Introduction and Objectives: One of the characteristic changes of tumor formation is accumulation of genetic disorders in mitochondrial and nuclear genome. Mitochondrial disorders, from its side, are responsible for failure of metabolism, apoptosis, cell growth, formation of reactive oxygen species, etc. Overprpoduction of reactive oxygen species (ROS) significantly impacts the respiration chain enzymes and entirely the antioxidant system of mitochondria. Finally this may become a favorable condition for normal cells transformation. The purpose of the presented work was to study the mitochondrial defects and to establish their role in prostate cancer development. Results: Experimental results demonstrate significant increase of the activity of mitochondrial succinate dehydrogenaze (complex II) of the malignant epithelial cells of prostate, and slight changes in cytochrome oxydase (complex IV) activity. Also significant activation of the antioxidant system (glutathione-dependant system) of mitochondria in prostate malignant epithelial cells was revealed. Cieľ: Cieľom práce bolo zistiť výskyt komplikácií v závislosti od veku a pohlavia pacientov, od dĺžky zavedenia stimulačnej elektródy, od zvolenej cesty prístupu zavedenia stimulácie u pacientov hospitalizovaných na Oddelení arytmií a koronárnej jednotky v Univerzitnej nemocnici Martin. Metodika: Pre zber potrebných údajov sme použili obsahovú analýzu zdravotnej dokumentácie, nemocničný informačný systém, pozorovanie na základe vypracovaného záznamového hárku. Údaje sme analyzovali pomocou deskriptívnej a induktívnej štatistiky. V sledovanom súbore bolo 38 pacientov od 58 rokov do 93 rokov. Pacienti boli akútne prijatí na Jednotku intenzívnej starostlivosti za účelom realizácie dočasnej kardiostimulácie, ktorá bola užívaná v priemere 5,28 ± 2,33 dní. Výsledky: Na základe spracovania údajov sme u 17 pacientov zistili výskyt komplikácií, pričom najčastejšou komplikáciou bola lokálna infekcia. Štatistickou analýzou sme zaznamenali významné vzťahy medzi dĺžkou zavedenia stimulačnej elektródy a výskytom lokálnej infekcie, kde nám vznikla silná signifikantná korelácia r(S) = 0,518 na hladine významnosti p = 0,001 a rovnako pri výskyte hematómu sme zaznamenali koreláciu r(S) = 0,329 na hladine významnosti p = 0,023. Záver: Analýzou získaných údajov sme identifikovali najčastejšie vyskytujúce sa komplikácie v spojitosti s dočasnou kardiostimuláciou. Najčastejšie sme zaznamenali výskyt lokálnej infekcie, ktorý súvisí s následnou ošetrovateľskou starostlivosťou, ale aj s významnými prediktormi, ktoré môžu do značnej miery ovplyvniť ich vývoj., Aim: The aim of our study was to determinate the incidence of complications depending on the age and gender of patients, duration of implantation of stimulation electrode, of selected access point of implementation stimulation in patients hospitalized at the Arrhythmia unit and Coronary care unit in University hospital in Martin. Methods: For the collection of required data we have used content analysis of medical records, hospital information system, surveillance based on own designed sheet. The data were analysed using descriptive and inductive statistic. The sample consisted of 38 patients in the range from 58 to 93 years of age. Patients were acutely admitted to ICU with the aim of realization of temporary cardiac pacing, which lasted 5.28 ± 2.33 days in average. Results: On the basis of processed data, we found incidence of complications in 17 patients, where the most frequent complication was local infection. Using statistical analysis we found a significant relationship between duration of implementation of stimulation electrode and incidence of local infection, which resulted in strong significant correlation (r(S) = 0.518, p = 0.001) and similarly, we noted correlation in the incidence of haematoma (r(S) = 0.329, p = 0.023). Conclusion: Using analysis of the obtained data we have identified the most frequently occurring complications in conjunction with the temporary cardiac pacing. Cíl: Cílem tohoto příspěvku je prezentovat výsledky pilotního šetření testování dotazníku PNAP „Hodnocení potřeb pacientů