We report the case of a 52year old man with severe acute pancreatitis. In this case report we discuss the undesirable effects of parenteral nutrition and the importance of nutritional support at patients with severe necrotizing pancreatitis. Severe acute pancreatitis is usually accompanied by systemic inflammatory response syndrome, which results in hypermetabolism with prominent protein catabolism. Providing nutrition to these patients is of paramount importance. An adequate nutritional support is crucial in patients with severe and complicated pancreatitis. Předpoklad: Studie IMProved Reduction of Outcomes: Vytorin Efficacy International Trial (IMPROVE‑IT) hodnotila potenciální prospěch na snížení velkých kardiovaskulárních (KV) příhod při přidání ezetimibu vs placebo k léčbě 40 mg simvastatinu u nemocných s akutním koronárním syndromem a nízkou hladinou low-density cholesterolu (LDL‑c) ≤ 125 mg/dl. Metodika: Primární smíšený cíl byl KV úmrtí, nefatální infarkt myokardu (IM), rehospitalizace pro nestabilní anginu pectoris (NAP) a koronární revaskularizace po 30 dnech. Ve větvi se simvastatinem v monoterapii byl cílový LDL‑c < 70 mg/dl. Předpokladem bylo, že ezetimib sníží LDL‑c o dalších 15 mg/ml s efektem léčby kolem 8–9 %. Cílový počet příhod byl 5 250. Výsledky: Bylo zařazeno 18 144 nemocných s ST elevacemi IM (STEMI, n = 5 192) nebo nonST elevacemi IM či NAP (NAP/nonSTEMI, n = 12 952) od října 2005 do července 2010. Nejvíce (40 %) bylo zařazeno v západní Evropě a Severní Americe (38 %). Pacienti se STEMI byli mladší a bylo zde více nemocných bez předchozí léčby statiny. Pacienti s nonSTEMI/NAP měli častěji diabetes mellitus, hypertenzi a předchozí IM. Medián LDL‑c při randomizaci byl 100 mg/dl u STEMI a 93 mg/dl u nonSTEMI/NAP. Primární cíl se vyskytl u 2 742 nemocných (34,7 %) na léčbě simvastatinem v monoterapii a u 2 572 nemocných (32,7 %) (p = 0,016) léčených kombinací. Methods: The primary composite end point was CV death, nonfatal myocardial infarction (MI), nonfatal stroke, re‑hospitalization for unstable angina (UA), and coronary revascularization (≥ 30 days postran‑domization). The simvastatin monotherapy arm's LDL‑C target was < 70 mg/dL. Ezetimibe was assumed to further lower LDL‑C by 15 mg/dL and produce an estimated ~8% to 9% treatment effect. The targeted number of events was 5,250. The STEMI cohort was younger and had a higher percentage of patients naive to lipid‑lowering treatment compared to the UA/NSTEMI cohort. The UA/NSTEMI group had a higher prevalence of diabetes, hypertension, and prior MI. Median LDL‑C at entry was 100 mg/dL for STEMI and 93 mg/dL for UA/NSTEMI patients. Primary endpoint occurred in 2,742 patients (34.7%) treated with simvastatin in monotherapy and in 2,572 patients (32.7%) (p = 0,016) treated with combination treatment. To avoid one primary outcome, treatment of 50 patients for seven years was necessary. Srdeční infarkty byly sníženy o 13 %, nefatální CMP o 20 %. Úmrtí z kardiovaskulární příčiny bylo v obou skupinách stejné. Průměrná doba sledování byla 6 let, nejdelší 8,5 roku. Průměrně 2 pacienti ze 100 předešli kardiovaskulární příhodě za 7 let (Number Needed to Treat – NNT) 50/7 let. Imunostimulace polybakteriálním lyzátem (Urivac®) v prevenci recidivujících infekcí dolních močových cest. Infekce močových cest (IMC) zaujímají v evropských zemích i u nás dlouhodobě druhé místo ve výskytu infekčních onemocnění v populaci a představují tak významný socioekonomický problém. Spolu s respiračními infekty se rozhodující měrou podílejí na preskripci antimikrobiálních přípravků. Extenzivní antimikrobiální léčba je jednou z nejvýznamnějších příčin nárůstu bakteriální rezistence. Určitou možnost prevence recidiv infekcí dolních močových cest a snížení spotřeby antimikrobiálních preparátů představuje imunostimulace. Prospektivní multicentrická studie hodnotila klinický účinek, snášenlivost a přínos k léčbě recidivujících zánětů při podávání imunostimulačního přípravku Urivac®. Počet atak infekcí dolních močových cest se po imunostimulaci významně snížil jak v jejím průběhu, tak po jejím ukončení (p = 2,2x10-16). V průběhu imunostimulace bylo bez příznaků 85 % nemocných a při dalším sledování 70,8 %. Snížil se také počet aplikovaných antimikrobiálních přípravků. Subjektivní zlepšení přetrvávalo ve více než 57 % i po ukončení léčby. Mikrobiologická odpověď dosáhla 77 %, v odstupu od ukončení léčby se snižuje. U exacerbací byla uropatogenní flóra detekována v 61 %. Nežádoucí účinky u dvou nemocných vedly k předčasnému ukončení terapeutického