Úvod: V České republice (ČR) jsou pacienti s diagnózou diabetes mellitus (DM) sledováni a léčeni převážně u specialistů (přibližně 80 % v odborné diabetologické ambulanci), menší část praktickými lékaři (do 20 %). Doposud nebyl zhodnocen dlouhodobý vývoj změn preskripce metforminu a sulfonylurey v České republice a jeho konkordance s doporučenými postupy. Cíl: Porovnání vývoje preskripce metforminu (MET) a sulfonylurey (SU) v letech 2002–2006 s vývojem v letech 2010–2014 v reprezentativním vzorku populace pacientů s DM v ČR vedených v databázi Všeobecné zdravotní pojišťovny České republiky (VZP), u které bylo v roce 2014 pojištěno 63 % populace ČR. Metodika: V databázi VZP jsme identifikovali všechny osoby, které měly v letech 2002–2006 a 2010–2014 záznam o diagnóze DM (E10–E16 podle MKN 10) nebo předpis jakékoliv antidiabetické terapie (ATC skupina A10). Pro analýzu byl extrahován soubor osob, které měly v daném roce předepsanou nejméně jednou látku ze skupiny A10 (308 962 evidovaných v roce 2002 a 426 695 evidovaných v roce 2014). Pro každý rok byl vyhodnocen počet osob, které měly předepsán nejméně jednou MET nebo SU. Počet osob léčených MET nebo SU byl pak vyjádřen v procentech ze všech, kteří měli v daném roce předepsanou jakoukoliv terapii ze skupiny A10. Výsledky: Preskripce metforminu stoupá lineárně ze 43 % na 77 %, preskripce sulfonylurey se lineárně snižuje z 65 % na 37 %. Závěr: Naše analýza je prvním zhodnocením vývoje preskripce metforminu v ČR a zhodnocením její konkordance s doporučenými postupy pro léčbu DM. Preskripce metforminu v ČR se zvýšila mezi roky 2002–2014 ze 43 % na 77 % a preskripce SU se snížila z 65 % na 37 %. Tento vývoj a aktuální poměr preskripce MET a SU je potvrzením implementace doporučených postupů do praxe a důkazem vysoké kvality péče o pacienty s DM2T v České republice. Goal: Comparison of the development of metformin (MET) and sulfonylurea (SU) prescriptions in the period of 2002–2006 with that of 2010–2014 in a representative sample of the patient population with DM kept in the database of the General Health Insurance Company of the Czech Republic (VZP) which provided health care coverage for 63% of Czech Republic population in 2014. Methodology: We identified all individuals in the VZP database who had a record of DM diagnosis (E10 – E16 based on ICD 10) or who had any antidiabetic therapy prescribed (ATC group A10) in the periods of 2002–2006 and 2010–2014. A cohort of patients was extracted for analysis, who had an agent from A10 group prescribed at least once in a relevant year (n = 308 962 in 2002; n = 426 695 in 2014). A number of patients was evaluated for each year, who had at least once MET or SU prescribed. The number of patients treated with MET or SU was then expressed as a percentage of all who had any therapy from A10 group prescribed in the year in question. Results: Metformin prescriptions have linearly risen from 43% to 77%, while sulfonylurea prescriptions have linearly decreased from 65% to 37%. Conclusion: The analysis presents the first evaluation of the development of metformin prescriptions conducted in the Czech Republic and evaluation of its concordance with the recommended procedures for the treatment of DM. The amount of metformin prescribed in the Czech Republic increased from 43% to 77% while the amount of SU prescribed decreased from 65% to 37% between 2002 and 2014. This development and the current ratio between the prescribed amounts of MET and SU demonstrate the implementation of the recommended procedures into practice and prove the high quality of care for patients with DM2T in specialists – diabetologists‘ surgeries. Pro populaci pacientů s diabetem 2. typu nejsou v současnosti v České republice dostupná data o distribuci hodnot glykovaného hemoglobinu. Plnému uplatnění výhod nových skupin antidiabetik pro časnou intervenci brání v současnosti indikační omezení, které omezuje úhradu této terapie pro nedostatečně kompenzované pacienty s hranicí definovanou hodnotou 60 mmol/mol (6 % IFCC). Analýza distribuce hodnot glykovaného hemoglobinu v náhodně vybrané populaci pacientů s diabetem 2. typu ve studii Valetudo ukazuje, že v rozmezí kritického intervalu od 43 mmol/mol do 80 mmol/mol je průměrná četnost pacientů z celé populace 1,88 %/1 mmol/mol glykovaného hemoglobinu. Interpretace dat z průřezových studií je velmi obtížná, jelikož zahrnuje pacienty před léčbou (případně před intenzifikací léčby) i pacienty úspěšně léčené., Currently, there are no available data on distribution of the values of glycated haemoglobin in patients with type 2 diabetes in the Czech Republic. More frequent, efficacy and early application of new antidiabetic agents is limited by indication criterion which restricts the payment of this therapy for patients with poorly compensated diabetes defined by threshold value of 60 mmol/mol (6% IFCC). The study Valetudo analyzed distribution of glycated haemoglobin values in the random population of patients with type 2 diabetes and showed that within the critical range from 43 mmol/mol to 80 mmol/mol is the average frequency of patients from the whole population of 1.88%/1 mmol/mol glycated haemoglobin. Úvod: Informace o prevalenci orgánových komplikací diabetu včetně syndromu diabetické nohy v České republice je možné získat z dokumentů Ústavu zdravotnických informací a statistiky. Cíl: Posoudit vývoj vysokých amputací a drobných výkonů na noze v letech 2010–2014 v reprezentativním vzorku populace pacientů s diabetes mellitus vedených v databázi Všeobecné zdravotní pojišťovny (VZP) České republiky. Metodika: V databázi VZP jsme identifikovali všechny osoby, které měly v letech 2010–2014 záznam o diagnóze DM (E10–E16 podle MKN 10) nebo předpis jakékoliv antidiabetické terapie (ATC skupina A10). Pro analýzu byl extrahován soubor osob, které měly v daném roce předepsanou nejméně jednou látku ze skupiny A10. Následně jsme identifikovali osoby, které v letech 2010–2014 současně prodělaly chirurgický výkon na noze z důvodu diagnózy diabetické nohy. Výsledky: Absolutní počet amputačních výkonů na končetině vykazuje stacionární úroveň. Závěr: Předkládaná analýza je prvním zhodnocením vývoje výkonů na diabetické noze v ČR. Je zřetelný setrvalý stav v počtu výkonů na diabetické noze, a to jak vysokých amputací, tak