v paliativní péči“ provedeného na onkologické klinice Fakultní nemocnice v Ostravě. Dále pak prezentovat zjištěnou důležitost bio-psycho-sociálně-spirituálních potřeb pacientů s ukončenou onkologickou léčbou. Metodika: Výzkumný soubor tvořilo 52 pacientů onkologické kliniky FN Ostrava, u nichž byla ukončena onkologická léčba v důsledku léčebně neovlivnitelné progrese onkologického onemocnění. Pro hodnocení potřeb byl použit dotazník - Hodnocení potřeb pacientů v paliativní péči (PNAP), který se skládá ze 42 položek sdružených do 5 domén. Výsledky: Za nejdůležitější považovali onkologičtí pacienti potřeby v doméně autonomie a fyzické potřeby. Nejméně byli respondenti spokojeni se saturací psychických a spirituálních potřeb. Pacienti mladší 60 let považovali sociální potřeby za signifikantně důležitější a současně byli méně spokojeni s jejich saturací (p<0,05). Ženy považovaly za důležitější psychické a spirituální potřeby. Se saturací těchto potřeb byly však více spokojeny (p<0,05) než muži. Z jednotlivých položek považovali respondenti nejčastěji za důležité: řešení bolesti, únavy, dušnosti, vyrovnat se s nemocí, komunikaci se zdravotníky a jejich podporu, rozhodování, respekt a úctu při ošetřování, vnitřní klid, naději a smysl života. Za nedostatečně saturované potřeby uvedli únavu, bolest, vyrovnanost s nemocí, strach ze závislosti, pokračování v obvyklých aktivitách, pozitivní náhled na život a naději. Závěr: Dotazník PNAP vychází z holistického pojetí péče. Zjištěné nedostatečně saturované potřeby by se měly stát předmětem ošetřovatelských intervencí., Aim: The aim of this contribution is to present the results of pilot research into a testing of the PNAP questionnaire “Patient needs assessment in palliative care“ implemented at the oncological clinic of the University Hospital Ostrava, as well as to present an established significance of bio-psycho-social-spiritual needs of patients with terminated oncological treatment. Methods: The sample was made up of 52 patients of the oncological clinic of the University Hospital Ostrava, whose oncological treatment was terminated as a result of an incurable progression of oncological disease. For needs assessment the questionnaire Patient needs assessment in palliative care (PNAP) was used, consisting of 42 items clustered into five domains. Results: Oncological patients considered the needs in the domains of autonomy and physical needs as the most important. They were the least satisfied with the saturation of psychological and spiritual needs. Patients under 60 years considered social needs as significantly more important and at the same time they were less satisfied with their saturation (p<0.05). Women considered psychological and spiritual needs as more important. However, they were more satisfied with the saturation of these needs than men (p<0.05). The following individual items were considered the most often as important: treating the pain, fatigue, dyspnoea, coming to terms with an illness, communication with health workers and their support, decision making, respect and dignity while being treated, inner peace, hope and the meaning of life. Insufficiently saturated needs were reported: fatigue, pain, reconciliation with an illness, fear from dependence, continuation in common activities, positive opinion of life and hope. Conclusion: The questionnaire PNAP is based on a holistic conception of care. According to our research, Uzbek individuals with Parkinson’s disease have high prevalence of 0/0 polymorphisms of GSTT1 and GSTM1 genes, as well as combinations of GSTT1 (0/0)/GSTM1/(0/0) genotypes. These mutations are associated with an earlier debut of the disease, its mixed form, and rapid rate of progression. Cíl: Cílem studie bylo vyhledat poznatky týkající se NANDA International ošetřovatelské diagnózy Porucha polykání (00103) se zaměřením na výskyt této ošetřovatelské diagnózy u různých skupin pacientů a její stanovení prostřednictvím znaků uvedených v klasifikačním systému dle NANDA International. Metodika: Senzitivní vyhledávací strategií byly vyhledány publikované informace k ošetřovatelské diagnóze dle NANDA International