Cílem článku je prezentovat současná doporučení k léčbě karcinomu močového měchýře po iniciální transuretrální resekci tumoru. Adjuvantní intravezikální léčba chemoterapií a imunoterapií prokazatelně snižuje recidivy karcinomů a v případě imunoterapie při udržovací dlouhodobé léčbě dokonce i oddaluje progresi onemocnění. Proto má svoji nezastupitelnou roli v komplexní péči pacientů s karcinomy měchýře neinfiltrujícími svalovinu., The aim of this article is to present current recommendations for treatment of bladder cancer after initial transurethral resection of tumor. Karcinom prostaty patří mezi nejčastější nádorová onemocnění u mužů. V časném a lokalizovaném stadiu je léčitelný, určité procento pacientů však relabuje nebo se diagnóza stanoví až v pokročilejších stadiích, kde jsou léčebné možnosti limitované. Imunoterapie je kromě klasické hormonální terapie, radioterapie a chemoterapie další perspektivní léčebnou modalitou v těchto stadiích. Vznik nádorového onemocnění je mnohastupňový proces komplexní povahy, v němž samotné nádorové buňky představují heterogenní populaci zasazenou do kontextu mikroprostředí nenádorový ch buněk a tkání tvořících složitý nádorový „ekosystém“. Imunitní systém je schopen kontrolovat růst nádorových buněk v raných stadiích vzniku nádoru. S pokročilým růstem nádoru jsou imunitní mechanizmy víceméně paralyzovány a mnohdy na proliferaci nádorových buněk mají paradoxně podpůrný účinek. Aby byla protinádorová terapie úspěšná, musí zohledňovat všechny komplexní aspekty interakce imunitního systému a nádorových buněk včetně mikroprostředí. It is treatable in the early and localized stage; however, a proportion of patients relapse or the diagnosis is made only in more advanced stages when the therapeutic options are limited. In addition to classic hormonal therapy, radiotherapy, and chemotherapy, immunotherapy is another promising treatment modality in these stages. The development of cancer disease is a multistage process of a comprehensive nature wherein the tumour cells themselves represent a very heterogeneous population put into the context of a microenvironment of non-tumour cells and tissues, thus forming a complex tumour "ecosystem". The immune system is capable of controlling tumour cell growth in the early stages of tumour development. With advanced tumour growth, immune mechanisms become more or less paralysed and, in fact, not infrequently have a promoting effect on tumour cell proliferation. In order to be successful, anticancer therapy must take into consideration all the comprehensive aspects of interaction of the immune system and tumour cells, including the microenvironment. Aktivace imunitního systému je velmi důležitým krokem v léčebné strategii metastazujícího melanomu a větší pochopení funkce kontrolních bodů imunity vedlo k vývoji řady checkpoint inhibitorů. Ipilimumab, monoklonální protilátka proti CTLA-4 byl první lék, který prokázal zlepšení přežití u pacientů s metastazujícím melanomem. Medián celkového přežití (OS) u pacientů léčených ipilimumabem byl, při porovnání s pacienty léčenými gp100 vakcínou, delší (10,9 proti 6,4 měsíce) a také pacienti léčení ipilimumabem a DTIC měli, proti pacientům léčeným samotným DTIC zlepšené OS (11,2 proti 9,1 měsíce). Většina nežádoucích účinků vyvolaných léčbou ipilimumabem jsou imunitně zprostředkované a mezi nejčastější patří gastrointestinální a kožní toxicita, endokrinopatie a hepatotoxicita. Koncem roku 2014 byly schváleny FDA pembrolizumab a nivolumab, nové monoklonální protilátky proti dalšímu významnému kontrolnímu bodu imunity PD-1 receptoru. Vstupujeme do nové éry v léčbě melanomu, ale nemůžeme říci, že je problematika léčby melanomu vyřešena. Je nutný další výzkum v oblasti biomarkerů a kombinované léčby, včetně optimálního dávkování i doby terapie., Activation of the immune system is very important step in the treatment strategy of metastatic melanoma and greater understanding of function of the immune checkpoints led to the development of several immune checkpoint inhibitors. Ipilimumab, an anti-CTLA-4 monoclonal antibody, was the first agent to demonstrate a survival benefit in patients with metastatic cutaneous melanoma. The median OS rate for patients treated with ipilimumab was, compared with patients receiving a gp100 vaccine longer (10.9 versus 6.4 months), and also patients treated with ipilimumab and DTIC compared with those treated with DTIC alone had improved OS (median OS 11.2 versus 9.1 months.) The majority of ipilimumab-related adverse events are immune related, and the most common are gastrointenstinal and skin toxicity, endocrinopathy and hepatotoxicity. New monoclonal antibody against other important immunity checkpoint PD-1 receptor, pembrolizumab and nivolumab, got FDA approval at the end of 2014. We are approaching the new era of the treatment of melanoma but we cannot say, that we resolved the problem. Zásadní význam T-lymfocytů pro kontrolu nádoru byl ověřen jak na