Text vykládá jednotlivé pojmy, které v Aristotelově díle představují kandidáty na protějšek moderního pojmu zdravého rozumu, a uvádí je do vzájemné souvislosti. Především v logických a rétorických textech Aristotelés pracuje s termínem endoxos, který používá pro výroky a osoby, které jsou v daném společenství uznávány. V epistemologii pracuje s termínem koiné aisthésis v souvislosti s vnímáním jako takovým. Autor poukazuje na vazbu, kterou u Aristotela díky jeho práci ve fyziologii má obecná schopnost vnímat třetí klíčový pojem, který patří do etiky, totiž na fronésis. V rámci výkladu fronésis má místo také poslední bezprostředně relevantní pojem – orthos logos. Aristotelova koncepce jednání, jež Aristotelés vykládá někdy pomocí tzv. praktického sylogismu, pak ukazuje na roli uznávaných výroků jako typických obecných premis v těchto sylogismech a roli osob uznávaných pro svou fronésis jako měřítek určujících kritéria pro to, co je dobré jednání., The text interprets the particular concepts in Aristotle’s work which present themselves as candidates for being the counterpart to the modern concept of common sense, and it introduces them in their mutual relatedness. Zdůvodňuje to lidskými právy a demografickou situací. Článek je analýzou vyslovených důvodů a možných konsekvencí. Text volně navazuje na moje dvě předchozí analýzy v minulých číslech ČZPB 2016, první řešící záměr rozšířit platbu ART zdravotními pojišťovnami a legislativně ukotvit náhradní mateřství, druhá zaměřená na věkové limity pro samoplátce. and In recent months, Czech Ministry of Labour and Social Affairs repeatedly informs the public of its intention to make assisted reproduction techniques (ART) to single persons, reasoning that human rights and demographic situation. The article is an analysis of reasons expressed by the Ministry and possible consequences of the steps. Text loosely builds on my two previous analyzes published in ČZPB 2016, the first dealing with the intention to extend the health insurence payment of ART and to anchore legislatively surrogacy in CR. The other analysis is focused on age limits for ART at private patients.
The article examines the meaning of the other for Kant's idea of autonomy. Autonomy is interpreted, in relation to the universal demand of the ethical, as governing the will by principles. Autonomy as principled self-determination by means of the practical law cannot be understood as the standpoint of an isolated subject. Instead we must understand it as a standpoint taken towards others, which we treat as the aspect of spontaneity, and at the same time as a standpoint taken thanks to others, since others make possible its awakening and development - here the aspect of receptivity is discussed. Cieľ: Cieľom štúdie bolo preskúmať, ako sestry vnímajú svoju úroveň dôstojného prístupu k pacientom v domovoch sociálnych služieb (DSS) a identifikovať, či existujú významné vzťahy medzi úrovňou dôstojného prístupu k pacientom v DSS a demografickými znakmi (vek sestier, dĺžka praxe, vzdelanie a pod.). Metodika: Výskumný súbor tvorilo 106 sestier pracujúcich v domovoch sociálnych služieb v zriaďovateľskej pôsobnosti Trnavského samosprávneho kraja. Výskum prebiehal v období december 2011 až február 2012. Na zistenie úrovne dôstojného prístupu sme využili autorizovaný dotazník: Posudzovanie etických problémov – dôstojný prístup k pacientovi. Tvorí ho 5 subškál (informovanosť pacientov; autonómia; dodržiavanie dôvernosti a ochrany informácií; princíp spravodlivosti; intimita). Štatistická analýza bola realizovaná softvérom SPSS, verzia 15.0. Výsledky: Svoju úroveň dôstojného prístupu k pacientom v domovoch sociálnych služieb hodnotili sestry ako primeranú. Medzi vnímanou úrovňou dôstojného prístupu sestier a demografickými údajmi boli nízke korelácie. Signifikantne vyššiu úroveň dôstojného prístupu sme zistili u sestier s vyšším vzdelaním (Bc.). Najnižšiu úroveň dôstojného prístupu k pacientom v domovoch sociálnych služieb sme zaznamenali v subškále zameranej na autonómiu pacienta. Záver: Na základe poznania týchto výsledkov môžeme podporovať zmeny, ktoré budú prispievať k zvýšenej kvalite poskytovanej ošetrovateľskej starostlivosti v domovoch sociálnych služieb., Aim: The aim of the study was to examine nurses’ perception of their own level of dignified approach to patients in social care homes and to identify potential statistically significant relationships between