Úvod: Bolesti v pravém podbřišku jsou nejčastějším důvodem hospitalizace na chirurgických pracovištích obecně. Zároveň akutní apendicitida je nejčastěji se vyskytující náhlá příhoda břišní. Včasná indikace k apendektomii je jedinou metodou léčby, obvykle s nekomplikovaným průběhem a krátkou délkou hospitalizace. V rámci diferenciálně-diagnostické rozvahy je nutné pátrat i po dalších symptomech, zejména dlouhodobých, při akutní exacerbaci často ustupujících do pozadí. Kazuistika: Prezentujeme případ 38leté pacientky, která byla přijata pro akutní bolesti břicha v pravém podbřišku. Klinický i laboratorní nález nasvědčoval pro typicky probíhající akutní apendicitidu, byla proto indikována k apendektomii. Při operaci byl nalezen hemorhagický výpotek v pánvi, na apendixu makroskopicky netypický nález pro akutní zánět. Překvapivý byl nález ztluštění stěny terminálního ilea a zvětšení pravých adnex. Nález si vyžádal rozšíření operačního výkonu na ileocekální resekci a pravostrannou adnexektomii. Definitivní diagnózu stanovilo histologické vyšetření potvrzující endometriózu apendixu, terminálního ilea a pravého ovaria. Dlouhodobou dispenzarizací nemocné byla následně zjištěna prekanceróza, resp. mikroadenokarcinom hrdla děložního. Závěr: Stanovení diagnózy endometriózy apendixu předoperačně je prakticky nemožné. V rámci diferenciální diagnostiky lze na ni pomýšlet u fertilních žen s chronickými gynekologickými obtížemi neobjasněné etiologie. Diagnostickou, resp. léčebnou metodou volby je laparoskopie, která dovolí přehlédnutí celé peritoneální dutiny, včetně malé pánve, provedení apendektomie a eventuálně excizi podezřelých ložisek z endometriózy. Definitivní stanovení diagnózy je doménou histopatologického vyšetření. Při průkazu endometriózy apendixu nebo jakékoli jiné lokalizace je nezbytné předání pacientky do péče gynekologa., Introduction: In general, pain in the lower right abdomen is the most frequent reason for hospital surgical admissions, acute appendicitis representing the most common cause of operation for acute abdomen. Timely appendectomy remains the only treatment in the early stages of inflammation and is usually uncomplicated, requiring only a short hospital stay. A differential diagnostic analysis necessitates a search for other, particularly long-term symptoms that might be driven to the background in cases of acute exacerbation. Case report: The case report presents a 38-year-old female patient who was admitted for lower right abdominal pain. Clinical examination and a blood test both suggested typical acute uncomplicated appendicitis, and therefore the patient underwent appendectomy. Haemorrhagic peritoneal fluid and nodularity of the appendix not typical for appendicitis was found. Oedema of the terminal ileum and a right adnexal tumour were a surprising finding. A more extensive surgical procedure involving ileocaecal resection and right-side adnexectomy was finally performed with regard to the intraoperative finding. The definitive diagnosis of appendiceal endometriosis, endometrial mass in the terminal intestine and ovarian endometriosis was established by histological evaluation. Long-term follow-up revealed microadenocarcinoma of cervix uteri. Conclusion: It is generally very difficult to confirm appendiceal endometriosis before operation, and revealing primary appendiceal endometriosis is virtually impossible. It is advisable to consider endometriosis in fertile women with chronic abdominal pain of unclear aetiology and gynaecological symptoms in their personal history. The best diagnostic and therapeutic method, respectively, is laparoscopy enabling exploration of the entire peritoneal cavity including the minor pelvis, and performing appendectomy as well as excision of suspicious endometrial lesions. Extramedulárne (EM) postihnutie pri akútnej myeloblastovej leukémii (AML) je relatívne vzácne a vyskytuje sa najčastejšie v 2 formách – ako myeloidný sarkóm (MS) alebo leukemia cutis (LC). Doposiaľ nebol presvedčivo objasnený molekulový mechanizmus EM postihnutia, je však pravdepodobné, že podiel chemokínových receptorov, adhezívnych molekúl a ich vzájomných interakcií, ktoré kontrolujú migráciu leukemických buniek do periférnych tkanív, je kľúčový. Vznik izolovaného EM relapsu (IEMR) po alogénnej transplantácii krvotvorných kmeňových buniek (ASCT) predstavuje menej častý klinický prejav s nepriaznivou prognózou, najmä v kombinácii s relapsom systémovým. Je pravdepodobne dôsledkom nepomeru medzi GvL efektom v kostnej dreni (KD) a periférnych tkanivách. Jeho liečba nie je jednoznačne definovaná. Predložená kazuistika ukazuje rýchly EM relaps v oboch prsných žľazách u pacientky s akútnou leukémiou so zmiešaným fenotypom (MPAL) po ASCT. Bioptická verifikácia pod ultrazvukovou kontrolou potvrdila prítomnosť leukemickej infiltrácie. Cieľom práce je ukázať menej obvyklý prípad IEMR s dôrazom na komplexnú diagnostiku a veľmi dobrú liečebnú odpoveď po ukončení imunosupresie., Extramedullary (EM) manifestation of acute myeloid leukemia (AML) is relatively rare, it occurs frequently in 2 forms: myeloid sarcoma (MS) or leukemia cutis (LC). The molecular mechanism of EM involvement has not been clearly explained yet, but it is obvious that homing of leukemic cells to peripheral tissues is controlled by chemokine receptors, adhesion molecules and their interactions. The isolated EM relapse (IEMR) after allogeneic haematopoietic stem cell transplantation (ASCT) is an uncommon clinical manifestation with an unfavourable prognosis, especially in case of systemic relapse. It is probably the result of