The purpose of this study was to test the hypothesis that more recently developed rubber dam systems (OptraDam ® Plus and OptiDam™) are faster and easier to handle, and that the quality of isolation is not decreased. The rubber dam systems were applied in standard conditions on a dental simulator in several model clinical situations. The time of preparation, application and removal were measured and the quality of isolation was evaluated. The median time of rubber dam placement was 51 s (Q1 = 38 s; Q3 = 79 s). The shortest median time of application was with OptiDam™ (42 s), followed by a conventional rubber dam (53 s), and finally the longest was with OptraDam® Plus (58 s). The median volume of fluid remaining in the isolated space after 5 minutes was 9.5 mL (Q1 = 8 mL; Q3 = 10 mL). The largest median volume of remaining water was with OptiDam™ (10 mL), followed by a conventional rubber dam (9.5 mL) and the least with OptraDam® Plus (8.5 mL). The afore-stated hypothesis about the advantages of modern rubber dam isolation systems was accepted for OptiDam™, but rejected for OptraDam® Plus. The aim of this study was to compare the isolation systems OptraDam® Plus and OptiDam™ with the conventional rubber dam in terms of objective and subjective parameters. The isolation systems were applied during the dental treatment of the patients. The time of preparation, placement, presence and removal were measured and the quality of isolation was evaluated. The median time of rubber dam placement was 76 s (Q1=62 s; Q3=111.25 s). The application time of OptraDam® Plus was significantly longer compared to the other systems (P ® plus. A cutaneous horn is a general name for cornified material protruding from skin. On the other hand, polydactyly is a common congenital anomaly of the hand and foot which is characterized by extra finger or toe. A cutaneous horn might mimick polydactyly by resembling an extra toe. Hereby, we present a 72-year-old white Caucasian male with an extra toe-like projection on his fourth toe. Initially, polydactyly was suspected, however a cutaneous horn was also considered. The lesion was surgically removed. Nevus lipomatosus superficialis is a rare hamartomatous malformation which is composed of ectopic adipocytes in the dermis. It was first reported in 1921 by Hoffmann and Zurhelle. Two clinical forms of nevus lipomatosus superficialis have been described: classical (multiple) and solitary. Classical form of nevus lipomatosus superficialis is usually found on pelvic girdle, trunk, buttocks and thighs as soft, skin colored papules or nodules. It is usually present at birth or it appears in the first two decades of life. The solitary form of lipomatosus superficialis appears as a solitary papule or nodule on the back, scalp and arms of the patients with late onset. The lesions are usually asymptomatic, however some patients may complain about pain and itching. Malignant transformation of nevus lipomatosis superficialis has not been reported yet. Therefore, surgical intervention is only necessary for the patients who have cosmetic concerns. Recurrence after surgical removal is very rare. Perineum is an uncommon localization for nevus lipomatosus superficialis. Hereby, we report a 55-year-old Caucasian female with a 6x5,5x4 cm mass in the perineal region. The patient had cosmetic concerns, therefore she wanted the lesion to be removed surgically. The lesion was surgically removed. The histopathological evaluation of the specimen revealed nevus lipomatosus superficialis. A solitary type of giant nevus lipomatosus superficialis in the perineal region of a patient over the age of 50 is a very rare condition. BACKGROUND: Radio frequency (RF) and chemical peels have been used for nonablative skin rejuvenation. Both of these cause collagen remodeling in the dermis and neo-collagen formation resulting in facial rejuvenation. There is limited literature on the evaluation of collagen remodeling by objective methods. OBJECTIVE: To compare the benefits of monopolar radiofrequency and glycolic acid peels in facial rejuvenation with regards to histopathology and Ultrabiomicroscopic sonography (UBM). METHODOLOGY: In this study, forty patients with mild to moderate photoaging received four treatments with 3 weeks interval of monopolar radiofrequency on one side of face and glycolic acid peels in increasing