Práce se zabývá statistickou analýzou cen léků v lékárnách okresu Kladno. Tato analýza byla provedena na modelové skupině 13 léků. Soustředila se zejména na rozdíly v cenách léků. Variační koeficient se pohyboval od 0,024 do 0,42. To ukazuje na velký rozdíl cen některých léků mezi lékárnami. Šetřením bylo dále prokázáno, že existuje závislost cen některých léků na umístění a bliž ší specifikaci lékáren kladenského ok re su. Statisticky významný rozdíl byl zjištěn mezi cenami léků v lékárnách umístěných v nákupních střediscích a mimo tato střediska (ve zdravotnických zařízeních, jinde). Vliv velikosti obce (Kladno – Slaný – ostatní) ani vzdálenosti od centra na ceny léků nebyl prokázán. Lékárníci kritizují systém jako příliš složitý a poukazují zejména na to, že mu pacienti nerozumí a nepřijímají ho., The paper deals with a statistical analysis of drug prices in pharmacies of the Kladno district. This analysis was carried out on a model set of 13 drugs. It was focused above all on differences in drug prices. The coefficient of variation varied between 0.024 and 0.42, which reflects large differences in prices of some drugs between pharmacies. The investigation has further shown that there is a dependency of prices of some drugs on the location and more detailed specification of pharmacies in the Kladno district. Statistically significant difference was proved between the prices in pharmacies within shopping malls, and outside them (in medical centres, elsewhere). Effects of the community size (Kladno – Slaný – other) and of the distance from the town centre were not established. The problem of prophylaxis of incompetence of bronchial stump and bronchial fistula is many sided. Notwithstanding the development of new technologies, perfection of surgical technique, its frequency still remains on high level that persistently requests the search of new ways for resolving this problem, which preserved its topicality till nowadays. We have found that: The most efficiency of BFS is achieved during treatment of bronchial fistulas with diameter up to 3 mm and with effectiveness at 83.2% patients after rightside PE and at 92.3% patients after leftside PE. In case of BPF with diameter more than 3 mm the number of satisfactory results of BFS reaches only 33.3%. but, it is necessary to mention that among patients with BPF sized over 3 mm after leftside PE the effectiveness of BFS significantly exceeds as general index (50.0% against 33.3%) so after rightside PE (50.0% against 25.0%)., F. V roce 2008 byly v českém zdravotnictví zavedeny regulační poplatky. V souvislosti s tímto opatřením se neustále diskutuje, zda jejich implementace neúměrně nezatěžuje rozpočty domácností a omezuje přístup pacientů ke zdravotní péči. Tento příspěvek se proto zaměřuje na dopady regulačních poplatků na rozpočty domácností a prozkoumání změny struktury ve výdajích domácností na zdraví. Pro analýzu dopadu na rozpočty domácností je použita metodologie Světové zdravotnické organizace a jsou použita data ze statistiky rodinných účtů v letech 2007 a 2008, které bezúplatně zveřejňuje Český statistický úřad. V příspěvku je sledováno 5 základních typů domácností (domácnosti zaměstnanců bez dětí, s dětmi, domácnosti s nezaměstnanou osobou v čele, domácnosti osob samostatně výdělečně činných a důchodců). Výsledky ukazují, že domácnosti důchodců čelily největšímu zvýšení výdajů na zdraví, a taktéž měly nejvyšší zdravotní výdaje v roce 2008. Významným výdajům na zdraví rovněž čelily domácnosti nezaměstnaných. Nejmenší nárůst výdajů byl pozorován u domácností s dětmi, tyto domácnosti platily nejmenší regulační poplatky v roce 2008. Co se týče změn ve struktuře výdajů na zdraví, u většiny sledovaných domácností došlo k poklesu výdajů na volně prodejná léčiva, což lze vysvětlit jako kompenzaci zavedení regulačních poplatků a růstu cen léků a doplatků na léky. Obecně lze říci, že poklesly výdaje na nadstandardní zdravotní služby., Background: The user fees were implemented into the Czech health care system in 2008. There has been a discussion whether the implemented user fees are possible to have a negative impact on the budgets of patients and can restrict the access to the consumption of health care. Therefore this paper focuses on the effect of user fees on the budget of the Czech households and the changes in the structure of their out-of-pocket payments. Methods: The methodology of the World Health Organization was used for the analysis of the impact on budgets. The used data was derived from the Household Budget Survey 2007 and 2008 published by