zvířecích modelech tak klinickými pozorováními. Adoptivní buněčná terapie dosáhla v poslední době výborné účinnosti, což podporuje zjištění, že imunitní systém může bojovat s nádorovým onemocněním. Použití buněčné imunoterapie bylo dříve omezeno technickými možnostmi izolace a expanze antigen specifických T-lymfocytů schopných selektivně zabíjet nádorové buňky. Jedna ze současných slibných strategií je založena na ex vivo manipulovaných pacientových T-lymfocytech, které jsou upraveny pomocí chimérického antigenního receptoru (CAR). Chimérické antigenní receptory obvykle kombinují vazebné místo pro antigen z monoklonální protilátky s efektorovou funkcí T-lymfocytů. Genetická manipulace dává T-lymfocytům požadovanou specificitu, ale také umožňuje ovlivnit jejich aktivační a proliferační potenciál. Takto upravené CAR T-lymfocyty mohou podstatně redukovat nádorovou masu, která exprimuje na nádorových buňkách cílový antigen. Byly testovány různé designy CAR receptorů, různé výrobní procesy a soubory pacientů. CAR T-lymfocyty představují efektivní terapeutický přístup, a to zejména u hematologických malignit. Podávání CD19 specifických CAR-modifikovaných T-lymfocytů, které se váží na B-lymfocyty, u nehodgkinovských lymfomů a chronické lymfocytární nebo akutní lymfoblastické leukémie, bylo v nedávných klinických studiích velice efektivní. Budoucí výzkum musí v oblasti designu a přípravy CAR T-lymfocytů identifikovat kritické parametry, které jsou nezbytné pro efektivní využití adoptivního přenosu T-lymfocytů v onkologické léčbě. Personalizovaná buněčná terapie se pak může rozšířit a stát se slibným léčebným postupem pro hematologické malignity i solidní nádory., A central role of T-cells in the control of cancer has been supported by both animal models and clinical observations. Adoptive cell therapy has recently achieved impressive efficacy, supporting the finding that the immune system can fight cancer. The broaden application of cell immunotherapy has been previously constrained by the technical abilities to isolate and expand antigen-specific T-cells potent to selectively kill tumor cells. One of the promising current strategies is based on ex vivo engineered patients T-cells, which are redirected by a chimeric antigen receptor (CAR). Chimeric antigen receptors usually combine the antigen binding site of a monoclonal antibody with the effector function of a T -cell. The genetic manipulation gives T-cells desired specificity but also enables to tailor their activation and proliferation potential. Such modified CAR T-cells can substantially reduce the tumor burden as long as the targeted antigen is present on the cancer cells. Various CAR designs, manufacturing processes, and study populations have been tested. CAR T-cells represent a powerful therapeutic approach, especially in hematologic malignancies. Administration of CD19-specific CAR-modified T cells, that target B-cell, in non-Hodgkin lymphomas and chronic lymphocytic or acute lymphoblastic leukemia, has been remarkably effective in recent clinical trials. The future research has to identify the critical parameters of CAR T-cells design and engineering that are necessary for effective utilization of adoptive T-cell transfer in cancer therapy. Pokrok zejména v oblasti molekulární imunologie otevřel nové možnosti, umožnil identifikaci nových cílů v imunitním systému a vedl k vývoji nových protinádorových látek. Některé z těchto látek jsou v současné době schváleny a jsou součástí standardní léčby, další jsou v posledních fázích klinického zkoušení. Tyto látky mají význam zejména v paliativní indikaci u nemocných s pokročilým stadiem onemocnění, kterým nemůžeme nabídnout mnoho léčebných možností. Očekává se rozšíření imunoterapie i do indikace adjuvantní. Potenciál imunoterapie v léčbě nádorových onemocnění je velký, pozornost se soustřeďuje na solidní nádory. Zejména maligní melanom hrál důležitou roli při rozvoji imunoterapie u nádorových onemocnění. Značné úsilí je také věnováno hledání klinicky dostupných prediktivních a prognostických faktorů, které by dokázaly vybrat pacienty vhodné k léčbě a které by usnadnily její průběžné hodnocení., Advances, particularly in the field of molecular immunology, have opened new opportunities, allowed identification of new targets in the immune system, and resulted in developing new anticancer agents. Úvod: Informace o prevalenci orgánových komplikací diabetu včetně syndromu diabetické nohy v České republice je možné získat z dokumentů Ústavu zdravotnických informací a statistiky. Cíl: Posoudit vývoj vysokých amputací a drobných výkonů na noze v letech 2010–2014 v reprezentativním vzorku populace pacientů s diabetes mellitus vedených v databázi Všeobecné zdravotní pojišťovny (VZP) České republiky. Metodika: V databázi VZP jsme identifikovali všechny osoby, které měly v letech 2010–2014 záznam o diagnóze DM (E10–E16 podle MKN 10) nebo předpis jakékoliv antidiabetické terapie (ATC skupina A10). Pro