A hybrid approach to elephant trunk technique for treatment of thoracic aortic aneurysms combines a conventional surgical and endovascular therapy. Compared to surgery alone, there is a presumption that mortality and morbidity is reduced. We present a case report of a 42-year-old man with a giant aneurysm of the entire thoracic aorta, significant aortic and tricuspid regurgitation and ventricular septum defect. The study aimed to investigate the role of aggression and impulsivity in smoking among male adult students. It was hypothesized that there would be significant difference between smokers and non- smokers on the traits of aggression and impulsivity. The sample of two hundred male adult students was selected from different universities of Islamabad. Two subscales of Impulsiveness and Aggression were administered to measure the personality traits of aggression and impulsivity. Descriptive statistics and t-test was calculated for analysis of data. Results showed that there is significant differences in aggression and impulsivity between smokers and non-smokers. Introduction: Obstruction of the appendiceal lumen is the primary cause of appendicitis. The most common causes of luminal obstruction are fecaliths and lymphoid follicle hyperplasia. Additionally, bacterial infections or enteric and systemic viral diseases can cause a reaction of the lymphoid follicle. Case presentation: An 11-year-old boy with active phase of chickenpox presented on our Pediatric surgery emergency department under the impression of acute appendicitis. An appendectomy was performed on the same day. An inflamed and edematous retrocecal appendix was removed during surgery. Histological investigation of the appendix revealed transmural acute inflammation, with diffuse proliferation of inflammatory cells, with characteristic intranuclear inclusion surrounded by a clear halo. The PCR analysis of peripheral blood and appendix tissue specimen revealed positive VZV DNA. In the last decade the health of young people in Ukraine as well as in many other countries of the world significantly deteriorated according to the results of screening tests. Because of this the main task of prophylactic medicine is development and introduction of new methods of early diagnostics of diseases. The aim of the work to assess the peculiarities of adaptation of students with different types of vegetative regulation. Material and methods: 415 second year students of Lugansk State Medical University were tested. The mechanisms of physiological functions of the body regulation were assessed by registration of cardiac rhythm variability (CRV). Analysis of CRV was performed by three methods: statistical, geometrical, and spectral analysis. The express method proposed by Shlyk, N.I. (2009) was later used to determine the prevailing type of vegetative regulation. To assess adaptability the method of R.M. Baevskij (1979) was used. Results: In the result of investigation 4 types of functional states of regulatory systems were determined: I type 295 (71 %) of tested, II in 14 (3 %), III in 96 (23 %) and IV only in 10 (2%). I type of functional state of regulatory systems in the wakeful state at rest was characterized by moderate prevalence of central regulation of heart rhythm (MPCR), decreased activity of autonomic regulation. It was noticed that some of the parameters of CRV (like RR, SDNN, RMSSD and pNN50) which characterize autonomic regulation, were statistically lowere (р≤0.01) compared to type III, and some (like AMo and SI) higher, in both female and male students. Summarized spectrum capacity and components of its wave structure (HF, LF, VLF) which characterize central regulation were statistically significantly lower (р≤0.01) for the I type of regulation compared to the III one which is characterized by moderate prevalence of autonomic regulation of the heart rhythm. In female students with type I autonomic regulation compared to male students such CRV parameters as RR and AМo were statistically significantly lower (р≤0.05), while the parameters of total potency (TP) and high frequency waves (HF) were higher (р≤0.05), which could point to more centralized heart rhythm regulation in male compared to female. The evaluation of regulatory systems overstress allowed to determine the state of adaptation and risk of overstress or breakdown in students with different types of autonomic regulation. Cíl: Průzkumné šetření zjišťovalo schopnosti spolupráce nemocných při léčbě Warfarinem a dodrţování léčebného reţimu v průběhu dlouhodobé antikoagulační terapie Warfarinem. Konkrétně byla mapována a vyhodnocena úroveň edukace