The management of chronic diseases shall be considered a priority in the work of the global international institutions, which are related to health policies. In the search for effective and efficient solutions for the treatment of chronic diseases, scientists are developing different programs. In Bulgaria the continuous monitoring of chronically ill people is defined with the term dispensarization. The objective of the research is to analyze the place and the role of patients in the monitoring of their chronic diseases and how the concept of quality of life relates to this process. Method The research in focus groups is planned within a research project on the monitoring of chronic diseases. Five group discussions were held. The number of participants in all focus groups was sixty. Results The two discussed directions - the active role of the patient and the focus on the results are weak links in the current organization of the care for the chronically ill people. The topic of creating and adopting an Act of the patient was also presented, in which to be paid attention to the evaluation of the quality of life. In general was brought the need for a clear delineation of the roles and the competencies of everyone involved in the monitoring process of chronic diseases. The chronic disease cannot be defined only as a medical problem, as there are economic and social, including political consequences. Conclusions The management of the chronic disease requires coordination of doctor-patient interaction, a model of partnership and trust in the relations, self-management of the condition by the patients and their inclusion in the health team. Electroantennogram responses of the polyphagous leafminer Liriomyza sativae and its generalist parasitoid Diglyphus isaea to host and non-host plant odours of L. sativae were investigated. The odours of healthy leaves can elicit distinct EAG responses in L. sativae. The EAG responses to the odours of the host plants, bean and tomato, were stronger than to non-host plants, Chinese rose and morning glory. Neither healthy host nor non-host plants of the leafminer elicited distinctive EAG responses in the parasitoid, D. isaea. Odours of physically damaged leaves, no matter whether of host or non-host plants, increased strongly the EAG responses of the leafminer and its parasitoid. We compared the EAG responses of D. isaea to bean leaves of different status. The odour of mined leaves elicited distinct EAG responses, which were weaker than those of physically damaged ones. No differences were detected in the EAG response to leaves with empty mines, healthy leaves or an air control. The role of plant odours in host location of the leafminer and its parasitoid is discussed.
Recent host records for Gephyraulus raphanistri (Kieffer), a flower-gall midge, show restriction to Raphanus raphanistrum throughout Europe. Gephyraulus raphanistri has never been reported infesting commercially grown Brassica crops. Historical records showing a broad host range appear to have resulted from confusion with new or as yet undescribed Gephyraulus spp. and Contarinia nasturtii (Kieffer), a known gall-former of Brassica and other related genera. This study tested host specificity of G. raphanistri in the field in Europe by manipulating host plant phenology of actual and potential hosts in the genera Raphanus and Brassica as part of a risk assessment of the insect as a potential biological control agent of R. raphanistrum, one of the most important weeds of crops in Australia. Raphanus raphanistrum raphanistrum (wild radish), R. raphanistrum landra (coastal wild radish), Raphanus sativus (radish) and Brassica napus (oilseed rape cultivar) were phenologically synchronised for initial flowering and planted out in a flowering time and species block design near a natural population of R. r. landra hosting a natural population of G. raphanistri. Three generation peaks in gall formation were observed in the experiment, with galls developing on all test plants with an apparent preference for R. r. landra. The high field specificity of this gall midge is driven by the synchrony of oviposition and flower availability, not host physiological incompatibility or behavioural unacceptability. Commercially grown Brassica spp. are not suitable hosts for G. raphanistri because in the field they differ in flowering phenology from Raphanus raphanistrum. The overlap in the flowering phenology of the crop and weed in Australia makes this insect unsuitable as a biological control agent.
The peroxisome proliferator-activated receptors (PPAR) belong to the nuclear superfamily of ligand-activated transcription factors. PPARγ acts as a nutrient sensor that regulates several homeostatic functions. Its disruption can lead to vascular pathologies, disorders of fatty acid/lipid metabolism and insulin resistance. PPARγ can modulate several signaling pathways connected with blood pressure regulation. Firstly, it affects the insulin signaling pathway and endothelial dysfunction by modulation of expression and/or phosphorylation of signaling molecules through the PI3K/Akt/eNOS or MAPK/ET-1 pathways. Secondly, it can modulate gene expression of the renin- angiotensin system – cascade proteins, which potentially slow down the progression of atherosclerosis and hypertension.
Thirdly, it can modulate oxidative stress response either directly through PPAR or indirectly through Nrf2 activation. In this context, activation and functioning of PPARγ is very important in the regulation of several disorders such as diabetes mellitus, hypertens
ion and/or metabolic syndrome.
