Introduction: Obstruction of the appendiceal lumen is the primary cause of appendicitis. The most common causes of luminal obstruction are fecaliths and lymphoid follicle hyperplasia. Additionally, bacterial infections or enteric and systemic viral diseases can cause a reaction of the lymphoid follicle. Case presentation: An 11-year-old boy with active phase of chickenpox presented on our Pediatric surgery emergency department under the impression of acute appendicitis. An appendectomy was performed on the same day. An inflamed and edematous retrocecal appendix was removed during surgery. Histological investigation of the appendix revealed transmural acute inflammation, with diffuse proliferation of inflammatory cells, with characteristic intranuclear inclusion surrounded by a clear halo. The PCR analysis of peripheral blood and appendix tissue specimen revealed positive VZV DNA. Plicní aspergilóza patří mezi zánětlivá onemocnění způsobená houbami rodu Aspergillus. V závislosti na stavu imunitního systému a plicního parenchymu nemocného existují invazivní, semi-invazivní a neinvazivní formy onemocnění. V diagnostice tohoto onemocnění hrají hlavní úlohu prostý rentgenový snímek hrudníku a HRCT plic. Prezentujeme kazuistiku pacienta s nově diagnostikovaným high-grade gliomem mozku, u kterého se v důsledku antiedematózní terapie kortikoidy rozvinula invazivní plicní aspergilóza včetně mimoplicní disseminace., Pulmonary aspergillosis belongs to inflammatory diseases caused by Aspergillus spp. Invasive, semi-invasive and non-invasive forms of the disease exist depending on the level of immunity and possible impairment of pulmonary parenchyma. Few studies concerning the occurrence of cow’s milk allergy with the use of double-blind, placebo controlled food challenge test in adolescents and adult patients suffering from atopic dermatitis exist. Aim: To evaluate the occurrence of cow’s milk allergy in adolescents and adults suffering from atopic dermatitis. Method: Altogether 179 persons suffering from atopic dermatitis were included in the study: 51 men and 128 women entered the study with the average age of 26.2 (s.d. 9.5 years). Complete dermatological and allergological examinations were performed. Results: The positive results in specificIgE and in skin prick tests were recorded in 12% of patients. According to the open exposure tests and double-blind, placebo controlled food challenge tests these patients are only sensitized to cow’s milk without clinical symptoms of allergy. Double-blind, placebo controlled food challenge test confirmed food allergy to cow milk only in one patient (worsening of atopic dermatitis), the oral allergy syndrome was observed in another one patient, occurrence of this allergy was altogether 1.1%. Účel studie: Hlavním cílem studie bylo odhadnout dosažení patologické kompletní odpovědi nemocných trpících lokálně pokročilým adenokarcinomem rekta po předoperační (neoadjuvantní) chemoradioterapii (CHRT) s využitím individuálních farmakokinetických parametrů 5-fluorouracilu (5-FU). Sekundárním cílem bylo hodnocení bezpečnosti a snášenlivosti léčby. Tato otevřená prospektivní studie podporovaná grantem IGA NS 9693–04/2008 zahrnula 34 dospělých s lokálně pokročilým karcinomem konečníku ozařovaných do celkové dávky 50,4 Gy ve 28 frakcích po 1,8 Gy 10–15 MV paprsky v režimu 5 + 2 (5 dní radioterapie, 2 dny volno) s kontinuálně podávaným 5-FU v i. v. infuzi 200–1000 mg/m2 po dobu 4–5 týdnů kontinuálně 7denní infuzí. Chirurgická resekce následovala 4–6 týdnů po ukončení CHRT a po klinickém restagingu. Hodnocena byla klinická a patologická odpověď na chemoradioterapii pomocí zobrazovacího (MR) a histopatologického vyšetření, vyjádřená v % jako reziduální choroba. Výsledky a závěr: Výsledek dokládá vztah mezi velikostí kumulativní dávky 5-FU a kumulativní AUC 5-FU (r = 0,61, p < 0,001). Podobný vztah byl prokázán mezi kumulativní AUC 5-FU a metabolickým poměrem (poměr dosažených plazmatických koncentrací neaktivního metabolitu dihydrofluorouracilu (5-FUH2) ku koncentracím 