Takotsubo kardiomyopatie je náhle vzniklé onemocnění charakterizované reverzibilní poruchou kinetiky levé srdeční komory. V typickém případě se rozvíjí následkem silného stresového podnětu a prezentuje se symptomy, EKG a laboratorním nálezem, které připomínají akutní koronární syndrom. Od toho se liší nepřítomností onemocnění koronárních tepen a ve většině případů nekomplikovaným průběhem s následnou velmi dobrou dlouhodobou prognózou. V kazuistice popisujeme případ 63leté ženy, u které se počátek onemocnění projevil fibrilací komor. Díky okamžité kardiopulmonální resuscitaci a následné odborné péči ve specializovaném kardiocentru, která mimo jiné zahrnovala implantaci implantabilního kardioverteru-defibrilátoru v rámci sekundární prevence náhlé srdeční smrti, se pacientka zotavila bez jakýchkoliv následků. Takotsubo kardiomyopatie je důležitou jednotkou v diferenciální diagnóze akutního koronárního syndromu. I přes dobrou prognózu, kterou se vyznačuje ve většině případů, ji nelze podcenit a zaslouží si náležitou pozornost a léčbu, která umožní předcházet případným komplikacím., Takotsubo cardiomyopathy (TTC) is an acute syndrome characterized by rapid onset of transient systolic dysfunction of the left ventricle. Symptoms, ECG and laboratory findings resemble acute coronary syndrome, from which TTC differs by the absence of coronary artery disease. In typical cases, TTC is triggered by exposure to unexpected stress and the clinical course and prognosis are very good. We present a case of a 63-year-old woman, in whom the onset of the disease was complicated by ventricular fibrillation. The patient recovered without sequelae thanks to immediate cardiopulmonary resuscitation and following care in a specialized cardiocenter, which involved also implantation of implantable cardioverter defibrillator to prevent sudden cardiac death. There are three basic procedures used for an assessment of the electrical heart field from the body surface: standard electrocardiography, vectorcardiography, and body surface potential mapping (BSPM). BSPM has two major advantages over both other methods: 1) it allows exploring the entire chest surface, thus providing all the information on the cardiac electric field available at the body surface; 2) it is more sensitive in detecting local electrical events, such as local conduction disturbances or regional heterogeneities of ventricular recovery. Nevertheless the results obtained using BSPM procedure cannot answer all questions about real causality of detected changes of the electrical heart field. We tried therefore to use mathematical model of the electrical field in order to answer these questions. A simple and anatomical forward calculation model was used to test the hypothesis whether the altered position of the heart could explain heterogeneity of repolarization at late stages of pregnancy in humans. Our aim was to test the hypothesis that the occurrence of extrasystoles in higher decennia is proportional to the altitude. The occurrence of supraventricular (SVPB) and ventricular (VEB) extrasystoles, values of systolic and diastolic blood pressure and the heart rate were studied in 20 healthy elderly men (50-64 years) during cable cabin transportation to a moderate altitude. These values were measured in stations located at 898 m, 1764 m, and 2632 m above sea level during the transportation in both directions. Our records show that the values of blood pressure and heart rate were within normal limits during the whole period of transportation. Both SVPB and VEB were increasing during the ascent and decreasing to the initial values during the descent compared to the values at altitude of 898 m. The highest values (6 to 7-times exceeding the initial ones) were measured at the summit. The results have demonstrated that the occurrence of SVPB and VEB is proportional to the altitude. V článku je představen systém pro mnohokanálové měření a povrchové mapování EKG potenciálů a možnosti jeho využití při diagnostice ischemické choroby srdeční. Systém může používat až 128 aktivních EKG elektrod. Diabetes mellitus (DM) has been known for many years to be associated with poor cardiovascular prognosis. Due to the sensitive neuropathy, the coronary artery disease in diabetic patients is frequently asymptomatic. Also twelve leads resting ECG can be within normal limits even in an advanced stage of coronary artery disease. Therefore in addition to the standard ECG other electrocardiographic procedures started to be studied in order to find some typical signs of myocardial damages caused by DM. Repeatedly reported results