Cíl: Práce se zabývá porovnáním posturální stability u dvou vybraných souborů probandů, a to u dětí s obezitou a u atletů. Metody: U obou skupin bylo provedeno posturografi cké vyšetření pomocí dvou testů – Limity stability v náklonu tzv. Limits of Stability (LOS) a Senzorický test – Sensory Organisation Test (SOT). Na základě takto získaných výsledků byly porovnány a statisticky vyhodnoceny rozdíly mezi jednotlivými soubory. Výsledky: Práce prokázala významný vliv obezity na individuální posturální stabilitu jedince. Prokázali jsme statisticky signifi kantní rozdíly jak v reakčním čase (RT) a maximální exkurzi při testu LOS, tak i ve volbě pohybových strategií pro udržení posturální stability při testu SOT. Závěry: U dětí s obezitou je, v rámci fyzioterapie, vhodné zaměřit pohybovou aktivitu na zlepšení celkové postury a posturální stability a to např. využitím funkčního tréninku., Aim: The paper deals with the comparison of postural stability in two selected groups of probands: children with obesity and athletes. Methods: Posturographic examination was carried out in both groups through two tests – Limits of Stability (LOS) and Sensory Organization Test (SOT). Variations between individual groups were compared and statistically evaluated based on the acquired results. Results: Obesity as well as overall posture has a signifi cant impact on postural stability. We demonstrated statistically signifi cant variations in reaction time (RT) and maximum excursion in LOS test, and in SOT test in the choice of movement strategies for maintaining postural stability. To study defects and chromosomal abnormalities of the fetus, we retrospectively analyzed results of comprehensive dynamic survey of 26,404 pregnant women aged 1850 years old at 6 to 40 weeks of pregnancy. Of them, 25,956 (98.3%) women had physiological course of pregnancy, 448 (1.7%) women had abnormal pregnancy. For the diagnosis of fetal defects, we carried out ultrasound, biochemical, invasive and cytogenetic studies. The results of study showed that the majority of fetal defects and pathological course of pregnancy was noted in women at the age of 2125 years old, since at this age period women have the highest number of pregnancies. At the older age, we noted a gradual decrease in the number of pregnant women, as well as the number of abnormalities of the fetus. Based on the analysis of the results obtained, we have developed an algorithm for early fetal ultrasound examination. Introduction Cutaneous malignant melanoma prevalence, incidence and mortality rates are increasing in white populations worldwide more rapidly than any other cancer site (American Cancer Society, 2006). Despite the potential importance of regular skin selfexamination and promotion of selfprotection practices, little is known about the prevalence of these practices in medical students in Albanian population. Methods This is a descriptive, quantitative crosssectional study. In this study were included a sample of 150 individuals chosen among the students of Faculty of Medicine based on their family history for skin cancer. This study was started on October the 3rd and finished on November the 12th. Subjects had to fill in a structured, selfadministered questionnaire. All participants lived within Republic of Albania but at the time of the study were students in University of Tirana, Faculty of Medicine. Results In this study that we conducted we included 150 individuals supposed to have a risk for skin cancer based on their family history. 200 individuals were approached for participation. Of these, 150 individuals returned questionnaire data, yielding a response rate of 75% among eligible, successfully contacted participants. The mean age of the sample was 20.05 years (Std. Dev. = 0.925), with males (22%) and females (78%) represented unequally, because the gender configuration of the faculty itself has gender disparities with more than 80% females and only 20% male students. The majority of the sample was born in city or town (85.3%), 14.7% was born in a village. In terms of income level, 79.3% of participants had a medium income family background, 15.3% high income and 5.3% had a low income level family background. Conclusion It is important for those individuals with family history for skin cancer (which inherently indicates risk for skin cancer) to develop selfexamination and SSE behaviours and practices in order to have a protection and at least an early detection (if onset) of the different forms of Melanoma. Cíl. Cílem práce bylo zjistit, zda je možné při použití úpravy CT obrazů hybridní iterativní technikou, označovanou iDose, snížit radiační zátěž při CT vyšetřování mozku u dětí a adolescentů o 30 %. Metoda. CT vyšetření mozku bylo provedeno