Jako Bouveretův syndrom je označována obstrukce v oblasti duodena impaktovaným žlučovým kamenem, který vycestuje cholecystoduodenální či cholecystogastrickou píštělí. Onemocnění je vzácné, vyskytuje se zejména u starších žen s četnými komorbiditami a vysokým operačním rizikem. K diagnóze se zpravidla dospěje endoskopicky nebo pomocí zobrazovacích metod. Pokud stav nelze řešit endoskopicky, je indikováno operační řešení. Autoři prezentují případ Bouveretova syndromu u 79leté pacientky, u níž se zdařilo zdokumentovat všechny fáze vyšetřovacího a léčebného procesu. Součástí sdělení je přehled literatury týkající se diagnostiky a terapie tohoto vzácného syndromu., Bouveret syndrome is a gastric outlet obstruction caused by impaction of a gallstone that passes through a cholecystoduodenal or cholecystogastric fistula. It is a rare disease, most common in elderly women with multiple comorbidities and high surgical risk. The diagnosis can be made either radiologically or endoscopically. Endoscopic extraction is the preferred therapeutic option. Surgical intervention is indicated when endoscopic methods fail. We describe a case of Bouveret syndrome in a 79 years old woman. Denosumab is a human monoclonal antibody representing a novel therapy of osteoporosis. Contrary to always other antiosteoporotic drugs, it is not contraindicated in advanced chronic kidney disease, as its pharmacokinetic does not differ from patients with normal kidney function. However, published case reports in chronic kidney disease (CKD) patients stopped the therapy after single dose because of hypocalcemia. Endometrióza znamená přítomnost tkáně endometria mimo dutinu děložní. Hlubokou infiltrující endometriózu (deep infiltrating endometriosis – DIE) představují rektovaginální léze, infiltrující onemocnění střev, močového měchýře, močovodu a DIE se vzácně objevuje i v dalších lokalizacích. Negynekologické komplikace DIE rozdělujeme do dvou hlavních kategorií: samotné onemocnění zahrnuje pánevní a břišní bolest, střevní striktury, opakující se krvácení z konečníku, dyschezii, chronickou anémii, dysurii, hematurii, stenózu ureterů, ztrátu ledvinných funkcí a komplikace léčby, ty jsou charakterizovány vznikem fistul, dysfunkcí močového měchýře a opakujícími se záněty urinárního traktu. DIE přináší riziko pro vznik malignit zasažených orgánů. DIE může být léčena konzervativně, nebo chirurgicky. Předkládaná práce je přehledem gastrointestinálních a urologických komplikací DIE a předkládá také kazuistiku pacientky centra pro léčbu endometriózy v nemocnici ve Znojmě (ČR)., Endometriosis is the presence of endometrial tissue outside the endometrial cavity. Deep infiltrating endometriosis (DIE) includes rectovaginal lesions as well as infiltrative disease to the bowel, urinary bladder, ureter and other rare locations. Non gynecological complications of DIE can be divided into two major categories: complications of the disease itself which include pelvic and abdominal pain, bowel stricture, cyclic rectal bleeding, dyschezia, chronic anemia, dysuria, hematuria, ureteral stenosis as well as loss of kidney function and complications of the treatment which include fistula formation, bladder void dysfunction and recurrent urinary tract infections. DIE has potential for malignant transformation in the affected organ. DIE may be managed conservatively or surgically. However definitive treatment is surgical. Infantilní hemangiomy jsou nejčastějšími benigními vaskulárními nádory novorozeneckého a kojeneckého věku. Jedná se o nádory s velmi dobrou prognózou. Většina infantilních hemangiomů spontánně zcela regreduje v prvních deseti letech života. Jen deset procent infantilních hemangiomů vyžaduje nějakou terapii. V současné době je preferováno použití b-blokátoru propranololu. Ostatní typy benigních cévních nádorů je zapotřebí diferenciálně diagnosticky odlišit pro zahájení vhodné terapie a určení prognózy. Předložený článek shrnuje základní poznatky o hemangiomech u novorozenců. V závěru článku je zmíněna krátká kazuistika, kdy byla léčba rozsáhlého hemangiomu zahajována po konzultacích na našem pracovišti za kontroly kardiology., Infantile hemangiomas are the most common benign vascular tumors of neonatal and infancy. These are tumors with a good prognosis. Most infantile hemangiomas spontaneously regress completely in the first decade of life. Only ten percent of infantile hemangiomas require some therapy. Currently it is preferred to use b-blocker propranolol. Other types of benign vascular tumors are needed to distinguish the differential diagnosis for initiation of appropriate therapy and prognosis. The present article summarizes the main findings of hemangiomas in infants. Angiofibromas are rare vascular tumors which originate predominantly in the nasopharynx and occur typically in male adolescents. Extranasopharyngeal sites such as nasal cavity and paranasal sinuses are less frequent. This review article was undertaken to evaluate the incidence, clinical features and management of extranasopharyngeal angiofibromas originating exclusivelly from nasal cavity structures. Our focus of interest was to evaluate the significance of immunohistochemical analysis in diagnosis of such extremely rare neoplasms. In the PubMed and Google Search, we found only 39 cases of nasal angifibroma, 27 males and 12 females from 1980 to 2012. The most prevalent site of origin was nasal septum, followed by inferior and middle turbinate. The commonest symptoms were nasal obstruction and epistaxis. Nasal angiofibromas are clinically distinct from nasopharyneal angiofibromas and can therefore be misdiagnosed. The differential diagnosis includes other vascular