showed in DM patients without cardiovascular complications the tachycardia, shortening of the QRS and QT intervals, increase of the dispersion of QT interval, decreased amplitudes of depolarization waves, shortened activation time of ventricular myocardium and a flattening of T waves confirmed by the lower value of maximum and minimum in repolarization body surface isopotential maps. Most of these changes are even more pronounced in patients with cardiac autonomic neuropathy. We studied the ability of the ECG to detect pathological changes in isoproterenol-induced remodeling of rat heart. Myocardial hypertrophy in rats was induced by repeated injections of isoproterenol (5 mg/kg s.c. 7 days, Iso5, n=7). Single overdose of isoproterenol (150 mg/kg s.c., Iso150, n=7) evoked myocardial infarction followed with ventricular remodeling. The electrocardiograms were recorded in anesthetized animals (thiopenthal 45 mg/kg i.p.) and myocardial contractile performance was analyzed in isolated hearts perfused according to Langendorff. The hypertrophic hearts were characterized by increased heart and left ventricular (LV) weight as well as by thicker LV free wall and interventricular septum. Mean values of LV contraction did not significantly differ from controls. Longer QT interval, QRS complex, negative Q and S waves, higher R amplitude were typical characteristics for Iso5 rats. Iso150 animals showed tendency to decreased systolic blood pressure and heart frequency. Decrease in the thickness of LV compared to Iso5 as well as impaired LV function were related to the dilated left ventricle. Iso150 ECG showed longer QRS and QT, deepened negativity of S wave and mild decrease of RII compared to Iso5. Terapie akutní fáze ischemické cévní mozkové příhody (CMP) je zaměřena jednak bezprostředně na omezení rozsahu poškození pomocí co možná nejčasněji zahájené reperfuzní strategie a dále nastavení léčby zajišťující farmakologickou sekundární prevenci recidivy onemocnění. Klíčový předpoklad správné a efektivní sekundární prevence představuje včasné stanovení příčiny ischemického iktu, protože riziko recidivy iktu je nejvyšší v prvních týdnech po prodělané CMP. Nedílnou součástí managementu pacienta s ischemickým iktem je tedy vyloučení potenciálních kardiálních a cévních zdrojů embolizace do mozkové cirkulace pomocí zobrazovacích metod. Hypertrofická kardiomyopatie (HCM) je nejčastější geneticky podmíněné srdeční onemocnění s prevalencí 1 : 500. Ve familiárních případech je dědičnost autozomálně dominantní s neúplnou penetrancí a různou expresivitou, může ovšem vznikat i de novo mutacemi. Nesmírná heterogenita projevů i prognózy HCM případ od případu značně komplikuje klinický management, a proto toto onemocnění stále často přináší mnoho otázek nejen pro lékaře prvního kontaktu, ale i pro kardiology. Stanovení rizika náhlé srdeční smrti u pacientů s HCM je integrální součástí klinického managementu a doporučení pro stratifikaci se neustále vyvíjí. První zkušenosti s ablací fokálního spouštěče fibrilace komor byly popsány u ojedinělých případů idiopatické fibrilace komor. Později byla popsána možnost zvládnutí elektrické bouře spouštěné ektopickou aktivitou u pacientů po infarktu myokardu. V obou případech bylo prokázáno, že zdroje extrasystol pocházejí téměř výlučně z převodního systému komor. Později byly popsány izolované případy i u dalších strukturních onemocnění. Důležité je, že urgentní katetrizační ablace dokáže odstranit spouštěcí fokus a přerušit tak elektrickou bouři. V mnoha případech jde o život zachraňující výkon. Tento přehled shrnuje současné znalosti o katetrizační ablaci fokálně spouštěné fibrilace komor u strukturního onemocnění srdce., First experiences with ablation of focus triggering ventricular fibrillation were reported in isolated cases of idiopathic ventricular fibrillation. Later, there were described the options in management of an electrical instability triggered by ectopic activity in patients after myocardial infarction. In both cases it was shown that the sources of extrasystoles originate almost exclusively from conducting system of chambers. Subsequently, isolated cases were also reported in other structural diseases. It is important that the urgent catheter ablation is able to remove focus which triggers electric instability. Cíl: Prospektivní sledování pacientů po prodělané akutní myokarditidě s cílem posoudit přínos a bezpečnosti pravidelného sledování pacientů pomocí magnetické rezonance (MR), laboratorních hodnot kardiospecifických enzymů a bicyklové ergometrie s ohledem na možné zkrácení doporučovaného přísného klidového