na přístroji CT 64 (Philips Haelthcare) u 40 pacientů různých věkových kategorií. Získaná data byla rekonstruována s různými kernely a následně iterativní technikou iDose (Philips Healthcare, dále ID) různé intenzity (ID30%, ID50%, ID70%) a s přidanou volbou MR (multi-resolution). Takto rekonstruované skeny byly porovnány s obrazy z rutinního vyšetření s konvenční rekonstrukcí FBP (filtered back projection). Hodnocení výsledných obrazů bylo provedeno jednak subjektivně, jednak objektivně. Výsledky. Použití filtrů ID30%, ID50% ID70% sníží šum a potlačí artefakty v obrazech, avšak při intenzitě 70% již snižuje ostrost a zhoršuje podání detailů. Kompenzaci tohoto nežádoucího jevu je možné dosáhnout zapnutím volby MR při rekonstrukci. Kombinace "ostřejšího" kernelu UB a ID rekonstrukce s přidanou volbou MR byla nejlepší pro zlepšení detailu obrazu a celkového užitku pro diagnostiku. Lepších výsledků bylo dosaženo u dospělých, u dětských pacientů byl přínos menší. Po redukci dávky záření při CT vyšetření o 30 % došlo ke zhoršení kvality obrazů, které bylo kompenzováno užitím kombinované rekonstrukce ID70UA + MR nebo ID70UB + MR. Při subjektivním hodnocení výsledných obrazů byla variabilita mezi hodnotícími 1,1-1,3 bodů. Závěr. iDose umožňuje provádět CT vyšetření s redukcí radiační dávky, aniž by došlo ke snížení diagnostické hodnoty obrazů., Aim. To evaluate the possibility of 30% radiation dose reduction using an innovative hybrid iterative reconstruction technique called iDose on image quality of CT brain imagesin children, adolescents and adults. Methods. Brain CT was done in forty patients of various age, scanned on CT64 (Philips Healthcare). Obtained data were reconstructed using various kernels and consequently by the new iDose (Philips Healthcare, NL) hybrid iterative reconstruction prototype on 3 different iD levels (ID30, ID 50, ID70 and multi-resolution /MR/ option). The images were compared to conventional FBP (filtered back projection). The assessment was subjective and objective. Results. Analysis of the variance showed a decrease of noise corresponding to ID levels (Fig. 1) and no differences to MR option. The combination of the filter UA in combination with ID70 led to smoother images but with fewer details, as shown in Fig. 1. An improvement of image quality has been seen when using the MR option. This combination using the higher kernel filters UB showed an improvement of smaller details and higher impact in the adult group while was less pronounced for the children group (Fig. 1). The reduction of dose decreased the image quality in both FBP and ID images, however the ID70UAMR and ID70UBMR reconstructions have compensated for this deterioration. The inter-observer variability was found at 1.1 and 1.3 points respectively. We studied the relationship of connective tissue state with magnesium and hydroxyproline levels in blood and urine of young women with primary dysmenorrhea. Study showed that in girls with dysmenorrhea and phenomena of connective tissue dysplasia hydroxyproline level in urine was increased which was associated with increased degradation of collagen and decreased level of magnesium in blood serum. Aim: The principal aim was to establish reference paediatric data for the serum homocysteine levels in Czech children and adolescents. Methods and Results: 144 children either healthy or not sufferig from acute or chronic inflammation, autoimmune disorders including rheumatic diseases, inflammatory musculoskeletal disorders, inflammatory bowel disease, diabetes mellitus, hypercholesterolemia, epilepsy, chronic renal failure, aged 0–19.9 years (0–6.9 years, n = 40; 7–10.9 y, n = 28; 11–15.9 y, n = 45; 16–19.9 y, n = 31) had their blood samples collected and the serum homocysteine level (S-homocysteine) was evaluated by chemiluminescence. A significant age dependence of the S-homocysteine levels was observed (R = 0.35, p < 0.01); with highest values of upper reference range in the 11–15.9 and 16–19.9 years’ group, respectively. Obezita dětí je v oblasti veřejného zdraví vážným celosvětovým problémem. Výživa, fyzická (in)aktivita a sedavý životní styl, ke kterému patří sledování televize, práce či hraní her na počítači, jsou nezávislými rizikovými faktory nárůstu BMI (nárůstu hmotnosti). Cílem předkládané práce bylo posoudit asociace mezi stravovacími návyky, fyzickou aktivitou, životním stylem, nadváhou a obezitou u dětí ve věku 8–14 let v Královéhradeckém, Libereckém a Pardubickém kraji. Cílovou skupinou (n = 2042) byli žáci ze základních škol z 2., 4., 6. a 8. ročníku. Všechny sledované parametry (vynechávání jídla, celkový energetický příjem, preference potravin a nápojů...) byly hodnoceny formou četnosti (%) v celé skupině, dle pohlaví a dle tělesné