drobných výkonů na noze. V kontextu s absolutním nárůstem pacientů, léčených pro diabetes mellitus, je stacionární stav známkou relativního poklesu, což je příznivé především v oblasti amputace dlouhých kostí. Goal: Assessment of the development of high amputations and minor surgical procedures on the lower limb from 2010 to 2014 in a representative sample of the population of patients with DM kept in the General Health Insurance Company of the Czech Republic database. Methodology: We identified all individuals in the VZP database who had a record of DM diagnosis (E10 – E16 based on ICD 10) or any antidiabetic therapy prescribed (ATC group A10) in the period of 2010 – 2014. A set of patients who had an agent from A10 group prescribed at least once in the given year was extracted for analysis. In the next step we identified individuals, who in the period of 2010 – 2014 also underwent a surgical procedure on the lower limb due to diabetic foot. Results: An absolute number of lower limb amputations remains at a stationary level. Conclusion: The submitted analysis presents the first assessment of the development of surgical treatment of diabetic foot in the Czech Republic. The amount of surgical procedures on the diabetic foot remains stable, regarding both high amputations and lower limb minor surgical procedures. In the context of an absolute increase of patients treated for diabetes mellitus, the stationary state is an indication of a relative decrease, which is favourable in particular with regard to the amputation of long bones. Vznik a progrese symptomatické diabetické neuropatie (SDN) se váže k hyperglykemii. Při terapii je proto velmi důležitá snaha o zlepšení kompenzace diabetes mellitus. Z tohoto důvodu je třeba kooperace v terapii mezi neurologem a diabetologem. Farmakologická terapie symptomatické senzitivní periferní diabetické neuropatie je svízelná a relativně málo účinná. K symptomatické terapii je užívána celá řada léčivých látek určených primárně k intervenci jiných patologických stavů. V doporučených postupech jsou uváděna antidepresiva, antikonvulziva, opiáty a jejich deriváty. Tato terapie je však zatížena relativně vysokou incidencí nežádoucích účinků, které snižují adherenci pacientů k léčbě. Výborných výsledků dosahuje terapie kyselinou tioktovou., The development and progression of symptomatic diabetic neuropathy (SDN) is linked to hyperglycemia. The effort to improve compensation of diabetes mellitus during therapy is therefore very important. This is where the cooperation between the diabetologist and neurologist within therapy plays an important role. The pharmacological therapy of symptomatic sensitive peripheral diabetic neuropathy is difficult and with a less than satisfactory effect. A variety of active substances is used in symptomatic therapy, primarily designed for intervention in other pathological conditions. The recommended guidelines include antidepressants, anticonvulsants, opiates and their derivatives. However this therapy brings with it a relatively high incidence of adverse effects which detract from patients’ adherence to treatment. V roce 2004 došlo v České republice k zásadním legislativním změnám v oblasti postgraduálního vzdělávání lékařů (zákon č. 95/2004 Sb.). Nahrazení dvojstupňového systému specializačního vzdělávání jednostupňovým s sebou přineslo problémy a podle našeho názoru nevedlo k jednoznačnému zkvalitnění postgraduálního vzdělávání, spíše naopak. K vytvoření vlastního lokálního systému postgraduálního vzdělávání na našem pracovišti nás vedla snaha podpořit kvalitu předatestační přípravy budoucích specialistů v interních oborech s uplatněním 4 základních principů: principu robustního interního základu jako platformy pro získání další specializace, principu opakování pro fixaci znalostí, principu kontroly znalostí a v neposlední řadě principu aktivní účasti v systému vzdělávání., In 2004 the principal legislative changes were accepted in the field of postgraduate medical education in Czech Republic (law No. 95/2004 Code). Úvod: Léčba inzulinovou pumpou (IP) je jedním ze způsobů intenzivní terapie inzulinem, určeným preferenčně pro pacienty s diabetem 1. typu. Cena léčby je vyšší než léčba klasickým bazálbolusovým a inzulinovým režimem pomocí opakované aplikace inzulinu dávkovačem. Cíl: Zhodnocení kvality monitorování terapie IP u pacientů s DM v reprezentativním vzorku populace pacientů s DM v ČR vedených v databázi Všeobecné zdravotní pojišťovny České republiky (VZP), u níž bylo v roce 2014 pojištěno 63 % populace ČR. Metodika: V databázi VZP jsme identifikovali všechny osoby, které měly v letech 2009–2013 záznam o diagnóze DM (E10–E16 podle MKN 10), nebo měly předepsanou jakoukoliv antidiabetickou terapii (ATC skupina A10). Za celé období 2009–2014 bylo v datech VZP identifikováno celkem 4 002 unikátních pacientů s diabetem, kteří byli v hodnoceném období léčeni IP. Incidence pro rok 2014 (nově léčení inzulinovou pumpou): všichni pacienti, kteří měli v roce 2014 vykázanou IP a zároveň v předešlém období 2009–2013 není záznam o vykázání IP. Prevalence pro rok 2014 (všichni léčení inzulinovou pumpou): všichni pacienti, kteří v období 2010–2014 mají alespoň jednou vykázanou inzulinovou pumpu a zároveň nezemřeli před rokem 2014. Parametry kontroly kvality (vyšetření HbA1c a spotřeba diagnostických proužků pro testování hladiny glukózy) u pacientů léčených IP byly podrobně hodnoceny pouze pro rok 2014, a to u všech pacientů léčených inzulinovou pumpou v roce 2014 po celý rok (tj. od 1. 1. 2014 do 31. 12. 