the Czech Statistical Office. Five types of households were monitored (households headed by employed person with and without children, households of unemployed, selfemployers and households of pensioners). Results: The results show that the households of pensioners faced to the highest increase in out-of-pocket payments and overall the highest out-of-pocket payments in 2008 as well. The household headed by unemployed person had also significant out-of-pocket payments in 2008. The smallest increase in out-of-pocket payments is observed by the households of employees with children. They paid the smallest amount of user fees in 2008. Regarding the changes in the structure of out-of-pocket payments, the majority of the observed households decreased their expenditure on medicaments sold without prescription as a result of user fees implementation and increase in medicament prices and copayments. In general, expenditure on above standard services dropped. Conclusion: The user fees implementation increased particularly the expenditure of vulnerable households (pensioners), however the effect on the changes in out-of-pocket payments structure in 2008 was not so significant., Veronika Krůtilová, and Lit.: 12
V české společnosti a českém zdravotnictví došlo od roku 1989 k hlubokým změnám. Financování zdravotní péče z daní bylo nahrazeno veřejným zdravotním pojištěním s veřejně-soukromým mixem poskytovatelů zdravotní péče. Dvacet let reforem lze považovat za úspěch, i když se zdravotní systém potýká s mnoha problémy. Radikální a rychlá řešení reformy zdravotního systému neexistují; přednost by měl mít konzervativní přístup. Zdravotní reformy ukazují, že společnost potřebuje více času na přijetí společenských změn. Systém zdravotní péče se složitými komplexními vztahy, není laboratoř pro experimenty na lidech. V textu jsou diskutována některá vybraná reformní opatření, která by měla vést ke zlepšení zdraví a zdravotního systému., Since 1989 the Czech society and the Czech health system have undergone deep changes. Health care financing out of taxation was replaced by public health insurance, with public-private mix of health providers. The twenty years of reforms can be viewed as a success, although the health system faces many problems. The radical and quick solutions do not exist in health system reform; the conservative approach should be preferred. Health reforms show that the society needs more time to accept social changes. The health system with complex relations is not a laboratory for pro experiments on humans. Před anesteziologickým výkonem je ve většině zdravotnických zařízení běžnou praxí provádět základní laboratorní vyšetření. Někdy pak (například z důvodu neprovázanosti jednotlivých informačních systémů jednotlivých zdravotnických zařízení) dochází k situaci, že jsou laboratorní vyšetření zbytečně opakována, čímž dochází ke zbytečné zátěži pacienta i k plýtvání finančních prostředků. I při velmi konzervativním odhadu jsme došli k závěru, že při důsledném sdílení dat mezi jednotlivými zdravotnickými zařízeními (například formou rozšíření systému IZIP) by bylo možné v českém zdravotnictví ušetřit nejméně 2,2 miliónu Kč ročně. I když se na první pohled může zdát tato částka jako zanedbatelná, je takovéto zjištění jasným signálem, ve kterém segmentu zdravotnictví je možné pomocí metod eHealth (zavádění výpočetní techniky do zdravotnictví) ušetřit. However, sometimes the examination is unnecessarily repeated due to an inaccessibility of original results. Even by taking a very conservative approach we have found possible savings of 2.2 million Czech crowns annualy by a rigorous date sharing among health care providers in the Czech Republic. Jako light chain escape fenomén (LCE) je označována změna biologického chování nádoru u pacientů s mnohočetným myelomem (MM). Při diagnóze jsou v séru přítomny kompletní monoklonální imunoglobulin (mIg) a monoklonální volné lehké řetězce (mFLC), při relapsu nebo progresi onemocnění dochází k prudkému vzestupu koncentrace pouze mFLC, obvykle při stabilní nebo klesající koncentraci mIg, společně s těžkým orgánovým postižením. Pro časnou diagnostiku LCE u nemocných s MM je doporučováno sériové vyšetřování FLC. LCE byl popsán v literatuře zatím pouze u pacientů po hematologické terapii. V tomto sdělení prezentujeme kazuistiku pacienta vstupně hodnoceného hematologem jako doutnající myelom, bez nutnosti hematologické terapie. Domníváme se, že vývoj nálezu odpovídal kritériím LCE., The light chain escape phenomenon is characterized as a change in tumor biological movement in