63letý pacient, hypertonik, celoživotní kuřák, byl na kliniku TRN FN Plzeň přijat pro tumor pravé plíce zjištěný pro bolesti v pravém hemitoraxu. Na základě stážování byla stanovena diagnóza adenokarcinomu pravé plíce s generalizací do obou plic a levé nadledviny, st. IV. Byla zahájena chemoterapie Paclitaxel (200 mg/m2) – Carboplatina (AUC) – Avastin (15 mg/kg) v rámci studie SAIL firmy Roche s velmi dobrou tolerancí. Po 3. cyklu chemoterapie byla potvrzena parciální remise dle CT hrudníku, dále pokračováno ve stejném schématu chemoterapie s Avastinem do 6. cyklu. Kontrolní CT hrudníku prokázalo další regresi tumoru. Vzhledem k velmi dobrému efektu podávané léčby bylo dále pokračováno v monoterapii Avastinem v třítýdenních intervalech zatím do 22 cyklů. Po 22 měsících léčby s Avastinem bylo dosaženo trvající parciální remise nálezu dle PET/CT trupu, pacient je klinicky zcela bez potíží, neomezen v běžném životě, tedy s velmi dobrou kvalitou života (KS 90 bodů), bez vedlejších nežádoucích účinků Avastinu. Chemoterapie již nemá potenciál pro další prodlužování přežití, je tedy potřebné hledání nových léčebných strategií. Avastin jako inhibitor angiogeneze vede poprvé k prodloužení celkového přežití nad 12 měsíců. Významné prodloužení přežití bez progrese onemocnění (PFS) umožňuje delší přežití bez zhoršení symptomů., A 63-year-old hypertensive patient, a life-long smoker, presented to the Department of Tuberculosis and Respiratory Disease of the Plzeň Teaching Hospital with a right-lung tumour detected due to pain in the right hemithorax. Based on staging, a diagnosis of stage IV right lung adenocarcinoma with generalization into both lungs and the left adrenal gland was made. Chemotherapy with Paclitaxel (200 mg/m2) – Carboplatin (AUC) – Avastin (15 mg/kg) was initiated within the SAIL study of the Roche company with very good tolerance. Following the third chemotherapy cycle, partial remission was confirmed based on CT of the chest; the same chemotherapy regimen with Avastin was continued for six cycles. Follow-up CT of the chest revealed further tumour regression. Given the very good effect of the treatment administered, monotherapy with Avastin was continued in three-week intervals for 22 cycles so far. Following 22 months of Avastin treatment, sustained partial remission was achieved according to PET/CT of the trunk; the patient is clinically free of complaints, without limitations in everyday life, thus with a very good quality of life (KS of 90) and free of adverse effects of Avastin. Chemotherapy has no more potential for further extension of survival; therefore, new therapeutic strategies have to be looked for. As an angiogenesis inhibitor, Avastin leads to an extension of overall survival beyond 12 months. Desmoplastický kulatobuněčný tumor (DSRCT) je raritní onemocnění patřící do skupiny sarkomů. V typickém případě se manifestuje rozsáhlou tumorózní infiltrací břišní dutiny bez evidentního primárního ložiska tumoru. Prognóza onemocnění je stále špatná, 5leté přežití je popisováno asi v 15 % případů i při užití kombinovaných protinádorových léčebných postupů. Uvádíme kazuistiku 34letého pacienta s DSRCT léčeného v našem centru. Zmiňujeme racionální postup v rámci diferenciální diagnostiky měkkotkáňové infiltrace v dutině břišní, podtrhujeme zásadní význam molekulárně genetické analýzy a zaměřujeme se na terapeutické možnosti tohoto zřídka se vyskytujícího onemocnění., Desmoplastic small round cell tumor (DSRCT) is a rare disease from sarcoma group family. Typically, there is an extensive infiltration of abdominal cavity, peritoneal effusion with no evident primary origin of tumor. Prognosis is unfavourable with 5-years overall survival described in 15 % of cases even though multimodal treatment strategy is performed. We introduce case report of 34-years old patient with DSRCT treated in our Oncology center. Reasonable schema of differential diagnosis of soft-tissue expansion in abdominal cavity is mentioned and we emphasize crucial role of molecular genetic analysis. Ve sdělení autoři prezentují kazuistiku duplicity primárních mozkových nádorů. Její výskyt je velmi vzácný, udává se méně než 1:1 000 000. U 69letého pacienta byl diagnostikován frontotemporálně vpravo lokalizovaný multiformní glioblastom způsobující klinickou symptomatologii a dále klinicky němý drobný meningeom v oblasti levého mostomozečkového koutu. Multiformní glioblastom byl subtotálně resekován z kraniotomie a drobný asymptomatický