režimu. Metodika: U 21 pacientů s diagnostikovanou akutní myokarditidou (včetně přítomnosti pozdního sycení - LGE při vstupní MR) byla v intervalu 1 a 6 měsíců provedena kontrolní MR, stanovení hladiny hypersenzitivního troponinu I (hs-Tn) a bicyklová ergometrie. Obrazová analýza byla zaměřena na vývoj rozsahu a intenzity LGE lézí v MR, výsledky byly porovnány s vývojem hs-Tn a výsledku bicyklové ergometrie po 1 měsíci. Výsledky: Předběžné výsledky studie prokázaly, že u všech pacientů byla prokázána regrese rozsahu i intenzity LGE při kontrolních vyšetřeních a zároveň nebyl prokázán rozvoj systolické dysfunkce levé komory srdeční u žádného pacienta. Tento vývoj korespondoval s vývojem průměrné hodnoty hs-Tn (z 9214,2 µg/1 na 25,4 µg/1). Při bicyklové ergometru nebyly prokázány závažné arytmie a všichni pacienti prokázali dobrou toleranci fyzické zátěže (minimálně 150 W). Závěr: Předběžné výsledky naší studie ukazují, že kombinace magnetické rezonance, hodnoty hs-Tn a bicyklové ergometrie je bezpečná a přínosná pro sledování pacientů po prodělané akutní myokarditidě. Důkladná analýza provedených výše uvedených vyšetření v delších intervalech je nezbytná pro možnou stratifikaci rizika u pacientů i s ohledem na délku doporučeného klidového režimu., Aim: Prospective study of patients following an acute myocarditis in order to assess the benefits and safety of regular monitoring using MRI, laboratory values of cardiac enzymes and exercise ECG. Methods: 21 patients with acute myocarditis (diagnosis including the presence of late gadolinium enhancement - LGE at acute MR) underwent in 1 and 6 months interval a follow-up MRI, level of hypersensitive tro-ponin I (hs-Tn) and the exercise ECG. Image analysis was focused on the development of LGE extent and intensity, results were compared with the development of hs-Tn and exercise ECG in 1 month follow-up. Results: Preliminary results of the study prove regression of the extent and intensity of LGE in all patients and normal left ventricular systolic function in all patients in 1 and 6 months follow-up. This trend corresponds with the development of the average value of hs-Tn (from 9214.2 µg/1 to 25.4 µg/1). During exercise ECG severe arrhythmia was not identified and all patients showed good tolerance of physical burden (at least 150 W). Conclusion: Preliminary results of our study indicate that the combination of MR, the value of hs-Tn and exercise ECG is safe and beneficial for follow-up of patients after acute myocarditis. Monophasic action potential (MAP) recording plays an important role in a more direct view of human myocardial electrophysiology under both physiological and pathological conditions. The procedure of MAP measuring can be simply performed using the Seldinger technique, when MAP catheter is inserted through femoral vein into the right ventricle or through femoral artery to the left ventricle. The MAP method represents a very useful tool for electrophysiological research in cardiology. Its crucial importance is based upon the fact that it enables the study of the action potential (AP) of myocardial cell in vivo and, therefore, the study of the dynamic relation of this potential with all the organism variables. This can be particularly helpful in the case of arrhythmias. There are no doubts that physiological MAP recording accuracy is almost the same as transmembrane AP as was recently confirmed by anisotropic bidomain model of the cardiac tissue. MAP recording devices provide precise information not only on the local activation time but also on the entire local repolarization time course. Although the MAP does not reflect the absolute amplitude or upstroke velocity of transmembrane APs, it delivers highly accurate information on AP duration and configuration, including early afterdepolarizations as well as relative changes in transmembrane diastolic and systolic potential changes. Based on available data, the MAP probably reflects the transmembrane voltage of cells within a few millimeters of the exploring electrode. Only limited data are available on body surface potential distribution during atrial activation. The aim of this study was to establish the distributions and to analyze chosen quantitative parameters of atrial isointegral maps recorded using a limited 24-lead system in a young healthy population. A total of 166 subjects underwent a procedure of body surface potential mapping. Isointegral maps during the P wave were constructed and qualitatively and quantitatively evaluated. Three types of atrial activation in individual maps were found according to the different