Porucha polykání (00103), k jejímu diagnostikování a k výskytu této diagnózy u pacientů. K vyhledání relevantních dat byly využity elektronické databáze: Bibliographia Medica Čechoslovaca, MEDLINE, Springer, ProQuest a vyhledávač Google. Výsledky: V odborných publikacích se autoři velmi často věnují problematice porušeného polykání a jejímu diagnostikování, nikoli však pomocí ošetřovatelské diagnostiky dle NANDA International. Ošetřovatelská diagnóza Porucha polykání (00103), diagnostikována prostřednictvím ošetřovatelské diagnostiky dle NANDA International, se vyskytovala ve velkém zastoupení u pacientů na jednotkách intenzivní péče, u pacientů s onkologickým onemocněním a u pacientů s onemocněním jícnu. Závěr: V ošetřovatelství je porucha polykání častým problémem, který lze diagnostikovat prostřednictvím klasifikačního systému NANDA International. Další výzkumy by mohly přispět ke zjištění, zda diagnostika poruchy polykání pomocí NANDA International je vhodná pro použití v klinické praxi., Aim: The aim of the study was to search for findings related to NANDA International nursing diagnosis Impaired Swallowing (00103), focusing on the incidence of this nursing diagnosis in different groups of patients and its establishment using the NANDA International classification system. Methods: Published information concerning NANDA International nursing diagnosis Impaired Swallowing (00103), its establishment and incidence in patients were found by a sensitive search strategy. The following electronic databases were searched for relevant information: Bibliographia Medica Čechoslovaca, MEDLINE, Springer and ProQuest, together with Google. Results: Although paper authors frequently mention impaired swallowing, it is rarely diagnosed using the NANDA International classification system. Nursing diagnosis Impaired Swallowing (00103) as diagnosed according to NANDA International was largely present in intensive care unit patients, oncology patients and those with oesophageal disease. Conclusion: Impaired swallowing is a frequent condition in nursing which can be diagnosed using the NANDA International classification system. Cíl: Hodnocení komunikačních a vybraných psychických oblastí (napětí, obavy, podráždění, deprimace) dispenzarizovaných nemocných se zhoubnými nádory hrtanu a slinných žláz, pomocí standardizovaných dotazníků kvality života. Východiska: Zhoubná nádorová onemocnění hlavy a krku se v četnosti výskytu řadí na šesté místo mezi zhoubnými nádory na celém světě. Otázka léčby a následné péče je velmi aktuální zvláště z hlediska rizika mutilujících léčebných výkonů a jejich vlivu na kvalitu života. Metody: Do výzkumu byli zařazeni aktivně dispenzarizovaní nemocní ORL kliniky se zhoubnými nádory hrtanu a slinných žláz. Vlastní šetření absolvovalo 113 respondentů, výzkum probíhal od ledna do prosince 2011. Pro sběr dat byly využity dva standardizované dotazníky kvality života (EQRTC QLQ-30, EQRTC QLQ-H&N35). Dále informace z nestandardizovaného dotazníku a zdravotnické dokumentace. Použité statistické metody – dvouvýběrový Wilcoxonův test a analýza rozptylu. Výsledky: U pacientů s nádorovým onemocněním hrtanu s tracheostomickou kanylou a bez ní byl statisticky významný rozdíl v komunikaci. Stádium nemoci dle klasifi kace TNM nemělo vliv na vybrané psychické oblasti u respondentů s nádorovým onemocněním hrtanu a slinných žláz. Závěry: Závažnost obtíží u dispenzarizovaných respondentů závisí na lokalizaci a velikosti nádoru, terapeutické strategii, věku pacienta, polymorbiditě a dalších faktorech. Trvalá tracheostomická kanyla má vliv na komunikaci respondentů s nádory hrtanu. V našem souboru respondentů jsme neprokázali vliv stádia nemoci dle klasifi kace zhoubných novotvarů TNM, na vybrané oblasti psychiky (napětí, obavy, podráždění, deprimace)., Aim: Assessment of communication and selected psychological areas (e. g. tension, concerns, irritation, depression) related to the dispensarized patients with malignant laryngeal and salivary glands tumors using standardized questionnaires assessing quality of life. Background: The head and neck malignant tumors rank sixth in frequency among malignant tumors worldwide. The question of treatment and follow-up care is very current especially in terms of the risk of mutilating surgical procedures and their impact on quality of life. Methods: In the research actively dispensarized patients of ENT clinic with malignant tumors of the larynx and salivary glands were included. 