analýzu byl extrahován soubor osob, které měly v daném roce předepsanou nejméně jednou látku ze skupiny A10. Následně jsme identifikovali osoby, které v letech 2010–2014 současně prodělaly chirurgický výkon na noze z důvodu diagnózy diabetické nohy. Výsledky: Absolutní počet amputačních výkonů na končetině vykazuje stacionární úroveň. Závěr: Předkládaná analýza je prvním zhodnocením vývoje výkonů na diabetické noze v ČR. Je zřetelný setrvalý stav v počtu výkonů na diabetické noze, a to jak vysokých amputací, tak drobných výkonů na noze. V kontextu s absolutním nárůstem pacientů, léčených pro diabetes mellitus, je stacionární stav známkou relativního poklesu, což je příznivé především v oblasti amputace dlouhých kostí. Klíčová slova: amputace – diabetes mellitus – syndrom diabetické nohy, Introduction: Information about the incidence of organ-affecting complications of diabetes, including the diabetic foot syndrome, can be obtained from the documents of the Institute of Health Information in the Czech Republic. Goal: Assessment of the development of high amputations and minor surgical procedures on the lower limb from 2010 to 2014 in a representative sample of the population of patients with DM kept in the General Health Insurance Company of the Czech Republic database. Methodology: We identified all individuals in the VZP database who had a record of DM diagnosis (E10 – E16 based on ICD 10) or any antidiabetic therapy prescribed (ATC group A10) in the period of 2010 – 2014. A set of patients who had an agent from A10 group prescribed at least once in the given year was extracted for analysis. In the next step we identified individuals, who in the period of 2010 – 2014 also underwent a surgical procedure on the lower limb due to diabetic foot. Results: An absolute number of lower limb amputations remains at a stationary level. Conclusion: The submitted analysis presents the first assessment of the development of surgical treatment of diabetic foot in the Czech Republic. The amount of surgical procedures on the diabetic foot remains stable, regarding both high amputations and lower limb minor surgical procedures. In the context of an absolute increase of patients treated for diabetes mellitus, the stationary state is an indication of a relative decrease, which is favourable in particular with regard to the amputation of long bones. Jako incidentalom se označuje náhodně zjištěná expanze nadledviny. Vzhledem k rozvoji zobrazovacích technik se s ní setkáváme stále častěji. Její prevalence je totiž více než 1 % dospělých a roste s věkem. Při vyšetření je nutno vyloučit malignitu. K tomu slouží klasicky vyšetření CT, ev. MRI. Nověji se stále více uplatňuje PET-CT, která má senzitivitu asi 97 % a specificitu 91 %. Hormonální vyšetření má vyloučit i subklinickou nadprodukci kortizolu (optimální je dexametazonový supresní test). Aldosteron je nutno vyšetřovat jen u nemocných s hypertenzí a katecholaminy u nádorů s vyšší nativní denzitou než 10 HU. Při sledování je nutnost opakovaných CT vyhrazena jen pro vybrané případy a rutinní kontrolní laboratorní vyšetření jsou také zpochybněna., Incidentaloma is an adrenal mass discovered serendipitously. Because of increasing use of imaging techniques it is a common finding, being present in more than 1 % of adults. During work-up malignancy has to be ruled out. Classically imaging using CT or MRI is used. Recently PET-CT with FDG has been used more often as its sensitivity for diagnosis of malignancy is about 97 % and specificity 91 %. Hormonal evaluation should diagnose subclinical hypercortisolism. (Dexamethasone Suppression Test is the method of choice). Aldosterone and Renin should be measured only in patients with hypertension and catecholamines in tumours with higher native density than 10 HU. The presence of a broad spectrum of autoantibodies in Sjögren's syndrome (SjS) patients is the result of abnormal B-cell regulation that can be at least partially explained by abnormal BAFF/BAFFR regulation. The objective of this study was to determine both membrane and intracellular expression of BAFF/BAFFR in monocytes and B-cells in peripheral blood of 19 primary Sjögren's syndrome patients and 20 healthy controls using flow cytometry. We also measured sBAFF in serum. Compared to healthy controls, both surface and intracellular expression of BAFF was significantly increased in monocytes and B-cells of SjS patients. Also serum sBAFF level was elevated. Expression of BAFFR on B-cells of SjS patients was surprisingly decreased, but there was no clear increase or decrease within monocytes. Our results indicate that activated monocytes communicate with B-cells via BAFF and BAFFR, so that B-cells are stimulated, but BAFF is also produced to stimulate cells in autocrine way. The decrease of BAFFR expression in SjS patients suggests that there is the mechanism that attempts to take over in order to balance the high level of BAFF. and J. Krejsek, M. Koláčková, I.