the level of dignified approach to patients in social care homes and demographic data (nurses’ age, duration of clinical experience, education etc.). Methods: The sample consisted of 106 nurses working in social care homes run by the Trnava Region. An authorized questionnaire called Assessment of Ethical Issues – Dignified Approach to a Patient was used to determine the level of dignified approach. The questionnaire consisted of five subscales (patient awareness; autonomy; respecting confidentiality; principle of justice; intimacy). Statistical analysis was carried out using the SPSS version 15.0 software. Results: The level of dignified approach to patients in social care homes was self-assessed by nurses as adequate. There was a low correlation between the perceived level of dignified approach to patients and demographic data. Nurses with higher education (bachelor’s degree) demonstrated significantly higher levels of dignified approach. The lowest level of dignified approach was found in the subscale focused on patient autonomy. Medicína je specifickým oborem, který čerpá nejen z empirických poznatků přírodních věd, ale též z principů věd humanitních, i skutečnosti, že zdraví a nemoc nejsou jen kategorie výlučně přírodovědné, ale i hodnotové a proto je nutno zachovávat proporcionalitu mezi oprávněnými požadavky biomedicínského výzkumu a etickými nároky na něj a mezi morálními imperativy lékařské praxe. To je právě prostor pro etiku a etický přístup k řešení nejasných nebo sporných otázek našeho konání, otázek veřejností dnes velmi citlivě vnímaných, často vedoucích ke ztrátě důvěry nejen ve zdravotníky a zdravotnictví, ale i v celý společenský systém. Principy etického rozhodování v medicíně, hygienu a epidemiologii nevyjímaje, spočívají na dvou základních předpokladech: prvým je profesionální etika odpovědnosti, která zajišťuje kvalitu péče s vysokým stupněm odbornosti lékaře při poskytování pomoci a zábraně možných škod (primum non nocere) a druhým je právo na sebeurčení (autonomie jedince) s právem na informovanost - to je souhlas jedince informovaného pro něj srozumitelným způsobem nejen o plánovaném diagnostickém, léčebném a příslušném preventivním opatření, ale i v případě epidemiologických studií vztahu prostředí a zdraví jeho informovaného souhlasu k jeho účasti ve studii, jejíž by měl být subjektem. Článek se zabývá metodickými otázkami etických přístupů ke zdravotním problémům s důrazem na hromadně se vyskytující infekční a chronické nesdělné nemoci z hlediska šancí jejich prevence., Medicine is a specific area that draws not only on empirical knowledge of sciences but also on principles from the humanities, and the fact that health and disease are not only exclusively categorised as natural science but as value based entities where it is necessary to maintain proportionality between the legitimate requirements of biomedical research and ethical requirements and the moral imperatives of medical practice. It is here that we find the niche for ethics and ethical approaches to solving unclear or disputed issues of our actions; questions that are very sensitively perceived by the public, often leading to a loss of confidence in both health care professionals and the health system but also in the whole social system as such. Ethical principles in medicine, including hygiene and epidemiology are based on two basic assumptions: the first is the professional ethic of responsibility, which ensures quality of care with a high degree of expertise from assisting physicians and prevention of possible harm (primum non nocere) and the second is the right to self-determination (individual autonomy) with the right to be informed. It likewise comprises informed consent based on clear information regarding planned diagnostic procedures, therapeutic and preventive measure and informed consent regarding epidemiological studies in which patient's cooperation is requested. Cíl: Cílem výzkumu bylo popsat zkušenosti hospitalizovaných dětí (adolescentů), doprovodu hospitalizovaných dětí předškolního a mladšího školního věku a všeobecné sestry s etickými problémy při hospitalizaci dítěte na standardním dětském lůžkovém oddělení. Metodika: Byl proveden kvalitativní výzkum. Pro sběr dat byl použit nestandardizovaný rozhovor. Výzkumný soubor tvořilo osm informátorů, osoby doprovázející dítě při hospitalizaci (n = 5), hospitalizované děti v adolescentním věku (n = 2) a dětská