sestrou, informovanost a adherence k léčbě u pacientů léčených Warfarinem. Metodika: Průzkumný soubor tvořilo 108 pacientů evidovaných v ambulantním léčení interní a kardiologické ambulance při interním oddělení Nemocnice ve Frýdku-Místku. Průzkumné šetření probíhalo v prosinci roku 2010 a lednu roku 2011. Pro sběr dat byla vyuţita metoda dotazníkového šetření. Výsledky: Výsledky průzkumného šetření ukazují, ţe naprostá většina pacientů léčených Warfarinem se aktivně podílí na léčebném procesu. Nemocným se od zdravotníků dostává poučení o léčbě, sestry se na edukaci podílejí v 75 %. Většina dotázaných dochází na pravidelné kontroly a zná zpaměti uţívanou dávku léku. Čtvrtina nemocných však není schopna uvést krvácení jako hlavní riziko léčby. Převáţná většina respondentů si je vědoma skutečnosti, ţe účinek léčby můţe být ovlivněn sloţením přijímané potravy, méně neţ polovina respondentů zná interakci Warfarinu s jinými léky či doplňky stravy. Nemocní mají na zřeteli, ţe je nutno informovat kaţdého lékaře o této léčbě, jen polovina respondentů však informuje i lékárníka. Závěr: Adherence k léčbě ze strany pacienta je základem úspěšné a bezpečné terapie Warfarinem. Cílem všech zdravotníků by měla být snaha o kvalitní a efektivní komunikaci s nemocnými, snaha o dosaţení co nejvyšší úrovně spolupráce, ochoty a schopnosti pacientů řídit se pokyny zdravotníků. Tím se výrazně zvýší úspěšnost léčby a eliminuje se riziko moţných komplikací léčby., Aim: Exploratory investigation ascertained capabilities of cooperation of patients when treated with warfarin and adherence to treatment regimen during long term anticoagulant therapy by warfarin. Specifically was mapped and assessed the level of education by nurse, foreknowledge and adherence to treatment in patients treated with warfarin. Methods: The exploratory set consisted of 108 patients, who were registered in ambulant treatment of internal and cardiac ambulance at internal department of Hospital in Frydek-Mistek. Exploratory investigation took place in December 2010 and January 2011. For data collection was used method of questionnaire survey. Results: The results of questionnaire survey show, that absolute majority of patients treated with warfarin is actively participating in the treatment process. The patients receive from the health care providers education about the treatment, nurses are participating in education in 75%. Majority of respondents have regular check ups and knows by heart the dose of the drug. A quarter of patients is not able to state bleeding as the main risk of treatment. The vast majority of respondents are aware of the fact, that the treatment effect may be influenced by the composition of the diet, less than half of the respondents know the warfarin interactions with other medication or supplements. Patients have in mind that every doctor should be informed about this treatment, only half of the respondents, however, informs as well the pharmacist. Conclusion: Adherence to treatment by the patient is the foundation of a successful and safe warfarin therapy. The goal of all health professionals should be to aim for quality and effective communication with patients, effort to achieve the highest level of cooperation, willingness and ability of patients to follow medical instructions. Cíl: Nepravidelné dávkování inzulinu je jedním z hlavních problémů spojených s inzulinovou terapií u pacientů s diabetem 2. typu; jeho skutečný rozsah však není přesně znám. Cílem průzkumu provedeného v ČR v rámci mezinárodního projektu GAPP2TM – Globální přehled přístupů pacientů a lékařů (Global Attitudes of Patients and Physicians) proto bylo ověřit, jaký je výskyt a dopad nepravidelného užívání bazálních inzulinových analog u pacientů s diabetem 2. typu, jakým způsobem a z jakých důvodů k těmto nepravidelnostem dochází, a jakým způsobem zdravotníci o nepravidelné aplikaci inzulinu s pacienty diskutují. Metodika: Projekt GAPP2TM je mezinárodní průřezovou studií provedenou on-line formou dotazníku prostřednictvím internetu u pacientů s diabetem 2. typu léčených inzulinovými analogy a u lékařů a zdravotníků, kteří tyto pacienty ošetřují. Průzkum proběhl ve dvou vlnách v celkem 17 zemích světa; do první vlny, která byla dokončena začátkem roku 2012, bylo zahrnuto 6 zemí, do druhé, dokončené v roce 2014, pak dalších 11 zemí včetně České republiky. Průzkum měl za cíl získat data o pohledech pacientů a lékařů na některé aspekty léčby diabetu 2. typu inzulinem a o přetrvávajících problémech v této oblasti v reálné každodenní praxi. Zabýval se zejména incidencí a zvládáním lehkých hypoglykemií a nepravidelnostmi v aplikaci inzulinu. V části zaměřené na adherenci k aplikaci bazálního inzulinu byly sledovány 3 typy nepravidelností v léčbě inzulinem: vynechání dávky, časový posun dávky (? 