režimu., In European countries, including the Czech Republic, urinary tract infections (UTIs) have long occupied the second place in the incidence of infectious diseases in the population, thus representing a major socioeconomic problem. Along with respiratory infections, they are the major contributors to the prescription of antimicrobial agents. Extensive antimicrobial therapy is one of the most important causes of increased bacterial resistance. Immunostimulation is a possible way of preventing recurrent lower urinary tract infections and reducing the consumption of antimicrobial agents. A prospective multicentre study evaluated the clinical effi cacy, tolerability, and benefi t to treatment of recurrent infections with the immunostimulatory drug Urivac®. The number of attacks of lower urinary tract infections following immunostimulation reduced signifi cantly both during its course and after its cessation (p = 2.2 x 10–16). During immunostimulation, 85 % of patients were symptom-free, and during subsequent follow-up it was 70.8 %. There was associated with a reduction in the number of the antimicrobial agents administered. Subjective improvement persisted in more than 57 % even after treatment cessation. Microbiological response reached 77 %; it decreases with increased time from treatment cessation. In the case of exacerbations, uropathogenic fl ora was detected in 61 %. Úvod: Axilární disekce lymfatických uzlin (ALND) je dnes jednoznačně doporučována pouze u 3 a více pozitivních sentinelových uzlin (SLN). Není indikována u negativní sentinelové uzliny a u přítomnosti pouze izolovaných tumorózních buněk. Přínos ALND je dále zkoumán u pacientek s méně než 3 pozitivními sentinelovými uzlinami a/nebo pouze s mikrometastatickým postižením. Cíl: Posouzení indikace axilární disekce uzlin při nálezu mikrometastatického postižení sentinelové uzliny u karcinomu prsu. Metoda: Biopsie sentinelové uzliny s periareolární aplikací Sentiscintu. Soubor 265 pacientek, u 82 pozitivní sentinelová uzlina, u 17 prokázáno pouze mikrometastatické postižení, u 4 byla nalezena kombinace mikrometastatického a makrometastatického postižení dvou různých sentinelových uzlin. U 7 pacientek s pouze mikrometastatickým postižením a u všech 4 pacientek s kombinací mikrometastatického a makrometastatického postižení sentinelových uzlin byla provedena ALND. Pacientky byly následně sledovány v průměru 41 měsíců. Výsledky: U všech pacientek byly vyjmuty při SLNB alespoň dvě sentinelové uzliny. U 7 pacientek s izolovaným mikroskopickým postižením SLN a alespoň jednou další vyjmutou a negativní uzlinou při SLNB byly po ALND nalezeny v axile pouze negativní lymfatické uzliny, u zbylých 10 pacientek nebyla prokázána axilární rekurence v průběhu sledování. U poloviny pacientek s kombinovaným mikrometastatickým a makrometastatickým postižením různých SLN bylo nalezeno po ALND metastatické postižení i dalších axilárních uzlin. Závěr: Při vyjmutí pouze jedné sentinelové uzliny s nálezem izolovaného mikrometastatického postižení není možno predikovat postižení dalších axilárních uzlin a upustit od ALND. To study efficiency of the combined treatment of advanced gastric cancer with inclusion Trastuzumab. Material: We present the intermediate analysis of the use of target therapy with Trastuzumab in patients with a HER2positive gastric cancer. Up to 01.10.2009y 118 patients have been tested for HER2 expression, and in 24 gastric cancer patients it is revealed HER2positive status of tumor. It is lead chemotherapy to all patients by the PLF regimen and Herceptin in doze 6 mg/kg once in 3 weeks (6 cycles). In control group in 26 patients it is lead only chemotherapy by the PLF regimen once in 3 weeks without addition Trastuzumab (6 cycles). Results: At the moment of the analysis of preliminary data, the median remission duration in compared groups has made 8.3 months, and 5.2 months, accordingly. This research paper presents the results of ovarian reserve estimation for 125 women with the Polycystic Ovary Syndrome (PCOS) who have undergone various methods of surgical treatment resection of the ovaries, thermokauterisation and drilling by laser (HoYag). Ovarian reserve was estimated according to the amount of antral follicles, level of follitropin and Müllerian inhibiting substance (MIS), also named antiMüllerian hormone (AMH). Blood flow in ovarian tissue was also examined after various methods of surgical treatment. Úvod: Kombinace monoklonální antiCD20 protilátky rituximabu s chemoterapií je vysoce účinná v léčbě chronické