2014), tedy celkem u 3 189 pacientů. Výsledky: V roce 2014 bylo celkem 247 incidentních pacientů a celkem 3 794 prevalentních pacientů. Nově byla IP nasazena téměř v 50 % u pacientů ve věkovém rozmezí 20–39 let. V roce 2014 byla průměrná frekvence vyšetření HbA1c 3,38/pacienta a nejméně jednou byl vyšetřen u 98,5 % pacientů. Průměrná spotřeba diagnostických proužků pro testování hladiny glukózy na jednoho pacienta byla 879 kusů. Závěr: Pilotní projekt hodnocení parametrů kvality monitorování terapie IP ukazuje, že pacienti léčení IP mají poměrně často kontrolovaný glykovaný hemoglobin (3,38 vyšetření u jednoho pacienta v roce 2014) a průměrně si měří glykemii 2–3 krát denně. The price of the treatment is higher than that of the conventional basalbolus and insulin regimens using repeated insulin application with a dose selector. Goal: Quality assessment of IP therapy monitoring in patients with DM in a representative sample of the patient population with DM kept in the database of the General Health Insurance Company of the Czech Republic (VZP) which provided health care coverage for 63% of Czech population in 2014. Methodology: We identified all individuals in the VZP database who had a record of DM diagnosis (E10 – E16 based on ICD 10) or who had any antidiabetic therapy prescribed (ATC group A10) in the period of 2009–2013. Over the whole period of 2009–2014 there were overall n = 4 002 unique patients with DM identified in the VZP data, who were treated with IP within the assessment period. Incidence for the year 2014 (the newly treated with an insulin pump): all patients who had IP recorded in 2014 while in the preceding period of 2009–2013 they had no record of IP use. Prevalence for the year 2014 (all treated with an insulin pump): all the patients who for the period of 2010–2014 had at least once insulin pump use recorded and who did not die before 2014. Quality control parameters (HbA1c examination and consumption of glucose level test strips) for patients treated with IP were only assessed in detail for the year 2014, namely for all patients undergoing insulin pump treatment in 2014 throughout the year (i.e. from 1 January 2014 to 31 December 2014), i.e. in n = 3 189 patients in all. Results: In 2014 there were 247 incident patients and 3 794 prevalent patients. IP was newly introduced for almost 50% of the patients aged 20–39 years. In 2014 an average frequency of HbA1c examination equaled 3.38 per patient and 98.5% patients were examined for HbA1c at least once. An average consumption of glucose level testing strips per patient was 879 pieces. Conclusion: The pilot project of assessment of quality parameters for IP therapy monitoring shows that the patients treated with IP have glycated hemoglobin checked quite frequently (3.38 checkups per patient in 2014) and they measure their blood glucose 2–3 times a day on average. Úvod: Každoročně jsou zveřejňována oficiální data, která popisují péči o pacienty s onemocněním diabetes mellitus v České republice. Je vždy uváděn celkový počet osob s diabetem, počet nově zachycených onemocnění a počet zemřelých. Z těchto dat však nelze zjistit, jaké jsou diference v mortalitě pro jednotlivé skupiny diabetiků ve vztahu k terapii. Cíl: Porovnat vývoj mortality v letech 2002 až 2006 s vývojem v letech 2010 až 2013 v reprezentativním vzorku populace pacientů s diabetes mellitus 2. typu léčených perorálními antidiabetiky v ČR vedených v databázi Všeobecné zdravotní pojišťovny České republiky, u níž bylo v roce 2013 pojištěno 63 % populace ČR. Metodika: Retrospektivní epidemiologická analýza. V databázi VZP jsme identifikovali všechny osoby, které měly v letech 2002–2008 a 2009–2013 záznam o diagnóze DM (E10-E16 podle MKN 10), nebo měly předepsanou jakoukoliv antidiabetickou terapii (ATC skupina A10). Pro vlastní analýzu jsme vybrali pouze pacienty, kteří byli léčeni perorálními antidiabetiky (v daném roce nebo v roce předcházejícím měli záznam o léčbě alespoň jedním preparátem ze skupiny A10B a současně neměli v obou letech záznam o léčbě preparáty ze skupiny A10A). Zvoleným kritériím odpovídalo v roce 2003 celkem 237 665 osob, a v roce 2013 celkem 315 418 osob. Výsledky: Mortalita se snížila napříč všemi věkovými skupinami (rok 2003–2013): věk 50–59 let 1,2 %-0,7 %; věk 60–69 let 2,6 %-1,6 %; věk 70–79 let 5,8 %-3,5 %. V roce 2013 se mortalita blížila obecné populaci, v níž byla pro tytéž věkové skupiny 0,6 %, 1,5 % a 3,4 %. Mortalita se snížila v relativním vyjádření oproti roku 2003 ve věkové skupině 50–59 let o 42 % (ČR o 25 %), ve věkové skupině 60–69 let o 39 % (ČR o 17 %) a ve věkové skupině 70–79 let o 40 % (ČR o 28 %). Pokles mortality u diabetiků léčených perorálními antidiabetiky byl v abslolutním i relativním vyjádření větší mezi roky 2003–2013, než v obecné populaci ČR. Závěr: Analýza mortality pacientů léčených perorálními antidiabetiky, kteří jsou v databázi VZP, prokázala jednoznačně příznivý trend snižování, který je rychlejší než v obecné populaci. Kriticky důležitým zjištěním je skutečnost, že se mortalita v tomto souboru přibližuje populaci obecné odpovídajícího věku. Současně lze oprávněně předpokládat, že při extenzivnějším využití dosavadních terapeutických postupů bude mortalita dále klesat. An overall number of individuals with diabetes, the number of newly reported cases and the number of patient deaths is always specified. However this data does not allow us to identify the differences in mortality between the individual cohorts of diabetic patients in relation to therapy. Goal: Comparison of the mortality development in the periods of 2002–2006 and 2010–2013 in a representative sample of the patient population with type 2 diabetes mellitus using oral antidiabetic drugs, kept in the database of the General Health Insurance Company of the Czech Republic (VZP) which provided health care coverage for 63% of Czech population in 2013. Methodology: A retrospective epidemiologic analysis. We identified all individuals in the VZP database who had a record of DM diagnosis (E10 – E16 based on ICD 10) or who had any antidiabetic therapy prescribed (ATC group A10) in the periods of 2002–2008 and 2009–2013. We only selected those patients for the analysis who were treated with oral antidiabetic medicines (in the given year or the preceding years they had a record of treatment with at least one medicine from A10B group, while having no record of treatment with medicines from A10A group within both years). 