meningeom byl ponechán k observaci., The authors present a case report of duplicity of primary brain tumours. It occurs very rarely with an incidence reported to be 1/1 000 000. A 69-year-old patient was diagnosed with a glioblastoma multiforme localized frontotemporally on the right which caused clinical symptomatology and with a clinically silent, small meningioma in the left pontocerebellar angle. Histiocytóza z Langerhansových buněk je vzácné onemocnění, vznikající proliferací Langerhansových dendritických buněk, jehož etiologie je nejasná. Vyskytuje se převážně v dětském věku. Vysoké procento pacientů má projevy nemoci v oblasti hlavy a krku. Nejčastěji postiženým orgánem je lebka, zejména temporální kost. V organizmu se může nacházet jedno nebo více oseálních i extraoseálních ložisek, nebo je postižen jeden nebo více orgánů. Klinické projevy imitují řadu jiných chorob, proto je diagnostika často obtížná a protrahovaná (1). Otorinolaryngolog se nejčastěji setkává se symptomy podobnými chronické externí otitidě, akutní mediotitidě, mastoiditidě, dermatitidě nebo onemocnění probíhá pod obrazem nádoru. V této práci přinášíme kazuistiku dospělého pacienta, který vyhledal ošetření pro otoreu nereagující na standardní léčbu., Langerhans´cells histiocytosis (LCH) is a rare disease, in which the proliferation of Langerhans´ cells is believed to be causative, affecting mainly children. The etiology still remains unclear. The head and neck region is frequently involved, the skull being the most often diseased site, especially the temporal bone. It may affect also another organs and systems with single or multiple osseal or extraosseal lesions. LCH can be challenging to diagnose with it´s varied presentations, which may resemble another common diseases. Jako light chain escape fenomén (LCE) je označována změna biologického chování nádoru u pacientů s mnohočetným myelomem (MM). Při diagnóze jsou v séru přítomny kompletní monoklonální imunoglobulin (mIg) a monoklonální volné lehké řetězce (mFLC), při relapsu nebo progresi onemocnění dochází k prudkému vzestupu koncentrace pouze mFLC, obvykle při stabilní nebo klesající koncentraci mIg, společně s těžkým orgánovým postižením. Pro časnou diagnostiku LCE u nemocných s MM je doporučováno sériové vyšetřování FLC. LCE byl popsán v literatuře zatím pouze u pacientů po hematologické terapii. V tomto sdělení prezentujeme kazuistiku pacienta vstupně hodnoceného hematologem jako doutnající myelom, bez nutnosti hematologické terapie. Domníváme se, že vývoj nálezu odpovídal kritériím LCE., The light chain escape phenomenon is characterized as a change in tumor biological movement in patient with multiple myeloma. Complete monoclonal immunoglobulin molecules (mIg) together with monoclonal free light chains (mFLC) are present in diagnosis, only mFLC great increase is observed in relapse or disease progression together with heavy organ impairment whether mIg concentration is usually stable or decreasing. Serum FLC monitoring is recommended for early diagnostics LCE phenomenon in patients with MM. To this time LCE phenomenon was described only in patients after hematological therapy. Autoři předkládají přehledový článek shrnující současný pohled na diagnostiku a léčbu neuroendokrinních nádorů pankreatu. Zmiňují aktuální TNM klasifikaci těchto neoplazií. Ze zobrazovacích metod podtrhují důležitost CT, MRI, EUS a octreotidové scintigrafie. V léčbě zdůrazňují snahu o radikální (R0) resekci, která přináší nejlepší dlouhodobé výsledky. Připouštějí i možnost debulking resekcí a transplantace jater v případě resekabilního origa a neresekabilních jaterních metastáz. Poukazují na rozdílnou prognózu pankreatických neuroendokrinních nádorů a karcinomu pankreatu. Zdůrazňují nutnost mezioborové spolupráce, kterou dokládají dvěma kazuistikami pacientů s lokálně neresekabilním a metastatickým nádorem., The authors put forward a review article summarizing current opinion in diagnostics and therapy of pancreatic neuroendocrine tumors. They present actual TNM classification of these tumors highlighting CT, MRI, EUS and octreoscan as standard imaging methods. In the field of therapy, radical, R0 resection, which brings the best overall survival, is accented. They also accept the possibility of debulking resections and liver transplantation in the cases of local advanced and metastatic disease respectively. They point out the different prognosis of pancreatic neuroendocrine tumors and pancreatic cancer.