Protease-activated receptors (PARs) belong to the G-proteincoupled receptor family, that are expressed in many body tissues especially in different epithelial cells, mast cells and also in neurons and astrocytes. PARs play different physiological roles according to the location of their expression. Increased evidence supports the importance of PARs activation during nociceptive signaling and in the development of chronic pain states. This short review focuses on the role of PAR2 receptors in nociceptive transmission with the emphasis on the modulation at the spinal cord level. PAR2 are cleaved and subsequently activated by endogenous proteases such as tryptase and trypsin. In vivo, peripheral and intrathecal administration of PAR2 agonists induces thermal and mechanical hypersensitivity that is thought to be mediated by PAR2-induced release of pronociceptive neuropeptides and modulation of different receptors. PAR2 activation leads also to sensitization of transient receptor potential channels (TRP) that are crucial for nociceptive signaling and modulation. PAR2 receptors may play an important modulatory role in the development and maintenance of different pathological pain states and could represent a potential target for new analgesic treatments., P. Deformability of red blood cells (RBC) is the ability of RBC to change their shape in order to pass through narrow capillaries in circulation. Deterioration in deformability of RBC contributes to alterations in microcirculatory blood flow and delivery of oxygen to tissues. Several factors are responsible for maintenance of RBC deformability. One of them is the Na,K-ATPase known as crucial enzyme in maintenance of intracellular ionic homeostasis affecting thus regulation of cellular volume and consequently RBC deformability. Decreased deformability of RBC has been found to be the marker of adverse outcomes in cardiovascular diseases (CVD) and the presence of cardiovascular risk factors influences rheological properties of the blood. This review summarizes knowledge concerning the RBC deformability in connection with selected risk factors of CVD, including hypertension, hyperlipidemia, and diabetes mellitus, based exclusively on papers from human studies. We attempted to provide an update on important issues regarding the role of Na,K-ATPase in RBC deformability. In patients suffering from hypertension as well as diabetes mellitus the Na,K-ATPase appears to be responsible for the changes leading to alterations in RBC deformability. Rheological, haemostatic, endothelial and platelet abnormalities appear to play a role in the thrombotic complications of hypertension. This prothrombotic/hypercoagulable state in hypertension may contribute to the increased risk and severity of target organ damage. It can be induced by the activated reninangiotensin system (RAS), with abnormalities in endothelial and platelet function, coagulation and fibrinolysis. Treatment of uncomplicated essential hypertension by RAS targeting antihypertensive therapy could result in a reversal of prothrombotic abnormalities, contributing to a reduction of thrombosis-related complications. Since angiotensin converting enzyme (ACE) inhibitors and angiotensin II receptor blockers (ARBs) have two distinct mechanisms of RAS interruption, it is hypothesized that each therapy might have different impact on the prothrombotic state in hypertensive patients. Some studies demonstrate a beneficial effect of both ACE inhibitors and ARBs on prothrombotic state, in addition to their efficacy to normalize elevated blood pressure. The potentially antithrombotic effect of the RAS inhibiting agents may in turn support the preservation of cardiovascular function. Available data may offer an additional explanation for the efficacy of the RAS targeting agents in the prevention of cardiovascular events in patients with atherosclerotic vascular disease., A. Results indicate that vascular responses to temperature stimulation are predominantly impaired in animals with 5-HT deprivation. A hypothesis is therefore raised that the 5-HT system participates in body temperature regulation in such a way asto link the regulatory output with vasomotor pathways. The 5-HT system in the spinal cord has been shown to inhibit the afferent transmission of temperature signals. Therefore, depletion of 5-HT does not prevent sensory transmission, at least at the spinal cord level.
The neonatally gonadectomized male rats failed to show typical male clasping of the flanks of the female rat in oestrous stage and also could not achieve intromission and ejaculation when studied at 90 days of age. The replacement of testosterone propionate did not abolish the deficit in sexual behaviour observed. The multiunit electrical activity (MUA) in the medial preoptic area (MPOA) of intact males as well as of oestrous females increased significantly in amplitude and frequency from the low basal levels during mounting, but reverted almost to the basal levels after mounting. The basal MUA in the gonadectomized male rats was of relatively high frequency and amplitude when compared to the intact males and oestrous females, and did not show significant changes during and after mounting. It is suggested that the dimorphic organization of MPOA, developing under the influence of neonatal testosterone, plays a significant role in the male sexual behaviour in adulthood.
This review summarizes recent trends in the construction of bioartificial vascular replacements, i.e. hybrid grafts containing synthetic polymeric scaffolds and cells. In these advanced replacements, vascular smooth muscle cells (VSMC) should be considered as a physiological component, although it is known that activation of the migration and proliferation of VSMC plays an important role in the onset and development of vascular diseases, and also in re stenosis of currently used vascular grafts. Therefore, in novel bioartificial vascular grafts, VSMCs should be kept in quiescent mature contractile phenotype. This can be achieved by (1) appropriate physical and chemical properties of the material, such as its chemical composition, polarity, wettability, surface roughness and topography, electrical charge and conductivity, functionalization with biomolecules and mechanical properties, (2) appropriate cell culture conditions, such as composition of cell culture media and dynamic load, namely cyclic strain, and (3) the presence of a confluent, mature, semipermeable, non-thrombogenic and non-immunogenic endothelial cell (EC) barrier, covering the luminal surface of the graft and separating the VSMCs from the blood. Both VSMCs and ECs can also be differentiated from stem and progenitor cells of various sources. In the case of degradable scaffolds, the material will gradually be removed by the cells and will be replaced by their own new extracellular matrix.
