Diagnosis of nerve agent intoxication is based on anamnestic data, clinical signs and laboratory examination. For acute poisoning, cholinesterase activity in the blood (erythrocyte AChE, plasma/serum BuChE) is sensitive, simple and most frequent laboratory examination performed in biochemical laboratories. Specialized examinations to precise treatment (reactivation test) or to make retrospective diagnosis (fluoride induced reactivation etc.) can be conducted. Úvod: Laparoskopická chirurgie jedním vstupem (SILS) je v současné době běžnou alternativou ke klasické laparoskopii. Materiálové náklady na SILS však výrazně limitují rozšíření metody na většině pracovišť v ČR. Proto jsme začali používat metodu 2 vstupů, která je sice technicky náročnější než přístup z 3–4 vstupů, zato s ním cenově zcela srovnatelná, a výsledkem se blíží kosmetickému efektu SILS. Metody: Popisujeme technický postup při laparoskopii ze 2 incizí. Hodnotíme vlastní soubor nemocných operovaných v období 2012−2014. Výsledky: V souboru 64 nemocných, kteří podstoupili laparoskopickou cholecystektomii, appendektomii či TAPP plastiku metodou DILS, jsme zaznamenali 9 komplikací, hodnocených stupni I-IIIb dle Claviena-Dindo. 4x jsme konvertovali DILS-cholecystektomii na laparoskopii z více portů. Závěr: Metoda 2 incizí není v našem souboru zatížena větším množstvím komplikací ve srovnání s laparoskopií z více vstupů. Z estetického hlediska je pacienty dobře hodnocená a ve srovnání se SILS je ekonomicky výhodná., Introduction: Single incision laparoscopic surgery (SILS) is a common alternative to classical laparoscopic surgery. However, material costs of SILS significantly limit the spreading of this method to most of surgical departments in the Czech Republic. Therefore we introduced DILS, which is technically more difficult than 3- to 4-incision laparoscopy but comparable cost-wise and its result approaches the cosmetic effect of SILS. Methods: We describe the technical approach for DILS. We evaluate our own group of patients who were operated on in 2012-2014. Results: Nine minor complications evaluated as Clavien-Dindo grade I-IIIb were seen in the group of 64 patients undergoing laparoscopic cholecystectomy, appendectomy or TAPP operation of groin hernias using DILS method. DILS-cholecystectomy was converted to multiple-ports laparoscopy 4 times. Conclusion: In our group, DILS did not present a higher complication rate than laparoscopic surgery using multiple incisions. Úvod: Video prezentuje naši současnou standardní/ nejčastější techniku provádění laparoskopické resekce (LR) tumoru ledviny (TL). Materiál: Od 8/2004 do 4/2015 jsme provedli 274 LR. Během let se metodika neustále vyvíjela se zručností operatérů a s využitím nových technických doplňků. Ruku v ruce s tím se rozšiřovalo i spektrum TL ošetřitelných laparoskopicky. Postupně se přidávaly nádory s vyšším RENAL nefrometrickým skórem (větší, centrálněji uložené, lokalizace i dorzálně a i na horním pólu, v kontaktu s cévami). Díky většímu rozsahu laparoskopicky ošetřitelných TL a také díky časnější diagnostice a posunu nově zachycených TL do nižších stadií se každoročně zvyšuje podíl TL ošetřených LR. V roce 2014 to bylo 31,9 % chirurgicky ošetřených TL. V absolutních číslech 48 výkonů za rok. Výsledky – popis metody: Standardní je poloha na boku, přístup transperitoneální ze čtyř portů (10, 2 × 5 a 12 mm). Vpravo někdy pro elevaci jater grasperem je užit další port 5 mm. Je otevřeno zadní peritoneum a Gerotova fascie. Je ozřejměn tumor a verifikován sonograficky laparoskopickou sondou (mj. určujeme ohraničení spodiny oproti parenchymu či sinu ledviny). Je-li tumor verifikován, jsou rozpreparovány hilové cévy. Krátkou laparoskopickou endosvorkou je klampována renální tepna či všechny její větve, či výjimečněji selektivně větev tepny zásobující tumor. Jde-li tumor do hloubky sinu, klampujeme i měkčí svorkou renální žílu. Pouze v případě velmi malých extrarenálně uložených tumorů hilus neklampujeme. U komplexních hilů, kde není možná selektivní preparace hilu, je možno použít