5-FU, r = -0,80, p < 0,0001). Kumulativní AUC korelovala se stupněm odpovědi na léčbu (r = – 0,53, p < 0,005) a určovala také stupeň toxicity léčby. Pokud bychom chtěli dosáhnout kompletní patologickou odpověď, pak by denní dávka 5-FU měla být u středně rychlého metabolizéra >350 mg/m2 a kumulativní AUC1–39 dní > 50 mg/L*h. U žádného z nemocných nebyla nalezena mutace v genu pro dihydropyrimidindehydrogenázu (DPD) a multidrug resistance-1 protein (MDR-1), přesto byla nalezena vysoká interindividuální variabilita v dosažených plazmatických koncentracích 5-FU, a to i s ohledem na cirkadiální rytmus kinetiky léčiva., Background and Purpose: The main goal of the present study was to estimate the early patients´response following neoadjuvant chemoradiotherapy (CHRT) based on 5-fluorouracil (5-FU) with curative aim in relation to plasma concentrations and pharmacokinetic parameters of 5-FU. Secondary objectives included evaluation of the safety and tolerability of the regimen. Patients and Methods: This open prospective study enrolles 34 adult patients with locally advanced rectal cancer, who received 5-FU 200 -1000 mg/m2 administered as a continuous i. v. infusion over 4–5 week and radiotherapy delivered with 10–15 MV photon beams at 1.8 Gy/fraction up o 50.4 Gy in 28 daily fractions for 5 days a week. Surgical resection with curative aim followed 4–6 weeks after the completion of CHRT and clinical restaging. Pathologic response evaluation and the rate of tumor regression was evaluated using tumor downstaging by MR, histopathological staging, and expressed as residual disease (%). Results and Conclusion: The outcome evidenced the correlation between the cumulative 5-FU dose and cumulative AUC of 5-FU (r = 0.61; p < 0.001). The similar relationship was demonstrated between the cumulative AUC and metabolic ratio (the plasma concentration od inactive metabolite dihydrofluotouracile 5-FUH2 to 5-FU; r = -0.80; p < 0.0001). The cumulative AUC was correlated with tumor regression rate (r = -0.53; p < 0.005) and determined toxicity grade. To reach pCR, the daily dose of 5-FU in patient with average metabolic ratio of 5-FUH2/5-FU should be >350 mg/m2 and the cumulative AUC1–39days > 50 mg/L*h. We report the case of a 52year old man with severe acute pancreatitis. In this case report we discuss the undesirable effects of parenteral nutrition and the importance of nutritional support at patients with severe necrotizing pancreatitis. Severe acute pancreatitis is usually accompanied by systemic inflammatory response syndrome, which results in hypermetabolism with prominent protein catabolism. Providing nutrition to these patients is of paramount importance. An adequate nutritional support is crucial in patients with severe and complicated pancreatitis. Nádory hlavy a krku jsou celosvětově 6. nejčastějším zhoubným nádorem. Histologicky se ve většině případů jedná o spinocelulární karcinomy s lokalizací dutiny ústní, orofaryngu, laryngu, hypofaryngu a dutiny nosní. Hlavním rizikovým faktorem je kouření a alkohol. Nově se diskutuje souvislost s infekcí lidského papilomaviru (HPV 16), zejména v oblasti orofaryngu. Pacienti mající infekci HPV 16 mají 15–50× vyšší riziko pro vznik karcinomu. Dle výsledků zkoumání se jedná o rozdílnou formu karcinomu s výskytem u mladších pacientů, lepšího sociálního stavu, nejčastěji v oblasti orofaryngu a báze jazyka, s pokročilým postižením uzlin, s lepší prognózou proti HPV negativním tumorům. Základem léčby je chirurgie a radioterapie, chemoterapie a nově cílená léčba. Ve většině případů (nad 50 %) se jedná o lokálně pokročilé onemocnění zasahující oblast polykání, mluvení – výrazně omezující denní aktivity. Chemoradioterapie s cisplatinou je standardem pro pacienty s pokročilým neresekabilním karcinomem hlavy a krku. Ve studiích fáze III byla srovnávána samostatná radioterapie proti přidání cetuximabu k radioterapii. Výsledky odpovědi pro kombinaci s cetuximabem 74 % vs. 64 %, s mediánem přežití 49 vs. 29,3 měs. (Bonner, 2006). Cetuximab je monoklonální protilátka