patient with multiple myeloma. Complete monoclonal immunoglobulin molecules (mIg) together with monoclonal free light chains (mFLC) are present in diagnosis, only mFLC great increase is observed in relapse or disease progression together with heavy organ impairment whether mIg concentration is usually stable or decreasing. Serum FLC monitoring is recommended for early diagnostics LCE phenomenon in patients with MM. To this time LCE phenomenon was described only in patients after hematological therapy. Stárnutí populace je považováno na celém světě za hlavní faktor, který je v pozadí růstu nákladů na zdravotní péči. V odborné literatuře se však poslední dobou množí důkazy, které naznačují, že stárnutí populace má jen malý vliv na růst těchto nákladů. Autoři textu předkládají rešerši literatury, analyzují náklady na zdravotnictví v České republice a upozorňují na oblasti zdravotní péče, které mají největší vliv na růst výdajů na zdravotnictví v ČR. V závěru konstatují, že v ČR je vliv stárnutí obyvatelstva na výdaje ve zdravotnictví relativně malý a podobá se situaci popsané v zahraniční literatuře. Vyslovují názor, že by centrálním mechanismem způsobujícím rychlý růst výdajů v ČR mohla být nabídkou indukovaná poptávka., Population aging is purported to be a major factor behind the rise of the health care cost worldwide. The increasing body of evidence available in the literature on the subject of aging population and its influence on health care cost suggests that demographic changes play a relatively minor role in driving up the health care bill. First, we review some available evidence. Second, we analyze the situation in the Czech Republic using financial and utilization data and demographic projections. Analýza dat ze druhé vlny šetření ESS vede k těmto zjištěním: (1) V evropských státech hodnotí zdravotnictví zpravidla příznivěji muži než ženy. Tento rozdíl je patrný především ve státech, které nemají komunistickou minulost. (2) Věk vykazuje s hodnocením zdravotnictví křivku ve tvaru otevřeného J: nejpříznivěji hodnotí zdravotnictví staří lidé, nejméně příznivě lidé ve středním věku. (3) V postkomunistických státech platí, že čím jsou jedinci – zejména ženy – vzdělanější, tím nepříznivěji hodnotí zdravotnictví. V ostatních státech je tento vztah mírně kladný. (4) Ve všech státech hodnotí respondenti zdravotnictví tím příznivěji, čím jsou celkově šťastnější a spokojenější se svými životy. (5) Tato souvislost je tím slabší, čím vyšší je kvalita života na úrovni států. (6) V postkomunistických státech hodnotí respondenti v průměru zdravotnictví méně příznivě než ve státech ostatních. V těchto státech je současně nižší průměrná kvalita života. Při kontrole kvality života na úrovni států se rozdíl v hodnocení zdravotnictví mezi postkomunistickými a ostatními státy výrazně sníží. (7) V naprosté většině evropských států hodnotí pravicově orientovaní jedinci zdravotnictví příznivěji než jedinci orientovaní levicově. Tento vztah je silnější ve státech, které nemají komunistickou minulost. Nelze ho vysvětlit rozdíly v kvalitě života v různých státech., The analysis of data from the second wave of the ESS (European Social Survey) leads to these findings: (1) Men in European states evaluate the health systems in their countries more favorably than women. (2) The relationship between age and evaluation of health system has the form of an open J: Old people evaluate the health system the best, people of middle age the worst. (3) The more educated are people in the post-communist states (esp. women), the less favorably they evaluate the health systems. In the other European states, the relationship is mildly positive. (4) The better the subjective well-being of individuals, the better their evaluation of the health system. (5) This relationship is the weaker, the better is the quality of life on the state-level. (6) Respondents in the post-communist states evaluate health systems less favorably than respondents in the other European countries. At the same time, the quality of life in the post-communist countries is worse than in the other countries. When statistically controlling for the difference between the quality of life in these two categories of countries, the difference in evaluations of the health systems in these two categories of countries is remarkably reduced. (7) In the majority of European states the more are the respondents politically to the right, the more favorably they evaluate the health system in their country. This relationship is stronger in non-postcommunist than in postcommunist states.