dlouhé cévní svorky zavedené přímo přes stěnu břišní. Následuje resekce tumoru nůžkami s lemem zdravé tkáně. Tumor je vložen do sáčku EndoCatch® Gold. Spodina je někdy ošetřena bipolární koagulací či argonovým koagulátorem (bezkontaktní, ale dražší) a to zejména v případech, že resekce byla blízko tumoru. Pomocí Vicryl® či nověji s V-Loc 90™ (oboje vstřebatelné) stehu je provedena sutura spodiny resekce (cévy event. i otevřený dutý systém), stehy jsou kotveny Hem-o-lok™ klipy velikosti ML. Další stejnou suturou jsou k sobě sešity okraje resekované ledviny. Klipy umožňují i u V-Loc™ lepší dotažení tkáně ledviny a vlákno se neprořezává. Hilové cévy jsou uvolněny. Je ošetřeno event. reziduální krvácení dalším stehem, někdy je přiložena tkaná celulóza. S V-Loc 90™ je provedena sutura obalů ledviny (Gerotova fascie, ev. i peritoneum). Laterálním portem je vložen páskový drén. Tumor je v sáčku extrahován lehce rozšířeným portem 12 mm. Spodina tumoru je pro patologa označena černým inkoustem. Závěr: LR je již standardizovaná metoda umožňující ošetřit téměř třetinu nádorů ledvin., This video presents our contemporary standard technique of laparoscopic resection (LR) of kidney tumours (KT), the method used most frequently by our group. Material: Between August 2004 and April 2015, we performed 274 LR. The surgical technique was continually developing along with the surgeons´ dexterity and usage of new technical accessories. At the same time, this surgical technique allowed for the treatment of a broader spectrum of KT, which are now managed using laparoscopic approach. This has gradually led to our management of KT with a higher R.E.N.A.L. nephrometry score (larger, more centrally or dorsally located lesions, and those affecting upper pole or hilus). There is an annual increase of KT managed by LR thanks to earlier diagnosis and an increased rate of detection of early (low) stage KT. In 2014, LR constituted 31.9 % of all surgically treated KT, amounting to 48 procedures. Results – our technique: The standard position is on the side with a transperitoneal approach using 4 ports (10mm, 2x5mm and 12mm). In select 105 Ces Urol 2015; 19(2): 103–105 video cases, an additional 5mm port on the right side is used for liver elevation with a grasper. Posterior peritoneum and Gerota´s fascia are opened. The tumour is detected and verified by a laparoscopic ultrasound probe (we define the borderline between tumour and parenchyma/renal sinus). After tumour verification, dissection of the renal hilar vessels is performed. Next the renal artery/arteries, or in rare instances only selected branch, are clamped using a laparoscopic bulldog endoclamp. In the case of tumour invasion into the sinus, we use a venous bulldog endoclamp to clamp the renal vein. In cases with a small extrarenal tumour we do not clamp the renal hilum. In cases with a complex renal hilum, where it is difficult to selectively dissect the vessel, it is possible to use extracorporeal clamping (inserted through the abdominal wall). This is followed by tumour resection into a healthy surgical margin using scissors. The specimen is inserted into the EndoCatch® Gold. In cases with a close resection margin, the resetected bed is managed by bipolar or Argon coagulation (the latter is contactless, but more expensive). The resected bed (vessels or opened collecting system) is sutured with Vicryl® or more recently V-Loc 90TM (both absorbable), with the sutures tightened with Hem-o-lok clipsTM ML. The same suture is used for closing of the parenchyma. Clips provide (even for V- LocTM) better tightening of kidney tissue and prevent the risk of kidney tearing. Hilar vessels are than released. Possible residual bleeding is managed by another suture or sometimes covered by oxidized cellulose. Suturing of the posterior peritoneum and Gerota´s fascia is done with V-Loc 90TM. An easyflow drain is placed through lateral 5mm port. The specimen is removed through incision of the 12mm port, which is slightly enlarged. The specimen margin is marked black for the