sestra (n = 1). Na základě kvalitativní analýzy dat byly vytvořeny 4 kategorie (1. informovaný souhlas, 2. přítomnost blízkého člověka, 3. srozumitelné informace a 4. péče sester), které byly dále členěny do podkategorií. Výsledky: Mezi nejčastější etické problémy při hospitalizaci dítěte na standardním lůžkovém oddělení respondenti uvedli: problémy s informovaným souhlasem v neúplných rodinách, nedostatečnou informovanost o nemoci a zdraví dítěte, deficit v oblasti edukace, nedostatek komunikace se sestrou a nedostatečnou péči. Adolescenti považovali za etický problém při hospitalizaci pocit malého respektování vlastní autonomie u rozhodování o sobě sama, potřebu většího soukromí a respektu k pocitům studu. Závěr: Většině etických problémů lze předcházet zvládnutím efektivní komunikace a dostatečné informovanosti a to již před hospitalizací dítěte v ambulanci praktického lékaře pro děti a dorost, případně v odborné ambulanci., Objective: The aim of the research was to describe the experience of hospitalized children (adolescents), accompaniment of hospitalized children preschool and younger school age and general nurses with ethical problems during the hospitalization of children on the standard children's ward. Methods: Qualitative research was conducted. For data collection was used nonstandardized interview. The research group consisted of eight informants, persons accompanying the child during hospitalization (n = 5), hospitalized children in adolescence age (n = 2) and child nurse (n = 1). Based on qualitative data analysis were created four categories (1. informed consent, 2. the presence of a loved one, 3. understandable information 4. care of nurses ), which were further divided into subcategories. Results: The most common ethical problems in the child's hospitalization on the standard ward respondents were stated: problems with informed consent in single-parent families, lack of information about the child's disease and child's health, the deficit in the area of education, lack of communication with the nurse and lack of care. Adolescents considered as the ethical problem during hospitalization the feeling of little respect for autonomy in deciding of themselves, the need for greater privacy and respect for the feelings of shame. The well-known book by Peter Singer The Liberation of Animals has not only inspired a series of texts defending the rights and interests of animals, but has also provoked a discussion about what humanity is, what meaning can our belonging to the human kind have for us, and whether Singer’ critique of the “human prejudice” is justified. The paper considers two important defenders of “human prejudice”, B. A. O. Williams and C. Diamond, who both claim the concept of human being to be a basic ethical concept. In the first part, we will present Williams’s argument that solidarity and identity with one’s species doesn’t have the structure of a blameworthy privilege similar to sexism and racism. In the second part, we will proceed to Diamond’s conception of human being that is founded in relations and responses towards the other. Just as our treatment of a human being depends on whether we see this person as our fellow, so our treatment of an animal depends on how we see it. A solution of the article (called „Ethics in the medical research with a present of human subjects from the point of view of medical and philosophical“) is to show some chosen aspects which relate crosswise to ethics in medical search. They are: Hippocratus oath as a deontological codex, a definition lege artis, bioethics and forming a structure of a relation between the doctor and the patient.
Health (medical) ethics is considered for a normative ethics. Its normativing comes out from settled rules and instructions which are inseparably connected with an application of inside morality in the area of medicine. Normative ethics tries to find adequate answers for questions – What is good? What´s bad? What are moral ideals and values of everything. What is right and human? What norms are closely connected with some feelings of duty, responsibility, accurancy, incorrectness, inclination, esteem, regret…? Generaly speaking, the whole ethics (not only health/medical ethics) stands and falls down on the principle, that there is a great need in the connection to other person to do only what is good (optimal, necessary) and delay what is bad (unoptimal, unnecessary).