shape of the zero isointegral line and to mutual positions of extrema. The most frequently occurring type resembled the group mean maps and was in good agreement with published data obtained from full lead systems. The highest extrema were found in the young men group, while, surprisingly, the lowest values in the young women group. All minima and the majority of maxima were recorded outside the ranges of standard chest leads. The usefulness of the limited lead system to record isointegral P wave maps was shown and new data were presented that can be useful in noninvasive evaluation of atrial pathologies., K. The 24-hour periodicity of supraventricular (SVPB) and ventricular (VEB) extrasystoles in healthy elderly men (age 49-69 years) was studied at two altitudes during 24 h Holter ECG monitoring. At the low altitude (200 m, n = 26), SVPB were more frequent than VEB. The highest occurrence of SVPB was at 17:00 h, the lowest at 01:00 and 02:00 h (P<0.001). The highest occurrence of VEB was at 09:00 h, the lowest one at 04:00 h (P<0.001). At 1350 m (n=9) the incidence of both SVPB and VEB was approximately twofold higher compared to that at the low altitude (P<0.001). The highest occurrence of SVPB was at 13:00 h, the lowest at 06:00 h (P<0.001). VEB were the most frequent at 10:00 h and 13:00 h, while the lowest frequency was observed at 06:00 h (P<0.001). Our results indicate that the incidence of SVPB and VEB in healthy persons at the moderate altitude is twofold and its periodicity is shifted compared to the low altitude. Here we report the case of a 31-year-old man admitted to our hospital with echocardiografic and Cardiac Magnetic Resonance signs of myocarditis complicated by ventricular tachycardia, initially resolved with direct current shock. After the recurrence of ventricular tachycardia the patient was submitted to electrophysiological study revealing a re-entrant circuit at the level of the medium segment of interventricular septum, successfully treated with transcatheter ablation. Ruptura aneuryzmatu Valsalvova sinu je vzácné, ale závažné srdeční onemocnění, které může ohrozit pacienta na životě. Včasná diagnostika a správná léčba může výrazně ovlivnit prognózu pacienta. V naší kazuistice popisujeme případ 44letého muže přijatého na Interní kardiologickou kliniku FN Brno pro typické bolesti na hrudi s nespecifickými změnami při elektrokardiografickém vyšetření a s pozitivním troponinem T. Vstupní transthorakální echokardiografické vyšetření odhalilo aneuryzma nekoronárního sinu a komunikaci mezi aortou a pravou síní, která byla potvrzena i CT angiograficky. Pacient byl poté odeslán do kardiochirurgického centra, kde byla následně provedena resekce aneuryzmatu a uzavřena komunikace mezi aortou a pravou síní záplatou z perikardu. Při vzniku aneuryzmatu nelze vyloučit podíl metabolického účinku kortikoidů, které pacient dlouhodobě užíval. Z uvedené kazuistiky plyne, že pečlivé klinické vyšetření a neinvazivní vyšetřovací metody zůstávají stále základním diagnostickým postupem. Early diagnosis and proper therapeutic treatment can significantly affect the patient’s prognosis. We describe the case of a 44‑year–old man admitted to the Cardiology Department of the University Hospital Brno with typical chest pain and non‑specific changes on electrocardiogram and positive troponin T. Transthoracic echocardiography revealed a non‑coronary sinus aneurysm and communication between the aorta and the right atrium, which was confirmed by computed tomography. The patient was referred to the cardiothoracic surgery centre and the aneurysm was resected. The communication between the aorta and the right atrium was closed via pericardial patch. The aneurysm formation can be caused by the metabolic effects of corticosteroids which the patient had been using for a long time. This case report suggests that careful clinical examination and noninvasive methods remain an essential diagnostic procedure. Úvod: Takotsubo kardiomyopatie (takotsubo cardiomyopathy – TCM) je vzácné srdeční onemocnění charakterizované v akutní fázi přechodnou dysfunkcí levé komory, které může být komplikováno fibrilací komor nebo srdečním selháním. V diferenciální diagnostice musí být vždy vyloučen akutní infarkt myokardu. Cílem práce je prezentovat charakteristiku pacientů s TCM, jejich jednoletou prognózu a přínos magnetické rezonance k diagnostice těchto pacientů v klinické praxi. Metodika: Vyhodnoceno bylo celkem 47 pacientů s prokázanou TCM. U všech pacientů byla vyloučena významná ateroskleróza koronárních tepen pomocí selektivní koronární angiografie. Celkem u 10 pacientů (21,3 %) bylo provedeno vyšetření magnetickou rezonancí (MRI) srdce. Pacienti byli prospektivně sledováni minimálně 12 měsíců. Výsledky: Soubor pacientů s TCM tvořily z 89,4 % ženy, průměrný věk pacientů byl 62,3 ? 