pathologist. Úvod: Obstrukční defekační syndrom (ODS) představuje častý medicínský problém, který může být podmíněn různými patofyziologickými poruchami pánve; často je diagnostikováno několik poruch současně. V současnosti je dostupné velké množství korekčních technik z různých operačních přístupů. Laparoskopická resekční rektopexe je miniinvazivní transabdominální technika, která sdružuje resekci redundantního sigmoidea s mobilizací a fixací rekta. Metoda: Cílem této práce je vyhodnotit bezpečnost a efektivitu laparoskopické resekční rektopexe v léčbě pacientů s ODS. Vyhodnocení je provedeno na základě analýzy vlastního souboru pacientů a na podkladě průzkumu dostupných publikovaných dat. Výsledky: Do studie bylo zařazeno celkem 12 pacientů s ODS, kteří ve FN Ostrava podstoupili laparoskopickou resekční rektopexi v letech 2012–2015. Průměrný věk pacientů byl 64,5 roku; průměrné BMI 21,9; v souboru bylo 11 žen (91,6 %). Všichni pacienti měli ODS na podkladě kombinované vady pánevního dna. U každého z pacientů bylo diagnostikováno dolichosigmoideum a vnitřní nebo zevní prolaps rekta; několik pacientů mělo navíc také enterokélu a rektokélu. Laparoskopická resekční rektopexe byla provedena u všech pacientů bez peroperačních komplikací s průměrným operačním časem 144 minut. Průměrná pooperační délka hospitalizace činila 7 dní. Pooperační 30denní morbidita byla 16,6 %. Všechny pooperační komplikace byly hodnoceny jako nezávažné – stupeň II dle Clavien-Dindo klasifikačního systému. Průměrné předoperační Wexnerovo skóre bylo 23,6 bodu; průměrné skóre 6 měsíců po výkonu bylo 11,3 bodu. Výrazné zlepšení symptomů bylo pozorováno u 58,3 % pacientů, mírné zlepšení u 16,7 % pacientů; u 25 % byla resekční rektopexe bez klinického efektu. Závěr: V léčbě ODS je nutné pečlivě selektovat pacienty, kteří by mohli profitovat z chirurgického řešení. Naše výsledky v souladu s výstupy publikovaných studií potvrzují, že laparoskopická resekční rektopexe představuje cennou operační metodu v léčbě pacientů s ODS na podkladě kombinované poruchy pánevního dna., Introduction: Obstructive defecation syndrome (ODS) presents a common medical problem, which can be caused by various pelvic disorders; multiple disorders are frequently diagnosed. At the present, a high number of corrective techniques are available via various surgical approaches. Laparoscopic resection rectopexy is a minimally invasive technique, which comprises redundant sigmoidal resection with rectal mobilisation and fixation. Methods: The aim of this paper was to evaluate the safety and effectiveness of laparoscopic resection rectopexy in the treatment of patients with ODS. The evaluation was performed via our own patients data analysis and via literature search focused on laparoscopic resection rectopexy. Results: In total, 12 patients with ODS undergoing laparoscopic resection rectopexy in University Hospital Ostrava during the study period (2012–2015) were included in the study. In our study group, mean age was 64.5 years and mean BMI was 21.9; the group included 11 women (91.6%). ODS was caused by multiple pelvic disorders in all patients. Dolichosigmoideum and rectal prolapse (internal or external) were diagnosed in all included patients. On top of that, rectocoele and enterocoele were diagnosed in several patients. Laparoscopic resection rectopexy was performed without intraoperative complications; mean operative time was 144 minutes. Mean postoperative length of hospital stay was 7 days. Postoperative 30-day morbidity was 16.6%. All postoperative complications were classified as grade II according to Clavien-Dindo classification. Mean preoperative Wexner score was 23.6 points; mean score 6 months after the surgery was 11.3 points. Significant improvement in ODS symptoms was noted in 58.3% of patients, and a slight improvement in 16.6% of patients; resection rectopexy provided no clinical effect in 25% of patients. Conclusion: It is fundamental to carefully select those patients with ODS who could possibly profit from the surgery. Our results, in accordance