237 665 individuals met the selected criteria in 2003 and 315 418 individuals in 2013. Results: Mortality rates dropped for all age groups (from 2003–2013): for 50–59 year olds by 1.2%-0.7%; in 60–69 year olds by 2.6%-1.6%; for 70–79 year olds by 5.8%-3.5%. In 2013 mortality rates came close to the general population where for the same age groups they reached 0.6%, 1.5% and 3.4% respectively. When expressed in relative terms, the mortality among 50–59 year olds declined by 42% (Czechia by 25%), among 60–69 year olds by 39% (Czechia by 17%) and among 70–79 year olds by 40% (Czechia by 28%) from the year 2003. The decline in mortality among the patients with DM treated with oral antidiabetic medicines was greater in both absolute and relative terms in the period of 2003–2013 than among the general population in the Czech Republic. Conclusion: The analysis of mortality among the patients treated with oral antidiabetic medicines, registered in the VZP database, has shown a clearly favourable trend of mortality decline which is faster than among the general population. The fact that mortality among this cohort is getting closer to that among the general population of the corresponding age is a finding of critical importance. There is a justified expectation that mortality, with increasingly extensive utilization of the present therapeutic procedures, will continue to decrease. Základním symptomem diabetes mellitus je hyperglykemie. Zvýšené kardiovaskulární riziko (KVR) u pacientů s diabetes mellitus 1. i 2. typu má však multifaktoriální etiologii. Současný negativní vliv hyperglykemie, hypertenze a hyperlipidemie na cévní systém se navzájem potencuje. Důsledná prevence kardiovaskulárních rizikových parametrů u pacientů s diabetes mellitus 2. typu (DM2T) významně snížila mortalitu a morbiditu na kardiovaskulární nemoci. Přesto zbývá „reziduální riziko“. Plné využití potenciálu statinů a jejich kombinace s ezetimibem může ještě více snížit KVR a prodloužit tak život pacientů s DM2T. Význam intervence inhibitory PCSK9 pro snížení kardiovaskulárního rizika u pacientů s diabetes mellitus je předmětem studia., The basic symptom of diabetes mellitus is hyperglycemia. However the increased cardiovascular risk in patients with both type 1 and type 2 diabetes mellitus has a multifactorial etiology. Concurrent negative effects of hyperglycemia, hypertension and hyperlipidemia on the vascular system reinforce one another. Consistent prevention of cardiovascular risk parameters in patients with type 2 diabetes mellitus (DM2T) has brought significant decline in mortality and morbidity due to cardiovascular diseases. Still there is “residual risk” remaining. The full use of the potential of statins and their combination with ezetimibe may lower the CV risk even more and extend the life of patients with DM2. Diabetes mellitus je závažným zdravotnickým a ekonomickým problémem současnosti. Terapie je vnímána primárně jako prevence pozdních komplikací diabetu. Většina farmakoekonomických analýz uvádí, že náklady spojené s terapií diabetes mellitus jsou z větší části spotřebovány na terapii komplikací. Projekt Národního diabetologického informačního systému vychází ze základního principu automatizovaného sběru dat, která jsou k dispozici v České republice v elektronické podobě a která současně popisují kvalitu a kvantitu péče poskytovaného pacientů s diabetes mellitus i s jejími výsledky. Výhodou postulovaného Národního diabetologického informačního systému je minimalizace selektivní ztráty dat, komplexní pohled na epidemiologii, terapii a její výsledky, možnost sledování trendů, zahrnutí nákladů spojených s hospitalizací a sociálními dávkami. Rizikem zůstává správná interpretace a možnost systémové chyby. Jestliže by se podařilo v České republice podobný systém uskutečnit, bude to první takto komplexní plně automatizovaný informační systém na světě. Závěr: Data popisující incidenci a prevalenci diabetes mellitus, strukturu terapie, náklady přímé na zdravotní péči i indukované na sociálních platbách jsou v naprosté většině k dispozici v elektronické podobě. Vzniká tak možnost založení Národního diabetologického informačního systému, který umožní kontinuální vyhodnocování kvality péče o pacienty s diabetes mellitus včetně farmakoekonomických a sociálních důsledků. Therapy is primarily perceived as prevention of the late complications of diabetes. Most pharmacoeconomic analyses state that the cost associated with the therapy of diabetes mellitus is largely spent on the therapy of its complications. The project of the National Diabetes Information System is based on the essential principle of automated collection of data which is available in digital form in the Czech Republic and which describes the quality and quantity of the care provided for patients with diabetes mellitus and its results. Benefits of the posited National Diabetes Information System embrace minimizing of selective loss of data, a comprehensive view of epidemiology, therapy and its results, possibility of following trends, inclusion of costs related to hospitalization and social benefits. Risks involve correct interpretation and a possible system error. If we succeeded in implementing such system in the Czech Republic, it would be the first fully automatic information system on such a comprehensive scale worldwide. Conclusion: The data describing the incidence and prevalence of diabetes mellitus, therapy structure, direct healthcare costs and induced costs of social payments is in the largest part available in digital form. This creates a basis for the foundation of the National Diabetes Information System which will allow for continuous quality assessment of care for patients with diabetes mellitus, including the related pharmacoeconomic and social impacts. Úvod: Náklady na léčbu diabetu 2. typu (DM2T) se zvyšují s nedostatečnou kompenzací a rozvojem komplikací. Nelze-li u pacientů dosáhnout kompenzace DM2T pomocí režimových opatření a podáváním perorálních antidiabetik (PAD), je doporučeno zahájení léčby inzulinem. Efektivita léčby DM2T bazálními inzuliny typu NPH nebo glargin v kombinaci s PAD byla zhodnocena řadou farmakoekonomických studií. Žádná z nich však nebyla provedena v České republice. Cílem projektu POET2 proto bylo porovnat přímé roční náklady na léčbu DM2T v podmínkách běžné klinické praxe v České republice, které vznikly v souvislosti se zahájením léčby bazálním inzulinem NPH nebo glargin v kombinaci s PAD. Metodika: Analyzována byla data od 1 967 pacientů, kteří splnili kritéria pro zařazení do neintervenčního prospektivního registru (DM2T, dosavadní léčba PAD, glykovaný hemoglobin HbA1c > 6 % IFCC) a kteří byli sledováni po dobu 12 měsíců od zahájení terapie inzulinem (glargin: n = 1 061 vs NPH: n = 906). Cena léčby byla hodnocena z pohledu zdravotních pojišťoven a zahrnovala úhrady léčiv, zdravotnických prostředků a pomůcek a zdravotnických výkonů. Výsledky: U obou léčebných skupin došlo k snížení HbA1c (skupina pacientů léčená inzulinem glargin: o 1,77 % IFCC vs skupina pacientů léčená inzulinem NPH: o 1,73 % IFCC) a glykemie nalačno (FPG) (glargin: o 3,67 mmol/l vs NPH: o 3,63 mmol/l). Léčba inzulinem glargin byla spojena s významně nižším výskytem dokumentovaných symptomatických hypoglykemií (glargin: 0,840 příhod na pacienta a rok léčby vs NPH: 1,053 příhod na pacienta a rok léčby; p < 0,05). Celkové přímé roční zdravotnické náklady vzniklé v souvislosti se zahájením léčby bazálním inzulinem byly v průměru o 2 547,07 Kč vyšší při léčbě inzulinem glargin (glargin: 12 173,09 ? 