Meduloblastom je embryonální neuroektodermální nádor mozečku, je nejčastějším maligním nádorem centrálního nervového systému v dětském věku. Ročně onemocní meduloblastomem v České republice přibližně 10 až 12 dětí. Současná léčba kombinuje chirurgickou resekci, radioterapii a chemoterapii. Významný pokrok v léčebných výsledcích byl dosažen zlepšením neurochirurgických technik, optimalizací dávek radioterapie a zavedením kombinované adjuvantní chemoterapie. Intenzita léčby se odvíjí od věku dítěte, rozsahu chirurgické resekce a přítomnosti metastáz. Současná léčebná schémata dosahují 5letého bezpříznakového přežití v 70–81 % u pacientů standardního rizika a 30–70 % u pacientů vysokého rizika. Porozumění nádorové biologii meduloblastomu umožní definovat terče pro cílenou biologickou léčbu i blíže určit riziko konkrétního onemocnění. Pokroky v léčbě Hodgkinova lymfomu (HL) patří k největším úspěchům moderní onkologie. V současné době dosahuje dlouhodobé remise (podle stadia a přítomnosti rizikových faktorů) 85–95 % pacientů, jejich životní naděje je nyní 40 let. Do popředí však začaly vystupovat pozdní následky léčby HL: kardiotoxicita, sekundární neoplazie, plicní toxicita, poruchy fertility (azoospermie, amenorea), poruchy funkce štítné žlázy, imunosuprese s infekčními komplikacemi, psychosociální problémy. Mezi další komplikace patří poškození měkkých tkání (svalů, orgánů gastrointestinálního traktu), kostí a neurologické komplikace. Současné znalosti o pozdní toxicitě léčby HL se zakládají na sledování pacientů, kteří byli léčeni terapeutickými modalitami, které se již delší dobu nepoužívají., Progress in the treatment of Hodgkin’s lymphoma (HL) is one of the huge successes in modern oncology. According to the stage and presence of risk factors, up to 85–90 % of patients achieve long-term remission with life expectancy of 40 years. New problem to deal with at the moment are the late adverse effects caused by preceeding therapy of HL: cardiotoxicity, secondary neoplasia, lung toxicity, impairement of fertility (azoospermia, amenorhea), impairement of the thyroid function, imunosupression with infectious complications, psychosocial problems. Other complications can occur: impairement of soft tissue (muscles, gastrointestinal organs), bones and neurological disorders. To improve the results of radiotherapy for cervical cancer used local and systemic radiomodification drugs. Materials and methods: 1984 patients with locally advanced cervical cancer (2b 3a stage), which at one stage of complex treatment by a course indoarterial of regional longterm polychemotherapy (EARDPHT) mode: methotrexate 50 mg/m2, the first 12 hours, fluorouracil 1000 mg/m2, over the next 12 hours and cisplatin 100 mg/m2, for 48 hours, 2 stage surgical treatment in the volume expanding hysterectomy 3 type (modification of Wertheim Meigs). Results: The results of treatment of locally cervical cancer using VADRPHT, depending on the unilateral and bilateral tumor invasion in parametrial tissue showed that under unilateral lesion of parametrium results of treatment were higher than for the bilateral involvement of parametrium. Medical pathomorphosis in tumors of different severity was observed in 86.9% of patients, metastasis to regional lymph nodes were detected in 36% of operated patients, and also studied the degree of therapeutic pathomorphosis in lymph nodes. There was revealed that the complete regression of tumors, 5 year survival rate is 98%, whereas in tumor regression of more than 50% of the original volume 78.5%, with stabilization of tumor no more than 58.4%.
