INTRODUCTION: Leptin is an adipokine which has a direct relationship to obesity. Our aim was to measure this hormone in pregnant women at three months intervals throughout their pregnancies to determine the serum value of those who developed preeclampsia. MATERIAL AND METHODS: We followed 19 women (median age 24.8 +/- 5.7 years) with pre-gestational Body Mass Index (BMI) less than 25 kg/m2, 21 (median age 26.1 +/- 4.6 years) with BMI higher than 25 kg/m2 and 16 (median age 30.9 +/- 5.8 years) with Gestational Diabetes Mellitus (GDM) (median age 30.9 +/- 5.8 years), recruited in the 1st trimester of pregnancy. Serum levels of leptin were measured with radioimmunoassay (RIA) technique. RESULTS: In the first trimester of pregnancy leptin levels showed statistically significant differences between normal weight and overweight-obese women (p < 0.001), diabetic women (p < 0.05) and the subgroup of preeclamptic women (p < 0.001). For those women with PGBMI > or = 40 kg/m2 and leptin > or = 40 ng/ml in the second trimester, the Odds Ratio (OR) to develop preeclampsia was of 47.95% CI (4.1-527.2). Analyzing leptin values with ROC curves, the greatest area under the curve (AUC) was for leptin in the second trimester (0.773, CI: 0.634-0.911). CONCLUSION: Women with morbid obesity (BMI > or = 40 kg/m2) had significantly higher levels of serum leptin (p < 0.01) and a value of 40 ng/ml of this hormone seems to be predictive of developing preeclampsia in this group of patients. and H. Hypertenze je nejčastějším kardiovaskulárním onemocněním, které se v mladém věku častěji vyskytuje u mužů; ve věkové skupině 45–64 let je hypertenze stejně častá u obou pohlaví, u starších osob je hypertenze častější u žen. Krevní tlak se více zvyšuje u žen v období kolem menopauzy. Podávání hormonální substituční léčby není provázeno vzestupem TK, ale vzhledem k vyššímu riziku koronárních, cévních mozkových a tromboembolických příhod není hormonální substituční léčba v prevenci kardiovaskulárních onemocnění (KVO) indikována. Pokles TK v důsledku léčby hypertenze a přínos z léčby hypertenze je podobný u mužů a u žen. Ženy však udávají zhruba dvojnásobné množství nežádoucích účinků. Užívání perorální antikoncepce je spojeno s mírným vzestupem TK u většiny žen a s rozvojem zjevné hypertenze přibližně u 5 % žen. There is a similar decrease in BP by antihypertensive drugs in both genders as well as benefit from antihypertensive treatment. Women report about a double rate of adverse events of antihypertensive drugs. Oral contraception use is associated with a mild BP increase in most women and development of overt hypertension in about 5 %. Erytrocytární aloimunizace (izoimunizace, hemolytické onemocnění plodu a novorozence – HON) se v současnosti díky rutinní imunoprofylaxi anti-D imunoglobulinem vyskytuje sporadicky. Výskyt atypických aloimunizací non-RhD-antigeny zůstává neměnný, poněvadž není známa profylaxe a ani prevence nezajišťuje stoprocentní účinnost. Pokud se imunoprofylaxe anti-D imunoglobulinem neprovede nebo selže, hrozí vznik hemolytického onemocnění plodu a novorozence. Každý případ je potom unikátní (specifita a typ protilátky, blokující protilátky v séru matky, antigenní výbava erytrocytů plodu, gestační stáří při začátku onemocnění atd). Pokroky v genové diagnostice, imunohematologii a dopplerovské ultrasonografii významně snižují frekvenci nutných invazivních zákroků v diagnostice a léčbě senzibilizovaného těhotenství. Intraumbilikální přísně intravenózní transfuze pod ultrasonografickou kontrolou je první metodou volby léčby případů s těžkou anémií plodu i při rozvinutém hydropsu. Konzultace a léčba se soustřeďují do center s perfektní porodnickou a neonatologickou péčí a nepřetržitou operační pohotovostí. The occurrence of atypical alloimmunizations related to non-RhD-antigens remains unchanged, as there is no prophylaxis known and even the prevention does not provide one hundred percent efficiency. When anti-D immunoglobulin immunoprophylaxis is not undertaken or fails, the threat of hemolytic disease of the fetus and newborn arises. Every case is then unique (specificity and type of antibody, blocking antibodies in maternal serum, antigenic make-up of fetal erythrocytes, gestational age at the disease onset etc). The advancements in genetic diagnostics, immunohematology and Doppler ultrasonography significantly reduce the frequency of necessary invasive interventions in the diagnostics and treatment of sensibilized pregnancy. Intraumbilical strictly intravenous transfusion under ultrasound guidance is the first-line method of therapy for cases of severe fetal anemia as well as advanced hydrops. Consultations and treatment are provided in the centres for comprehensive obstetrical and neonatal care with continuous intervention preparedness. Inversion of uterus is very rare and serious complication of childbirth. It is a situation with very high maternal morbidity and mortality, whitch can lead to developement of very serious periparthal bleeding, DIC, neurogenic and haematologic shock. In 50% without any risk factor. This case report demonstrates this acute case which was caused by then physiologic partus until then and it shoud demonstrate some eventuality to solve this emergent situation including the method used in this case - partially successful reposition attempt of uterus using vaginal path and resulting laparotomia., Inverze dělohy je velmi závažnou a naštěstí poměrně vzácnou komplikací III. doby porodní. Jedná se o stav s vysokou mateřskou morbiditou i mortalitou, při němž může velmi snadno dojít ke vzniku život ohrožujícího krvácení, diseminované intravaskulární koagulopatie a rozvoji hemoragického a neurogenního šoku. V 50 % případů vzniká bez jakéhokoli rizikového faktoru.