4 169,44 Kč vs NPH: 9 626,02 ? 3 432,79 Kč; p < 0,001) v důsledku vyšší ceny farmakoterapie (glargin: 7 992,97 ? 4 001,81 Kč vs NPH: 3 784,2 ? 3 181,48 Kč; p < 0,001). Při léčbě inzulinem glargin však byly významně nižší náklady spojené s poskytováním zdravotnických prostředků a pomůcek (glargin: 2 332,08 ? 917,84 Kč vs NPH: 3 893,95 ? 989,79 Kč; p < 0,001) a náklady na zdravotnické výkony (glargin: 1 848,04 ? 684,89 Kč vs NPH: 1 947,87 ? 685,43 Kč; p < 0,001). Závěr: Léčba DM2T bazálním inzulinem v kombinaci s PAD byla v podmínkách běžné klinické praxe v České republice efektivní terapeutickou intervencí. Celkové přímé roční zdravotnické náklady byly vyšší při léčbě inzulinem glargin než při léčbě inzulinem NPH. Náklady spojené s poskytováním zdravotnických prostředků a pomůcek a náklady na zdravotnické výkony však byly při léčbě inzulinem glargin nižší. Také výskyt hypoglykemií byl významně nižší u pacientů léčených inzulinem glargin., Introduction: Poor glycemic control and the resulting development of complications of type 2 diabetes (DM2T) increase treatment costs. If adequate glycemic control cannot be achieved by lifestyle modifications and oral antidiabetic (OAD) therapy, initiation of insulin therapy is recommended. Cost effectiveness of basal insulins of the type NPH or glargine in combination with OAD for the treatment of DM2T was examined in a number of pharmacoeconomic studies. However, none of these studies were conducted in the Czech Republic. Therefore, the aim of the project POET2 was to compare annual direct medical costs of treating DM2T after addition of insulin NPH or glargine to OAD therapy in a clinical practice setting in the Czech Republic. Methodology: Data collected from 1 967 patients who met the criteria for inclusion into the non-interventional prospective registry POET2 (DM2T, ongoing OAD therapy, glycated hemoglobin HbA1c > 6 % IFCC) and who were observed for 12 months following the start of insulin therapy (glargine: n = 1 061 vs NPH: n = 906) were analysed. Costs of treatment were analysed from the perspective of health insurance companies and included costs of medication, medical devices and medical procedures. Results: In both treatment groups a reduction of HbA1c (glargine group: by 1.77 % IFCC vs NPH group: by 1.73 % IFCC) and fasting plasma glucose (glargine group: by 3.67 mmol/l vs NPH group: by 3.63 mmol/l) was observed. Insulin glargine therapy was associated with the incidence of a significantly lower number of documented symptomatic hypoglycemic events (glargine group: 0.840 events per patient and year of treatment vs. NPH group: 1.053 events per patient and year of treatment; p < 0.05). Overall annual direct medical costs associated with the initiation of basal insulin treatment were higher on average by 2547.07 CZK among patients treated with insulin glargine (glargine group: 12 173.09 ? 4 169.44 CZK vs NPH group: 9 626.02 ? 3 432.79 CZK; p < 0.001) due to higher costs of medication (glargine group: 7 992.97 ? 4 001.81 CZK vs NPH group: 3 784.2 ? 3 181.48 CZK; p < 0.001). Diabetes mellitus je onemocněním, jehož některé projevy mohou zvýšit riziko dopravní nehody, ovlivňuje tedy způsobilost k držení řidičského průkazu. Za nejrizikovější situaci je v tomto ohledu považována hypoglykemie během řízení, diabetes však uvedené riziko zvyšuje např. i díky možnému zhoršení zrakových schopností při případné retinopatii. Za nejrizikovější skupinu řidičů diabetiků stran nehod lze považovat ty, kteří mají v anamnéze těžkou hypoglykemii, hypoglykemii proběhlou při řízení, či silniční nehodu. Zásadním preventivním prvkem v rámci bezpečného silničního provozu je u diabetika léčeného inzulinem především měření glykemie před jízdou a znalost, jak hypoglykemii předcházet a léčit., Diabetes mellitus is a disease which may affect the eligibility to hold a driving license and increase the risk of a road accident. Hypoglycemia while driving is considered to be the most risky situation, with diabetes increasing the mentioned risk for instance due to impaired vision in the case of possible retinopathy. The group of drivers with diabetes being at the greatest risk as to accidents are those with a case history of severe hypoglycemia or hypoglycemia occurred while driving, or possibly of a road accident. Diabetes mellitus a zejména diabetes mellitus 2. typu představuje jeden z nejvýznamnějších rizikových faktorů kardiovaskulárních onemocnění. Z hlediska ovlivnění KV rizika má enormní význam nejen pozitivní ovlivnění glykemie, ale také léčba diabetické dyslipidemie a arteriální hypertenze. Předložená práce poskytuje analýzu terapie antihypertenzivy a hypolipidemiky u všech diabetiků registrovaných u VZP v období let 2010–2013. V uvedeném období bylo s diagnózou diabetes mellitus léčeno 866 570 pojištěnců VZP, přičemž většinu tvořili diabetici nezávislí na inzulinu. Přibližně polovina pacientů byla dispenzarizována diabetology a polovina lékaři jiných specializací. Z antihypertenziv byli pacienti nejčastěji léčeni léky ovlivňujícími systém renin-angiotenzin, dále pak betablokátory a diuretiky. Preskripce hypolipidemické léčby, zejména preskripce statinů, v souladu s platnými doporučeními, pozvolna narůstá, nicméně i nadále zůstává nedostatečná (v roce 2013 zůstalo bez hypolipidemické léčby 43,6 % diabetiků z ambulancí diabetologů a 51,3 % diabetiků z ambulancí praktických lékařů!). V doplňující analýze dat za rok 2014 jsme zjistili, že v tomto roce bylo léčeno z celkového počtu osob s farmakologickou léčbou diabetu nějakým hypolipidemikem 57,3 % osob. Nedostatečné využití zaznamenává rovněž