2 hodiny oproti předepsanému času) a snížení dávky, ve všech v případech během uplynulých posledních 30 dnů před vyplněním dotazníku. Dále bylo zkoumáno chování pacientů ve vztahu k této problematice a jejich vnímání těchto problémů. Výsledky: Výsledky průzkumu ukázaly, že nepravidelné dávkování inzulinu je v České republice méně časté než v ostatních zemích, v nichž probíhal výzkum GAPP2TM. I tak se ovšem týká cca pětiny všech nemocných s DM 2. typu léčených inzulinovými analogy v režimu bazál-bolus nebo pouze bazál. Při poslední nepravidelné aplikaci bazálního inzulínu se jednalo o záměr pacienta při úplném vynechání dávky ve 13 % případů, posun aplikace byl záměrný ve 23 % a snížení dávky inzulínu v 61 % případů. Nejčastěji hlášenými důvody bylo snížení rizika hypoglykemie a dodržování doporučení zdravotníků. Obavu z vynechání dávky bazálního inzulinu má 40 % českých pacientů a 35 % by mělo pocit viny, pokud by svou dávku bazálního inzulinu vynechali (u pacientů s intenzivním inzulinovým režimem je to 47 %). Pouze 60 % pacientů si uvědomuje, že vynechávání dávek bazálního inzulinu může mít negativní dopad na jejich dlouhodobý zdravotní stav. Dotazovaní lékaři měli podezření, že během pravidelných kontrol hlásí nižší počet vynechaných dávek bazálního inzulínu nebo vynechávání zcela zatajuje přibližně třetina pacientů. Tuto skutečnost však připustilo jen 11 % pacientů užívajících bazální inzulin a 15 % pacientů léčených intenzifikovaným režimem. Čtvrtina předepisujících lékařů navíc uvádí, že se svými pacienty nepravidelné užívání bazálního inzulinu během kontrol běžně neprobírá. Závěry: Ačkoli pacienti s diabetem 2. typu dodržují v České republice předepsané schéma léčby bazálním inzulinem častěji než v jiných zemích účastnících se průzkumu GAPP2TM, vynechání, časový posun a snížení dávky inzulinu je poměrně časté a ze strany zdravotníků si zasluhuje během pravidelných kontrol více pozornosti a cílenou edukaci., Objective: Irregular insulin dose is one of the main problems associated with insulin therapy in patients with type 2 diabetes; its extent is not known precisely. The aim of survey conducted in the Czech Republic in the international project GAPP2™ – Global Attitudes of Patients and Physicians was to determine the incidence and the impact of irregular use of basal insulin analogues in patients with type 2 diabetes, to point out the reasons for these irregularities and to focus on how physicians discuss irregular application of insulin with patients. Methods: The project GAPP2™ is an international cross-sectional study performed on-line via the Internet using a questionnaire filled by diabetic patients treated with insulin analogues and physicians who treat these patients. The survey was conducted in two steps in 17 countries; the first step included 6 countries and was completed in the beginning of 2012, the second step involved 11 other countries including the Czech Republic with termination in 2014. The survey was designed to obtain the views of patients and physicians on certain aspects of insulin treatment and persistent issues in this field in the real daily practice. Special focus was on the incidence and management of hypoglycaemia as well as on irregularities of insulin application. In the part dedicated to adherence to basal insulin application were observed three types of irregular insulin therapy: missed dose, time imprecision of dose (? 2 hours vs. the prescribed time) and dose reduction in all cases in the past 30 days before completing the questionnaire. In addition, it was investigated the attitude and relation of patients to these issues. Results: The results have shown that irregular insulin dose in the Czech Republic is less frequent than in other countries involved in the GAPP2™ research. Nevertheless, approximately one fifth of diabetic patients using insulin analogues in basal-bolus or only basal therapy regimen is related to this problem. The last irregular insulin application was due to missed dose in 13% of cases, time imprecision in 23% and reduction of dose in 61% of cases. The most commonly reported reason was risk reduction of hypoglycaemia and the recommendations of health professionals. Fear of missed dose is present in 40% Czech patients and 35% would feel guilty if their insulin dose is missed (up to 47% in patients with intensified insulin regimen). Only 60% patients are aware of negative impact on their long-term health after missed dose of basal insulin. Questioned doctors have