The doctor´s ethics, but ethics of other health service assistants, comes out from a relationship „Me“ and „the other.“ It is so called relationship of a helping man and a suffearing one. The doctor takes care of an ill person, tries to ofer (if it is possible) the best solution of his/her situation. A great attention deserves the doctor´s moral obligation, to act with a patient as a man who is in a difficult life situation. Except of this, the doctor is obliged to respect principles that correspond patient´s health disease. Realization of the disease is quite large problem of the patients.
The emphasis of delicacy and complexity of doctor´s and patient´s respects are the aim of the article. The doctor and the patient are the main subjects who stand in the centre of view of health (medical) ethics. The patient, as a subject of a medical search, always and at any circumstances distributes himself or herself. The doctor is admittedly determining element of his/her disease, tries to make an ill patient to self – filling of the basic cure rules, but first of all it is the patient who primarily decides about his/her health.
The doctors have a large privilege – and that is – to help people. The patients should believe the doctors and they should turn to them in their health problems. and Východiskem článku (s názvem „Etika v lékařském výzkumu za účasti lidských subjektů z pohledu lékařského a filozofického“) je poukázat na vybrané aspekty, které se napříč vztahují k etice v lékařském výzkumu. Jsou to: Hippokratova přísaha jako deontologický kodex, definice lege artis, bioetika a strukturování vztahu lékaře a pacienta.
Zdravotnická (lékařská) etika je považována za normativní etiku. Její normativnost vychází z pevných zásad a instrukcí, které jsou nerozlučně spojeny s aplikací vnitřní mravnosti v oblasti medicíny. Normativní etika se neustále snaží nalézat vhodné odpovědi na otázky – Co je dobré? Co je zlé? Jaké jsou mravní ideály a hodnoty všeho správného a humánního? Jaké normy jsou úzce spojeny s pocity povinnosti, odpovědnosti, správnosti, nesprávnosti, náklonnosti, úcty, lítosti…? Obecně se dá říci, že celá etika (nejenom zdravotnická/lékařská etika) stojí a padá na zásadě, že je potřeba ve vztahu k druhému člověku konat jen to, co je dobré (optimální, žádoucí), a zdržet se toho, co je zlé (neoptimální, nežádoucí).
Etika lékaře, ale i dalších zdravotnických pracovníků, vychází ze vztahu „Já“ a ten „druhý.“ Jedná se o vztah mezi člověkem pomáhajícím a člověkem trpícím. Lékař o nemocného člověka pečuje, snaží se mu nabídnout (pokud je to možné) co nejlepší řešení jeho situace. Velkou pozornost zasluhuje i lékařův morální závazek jednat s pacientem jako s člověkem, který se ocitl v obtížné životní situaci. Kromě toho lékaře morálně zavazuje i to, že musí plně respektovat zásady, které odpovídají povaze pacientova onemocnění. Uvědomění si nemoci bývá u pacientů dosti velkým problémem.
Cílem článku je zvýraznění křehkosti a komplikovanosti vztahu mezi lékařem a pacientem. Lékař a pacient jsou hlavními subjekty, které jsou středem pozornosti zdravotnické (lékařské) etiky. Pacient jako subjekt lékařského výzkumu vždy a za všech okolností rozvrhuje sám sebe. Lékař je sice rozhodujícím činitelem v ekvivalenčním vztahu, působí na nemocného, snaží se nemocnému vysvětlit závažnost jeho onemocnění, pokouší se nemocného přimět k samovolnému plnění základních léčebných pravidel, ale je to především pacient, který primárně rozhoduje o svém zdraví.
Lékaři mají jedno obrovské privilegium – a to takové, že se snaží pomáhat lidem. Pacienti by měli lékařům důvěřovat a měli by se na ně jako na odborníky obracet ve svých zdravotních nesnázích.
A workshop on Ethics of Science in Czech Republic - the present state and its historical roots was organised by the Institute of Philosophy of the ASCR on February 9, 2010.