with published data, suggest that laparoscopic resection rectopexy is a valuable surgical technique in the treatment of patients with ODS caused by multiple pelvic disorders., and P. Úvod: V dnešní době laparoskopické operace představují běžnou součást chirurgické praxe. Mají však svá specifika a vyžadují zvláštní způsob přípravy. K nácviku dovedností se v poslední době stále více využívají simulační techniky. Cílem tohoto systémového přehledu je zpracovat dostupnou literaturu na téma laparoskopických simulátorů, zhodnotit jejich přínos při edukaci chirurgů a identifikovat nejefektivnější typ simulace. Metody: K vyhledávání relevantních publikací jsme použili databáze PubMed, Web of Science, Cochrane Library. Při vyhledávání byla použita hesla „laparoscopy, simulator, surgery, assessment“. Vyhledávání bylo omezeno na prospektivní studie publikované v posledních 5 letech v anglickém jazyce. Výsledky: Z celkového počtu nalezených 354 studií bylo do přehledu zařazeno 26, které odpovídaly naším kritériím. Devět studií porovnávalo jednotlivé simulátory mezi sebou. Pět studií vyhodnotilo „high a low fidelity“ (virtuální a box simulátory) jako stejně účinné (EBM 2a). Ve třech případech byl shledán efektivnější „low fidelity“ krabicový simulátor (EBM 2a−3b). Pouze jedna studie preferovala virtuální simulátor (dále jen VR) (EBM 2b). Třináct studií hodnotilo přínos simulátorů do praxe. Dvanáct shledalo trénink na simulátoru jako efektivní způsob přípravy (EBM 1b−3b). Jedna studie naproti tomu nenašla rozdíl mezi tréninkem na simulátoru a tradičním způsobem přípravy (EBM 3b). Devět studií hodnotilo přímo některou z metod hodnocení laparoskopických dovedností. Tři studie zhodnotily VR simulátor jako vhodný nástroj hodnocení. Další studie hodnotily jako úspěšný skórovací systém GOALS-GH. V jednom případě byl úspěšný model analýzy pohybu rukou. Většina studií byla observačních (EBM 3b), pouze 2 studie byly vyšší kvality (EBM 2b). Závěr: Simulátory jsou efektivním nástrojem k nácviku laparoskopických technik (EBM: 1b). Z dostupných dat nelze jednoznačně určit, který ze simulátorů je nejefektivnější. Virtuální simulátor však představuje dosud nejsoběstačnější jednotku vhodnou k výuce i k hodnocení laparoskopických technik (EBM 2b−3b). Je potřeba dalších studií k nalezení účinného systému a parametrů k objektivnímu hodnocení dovedností., Introduction: Nowadays, laparoscopic operations are a common part of surgical practice. However, they have their own characteristics and require a specific method of preparation. Recently, simulation techniques have been increasingly used for the training of skills. The aim of this review is to provide a summary of available literature on the topic of laparoscopic simulators, to assess their contribution to the training of surgeons, and to identify the most effective type of simulation. Methods: PubMed database, Web of Science and Cochrane Library were used to search for relevant publications. The keywords “laparoscopy, simulator, surgery, assessment” were used in the search. The search was limited to prospective studies published in the last 5 years in the English language. Results: From a total of 354 studies found, we included in the survey 26 that matched our criteria. Nine studies compared individual simulators to one another. Five studies evaluated „high and low fidelity“ (a virtual box simulator) as equally effective (EBM 2a). In three cases the „low fidelity“ box simulator was found to be more efficient (EBM 2a−3b). Only one study preferred the virtual simulator (VR) (EBM2b). Thirteen studies evaluated the benefits of simulators for practice. Twelve found training on a simulator to be an effective method of preparation (EBM 1b−3b). In contrast, one study did not find any difference between the training simulator and traditional preparation (EBM 3b). Nine studies evaluated directly one of the methods of evaluating laparoscopic skills. Three studies evaluated VR simulator as a useful assessment tool. Other studies evaluated as successful the scoring system GOALS-GH. The