kombinační hypolipidemická léčba, dosud zastoupená zejména kombinací statinu s fibrátem. Nicméně v následujících letech očekáváme na základě pozitivních výsledků subanalýzy studie IMPROVE-IT nárůst kombinační hypolipidemické terapie, resp. kombinační léčby statiny v kombinaci s ezetimibem. To influence cardiovascular risk there is enormous important not only positive influence of glycemia, but also the treatment of diabetic dyslipidemia and hypertension. The present work provides an analysis of lipid-lowering and antihypertensive therapy for all diabetics registered with General Health Insurance Company in the period 2010–2013. In this time 866 570 patients with diabetes mellitus registered with General Health Insurance Company were treated, the majority of them were diabetics independent on insulin. Approximately half of the patients were observed by dialectologists and half of them by the doctors of other specialization. Out of antihypertensive medication, patients were most often treated by drugs that affect the renin-angiotensin system, as well as beta-blockers and diuretics. Prescription of the lipid-lowering therapy, especially prescription of statins, in accordance with the guidelines, is increasing, but remains insufficient (at 2013 43.6% diabetics treated by dialectologists and 51.3% diabetics treated by GP´s didn´t have lipid lowering therapy). Inadequate use of combination lipid-lowering therapy was recorded too, still represented mainly by combination of statin and fibrate, but in coming years we expect (based on the positive results of the subanalysis IMPROVE-IT study), an increase of combination therapy statin and ezetimibe. Cíl práce: Zhodnotit bezpečnost a účinnost léčby bazálním inzulinem glargin v režimu bazál/bolus v podmínkách běžné klinické praxe v České republice. Soubor a metodika: Projekt LINDA byl neintervenční, multicentrický (n = 255), národní observační projekt, který probíhal v podmínkách běžné klinické praxe. Do sledování bylo zařazeno celkem 4 998 pacientů s diabetes mellitus 1. a 2. typu (DM1T, DM2T) s převážně inzulinovou terapií (99,7 %), po následném zahájení léčby inzulinem glargin v režimu bazál/bolus. Pacienti byli sledováni po dobu 6 měsíců od zahájení této terapie. Primárním cílem projektu bylo sledování četnosti výskytu závažných hypoglykemických příhod při léčbě bazálním inzulinovým analogem glargin (Lantus?) v podmínkách běžné klinické praxe. Sekundárními sledovanými parametry byla změna glykovaného hemoglobinu (HbA1c), glykemie nalačno (FPG), změna tělesné hmotnosti, změna dávek inzulinu, změna počtu hypoglykemických příhod ve srovnání s předchozí léčbou a sledování četnosti nežádoucích účinků. Výsledky: Závažné hypoglykemie byly pozorovány během léčby inzulinem glargin u 0,8 % pacientů. Při srovnání výskytu hypoglykemií s předchozí terapií bylo prokázáno klinicky a statisticky významné snížení jejich četnosti. Procento pacientů s hypoglykemickou příhodou (17,6 %), závažnou (0,8 %) a závažnou noční hypoglykemií (0,3 %) bylo za poslední měsíc léčby s inzulinem glargin v režimu bazál/bolus konzistentně menší ve srovnání s posledním měsícem léčby před zahájením této terapie (42,5 %; 17,6 %; 13,8 % pacientů). U pacientů s DM1T došlo k poklesu všech hypoglykemií z hodnoty 37,80 ? 15,95 příhod/pacient/rok na 8,76 ? 4,38 příhod/pacient/rok (p < 0,001), závažných hypoglykemií z hodnoty 5,64 ? 3,27 příhod/pacient/rok na 0,0396 ? 0,012 příhod/pacient/rok (p < 0,001) a závažných nočních hypoglykemií z hodnoty 3,84 ? 2,04 příhod/pacient/rok na 0,0096 ? 0,003 příhod/pacient/rok (p < 0,001). U pacientů s DM2T poklesla četnost všech hypoglykemií z hodnoty 12,48 ? 7,57 příhod/pacient/rok na 1,68 ? 0,78 příhod/pacient/rok (p < 0,001), závažných hypoglykemií z hodnoty 2,04 ? 0,94 příhod/pacient/rok na 0,0132 ? 0,005 příhod/pacient/rok (p < 0,001) a závažných nočních hypoglykemií z hodnoty 1,32 ? 0,77 příhod/pacient/rok na 0,0048 ? 0,0008 příhod/pacient/rok (p < 0,001). Bylo prokázáno statisticky významné zlepšení metabolické kompenzace při užití inzulinu glargin. U DM1T došlo ke snížení hladiny glykovaného hemoglobinu (HbA1c) z hodnoty 7,74 ? 1,71 % na 6,43 ? 1,39 % (∆ -1,31 ? 0,32 %; p < 0,001) a u DM2T z hodnoty 8,13 ? 1,56 % na 6,72 ? 1,40 % (∆ -1,41 ? 0,28 %; p < 0,001). Statisticky významně (p < 0,001) se zvýšil počet pacientů s hodnotami HbA1c < 5,4 %. Statisticky významný byl také pokles glykemie nalačno (FBG) a hodnot 6bodového glykemického profilu, a to u obou typů diabetu (p < 0,001). Změna terapie a následná léčba inzulinem glargin byla lékaři a pacienty vnímána pozitivně. Závěr: Za podmínek běžné klinické praxe zahájení léčby inzulinem glargin v režimu bazál/bolus u pacientů s předchozí inzulinovou terapií vedlo ke snížení četnosti výskytu hypoglykemických příhod včetně závažných a závažných nočních hypoglykemií a ke zlepšení metabolické kompenzace pacientů s diabetem (snížení glykovaného hemoglobinu, glykemie nalačno a hodnot 6bodového glykemického profilu). Byla zaznamenána větší spokojenost se stávající léčbou ze strany pacientů a lékařů. Patients and methods: The LINDA project was a non-interventional, multicenter (n = 255), national, observational project. A total of 4,998 patients with Type 1 and 2 diabetes mellitus (T1DM, T2DM) with predominantly insulin therapy (99,7 %), after switch on insulin glargine at basal-bolus regimen, were enrolled in this project. The patients were followed up for 6 months after initiation of the therapy with insulin glargine. The primary objective of the project was to investigate the incidence of severe hypoglycemic episodes during the treatment with basal insulin analogue glargine (Lantus?) in a common clinical practice setting. The secondary endpoints were changes in glycosylated hemoglobin (HbA1c) levels, fasting plasma glucose (FPG), body weight, insulin dose, change of number of hypoglycemic episodes in comparison the previous therapy and the frequency of adverse effects. Results: Severe hypoglycaemia were observed during treatment with insulin glargine at 0.8 % patients. When comparing the incidence of hypoglycemia with the previous therapy, we demonstrated a clinically and statistically significant reduction in their frequencies. The percentage of patients with hypoglycemic episodes (17.6 %), severe hypoglycemia (0.8 %) and severe nocturnal hypoglycemia (0.3 %) over the last month of treatment with insulin glargine using the basal-bolus regimen was consistently lower compared to the last month of treatment before initiation of this therapy (42.5 %, 17.6 %, and 13.8 % of the patients, respectively). In patients with T1DM, the incidence of hypoglycemia decreased from 37.80 ? 