Motivation. Our previous study showed differences in the atherosclerosis phenotype between Lithuanian and Swedish men that could be influenced by complementary factors, namely oxidation processes and/or oxidative stress. The goal of this study was to evaluate the mainstream biological pathways inducing and maintaining the atherosclerotic process by analyzing genetic biomarkers particularly in inflammatory and metabolic pathways where the main focus is laid on the oxidation process. Methods. There were 32 families recruited for the study and clinical as well as biochemical analyses were performed. For genetic analysis 150 SNPs in 89 genes were selected in order to construct a microarray based on Arrayed Primer Extension (APEX) genotyping technology. Genotyping was carried out in 28 families and transmission disequilibrium test (TDT), siblingTDT (STDT), and combined analysis were performed. Results. Clinical and biochemical analysis revealed that probands with premature CAD were more likely to have diabetes mellitus, arterial hypertension, dyslipidemia and were male with high body mass index. Genetic analysis showed six SNPs statistically significantly associated with the atherosclerosis phenotype in the candidate genes ITGA2, IL1B, ALOX5A, OR13G1, MMP9 and NFKB1. These genes belong to different biological pathways: trombocyte adhesion and vessel damage, inflammation response, cholesterol and lypoxygenase metabolic pathway and nutrition. Conclusions. Chronická myelomonocytová leukémia (CMML) je ochorením krvotvornej kmeňovej bunky, ktoré nesie zároveň črty myelodysplázie i myeloproliferácie. Vyskytuje sa relatívne zriedkavo, najmä u starších osôb, a môže prechádzať do akútnej myeloblastovej leukémie. V poslednom období sa pomocou sofistikovaných molekulovo-biologických technológií podarilo určiť, že veľká časť pacientov nesie mutácie v epigenetických regulačných génoch (napr. TET2, ASXL1, …), ktoré modifikujú kondenzáciu chromatínu a v konečnom dôsledku i génovú expresiu. Tieto skutočnosti umožnili zaradenie hypometylačných látok (HMA) do štandardnej liečby CMML a otvárajú bránu pre využitie ďalších molekúl. Cieľom príspevku je ukázať na konkrétnych príkladoch súčasné možnosti diagnostiky a terapie CMML a tiež zmieniť niektoré úskalia spojené s využitím nových postupov., Chronic myelomonocytic leukemia (CMML) is a hematopoietic stem cells disorder, it carries myelodysplastic and myeloproliferative features. CMML occurs rarely, especially in the elderly, and often transforms into acute myelogenous leukemia. Recently, in a large proportion of patients sophisticated molecular-biology technologies can determine mutations in epigenetic regulatory genes (e.g. TET2, ASXL1, …) that modify chromatin condensation and also gene expression. These facts support the inclusion of hypomethylating agents (HMA) in the standard treatment of CMML and opened the gates for the use of other new molecules. Neurologické postižení je poměrně vzácná komplikace hepatitidy E. Uvedená kazuistika popisuje 52letého imunokompetentního muže s aseptickou meningitidou a mnohočetným postižením periferních nervů na horních končetinách typu mononeuropatia multiplex, u kterého byla sérologickým vyšetřením prokázána akutní virová hepatitida E. Navíc byl virus hepatitidy E zjištěn polymerázovou řetězovou reakcí v séru i likvoru (genotyp 3f). Pacient byl léčen pouze symptomaticky, po půl roce přetrvával mírný neurologický deficit. Infekci virem hepatitidy E je vhodné zvažovat u pacientů se současným neurologickým a jaterním postižením., Neurological complications of acute hepatitis E are relatively rare. The case report describes mononeuropatia multiplex with aseptic meningitis in a 52‑year‑old immunocompetent man. Viral hepatitis E was confirmed serologically and by polymerase chain reaction from the blood and cerebrospinal fluid (genotype 3f). Symptomatic treatment was given. Residual neurological deficit persisted six months after the onset of the disease. Diagnostic of hepatitis E should be considered in patients with concurrent neurological symptoms and liver cytolysis. Autoři prezentují případ 73leté ženy léčené v posledních 15 letech pro arteriální hypertenzi diuretiky (tiazidy, amiloridem a indapamidem) a současně antidepresivem trazodonem. Po 6 letech této léčby začala pacientka trpět manifestní osteoporózou s vícečetnými frakturami i po banálním traumatu a současně byla po celou tuto dobu dokumentována těžká hyponatremie bez adekvátní suplementace. Osteoporóza a hyponatremie u této pacientky spolu s největší pravděpodobností těsně souvisely – autoři diskutují možné patofyziologické mechanizmy na úrovni renálních tubulů, osteoblastů a osteoklastů, hypofyzárně-skeletální osy, syndrom neadekvátní sekrece adiuretinu (SIADH) a možný vliv porušené acidobazické rovnováhy., The authors present a case-report of a 73years old woman treated for arterial hypertension within past 15 years with diuretics (thiazides, amiloride and indapamide) and concomitantly with antidepressive trazodone. The patient developed severe osteoporosis after 6 years of such a treatment with multiple fractures even after minor trauma; during whole this period severe hyponatremia without adequate supplementation was documented.
