal hydrops was described in 29% of cases, therapeutic amnio drainage performed in 71% and prenatal thoracentesis in 21% of cases. 78% of fetuses were delivered spontaneously; thoracic drainage was necessary nine days after birth in average. Mortality reached 29%. In view of the high anti-oxidative potential oftocotrienol, the role of the tocotrienol-rich fraction (TRF) of palm oil in preventing pregnancy induced hypertension (PIH) was explored in a randomized double-blind placebo-controlled clinical trial in an urban teaching hospital. Healthy primigravidae were randomized to receive either oral TRF 100 mg daily or placebo, from early second trimester until delivery. Out of 299 women, 151 were randomized into the TRF arm and 148 into the placebo arm. A total of 15 (5.0%) developed PIH. Although there was no statistically significant difference in the incidence of PIH (4/151 or 2.6% in the TRF arm vs. 11/148 or 7.4% in the placebo arm, p = 0.058) between the two arms, there was a tendency towards a lower incidence of PIH in the TRF arm compared to the placebo arm. With TRF supplementation, the relative risk (RR) of PIH was 0.36 (95% CI 0.12-1.09). In conclusion, although TRF from palm oil does not statistically significantly reduce the risk of development of PIH in the population studied, the 64% reduction in incidence of PIH is substantial. Závislost na tabáku je příčinou každého šestého úmrtí v ČR a souvisí s nemocemi celého těla. Léčba závislosti na tabáku by měla být samozřejmou součástí léčebně preventivní péče v rozsahu dle časových možností – od krátké intervence při každém klinickém kontaktu s pacientem až po intenzivní léčbu. Zahrnuje psychosociobehaviorální podporu a farmakoterapii. Měla by se týkat všech profesí v klinické medicíně – dle doporučení WHO především lékařů, sester, lékárníků a dentistů, a měla by být hrazena v rámci zdravotnických systémů. Z ekonomického hlediska je to jedna z nejvýhodnějších intervencí v medicíně. V naší populaci kouří asi 30 % populace nad 15 let (asi 2 200 000 osob), diagnóza F17 se týká asi 1,75 milionu osob (zhruba 80 % kuřáků)., Tobacco dependence causes every sixth death in the Czech Republic and is associated with diseases of the whole body. Treatment of tobacco dependence should be a standard part of clinical care to the extent of time available – from brief intervention at each clinical contact with patients up to intensive treatment. It includes psycho-socio-behavioural support and pharmacotherapy. It should apply to all professions in clinical medicine – as recommended by WHO mainly doctors, nurses, pharmacists and dentists, and should be covered within health care systems. Economically, it is one of the most cost-effective interventions in medicine. Gestageny mají různé biologické vlastnosti, na které bereme ohled při jejich volbě. V hormonální substituční terapii volíme progestin dle jeho metabolické neutrálnosti a bezpečnosti vzhledem k možnému riziku onemocnění karcinomem prsu. Mikronizovaný progesteron a dydrogesteron mají pravděpodobně lepší bezpečnostní profil než syntetické progestiny. Také v suplementaci gestagenů v těhotenství dáváme přednost přirozeným gestagenům před syntetickými progestiny. The best progestogen for the hormone replacement therapy could be metabolic neutral and safe for breast tissue. Micronised progesterone and dydrogesterone have better safety profile than synthetic progestagens. Also during pregnancy we prefer natural gestagens to synthetic ones.