suspected that the patients report lower number of missed doses during regular medical check because one third of patients doesn´t admit missed dose. However, this fact conceded only 11% of patients on basal insulin and 15% of patients on intensified insulin therapy. Quarter of prescribing doctors admit that they usually don´t discuss with patients irregularities in basal insulin treatment. Conclusion: Although, type 2 diabetes patients in the Czech Republic follow prescribed basal insulin therapy scheme more often than patients in other countries participating in the survey GAPP2 ™, missed dose, time imprecision and reduction of dose is quite common and it deserves more attention from medical side during regular medical check together with appropriate education of patients., and Martin Prázný
Východiska: Vzhledem ke geriatrizaci ošetřovatelské péče se ageismus stává často diskutovaným tématem. Z tohoto důvodu je velmi důležité, aby nelékařští zdravotničtí pracovníci byli v tomto ohledu dostatečně vzděláváni, protože právě dostatečná informovanost a praktická zkušenost může ovlivnit míru inklinace k ageistickým postojům. Cíl: Cílem výzkumného šetření bylo zhodnotit ageistické postoje u studentů středních zdravotnických škol v závislosti na škole a ročníku, kterou respondenti navštěvovali a zjistit rozdíly ve vnímání seniorské populace. Metody: V souladu s cíli práce byl proveden kvantitativní výzkum formou dotazníkového šetření. Na základě bodového ohodnocení odpovědí v dotazníku byla zjištěna míra ageistických postojů u respondentů. Výsledky: Práce prokázala, že je statisticky významný vztah mezi ageistickými postoji u studentů a jejich příslušností ke střední škole. Stejně tak byl prokázán rozdíl mezi ageistickými postoji u studentů druhých a čtvrtých ročníků středních zdravotnických škol. Závěry: Z práce vyplývá, že ve sledovaném souboru se u většiny studentů objevuje střední inklinace k ageistickým postojům s ohledem na navštěvovanou školu. Dále pak, že k ageistickým postojům inklinují více studenti čtvrtých ročníků, kteří již absolvovali odbornou praxi u lůžka nemocného, než studenti druhých ročníků, kteří tuto praxi ještě neabsolvovali., Background: Due to geriatrization of nursing care becomes oen discussed theme. Because of this it’s very important that non-medical health staff will be enough educated. It is that sufficient awareness and practical experience that can affect the rate of inclination to ageist attitudes. Objective: e objective of this research was to evaluate ageist attitudes of nursing school students according to school which the respondents visited and find out the differences in perception of senior population among the students of second and fourth grade. Methods: Due to the aims of this work the quantitative research has been done through a questionnaire survey. e rate of ageist attitudes was observed based on scoring responses. Results: A statistically significant relationship between symptoms of ageist attitudes this work proved as well as the differences between symptoms of ageist attitudes among second and fourth grade students. Conclusions: e work shows that in the sample survey, the majority of students appearing middle inclination to the myths of old age. Karcinom prostaty i přes stále se zlepšující diagnostiku patří ke druhé nejčastější příčině úmrtí na maligní onemocnění u mužů v ČR. Již od 80. let minulého století jsou vidět první pokusy o systematický screening zavedením pravidelných kontrol digitálním rektálním vyšetřením a ultrazvukem, poté se v 90. letech objevuje na scéně prostatický specifický antigen (PSA), který znamená opravdový průlom v časné detekci karcinomu prostaty. V současnosti je screening rutinně prováděn pomocí vyšetření per rektum, PSA doplněného o stanovení free PSA a v indikovaných případech odběru biopsie. Cílem práce je podat základní shrnutí současných možností screeningu a časné detekce karcinomu prostaty., Active approach to prostate cancer detection – what is reasonable and what may cause harm Despite the continuously improving diagnostics, prostate cancer represents the second most common cause of death from malignancy in men in the Czech Republic. Beginning in the 1980´s the first attempts of systematic screening were made by introducing regular check-ups using digital rectal examination and ultrasound screening, then in the 1990´s prostate specific antigen (PSA) appeared on stage, which meant a real breakthrough in the early detection of prostate cancer. Currently screening is routinely performed using rectal examination, PSA test supplemented by determination of free PSA and prostate biopsy, when indicated.
