Primary graft dysfunction (PGD) is a life-threatening complication among heart transplant recipients and a major cause of early mortality. Although the pathogenesis of PGD is still unclear, ischemia/reperfusion injury has been identified as a predominant factor. Both necrosis and apoptosis contribute to the loss of cardiomyocytes during ischemia/reperfusion injury, and this loss of cells can ultimately lead to PGD. The aim of our prospective study was to find out whether cell death, necrosis and apoptosis markers present in the donor myocardium can predict PGD. The prospective study involved 64 consecutive patients who underwent orthotopic heart transplantation at our institute between September 2010 and January 2013. High-sensitive cardiac troponin T (hs-cTnT) as a marker of minor myocardial necrosis was detected from arterial blood samples before the donor’s pericardium was opened. Apoptosis (caspase-3, active + pro-caspase-3, bcl-2, TUNEL) was assessed from bioptic samples taken from the right ventricle prior graft harvesting. In our study, 14 % of transplant recipients developed PGD classified according to the standardized definition proposed by the ISHLT Working Group. We did not find differences between the groups in regard to hs-cTnT serum levels. The mean hs-cTnT value for the PGD group was 57.4±22.9 ng/l, compared to 68.4±10.8 ng/l in the group without PGD. The presence and severity of apoptosis in grafted hearts did not differ between grafts without PGD and hearts that subsequently developed PGD. In conclusion, our findings did not demonstrate any association between measured myocardial cell death, necrosis or apoptosis markers in donor myocardium and PGD in allograft recipients. Srdeční transplantace je v posledních letech metodou volby léčby pacientů s těžkým srdečním selháním. Tato léčba v sobě zahrnuje doživotní imunosupresivní terapii, která je kombinační. Klasickou imunosupresi tvoří cyklosporin nebo tacrolimus, mykofenolát mofetil a kortikoidy. V posledních letech se u pacientů po ortotopické transplantaci srdce nejčastěji s poruchou renálních funkcí nebo vaskulopatií uplatňuje kombinace nízkých dávek kalcineurinových inhibitorů s nízkými dávkami mTOR inhibitorů nebo režimy bez kalcineurinových inhibitorů (cyklosporinu či tacrolimu), které jsou nefrotoxické. Po prvním roce se snažíme ve všech režimech vysadit kortikoidy a výslednou doživotní léčbou je dvojkombinace. In the last few years, new regimes of immunosuppressive drugs have been used in post-heart transplant patients suffering from renal dysfunction or allograft vasculopathy: a combination of low doses of calcineurin inhibitors with low doses of mTOR inhibitors or calcineurin-free regimes, which are not so nephrotoxic. There has been an effort to withdraw corticosteroids from all regimes after the first year and to have life-long therapy based on a combination of two drugs. Solid organ transplantation is an established treatment modality in patients with end-stage organ damage in cases where other therapeutic options fail. The long-term outcomes of solid organ transplant recipients have improved considerably since the introduction of the first calcineurin inhibitor (CNI) - cyclosporine. In 1984, the potent immunosuppressive properties of another CNI, tacrolimus, were discovered. The immunosuppressive effects of CNIs result from the inhibition of interleukin-2 synthesis and reduced proliferation of T cells due to calcineurin blockade. The considerable side effects that are associated with CNIs therapy include arterial hypertension and nephrotoxicity. The focus of this article was to review the available literature on the pathophysiological mechanisms of CNIs that induce chronic nephrotoxicity and arterial hypertension. CNIs lead to activation of the major vasoconstriction systems, such as the reninangiotensin and endothelin systems, and increase sympathetic nerve activity. On the other hand, CNIs are known to inhibit NO synthesis and NO-mediated vasodilation and to increase free radical formation. Altogether, these processes cause endothelial dysfunction and contribute to the impairment of organ function. A better insight into the mechanisms underlying CNI nephrotoxicity could assist in developing more targeted therapies of arterial hypertension or preventing CNI nephrotoxicity in organ transplant recipients, including heart transplantation., L. Ortotopická transplantace srdce je zavedenou metodou léčby pacientů v terminálním stadiu srdečního selhání, která zlepšuje přežívání těchto pacientů. Cenou za podávání imunosupresivní léčby je řada potenciálních rizik, jedněmi z nejzávažnějších jsou infekční komplikace. Kazuistika popisuje případ pacientky se srdečním selháním na podkladě primární amyloidózy, která podstoupila srdeční transplantaci. Po transplantaci byla v plánu kurativní hematoonkologická léčba, která však vzhledem ke kompletní remisi základního onemocnění nebyla uskutečněna. V časném potransplantačním období pacientka prodělala řadu komplikací, z nichž nejzávažnější byla invazivní plicní aspergilóza. Tato infekce byla úspěšně zaléčena vorikonazolem a pacientka je nyní 28 měsíců po srdeční transplantaci ve velmi dobrém klinickém stavu bez známek aktivní infekce., Orthotopic heart transplantation is an established method of treatment for patients with end‑stage heart failure, which improves the survival of these patients. Our case report describes a patient with heart failure on the basis of primary amyloidosis who underwent cardiac transplantation. Hemato‑oncological curative treatment was planned after transplantation, but has not been applied due to the complete remission of the primary disease. In the early post‑transplant period, the patient experienced a number of complications, the most serious being invasive pulmonary aspergillosis.