15.95 episodes/patient/year to 8.76 ? 4.38 episodes/patient/year (p < 0.001) for all hypoglycemic episodes; from 5.64 ? 3.27 episodes/patient/year to 0.0396 ? 0.012 episodes/patient/year (p < 0.001) for severe hypoglycemia; and from 3.84 ? 2.04 episodes/patient/year to 0.0096 ? 0.003 episodes/patient/year (p < 0.001) for severe nocturnal hypoglycemia. In patients with T2DM, the incidence of hypoglycemia decreased from 12.48 ? 7.57 episodes/patient/year to 1.68 ? 0.78 episodes/patient/year (p < 0.001) for all hypoglycemic episodes; from 2.04 ? 0.94 episodes/patient/year to 0.0132 ? 0.005 episodes/patient/year (p < 0.001) for severe hypoglycemia; and from 1.32 ? 0.77 episodes/patient/year to 0.0048 ? 0.0008 episodes/patient/year (p < 0.001) for severe nocturnal hypoglycemia. A statistically significant improvement in the metabolic control was demonstrated when using insulin glargine. The glycated hemoglobin (HbA1c) decreased from 7.74 ? 1.71 % to 6.43 ? 1.39 % (∆ -1.31 ? 0.32 %, p < 0.001) in patients with T1DM, and from 8.13 ? 1.56 % to 6.72 ? 1.40 % (∆ -1.41 ? 0.28 %, p < 0.001) in patients with T2DM. A statistically significant (p < 0.001) increase in the number of patients with HbA1c < 5.4 % was further demonstrated. The decrease in fasting blood glucose (FBG) and 6-point blood sugar profile was also statistically significant in both types of diabetes (p < 0.001). Changes in therapy and subsequent treatment with insulin glargine were perceived positively by both physicians and patients. Conclusion: In the common clinical practice setting, the initiation of treatment with insulin glargine using the basal-bolus regime in patients with previous insulin therapy resulted in a reduction in the incidence of hypoglycemic events, including severe hypoglycemia and severe nocturnal hypoglycemia, and improved metabolic control in patients with diabetes (reduced glycated hemoglobin, fasting glucose values and 6-point blood glucose profile). Greater satisfaction with the current treatment was reported by both patients and physicians. Toxická epidermální nekrolýza je vzácné, akutní, život ohrožující onemocnění projevující se rozsáhlým odlučováním epidermis a slizničních povrchů. Jde o závažnou nežádoucí reakci na podávané léky, nejčastěji antibiotika, antikonvulziva či nesteroidní antirevmatika. Jako první ji popsal skotský dermatolog Alan Lyell v roce 1956, proto je též nazývána Lyellův syndrom. Mortalita tohoto onemocnění je vysoká, proto je klíčová včasná diagnostika a neodkladné zahájení adekvátní péče o pacienta. Jedná se o vzácné onemocnění s velmi malou incidencí a závažnou prognózou, což značně ztěžuje provedení velkých randomizovaných klinických studií. Je tedy obtížné hodnotit efektivitu jednotlivých terapeutických možností. Jako nejperspektivnější se prozatím jeví nitrožilní podávání imunoglobulinů se zřejmým zlepšením klinického stavu pacienta, dobrou tolerancí a minimálními vedlejšími nežádoucími účinky. Nové experimentální postupy se snaží hledat cestu terapie cíleným ovlivněním granulyzinu jakožto hlavního cytotoxického mediátoru. It is a serious adverse response to administered drugs, mostly antibiotics, anticonvulsants or NSAIDs. First described by Scottish dermatologist Alan Lyell in 1956, it is also known as Lyell’s syndrome. Mortality of the disease is high, therefore its early diagnosis is crucial and immediate initiation of appropriate patient care necessary. It is a rare disease with a very low incidence and serious prognosis, which is a considerable hindrance to undertaking large randomized clinical studies. It is therefore difficult to evaluate the effectiveness of various therapeutic options. As the most promising so far appears the administration of intravenous immunoglobulins, apparently reaching improvement in the clinical condition of the patient, with a good tolerances and minimal side adverse effects. New experimental techniques endeavour to seek a way to the therapy through targeted influencing of granulysin as the major cytotoxic mediator. Úvod: Projekt PROROK (Prospektivní observační projekt významu diference glykemie nalačno a postprandiální glykemie pro odhad úspěšnosti terapie diabetu 2. typu) měl charakter neintervenčního, prospektivního, multicentrického observačního projektu s délkou trvání 6 měsíců, jehož cílem byla kvantifikace významu rozdílu mezi glykemií nalačno a postprandiálně pro úspěšnost léčby agonisty pro GLP1 resp. léčby inzulinem bazálním, premixovaným, nebo kombinací bazálního a bolusového inzulinu. Lékaři vybírali léčbu pro neuspokojivě kompenzované pacienty dle vlastního uvážení, zařazeno bylo celkem 4 972 pacientů. Cíl: Cílem práce bylo zhodnocení rozdílů v základních antropometrických a biochemických parametrech mezi skupinami pacientů zařazených do projektu PROROK se zřetelem k volbě terapie ošetřujícím diabetologem. Metodika a výsledky: Pacienti léčení agonisty receptoru GLP1 byli významně mladší, trpí diabetem kratší dobu a zároveň byli obéznější a měli nejvyšší koncentraci triacylglycerolů. Pacienti, u nichž byla volena terapie bazálním inzulinem, měli nejvyšší glykemii nalačno. Pacienti, u nichž byla volena terapie premixovaným inzulinem nebo režimem bazálního a bolusového inzulinu, měli nejvyšší glykemii postprandiálně, pacienti s režimem bazálního a bolusového inzulinu měli nejvyšší vstupní glykovaný hemoglobin. Rozdíl mezi glykemií nalačno a postprandiálně byl nejmenší ve skupině, u níž byla volena terapie bazálním inzulinem a nejvyšší ve skupině vybrané k terapii premixovaným inzulinem resp. kombinací bazálního a bolusového inzulinu. Průměrné zlepšení glykovaného hemoglobinu bylo v celém souboru 1,6 %, medián výsledného glykovaného hemoglobinu byl 5,9 % resp. 5,8 % (léčení agonisty receptoru pro GLP1). Všechny rozdíly p < 0,001. Závěr: Při vědomí skutečnosti, že rozdíly nalezené v parametrech popisujících soubory jsou sice statisticky významné, ale klinicky jsou méně