lymfocytární leukemie (CLL). Hlavním nežádoucím účinkem rituximabu jsou infuzní reakce, např. syndrom uvolnění cytokinů (cytokine release syndrome – CRS), který zpravidla doprovází první podání rituximabu. Cílem naší retrospektivní analýzy bylo zhodnotit četnost výskytu infuzních reakcí v rutinní klinické praxi, jejich vztah k parametrům nádorové nálože a zjistit možnou spojitost infuzních reakcí s účinností léčby. Dále byla posouzena bezpečnost rychlé infuze s rituximabem u nemocných s CLL. Charakteristika souboru: Hodnoceno bylo 108 nemocných s CLL léčených chemoimunoterapií s rituximabem od května roku 2005 do března roku 2011 v našem centru. V rámci 1. linie (n = 66, 71 % mužů, věkový medián 63 let) byly použity tyto léčebné režimy: fludarabin, cyklofosfamid, rituximab – FCR (n = 43) a nízkodávkovaný FCR (n = 13); 10 nemocných bylo léčeno jinými protokoly. 42 nemocných (76 % mužů, věkový medián 65 let) podstoupilo 2. linii: rituximab-dexametazon (n = 18), FCR/nízkodávkované FCR (n = 14) a 10 nemocných jiný režim. Součástí premedikace před infuzí rituximabu byl metylprednisolon 80 mg i.v., paracetamol 1 000 mg p. o. a bisulepin 1 mg i. v., všichni nemocní měli nitrožilní hydrataci minimálně 2 000 ml/den a alopurinol 300–600 mg/den. V 1. cyklu byl rituximab podáván frakcionovaně v celkové dávce 375 mg/m2 a od 2. cyklu formou rychlé infuze v dávce 500 mg/m2. Výsledky: K rozvoji infuzní reakce po rituximabu došlo u 32 % nemocných v 1. linii a 19 % v 2. linii. Jednalo se převážně o mírné reakce a k závažné toxicitě (stupeň III/IV dle NCI CTCAE – National Cancer Institute – The Common Terminology Criteria for Adverse Events) došlo výjimečně (3 % nemocných v 1. linii a 2 % v 2. linii). Reakce se nejčastěji projevila třesavkou, zimnicí, dušností, horečkou či hypotenzí. U všech nemocných bylo možné dokončit infuzi rituximabu v plánované dávce ve stejný den. Hodnocení léčebné odezvy (overall response rate – ORR) neprokázalo statisticky významné rozdíly mezi nemocnými s reakcí či bez infuzní reakce. Nemocní s infuzní toxicitou měli vyšší absolutní počet lymfocytů (1. linie, 87 vs 56 × 109/l, p = 0,21; 2. linie, 101 vs 14 × 109/l, p = 0,043). Při mediánu sledování 36 měsíců nebyl prokázán statisticky významný rozdíl v PFS (doba přežití bez progrese – progression-free survival) a OS (celkové přežití – overall survival) mezi oběma skupinami. Závěry: Výskyt infuzních reakcí u nemocných s CLL léčených rituximabem je poměrně běžný (32 %), avšak při pečlivé premedikaci jsou reakce velmi dobře zvladatelné a těžká forma je vzácná. Potvrdili jsme velmi dobrou snášenlivost rychlé infuze rituximabu. Naše data neprokázala statisticky významnou souvislost mezi výskytem infuzních reakcí a efektem léčby., Background and Aims: Rituximab in combination with chemotherapy is an effective treatment of patients (pts) with chronic lymphocytic leukemia (CLL). The most frequent adverse event of rituximab is infusion-related toxicity, e.g. cytokine-release syndrome that occurs usually during the first infusion. However, there is scarce data on feasibility and tolerability of rituximab infusions in CLL outside clinical trials. Therefore, we performed a single-center retrospective analysis of the frequency of rituximab infusion-related adverse events during the first- and the second line CLL treatment administered in the routine practice. We also analyzed its relation to parameters of tumor load and possible association with treatment efficacy. The safety of rapid infusion of rituximab in CLL pts was also evaluated. Patients and Methods: We analyzed 108 pts with CLL treated with rituximab-containing regimens between March 2005 and May 2011 at our institution. The most common first-line regimens (n = 66, 47 males, median age 63 years) were FCR (43 pts) and low-dose FCR (13 pts); 10 pts were treated by other protocols. Forty-two pts (32 males, median age, 65 years) underwent second line treatment: 18 pts rituximab-dexamethasone, 7 pts FCR, 7 pts low-dose FCR and 10 pts other regimens. Intravenous hydration (2 000 ml daily on days 0 and 1 of cycle), allopurinol 300–600 mg p. o. daily and premedication with methylprednisolone 80 mg i. v., acetaminophen 1 000 mg p. o. and bisulepine 1 mg i. v. were administered before rituximab infusion. Rituximab was given by fractionated infusion (100 mg for 2 hours, then if tolerated well, the rest of the dose with infusion rate escalation from 100 mg/hour up to 400 mg/hour) at the dose of 375 mg/m2 in the first cycle. Subsequent doses (500 mg/m2) were administered by rapid-infusion