lesions, such as lobular capillary hemangioma and sinonasal-type hemangiopericytoma. Although immunohistochemistry is not necessary for differentiation between angiofibroma and capillary hemangioma, that diagnostic procedure may be helpful in distinction from sinonasal hemangiopericytoma. As an ilustration for immunohistochemical analysis, we presented a case of an elderly woman with tumor arising from the middle turbinate, diagnosed as angiofibroma. Juxtapapilární divertikl duodena je literárně uváděn jako možná příčina řady pankreatobiliárních onemocnění. Etiologická souvislost však často není považována za jednoznačnou a v reálné klinické praxi bývá možná role divertiklu podceňována, až popírána. Tento článek na základě kazuistiky a přehledu literatury na tuto problematiku poukazuje. Za klinicky významnou známku možného podílu divertiklu na potížích lze považovat vyplnění lumen divertiklu bezoárem. Tento nález by, po vyloučení jiných příčin, měl vést ke zvažování etiologické souvislosti divertiklu s pankreatobiliárními potížemi. Řešení představují endoskopická papilosfinkterotomie jako metoda první volby a chirurgický výkon (divertikulektomie, biliodigestivní spojka či kombinace) v případě recidiv potíží a selhání méně invazivního postupu., Juxtapapillary duodenal diverticula are reported as a potential cause of many pancreatobiliary diseases. However, data concerning this association is inconsistent and the role of the diverticulum is often underestimated or even denied in clinical practice. This case report and literature review is aimed at pointing out this problem. Obstruction of the diverticulum with a food bezoar can be considered as an important clinical clue of the etiological role of the diverticulum in pancreatobiliary disease development. Léčba kolorektálního karcinomu (KRK) vyžaduje multidisciplinární přístup s účastí gastroenterologa, chirurga, patologa, onkologa a radiologa. Možnosti léčby jsou závislé na stadiu nemoci, celkovém stavu pacienta a také molekulární charakteristice nádoru. V rámci předoperační péče se léčebné postupy u karcinomu konečníku mírně liší od léčby karcinomu tlustého střeva. V případě metastatického KRK jsou možnosti léčby jednotné, použití kombinace chirurgické a cílené systémové léčby. Každá léčebná modalita KRK je zatížená rizikem komplikací. Proto je nezbytné s každým pacientem probrat prospěch a rizika navrhované léčby. Prezentujeme dvě kazuistiky pacientů s KRK, u kterých léčba vedla k dosažení remise i za cenu významných komplikací., Treatment of colorectal cancer (CRC) requires a multidisciplinary approach involving a gastroenterologist, surgeon, pathologist, oncologist and a radiologist. Treatment options depend on the stage of the disease, the overall condition of the patient and the molecular characteristics of the tumor. In the context of preoperative care, treatment procedures for cancer of the rectum is slightly different from the treatment of the colon cancer. In the case of metastatic CRC, there are unified treatment options, using a combination of surgical and targeted systemic therapy. Each treatment modality is at risk of complications. Therefore it is necessary to discuss the benefits and risks of the proposed treatment with each patient. Low-grade myofibroblastic sarcoma (LGMS) is a very rare, atypical myofibroblastic tumor with fibromatosis-like features with predilection mostly in head and neck region. LGMS occurs primarily in adult patients with a slight male predominance. Only few cases of LGMS affecting the larynx have been reported in literature to this date. We describe a case of low-grade myofibroblastic sarcoma of the larynx in a 40-year-old male patient. Závažný defekt koagulačného faktora XII (FXII) je veľmi zriedkavou, záhadnou a málo známou vrodenou poruchou. Na rozdiel od defektu ostatných koagulačných faktorov je úplne asymptomatický. Prekvapivo nevedie k abnormálnemu krvácaniu, dokonca ani pri rozsiahlejších chirurgických výkonoch. Vysvetlenie pre chýbanie krvácavých prejavov nie je známe. Predpokladá sa, ale nie je to dokázané, že pacienti nie sú dostatočne chránení pred trombózou. Defekt FXII sa obyčajne odhalí náhodou rutinným hemokoagulačným vyšetrením uskutočneným pred operáciou. Keďže FXII zohráva dôležitú úlohu pri tvorbe koagula in vitro, jeho defekt spôsobuje pri laboratórnom vyšetrení výrazné predĺženie aktivovaného parciálneho tromboplastínového času. Hlavným problémom spojeným s defektom FXII sú potom ďalšie zbytočné vyšetrenia, strata času pri poskytovaní zdravotnej starostlivosti a obavy z chirurgických výkonov, ktoré môžu vyplývať z abnormálneho laboratórneho výsledku. Kľúčové slová: aktivovaný parciálny tromboplastínový čas – faktor XII – hemokoagulácia – krvácanie, Severe coagulation factor XII (FXII) deficiency is a very rare, mysterious and not well known inherited condition. Unlike other coagulation factor deficiencies, it is totally asymptomatic. Surprisingly, it does not lead to abnormal bleeding, even with major surgical procedures. The explanation for the lack of bleeding manifestations is unknown. It is suggested, but unproven, that patients are not sufficiently protected from thrombosis. FXII deficiency is usually discovered by accident through a routine coagulation testing done prior to surgery. Since FXII plays an important role in clot formation during in vitro measurements, its deficiency causes a marked prolongation of the activated partial thromboplastin time in the laboratory examination. The main concern related to FXII deficiency is the unnecessary testing, delay in health care and worry of surgical interventions that may be prompted by the abnormal laboratory result.