113 respondents took part in the research, which was conducted from January to December 2011. Standardized (EQRTC QLQ-30, EQRTC QLQ-H&N35) and non-standardized questionnaires assessing quality of life and medical records were used for data collection. Statistical methods such as double selection Wilcoxon test and analysis of dispersion were used. Results: There was a statistically signifi cant diff erence in communication between patients with tracheostomy cannula and without it. Stage of the illness according to TNM classifi cation did not infl uence selected psychological areas of the respondents with cancer of the larynx and salivary glands. Conclusions: The seriousness of the diffi culties of the dispensarized patients depends on localization and size of the tumor, therapeutic strategy, age of the patient, polymorbidity and other factors. Permanent tracheostomy cannula has an impact on communication of the respondents with laryngeal tumors. In the department of coloproctology of NORC MH RUz 17 patients with disseminated forms of colorectal cancer was made the study of oncogenes and complex treatment by 2 protocols using FOLFOX4 regime and FOLFIRI regime. In second protocol there used 2 sessions of endolymphatical polychemotherapy FOLFOX4 regime against EHFhyperthermia. All patients were performed additional investigations directed to study the presence of multiple drug resistance in them where definition of р53, bcl2 oncogene expression. In our observations we followed resistance to FOLFOX4 scheme in 4 patients, and to FOLFIRI scheme in 2 cases. Purpose: To investigate the protective action of Ceruloplasmin (Cp) on the lysis of erythrocytes in men with prostate tumors. Material and Methods: The blood erythrocytes of the patients with benign hyperplasia of prostate (BHP) and prostate adenocarcinoma (CaP) were studied. Patients at the age 60-75 with early stages of a cancer have been investigated. The control group was consisted of apparently healthy males with the compatible age. n=15 for each group. The clinical stage of the disease was diagnosed by means of rectal, histological and echographic examination of the prostate gland. Photometric methods were applied to register lysis dynamics in order to test out the protective action of Cp. Results: The nonspecific protective function of Cp preparation on BHP and CaP erythrocytes as well as on the control group erythrocytes was revealed. CaP erythrocytes have shown more sensitivity to the lysis provocative factor than BHP and the control group erythrocytes, that was presumably attributed to the structural and functional changes of the erythrocytes developed in the presence of malignant tumor., N. Cíl: Práce se zaměřuje na zjištění vlivu sociální opory jako protektivního faktoru zdraví na míru deprese, úzkosti a životní spokojenosti u pacientů léčených pro lékařskou diagnózu F10.2 Závislost na alkoholu a F32 Depresivní epizoda na odděleních psychiatrie. Metody: Pro účely výzkumu byl zvolen kvantitativní přístup. Ke sběru dat byla použita dotazníková baterie. Baterii tvořily čtyři standardizované dotazníky: PSSS (Perceived Social Support Scale) J. A. Blumenthala, BDI-II (Beck Depression Inventory) Becka a kol., SAS (Self Rating Anxiety Scale) Zungova sebeposuzovací stupnice úzkosti a DŽS – Dotazník životní spokojenosti Fahrenberga et al. Výsledky: Z výsledků výzkumného šetření vyplynulo, že existuje statisticky významný rozdíl v míře sociální opory mezi skupinou kontrolní a zkoumanou (pacienti s lékařskou diagnózou deprese a závislost na alkoholu). Potvrdil se předpoklad o existenci signifi kantního negativního vztahu mezi mírou úzkosti a sociální opory u pacientů s lékařskou diagnózou závislost na alkoholu. Výsledek potvrzuje protektivní vliv sociální opory ve vztahu ke zdraví. Nebyla prokázána existence signifi kantního vztahu