protocol. Results: Rituximab infusion-related toxicity occurred in 32 % pts (n = 21) during the first line treatment and 19% pts (n = 8) during the second line treatment. Adverse events were predominantly mild and NCI CTCAE grade III/IV occurred rarely (3% in the first line, 2% in the second line). Infusion toxicity manifested predominantly as rigors, chills, fever and hypotension. All patients with adverse events could finish rituximab infusion as initially planned on the same day. Treatment response analysis did not demonstrate statistically significant differences between patients with and without rituximab infusion toxicity. Patients who developed rituximab infusion toxicity had higher absolute lymphocyte count (first line, 87 vs 56 × 109/l, p = 0.21; second line, 101 vs 14 × 109/l, p = 0.043). At the median follow-up of 36 months, there were no statistically significant differences in PFS or OS in both cohorts. Conclusions: Rituximab infusion-related toxicity in pts with CLL is relatively frequent (32%). However, occurrence of infusion-related symptoms can be reduced by proper premedication and severe adverse events are uncommon. In our experience, all patients were able to receive the planned dose of rituximab. Subsequent doses of rituximab could be safely administered by rapid-infusion protocol. Cíl: Sémontův repoziční manévr se v praxi užívá k léčbě pacientů s benigním paroxyzmálním polohovým vertigem zadního polokruhového kanálku. Cílem studie bylo zhodnotit interindividuální a intraindividuální variabilitu při provádění tohoto manévru a definovat parametry, které jeho provedení nejvíce ovlivňují. Metody: Tři zkušení terapeuti provedli manévr oboustranně u 10 zdravých jedinců. Inerciální měřicí jednotka složená z akcelerometru a gyroskopu získávala informace o trajektorii pohybu hlavy v prostoru a její rychlosti. Úhlové změny byly zaznamenány v souřadnicovém referenčním systému. Z odchylek od uvažované roviny polokruhového kanálku byla vyhodnocena data o přesnosti provedení manévru. Výsledky: Parametry ovlivňujícími přesnost manévru jsou výška probanda (p = 0,0252), fáze pohybu (p < 0,0001) a dále terapeut a strana pohybu, přičemž tyto faktory vykazují vliv především ve své interakci. Byl hodnocen vztah fáze pohybu k výšce probandů (p = 0,0130), terapeuta k fázi pohybu (p = 0,0001), terapeuta k výšce probandů (p < 0,0252). Největší zaznamenaný rozsah odchylky od senzorické roviny byl v rozmezí –37,17° až 31° se standardní odchylkou 16,6°. Závěry: Získaná data poukazují na překvapivě vysokou variabilitu při provádění Sémontova manévru ať v porovnání mezi jednotlivými měřeními u jednoho terapeuta nebo v porovnání mezi terapeuty. Analýza vlivu těchto odchylek na úspěšnost terapie benigního polohového paroxyzmálního vertiga a bližší analýza ovlivňujících parametrů by měly být předmětem dalšího výzkumu., Aim: Sémont liberatory manoeuvre is used in the treatment of posterior semicircular canal in patients with benign paroxysmal positional vertigo. The aim of the study was to determine the inter-individual and intra-individual variability in the processing of the manoeuvre and to define the parameters that affect it the most. Methods: Three experienced therapists applied the manoeuvre bilaterally in 10 healthy probands. Inertial measurement unit consisting of the accelerometer and gyroscope recorded the trajectory and speed. Angle changes were placed in the coordinate reference system. Accuracy of the manoeuvre was evaluated based on the deviations from the projected sensory plane. Results: Parameters affecting the accuracy of the manoeuvre included height of a proband (p = 0.0252), phase of the movement (p < 0.0001), therapist and the side of the movement. The effect of these factors is the most pronounced when combined. We assessed interactions of the phase of the movement and the height of the probands (p = 0.0130), the therapist and the phase of movement (p = 0.0001), the therapist and the height of the probands (p < 0.0252). The largest magnitude of deviation from the sensory plane was in the range of –37.17° to 31° with a standard deviation of 16.6°. Conclusions: The data highlight high variability in the implementation of the Sémont liberatory manoeuvre, whether measurements by a single therapist or inter-individually between the therapists are compared. Analysis of the impact of these deviations on therapeutic efficacy in patients with benign paroxysmal positional vertigo and detailed analysis of influencing parameters should be the subject of further research.