hand motion analysis model was successful in one case. Most studies were observational (EBM 3b) and only 2 studies were of higher quality (EBM 2b). Conclusion: Simulators are an effective tool for practicing laparoscopic techniques (EBM: 1b). It cannot be determined based on available data which of the simulators is most effective. The virtual simulator, however, still remains the most self-sufficient unit suitable for teaching as well as evaluation of laparoscopic techniques (EBM 2b−3b). Laryngeal electromyography is used in the evaluation of vocal cord paralysis to confirm the diagnosis, to guide the diagnostic work-up for etiology, to provide prognostic information and to help choose the correct treatment for the patient. Cardiovocal syndrome is characterised by vocal cord paralysis due to a cardiovascular disease. A wide spectrum of conditions can result in this syndrome. Here we present a case of cardiovocal syndrome in association with primary pulmonary hypertension. Aspergillus sp. are ubiquitous mould infections and in most patients, the source is presumed to be air-borne infections during surgical procedures. Prevention of these infections requires special attention of ventilation systems in operating rooms. Post-operative aspergillosis occurs mainly in immunocompromised patients as well as those who receive corticosteroids temporarily. Capsular bag distension syndrome is a very rare complication of cataract surgery. Most cases occur after uneventful phacoemulsification with continuous curvilinear capsulorhexis and implantation of an intraocular lens in the capsular bag. The entity presents with reduction of visual acuity and myopic shift in the early-to-late postoperative period. Characteristic findings include the distension of the capsular bag due to the accumulation of milky material and the forward displacement of the intraocular lens. We present two cases with an unusually delayed presentation of 6 and 8 years respectively following phacoemulsification, and describe their successful management with Nd:YAG laser posterior capsulotomy. and P. Většina působí jako konvenční cytostatika modulující mikrotubuly (vinkristin, vinblastin, taxany), nebo inhibitory topoizomeráz jako např. deriváty epipodofylotoxinu a kamptotecinu (etopozid, irinotecan). Z rostlin či mořských organizmů jsou odvozeny také látky s účinkem na cílené nitrobuněčné struktury (flavopiridol, kombrestatin aj). Řada látek obsažených v ovoci a zelenině může interferovat z různými etapami kancerogeneze. LDL-aferéza je extrakorporální eliminační metodika, která usiluje o pokud možno specifické odstranění LDL-cholesterolu z cirkulující krve. V současnosti je používáno 6 účinných metod k selektivnímu odstranění LDL-cholesterolu. Mezi hlavní indikace k léčbě LDL-aferézou patří diagnózy homozygotní familiární hypercholesterolemie, heterozygotní familiární hypercholesterolemie refrakterní nebo při intoleranci k standardní léčbě, a dále pacienti se zvýšením lipoproteinu(a) refrakterní nebo intolerantní k farmakoterapii. Dosud však není jednoty o tom, jaká hladina LDL-cholesterolu je rozhodující pro zahájení LDL-aferézy, názory se liší i v jednotlivých státech. I když neexistují klasické velké randomizované studie, existuje dostatečný počet dobře řízených studií k tomu, aby v celém světě bylo uznáváno, že je efekt LDL-aferézy na kardiovaskulární prognózu u závažné hypercholesterolemie významný., LDL-apheresis is an extracorporeal elimination technique, which specifically removes LDL-cholesterol from the circulation. There are six methods for the selective LDL-cholesterol removal these days. The main indications for LDL-apheresis are the diagnosis of homozygous familial hypercholesterolemia, heterozygous familial hypercholesterolemia which is refractory the standard care and intolerance of routine care, and also patients with lipoprotein(a) increase resistant to the farmacotherapy. There is still debate which LDL-cholesterolemia is indication for LDL-apheresis therapy, and the recommendation differs among various countries.