třídy IgG1, blokující vazbu endogenních EGFR ligand, což vede k inhibici funkce receptoru. EGFR je exprimován u nádorů epiteliálního původu. U nádorů hlavy a krku se vyskytuje téměř u všech – nad 90 %., Worldwide, head and neck tumours are the sixth leading malignant tumour. Histologically, the majority of cases are squamous cell carcinomas of the oral cavity, the oropharynx, the larynx, the hypopharynx, and the nasal cavity. Smoking and alcohol are the major risk factors. Recently, an association with the human papillomavirus (HPV 16) infection, particularly in the area of the oropharynx, has been discussed. Patients with the HPV 16 infection have a 15–50-fold higher risk of developing carcinoma. According to research results, it is a distinct form of carcinoma occurring in younger patients, of a higher social status, most frequently in the area of the oropharynx and the base of tongue, with advanced lymph involvement, and with a better prognosis compared to HPV-negative tumours. The mainstay of treatment is surgery and radiotherapy, chemotherapy, and, more recently, targeted therapy. In most cases (over 50%), there is locally advanced disease involving the area of swallowing and speaking, thus significantly limiting daily activities. Chemoradiotherapy with cisplatin is the standard treatment for patients with advanced unresectable head and neck cancer. Radiotherapy alone versus adding cetuximab to radiotherapy was compared in phase III trials. Plicní karcinomy jsou z onkologických onemocnění nejčastější příčinou smrti, nemalobuněčný plicní karcinom je nejčastějším histologickým typem plicního tumoru. Gefitinib je prostředkem biologické léčby pokročilého a metastatického nemalobuněčného plicního karcinomu u pacientů s aktivační mutací receptoru pro epidermální růstový faktor v první linii léčby, tzn. u chemonaivních pacientů. Kazuistika se zabývá dvěma případy pacientů léčených gefitinibem s podobnými vstupními parametry a odlišným efektem léčby., Lung carcinoma is the most common cause of death among cancer patients. Cílem našeho sdělení je demonstrovat možnosti základního vyšetření likvoru (CSF, z angl. cerebrospinal fluid = mozkomíšní mok) při sledování následného vývoje stavu centrálního nervového systému (CNS) u pacienta po subarachnoidálním krvácení. Klíčovou problematikou je především odlišení purulentních zánětlivých změn bakteriální etiologie a neinfekčního serózního zánětu ve smyslu úklidové reakce v CNS při opakovaně negativním mikrobiologickém vyšetření CSF. Možným řešením je společné hodnocení cytologického obrazu CSF a energetických poměrů v likvorovém kompartmentu vedoucí k určení charakteru zánětlivého procesu v CNS. Informační potenciál základní likvorologie však tímto není vyčerpán. Na případu dále předvádíme sledování poruchy cirkulace CSF vedoucí ke vzniku hydrocefalu, detekci probíhající destrukce tkáně při ischemizaci mozkového parenchymu a hodnocení přítomnosti krvácení do likvorových cest. Vývoj komplexního likvorového obrazu je pak průběžně konfrontován s výsledky zobrazovacích vyšetření a přispívá k optimalizaci terapeutických postupů až do ukončení intenzivní péče o pacienta., Our paper demonstrates the possibilities of basic examination of the cerebrospinal fluid (CSF) when monitoring the central nervous system (CNS) in a patient after subarachnoid hemorrhage. The key problem is to differentiate between purulent inflammation of bacterial etiology and non-infectious serous inflammation, the so called „clean-up reaction”, in the CNS during repetitively negative microbiological examinations of the CSF. Combined evaluation of the cytological image and energy turnover in the CSF compartment can help in specifying the character of inflammation of the CNS. The information potential of the CSF examination, however, is not depleted yet. In this case report we further demonstrate the failure of the CSF circulation leading to hydrocephalus, detection of tissue destruction during brain ischemia and detection of hemorrhage into the cerebrospinal fluid pathways.
