Akutní symptomatické záchvaty představují až 40 % všech prvních epileptických záchvatů a vznikají v těsné časové souvislosti s probíhajícím postižením centrálního nervového systému (CNS) různé povahy. Z hlediska posouzení rizika opakování záchvatu má kromě anamnestických údajů význam i případná abnormita v EEG či odchylky v objektivním nálezu svědčící pro postižení CNS. Zahájení léčby je po prvním záchvatu indikováno při vysokém riziku opakování či při zvýšené pravděpodobnosti závažných komplikací dalšího záchvatu., Following a first seizure, it is important to establish whether this was an epileptic or non-epileptic seizure, and to ascertain possible cause of an epileptic seizure. With respect to further management, diagnostic or therapeutic, it is crucial to differentiate non-epileptic from epileptic seizures and to identify provoked epileptic seizures (acute symptomatic). Patient history with a detailed description of the course of a seizure and circumstances under which it happened is instrumental in discriminating between the different types of seizures. Complementary investigations can further support the diagnostic process. Acute symptomatic seizures represent up to 40% of all single epileptic seizures and occur in close chronological proximity to an ongoing central nervous system injury of various origins. The source can easily be identified – most frequently this is an acute cerebrovascular event, craniocerebral injury, inflammatory CNS disorder, while primarily extracerebral effects are also important – toxic and metabolic disorders and drug or alcohol abuse. Unprovoked or reflex single epileptic seizures in patients with persisting predisposition and a high risk (> 60%) of relapse may, according to the new definition, be regarded as epilepsy and long-term antiepileptic treatment may be initiated. MRI under appropriate protocol should always be performed after the first unprovoked seizure. The risk of seizure recurrence can be estimated based on patient history as well as any EEG or physical examination abnormity that suggests an injury of the CNS. Antiepileptic treatment is indicated in patients with a single seizure if there is a high risk of recurrence or an increased likelihood of severe complications of further seizures. The main aim of the initial examination is to decide whether we are facing a patient with life-threatening disease, e.g. stroke, or a benign disease such as vestibular neuritis. This decision is usually made under a time pressure and knowing that neuroradiology examination may not provide sufficient information. Clinical examination of the vestibular and oculomotor function is sufficient to make the initial decision. Vestibular examination requires basic knowledge of the vestibular system function and reflex circuits involved in the organization and control of the multisensoric balance system. The subject of liability for damage to client caused during performance of social services in residential establishments becomes increasingly relevant. As opposed to the liability for damage caused during performance of health services, this subject is rather underappreciated by general members of the legal profession. For this reason RILSA produced a study „Legal aspects of liability for damage caused to a client while providing a social-health care in residential establishment for social services“ that aimed to find out the level of knowledge among the providers of residential social services in the area of liability for damage. To find out the level of legal knowledge of providers of residential social services, a question form was distributed randomly in the Czech Republic. We found out, that providers of residential social services know about the existence of liability for damage and that they are insured. But in the case of significant incurred damage or in the case of more damages within one year the insurance coverage limit might not be sufficient. However, the knowledge of legal terms of liability for damage eventually caused to their clients is limited and current way of obtaining the knowledge by individual study is insufficient., Daniela Bruthansová, Věra Jeřábková, and Literatura
Zvláštní pozornost je věnovaná fenoménu role playing games a dnešní podobě literárního upíra, přičemž jsou zde zohledněny i genderové rozdíly v přístupu k těmto fantastickým bytostem., The theme of this contribution is an analysis of searching magic beings, dragons and vampires, in the postmodern world of science and technique. At first sight, it could seem these beings, so far away from rationalism of present time, cannot find their real expression. The aim of this study is to describe which way the stories about monsters, their victims and heroes, who come to save them, has adapted to the cultural code of present time. How much have dragons and vampires kept their former identity and how much have the differences between a h ero and a m onster started to remove? Special attention is paid to the phenomenon of the role playing games and current image of a li terary vampire, and even gender differences in the approach to the fantastic beings was taken into account., Markéta Poláková, and Literatura
Rozdílný pohled na některá témata vnímám jako podněcující, protože člověku skýtá příležitost přemýšlet o věcech novým, obohacujícím způsobem., The purpose of my work is to think of the existentialism and the postmodern approach from the view of their mutual similarities and differences. In my opinion, the existential analysis stands between the modernism and the postmodernism, because on the one hand it is based on the cognition of the objective world (the modernism) and on the other hand, there is also the requirement of subjective experience there (the postmodernism). I came to the following similar premises of the existential and postmodern approach: the return to subjective experience, non-reductive attitude to the life experience, the ability of free choice, the principle of personal responsibility, the accent on inner sources of the client and the focus on the future, broader context of the interpersonal relationships or the given culture, the absence of the only truth, the accent on the passage of time, the return to the language and the accommodation to the language of the client. On the contrary, both of the directions differ in the theories of cognition, in different focus on the meaning and goal of life, in different conception of emotions and consequent grasping of unpleasant experiences. I percieve the different view on some subjects as incitory, because it offers the opportunity of thinking about things in a new, enriching way., Slabý M., and Literatura
Některé z nich prokázaly částečný funkční a morfologický efekt u laboratorních zvířat. V posledních 20 letech pak byly tyto experimentální postupy aplikovány i v klinických studiích, zatím však bez jednoznačných výsledků. V této práci poskytujeme souhrn současného stavu klinické a především experimentální terapie míšního poranění., Spinal cord injuries still remain incurable in current clinical practice. Standard therapy focuses on early decompression and stabilization of the injured spine together with prevention of a secondary injury. On the other hand, several experimental therapies are being developed in laboratories. Some of these showed partial functional and morphological effect in laboratory animals. Over the last 20 years, some of the experimental therapies have been applied in clinical studies but did not provide unambiguous results. In the present paper, we provide an overview of current clinical and experimental therapies of spinal cord injuries.