mezi mírou deprese a mírou sociální opory. Výsledky výzkumu rovněž potvrzují, že existuje statisticky významný rozdíl v míře životní spokojenosti mezi skupinou kontrolní a zkoumanou (osoby se závislostí na alkoholu). Na základě výsledků výzkumu lze konstatovat, že existuje signifi kantní pozitivní vztah mezí mírou sociální opory a mírou životní spokojenosti u osob se závislostí na alkoholu. Závislost na alkoholu vede ke snížení životní spokojenosti. Závěr: Výzkum prokázal význam sociální opory jako protektivního faktoru zdraví. Potvrdilo se, že se zvyšující se mírou sociální opory se snižuje míra úzkosti a naopak se zvyšuje míra celkové životní spokojenosti u osob se závislostí na alkoholu., Aim: The article focuses on the effect of social support as a health protective factor against depression, anxiety and life satisfaction in patients treated for the diagnosis F10.2 Alcohol dependence and F32 Depression on psychiatric wards. Methods: A quantitative approach was chosen for the research. To collect data, a questionnaire battery comprising four standardised questionnaires was used: PSSS (Perceived Social Support Scale) J. A. Blumenthal, BDI-II (Beck Depression Inventory) Beck et al., SAS (Zung’s Self Rating Anxiety Scale) and Questionnaire of Life Satisfaction by Fahrenberg et al. Results: The results of the research show that there is a statistically signifi cant diff erence in the degree of social support between the control and target group (patients with medical diagnosis of depression and alcohol dependence). It confi rmed the assumption about signifi cant negative relation between the degree of anxiety and social support in patients with medical diagnosis of alcohol dependence. The existence of a signifi cant relationship between the level of depression and level of social support was not confi rmed. The research results also confi rm the existence of a statistically signifi cant diff erence in the degree of life satisfaction between the control and target group (people with alcohol dependence). Based on the results, we can claim there is a signifi cant positive relation between the degree of social support and the degree of life satisfaction in people with alcohol dependence. Alcohol dependence leads to lower life satisfaction. The Purpose of the study: define the indications and contraindications to sphincterpreserving operation in lowerampullar cancer of rectum. The Materials and methods: In coloproctological branch of National Oncological Scientific centre (NOSC) in 20052009 years 142 patients were performed radical operations, 64 (45,1%) patients were performed sphincterpreserving operations abdominoanal resection of rectum (AARR), medium age of patients 59,8 years, beside 78 (54,9%) patients were performed abdomeno perineal extirpation of rectum (APER), medium age 54,2 years. The Results: Studied results of the morphological research of removed preparation by histological structure of tumors and by form of the growing, depending on locations of the tumors on toothed line. The remote postoperative results studied beside 51 (79,7%) patient with AARR and beside 63 (80,1%) patients with APER. The Findings: sphincterpreserving operations (SPO) of the rectum can be the operation of the choice in treatment of high differentiated adenocarcinoma (HDA) and medium differentiated adenocarcinoma (MDA) in stages T23 in localization of the lower pole of tumors, not below 1 centimeter of toothed line (TL) and carries combined character, in location of the lower edge of tumors on toothed line in patients with HDA MDA in 4% and 2,2% cases are shown that completing intrasphincternal resections of rectum, in connection with particular aggressiveness LDA question about choice of the method of the treatment under their localizations directly on TL must be solved in favour of complete APER, in exophytic tumor located onto and above TL is indication for sphincterpreserving operation in type of AARR, indices recurrencefree and not metastatic period, oneyear deathrate, threeyear survivability after SPO at inferior ampular cancer of rectum has not an essential distinction from results after APER., A.