11,1 let. Třicet devět pacientů (83,0 %) mělo apikální a 8 midventrikulární formu TCM. Zjevný provokující stresový faktor jsme identifikovali u 18 pacientů (38,3 %). Časté byly další komorbidity, zejména onemocnění štítnice, psychiatrická, plicní nebo alergická onemocnění. U 42 pacientů (89,4 %) byla přítomna dysfunkce levé komory, 4 pacienti (8,5 %) byli v kardiogenním šoku. Čtyři pacienti (8,5 %) prodělali během hospitalizace fibrilaci komor. Roční mortalita v našem souboru činila 14,9 %, jedna pacientka (2,1 %) prodělala v průběhu sledování recidivu ataky TCM. Závěr: Přestože u pacientů s TCM dochází k úpravě funkce levé komory, akutní fáze může být komplikována rozvojem kardiogenního šoku nebo fibrilací komor. U části pacientů je indikováno vyšetření MRI k vyloučení ischemické nebo zánětlivé etiologie., Introduction: Takotsubo cardiomyopathy (TCM) is a rare cardiac disease. In the acute phase it may imitate acute myocardial infarction of the anterior wall with ST elevations. This can be complicated by ventricular fibrilation or heart failure. The aim of this study is to show the characteristics of patients with takotsubo cardiomyopathy, the one year outcome and the application of magnetic resonance imaging of the heart in the diagnosis of this disease. Methods: Forty seven patients with takotsubo cardiomyopathy were enrolled in this study. Severe stenosis of coronary arteries was excluded by selective coronarography. In twelve patients the magnetic resonance imaging of the myocardium was performed. The patients were observed for at least 12 months. Results: 89.4 % of takotsubo patients were women. Average of age was 62.3 ? 11.1 years. We found obvious stress factors in 18 patients (38.3 %). Many patients suffered from thyroid disorders, psychiatric, lung or allergic diseases. Thirty nine patients (83.0 %) had the apical and eight the midventricular type of takotsubo cardiomyopathy. In 42 patients (89.4 %) developed systolic left ventricular dysfunction and four patients (8.5 %) were in cardiogenic shock. Four patients (8.5 %) manifested ventricle fibrilation. One patient (2.1 %) had recurring takotsubo cardiomyopathy. Conclusion: Left ventricle function is mostly recovered in patients with takotsubo cardiomyopathy, but the acute phase can be complicated by cardiogenic shock or ventricular fibrilation. The long-term electrocardiographic recording with retrospective evaluation (Holter system) has been widely used not only in cardiology, but also in other disciplines of internal medicine and in pharmaceutical research. The Holter system can be used in mini-pig, sheep, dog, cat, rabbit, ferret, and rat. In this paper hardware, software, and anesthesia requirements are summarized with respect to the experimental work with various species. As the Holter systems work in bipolar mode, the use of bipolar leads in sagittal and transversal planes has been proved to be the most appropriate because of large amplitude of QRS complex and uncomplicated consequent automatic analysis of the record. Aims of the study were to compare the development of electrocardiographic responses of the ischemia-induced heterogeneities of activation and repolarization in the ventricular myocardium of normal and diabetic animals. Body surface ECGs and unipolar electrograms in 64 epicardial leads were recorded before and during 20 min after the ligation of the left anterior descending artery in diabetic (alloxan model, 4 weeks, n=8) and control (n=8) rabbits. Activation times (ATs), end of repolarization times (RTs) and repolarization durations (activation-recovery intervals, ARIs) were determined in ischemic and periischemic zones. In contrast to the controls, the diabetic rabbits demonstrated the significant prolongation of ATs and shortening of ARIs (P<0.05) during ischemia in the affected region resulting in the development and progressive increase of the ARI and RT gradients across the ischemic zone boundary. The alterations of global and local dispersions of the RTs in diabetics correlated with the Tpeak-Tend interval changes in the limb leads ECGs. In the ischemic conditions, the diabetic animals differed from the controls by the activation delay, significant repolarization duration shortening, and the increase of local repolarization dispersion; the latter could be assessed by the Tpeak-Tend interval measurements in the body surface ECGs., K.