relevantní, považujeme za důležitou skutečnost, že výběr terapie je v konkordanci se základními poznatky o patofyziologii diabetu 2. typu a možnostmi individuálně volené cílené intervence antidiabetickou terapií. Lékaři účastnící se v projektu PROROK tedy volí většinou terapii racionálně., Introduction: The PROROK project (A prospective observation project to assess the relevance of the difference between fasting glycemia and postprandial glycemia to estimation of success of type 2 diabetes therapy) had a character of a non-interventional, prospective, multicentric observation project conducted for a period of 6 months, whose aim was to quantify the relevance of the difference between fasting and postprandial glycemia to the success of GLP1 receptor agonist treatment, or insulin therapy with basal or premixed insulin, or a combination of basal and bolus insulin. Physicians chose therapy for inadequately compensated patients at their own discretion, with 4 972 patients included. Aim: The study aimed at the assessment of the differences in basic anthropometric and biochemical parameters between the patient cohorts included in the PROROK project with regard to the therapy selected by the treating diabetologist. Methodology and results: The patients treated with GLP1 receptor agonists were quite young, they have suffered from diabetes for a shorter period of time and at the same time were more obese and had the highest concentration of triacylglycerols. The patients who underwent basal insulin therapy, had the highest fasting glycemia. The patients for whom premixed insulin therapy or basal/bolus insulin regimen were chosen, manifested the highest postprandial glycemia, those with basal/bolus insulin regimen had the highest initial glycated haemoglobin. The difference between fasting and postprandial glycemia was the smallest in the cohort for which basal insulin therapy was chosen and the greatest in the cohort chosen for the therapy with premixed insulin, or with the basal/bolus insulin combination. Average improvement in glycated haemoglobin values reached 1.6 % within the whole cohort, a median of the resulting glycated haemoglobin reached 5.9 % or 5.8 % (GLP1 receptor agonist treatment). All the differences amounted to p < 0.001. Conclusion: Bearing in mind that the differences established in the parameters describing the cohorts, although statistically relevant, are of smaller clinical relevance, we regard as an important finding that the choice of therapy is in accordance with the basic knowledge about the pathophysiology of type 2 diabetes and possibilities of an individually chosen targeted intervention with antidiabetic therapy. Cíl: Cílem bylo zhodnocení výsledků intervence u pacientů zařazených do observačního projektu PROROK se zřetelem k volbě terapie ošetřujícím diabetologem po 6 měsících sledování. Výsledky: Průměrné zlepšení glykovaného hemoglobinu činilo v celém souboru 1,6 %, medián výsledného glykovaného hemoglobinu byl 5,9 %, resp. 5,8 % (bazální inzulin). Medián hmotnosti statisticky významně ve skupině osob léčené agonisty receptoru pro GLP1 ze 105 kg na 100 kg, v ostatních skupinách se nezměnil významně. Změna obvodu pasu v čase u všech pacientů a u jednotlivých skupin odpovídala změně hmotnosti. Medián změny glykemie nalačno byl v celém souboru po 3 měsících -1,7 mmol/l, po 6 měsících -2,4 mmol/l (p < 0,001). Největší absolutní pokles byl ve skupině léčených bazálním inzulínem (-2,8 mmol/l). Medián změny glykemie postprandiálně byl po 3 měsících -2,4 mmol/l, po 6 měsících -3,3 mmol/l. Největší absolutní pokles byl zaznamenán ve větvi léčené kombinací prandiálního a bazálního inzulinu (-3,9 mmol/l). Všechny rozdíly p < 0,001. Závěr: Výběr terapie v projektu PROROK je v konkordanci se základními poznatky o patofyziologii diabetu 2. typu a možnostmi individuálně volené cílené intervence antidiabetickou terapií. Výsledky 6měsíční observace potvrzují správnost individuální volby terapie. U skupiny diabetiků lišících se na začátku hmotností, obvodem pasu, glykemií nalačno a rozdílem mezi glykemií nalačno a postprandiálně vedla diferencovaně volená terapie ke stejnému konečnému výsledku, přitom absolutní změna sledovaných parametrů se lišila v jednotlivých skupinách. Physicians chose a therapy for patients with insufficiently compensated problems as they considered appropriate; 4 972 patients were included. Goal: Evaluation of the intervention results for the patients included in the PROROK observation project with a focus on the choice of therapy by the treating diabetologist after 6 months of observation. Results: An average improvement of the glycated hemoglobin values in the whole cohort reached 1.6%, the median of the resulting glycated hemoglobin reached 5.9% and 5.8% resp. (basal insulin). Statistically significant was the change in the median weight in the cohort treated with GLP-1 receptor agonists, from 105 kg to 100 kg; this did not significantly change in the other cohorts. The change of waist circumference over time in all patients and in the individual cohorts was consistent with the change of weight. The median change of fasting blood glucose levels in the whole cohort was -1.7 mmol/l after 3 months and -2.4 mmol/l (p < 0.001) after 6 months. The greatest absolute decrease was recorded in the cohort treated with basal insulin (-2.8 mmol/l). The median change of postprandial blood glucose levels was -2.4 mmol/l after 3 months and -3.3 mmol/l (p < 0.001) after 6 months. The greatest absolute decrease was recorded in the branch treated with a combination of prandial and basal insulin (-3.9 mmol/l). All differences p < 0.001. Conclusion: The choice of therapy in the PROROK project is in agreement with the basic findings in pathophysiology of type 2 diabetes and with the options of an individually chosen targeted intervention involving antidiabetic therapy. The results of the six-month observation have proven the individual choice of therapy correct. In the cohort of diabetic patients differing at the beginning in weight, waist circumference, fasting blood glucose and the difference between fasting and postprandial glucose levels, an individually chosen therapy led to the same final result, while an absolute change in the followed parameters differed in the individual groups.