concentrations (NeostrataR) on the other side. Pre and post treatment, 2 mm biopsies were taken from both preauricular areas and Ultrasonography using a 35 MHz probe was done from outer canthus of eye and nasolabial folds from both sides of face. A blinded assessment was done to measure the increase in the grenz zone and dermal thickness. RESULTS: In 35/40 patients there was a significant increase in the grenz zone on histopathology and decrease in subepidermal low-echogenic band (SLEB) on UBM of the nasolabial folds on both sides of the face (p < 0.05). Cíl: Publikace případu falešně zvýšené koncentrace bilirubinu u pacientky s mnohočetným myelomem. Metoda: U pacientky s diagostikovaným a léčeným mnohočetným myelomem byla stanovena atypicky vysoká hodnota sérového bilirubinu diagnostickým kitem Roche na přístroji Modular a normální, o dva řády nižší hodnota téhož analytu diagnostickým kitem Olympus na přístroji Olympus 400. Výsledky: Pacientka měla v krvi koncentraci paraproteinu IgG kappa 61,6 g/l. Sérový bilirubin, stanovený na přístroji Modular měl hodnotu 546 μmol/l, zatímco při opakovaném měření na přístroji Olympus 400 byla hodnota u stejné pacientky 6 μmol/l. Nižší hodnota korespondovala s klinickým stavem pacientky. Diskuse a závěr: Oba výrobci upozorňují ve svých pracovních návodech na potenciální možnost pozitivních interferencí v přítomnosti IgM lambda. Zjistili jsme extrémní pozitivní chybu o velikosti dvou řádů v přítomnosti paraproteinu IgG kappa u jednoho z nich (Roche). Chyba je snadno detekovatelná inspekcí záznamu kinetiky reakčního průběhu a neměla by ujít pozornosti při vydávání výsledků., Objective: Falsely elevated serum bilirubin in patient with multiple myeloma. Method: We determined serum bilirubin in a patient with multiple myeloma by using Modular and we repeated analysis by using Olympus 400. Results: Concentration of IgG kappa was 61.6 g/l. Serum bilirubin value measured on the Modular Roche has been 546μmol/l, while repeated measurement on Olympus 400 provided value only 6 μmol/l. Lower Olympus value corresponded to clinical status of the patient. Discussion and conclusion: Both producers introduce in their working manual the possibility of IgM lambda interference for bilirubin measurement. We found extremely high systematic error in presence IgG kappa only in Modular Roche, not in Olympus 400. Hereditární hemoragická teleangiektázie neboli morbus Rendu‑Osler‑Weber je fibrovaskulární dysplazie s autozomálně dominantní dědičností charakteristická přítomností teleangiektázií a arteriovenózních malformací kůže, sliznic a vnitřních orgánů. Plicní arteriovenózní malformace jako zdroj septických embolů mohou zapříčinit vznik mozkového abscesu, který tak bývá prvním a často jediným projevem heriditární hemoragické teleangiektázie. V práci jsou prezentovány tři kazuistiky pacientů trpících touto chorobou, kde prvním a závažným příznakem byl vzniklý mozkový absces. Protože se jedná o relativně vzácné onemocnění, nebývá vždy zvažováno v rámci diferenciální diagnostiky a tím pádem ani diagnostikováno. U každého pacienta s mozkovým abscesem je nezbytné na toto onemocnění pomýšlet a v případě podezření je nutné jej řádně vyšetřit. Při verifikované plicní arteriovenózní malformaci je třeba ji ošetřit a zabránit tak vzniku recidivujících mozkových abscesů. Péče o tyto pacienty je komplexní a multioborová., Hereditary hemorrhagic telangiectasia or Rendu‑Osler‑Weber disease is an autosomal‑dominant disorder that results in fibrovascular dysplasia and is characterized by telangiectases and arteriovenous malformation of the skin, mucosa and viscera. Pulmonary arteriovenous malformations as a source of septic emboli may give rise to brain abscess that so often is the first and only clinical manifestation of hereditary hemorrhagic telangiectasia. Three cases of patients suffering from this disease with brain abscess as the first and serious symptom are presented. Since this is a relatively rare disease, it is not always considered in the differential diagnosis and thus not diagnosed. This disease has to be considered in patients with brain abscess and, in case of a suspicion, it must be properly examined. When pulmonary arteriovenous malformation is verified, it has to be treated in order to prevent recurrent brain abscesses. Care of these patients should be comprehensive and multidisciplinary.