hmotnosti. Ve všech sledovaných parametrech měly děti s nadváhou/obezitou vždy horší výsledky než děti s normální hmotností, i když ne vždy signifikantní. Největší rozdíly byly zjištěny při dodržování pitného režimu, kde děti s nadváhou/obezitou pijí především slazené nápoje., Childhood obesity is a serious public health concern worldwide. Dietary behaviour, physical activity and a sedentary lifestyle which includes television viewing, working or playing games on the computer are independent risk factors for increased BMI (increasing weight). The aim of this study was to explore dietary habits, physical activity and lifestyle associated with overweight and obesity among children 8–14 years of age, in Hradec Králové, Liberec and Pardubice district. The target population (N = 2042) comprised elementary school children in the 2nd, 4th, 6th and 8th grade. All monitored parameters (skipping meals, total energy intake, food and beverage preferences...) were evaluated by frequency (%) in the group, by gender and according to body weight. In all monitored parameters overweight/obese children have worse results, although not always significantly, than children with normal weight. Cíl. Shrnout dostupné informace a analyzovat zobrazené nálezy na námi zachycených běžných i méně běžných případech tarzální koalice. Budeme se zabývat primárními i sekundárními nálezy při vlastním zobrazení i korelací s literaturou, udávanými histologickými nálezy, zobrazovacími metodami a protokoly, diferenciální diagnózou a krátkou informací o klinickém nálezu a léčbě. Metody. Vyšetřili jsme osm pacientů s různými typy tarzální koalice - s koalicí talokalkaneální mediální i dorzální, s koalicí kalkaneonavikulární, s oboustrannou talokalkaneální koalicí i s koalicí talokalkaneokuboidní, která byla součástí komplexnější dysplazie. Nálezy jsou analyzovány především na CT a MR, jsou doplněny RTG snímky. Výsledky. V závislosti na typu koalice a zvolené projekci, jsou známky na RTG snímku buď patognostické (u šikmé projekce a kalkaneonavikulární koalice), nebo diskrétní a nepřímé (u ostatních projekcí a ostatních typů koalice). CT a MR zobrazí tuto vadu při vhodně zvolených rovinách zobrazení a sekvencích nepochybně. Mezi nejvýznamnější nálezy patří zúžená nebo přemostěná kloubní štěrbina, nerovná kortikální kost, edém, osteoskleróza a změna tvaru kostí. Časté jsou sekundární degenerativní změny okolních kloubů. Závěr. Povědomí o známkách této vady umožňuje její rozpoznání i přes její málo frekventní výskyt, a to i v případech, kdy na ni klinický lékař nevysloví podezření. Na CT a MR je třeba při vyšetřování hlezna rutinně zachycovat i oblast subtalárních kloubů a tarzu., Aim. To summarize and display the imaging findings in patients with different types of both frequent and less frequent types of tarsal coalition, to discuss the imaging protocols, differential diagnoses and give brief information on clinical findings and treatment. Method. The authors display CT, MRI and X-ray findings in 8 patients with the main types of coalition (talocalcaneal, calcaneonavicular), with a bilateral talocalcaneal coalition and in a patient with a complex synostosis. Results. The encountered findings were: narrowed or absent joint space, thinned irregular cortical line, adjacent oedema and sclerosis and an unusual shape of the involved bones. Secondary degenerative changes in the adjacent joints were often encountered. Depending on the type of coalition and on the projection, this diagnosis presents with X-ray findings, which are either prominent (in calcaneonavicular coalition and an oblique X-ray) or subtle and indirect (all other cases). In MRI and CT the diagnosis is much more obvious. The pathognomonic signs as well as secondary degenerative signs will be pointed out together with the appropriate imaging protocols. A differential diagnosis will be discussed. The findings of the authors will be correlated with the literature. Conclusion. It is necessary to be aware of the findings in tarsal coalition, in order to establish the diagnosis even in cases unsuspected by the clinitian. Sometimes intraorbital foreign bodies lead to unexpected findings. A 16-year old boy was referred due to ocular surface trauma. A conjunctival laceration was detected at the level of the left caruncle with associated left exotropia, reduced adduction as well as a preretinal hemorrhage along the nasal periphery of the fundus. A blow-out fracture of the medial orbital wall was suspected and a CT scan of the orbits was scheduled which revealed the presence of a large intraorbital foreign body. The removal of the intraorbital foreign body (which proved to be a bullet) and precautionary laser photocoagulation along the nasal periphery of the left eye were performed.
