Náplní sdělení je souhrnná informace o klinickém významu kvantitativního vyšetřování sérových hladin volných lehkých řetězců (S-VLŘ) imunoglobulinu a jejich vzájemného poměru (index ?/?) u jednotlivých forem monoklonální gamapatie (MG). Autoři popisují metodické aspekty automatizované kvantitativní imunochemické analýzy, využívající vysoce avidní polyklonální protilátku proti vnitřnímu epitopu molekul LŘ ? a ?, která umožňuje spolehlivé stanovení VLŘ. Je popsán stěžejní přínos vyšetřování S-VLŘ a indexu ?/? pro diagnostiku Bence-Jonesova ne- a hyposekretorického i IgD typu mnohočetného myelomu (MM), ale i pro průběžné hodnocení vývoje, hloubky léčebné odezvy, trvání stabilní/plateau fáze pro časné rozpoznání relapsu/progrese a léčebné rezistence i pro vymezení hloubky kompletní remise tzv. strindgent KR (sKR) u všech forem MM. Popsán je také význam pro diagnostiku a hodnocení léčebné odezvy u systémové AL-amyloidózy, primární makroglobulinémie a solitárního plazmocytomu; diskutována je i monoklonální gamapatie nejistého významu včetně její stratifikace do 3 rizikových skupin (low, intermediate, high risk), s odlišným prognostickým výhledem. Ze sdělení vyplývá, že zavedení této moderní imunochemické metody umožňující kvantifikaci S-VLŘ otevřelo bránu k novým klinickým aplikacím, zejména však k přesnější diagnostice a monitorování průběhu prakticky všech typů MG., The aim of the presented study is to display general information about the clinical significance of quantitative assessment of serum free light chains (S-FLC) of immunoglobulins and their mutual ratio (index ?/?) in individual forms of monoclonal gammopathies (MG). The study describes methodical aspects of automatic quantitative immunochemical analysis using high avidity polyclonal antibody against the internal epitope of the molecules of LC ? and ? enabling reliable detection of FLC. It also demonstrates the fundamental contribution of assessment of S-FLC and the ?/? index for the diagnostics of Bence-Jones, non- and hyposecretory and IgD multiple myeloma, and also for continual evaluation of the course of the disease, depth of therapeutic response, duration of stable/plateau phase, for early recognition of relapse/progression and therapy resistance as well as the assessment of the depth of therapeutic response (stringent complete remission, sCR) in all forms of MM. We also describe the significance for the diagnostics and evaluation of therapeutic response in systemic AL-amyloidosis, primary macroglobulinemia and solitary plasmocytoma, we also discuss the contribution for evaluation of monoclonal gammopathy of undetermined significance including its stratification to three groups (low, intermediate and high risk) with different prognosis. Onemocnění z depozice lehkých řetězců (light chain deposition disease – LCDD) patří mezi vzácná systémová onemocnění způsobená monoklonální proliferací terminálně diferencovaných B-lymfocytů s produkcí volných lehkých řetězců imunoglobulinu a jejich ukládáním především v ledvinách či v dalších tkáních. Náplní sdělení je poukázat na úskalí diagnostiky této klinické jednotky a na příkladu série 4 nemocných s LCDD předložit důkazy o vysoké účinnosti léčby s bortezomibem z pohledu hematologické i orgánové terapeutické odezvy. Z našeho hodnocení vyplývá, že léčba bortezomibem je u LCDD velmi rychlá a účinná, bývá však při intenzivním dávkovacím schématu doprovázena řadou nežádoucích účinků. Z nich je nejzávažnější periferní neuropatie, ohrožující i případné setrvání nemocného v léčbě. Méně intenzivní režimy (tzv. bortezomib weekly) představují alternativu s nižším výskytem nežádoucích účinků. Vysokodávkovaná chemoterapie s podporou autologní transplantace krvetvorných buněk je u vhodných kandidátů doporučeným a relativně bezpečným postupem. Orgánová léčebná odezva se významně opožďuje za hematologickou odezvou a zůstává otázkou, zdali je postižení tkání depozity lehkých řetězců plně reverzibilní. Klíčová slova: autologní transplantace krvetvorných buněk – bortezomib – hematologická léčebná odezva – onemocnění z depozice lehkých řetězců – orgánová léčebná odezva, Light chain deposition disease (LCDD) is a rare systemic condition caused by monoclonal proliferation of terminally differentiated B-lymphocytes with production of free light chains and their deposition in kidneys or other organs. The aim of our study is to show the pitfalls of the diagnostics, and to demonstrate the effect of bortezomib-based therapy on a series of 4 patients with LCDD, from the point of hematological and organ therapeutic response. We include that bortezomib based treatment provides rapid and effective hematological response. It is, however, often accompanied by adverse events, especially within intensive treatment schedules. The most serious adverse effects includes peripheral neuropathy, which might be dose or treatment-limiting. Less intensive regimens („bortezomib weekly“) suggest an alternative with expectation of lower incidence of adverse effects. Autologous stem cell transplantation is a recommended and relatively safe approach in convenient candidates. Organ response is significantly delayed after hematological response, and organ damage by light chain deposits might not be fully reversible. Náplní sdělení je, na pozadí rozboru diagnostiky a léčby vlastních tří nemocných, podání současných poznatků o poměrně vzácném a v běžné klinické praxi opomíjeném onemocnění, vyznačujícím se depozicí lehkého řetězce imunoglobulinu v tkáních životně důležitých orgánů. Onemocnění z depozice lehkých řetězců imunoglobulinu postihuje především ledviny a vede k progresivní poruše funkce s konečným orgánovým selháním. Kromě etiopatogeneze, odlišení idiopatické formy od formy provázející mnohočetný myelom a vzácně i jiné B-lymfoproliferativní stavy, je nastíněna problematika klinických projevů, diagnostiky, diferenciální diagnostiky a terapie včetně přínosu biologické léčby bortezomibem a vysokodávkované chemoterapie s podporou autologních kmenových buněk, vedoucí k podstatnému zlepšení kvality i celkové délky života nemocných s touto prognosticky stále závažnou nemocí., The aim of the paper within the background of the diagnostics and treatment of three patients is the summary of recent knowledge about a relatively rare disease which is characterized by the deposition of immunoglobulin light chains in vital tissues and organs, especially the kidneys, leading to progressive deterioration of their function with end stage organ failure.