GvL imbalance in the bone marrow and peripheral tissues. The treatment of IEMR is not clearly defined. We present a case report of the rapid EM involvement in both breasts in patient with acute leukemia (mixed phenotype) (MPAL) after ASCT. Verification biopsy under ultrasound-guidance confirmed the presence of leukemic infiltration. Osteochondrom (exostóza) je nejčastějším benigním kostním tumorem. Ve většině případů jde o solitární lézi. Mnohočetná hereditární osteochondromatóza představuje vzácné autozomálně dominantně dědičné onemocnění charakterizované růstem vícečetných exostóz především na metafýzách dlouhých kostí, lopatách kosti kyčelní, žebrech, obratlích a na lopatkách. Cévní komplikace mnohočetné osteochondromatózy jsou velice vzácné, přičemž nejčastěji postihují popliteální tepnu. Autoři popisují případ 27leté pacientky s hereditární mnohočetnou osteochondromatózou, která byla komplikována femoropopliteální žilní trombózou a rozsáhlým pseudoaneuryzmatem popliteální tepny., Osteochondromas (exostoses) are the most common benign bone tumours. Vascular complications of multiple osteochondromatosis are very rare, most frequently affecting the popliteal artery. Prezentujeme případ 26leté mladé ženy s náhodně ultrasonograficky nalezeným tumorem pod levým lalokem štítné žlázy, jehož velikost během roku mírně zprogredovala; při aspirační cytologii bylo vysloveno podezření na nádor neuroektodermového původu a bylo indikováno jeho operační odstranění. Nádor byl hladce a šetrně odstraněn, přesto se v bezprostředním pooperačním období objevuje Hornerův syndrom. Histologicky byl verifikován ganglioneurom., We present the case of a 26-year-old female patient with coincidental ultrasound finding of a neck tumour located below the left thyroid lobe. The tumour slightly increased in size over a year. Fine needle aspiration indicated the neuroectodermal origin of the tumour and surgical removal was performed. Endometrióza znamená přítomnost tkáně endometria mimo dutinu děložní. Hlubokou infiltrující endometriózu (deep infiltrating endometriosis – DIE) představují rektovaginální léze, infiltrující onemocnění střev, močového měchýře, močovodu a DIE se vzácně objevuje i v dalších lokalizacích. Negynekologické komplikace DIE rozdělujeme do dvou hlavních kategorií: samotné onemocnění zahrnuje pánevní a břišní bolest, střevní striktury, opakující se krvácení z konečníku, dyschezii, chronickou anémii, dysurii, hematurii, stenózu ureterů, ztrátu ledvinných funkcí a komplikace léčby, ty jsou charakterizovány vznikem fistul, dysfunkcí močového měchýře a opakujícími se záněty urinárního traktu. DIE přináší riziko pro vznik malignit zasažených orgánů. DIE může být léčena konzervativně, nebo chirurgicky. Předkládaná práce je přehledem gastrointestinálních a urologických komplikací DIE a předkládá také kazuistiku pacientky centra pro léčbu endometriózy v nemocnici ve Znojmě (ČR)., Endometriosis is the presence of endometrial tissue outside the endometrial cavity. Deep infiltrating endometriosis (DIE) includes rectovaginal lesions as well as infiltrative disease to the bowel, urinary bladder, ureter and other rare locations. Non gynecological complications of DIE can be divided into two major categories: complications of the disease itself which include pelvic and abdominal pain, bowel stricture, cyclic rectal bleeding, dyschezia, chronic anemia, dysuria, hematuria, ureteral stenosis as well as loss of kidney function and complications of the treatment which include fistula formation, bladder void dysfunction and recurrent urinary tract infections. DIE has potential for malignant transformation in the affected organ. DIE may be managed conservatively or surgically. However definitive treatment is surgical. Hypertrofická kardiomyopatie (HCM) je nejčastější geneticky podmíněné srdeční onemocnění s prevalencí 1 : 500. Ve familiárních případech je dědičnost autozomálně dominantní s neúplnou penetrancí a různou expresivitou, může ovšem vznikat i de novo mutacemi. Nesmírná heterogenita projevů i prognózy HCM případ od případu značně komplikuje klinický management, a proto toto onemocnění stále často přináší mnoho otázek nejen pro lékaře prvního kontaktu, ale i pro kardiology. Stanovení rizika náhlé srdeční smrti u pacientů s HCM je integrální součástí klinického managementu a doporučení pro stratifikaci se neustále vyvíjí. V této kazuistice prezentujeme případ familiární HCM u dvou bratrů nesoucích stejnou mutaci s velmi rozdílným klinickým nálezem a průběhem., Hypertrophic cardiomyopathy (HCM) is the most common genetic cardiac disease with a prevalence of 1 : 500. In familial cases the inheritance is autosomal dominant with non‑complete penetrance and variable expression; however, it can also be caused by de novo mutations. The heterogeneity of both its presentation and prognosis from case to case largely complicates clinical management, and therefore the disease often represents a dilemma for primary care clinicians as well as cardiologists. An estimation of sudden cardiac death risk is an integral part of clinical management and the stratification guidelines are continuously developing. Heterotopická mezenteriálna osifikácia (HMO) je zriedkavé ochorenie podmieňujúce patologickú tvorbu kostného tkaniva v dutine brušnej po opakovaných operačných traumách. Autori prezentujú neobvyklý prípad vzniku mezenteriálnej osifikácie u 30ročného muža po operácii pre ileózny stav na podklade tenkočrevnej inkarcerácie. Po prepustení do domáceho liečenia musel byť znovu hospitalizovaný a reoperovaný. V priebehu 6 týždňov bol ešte 3krát reoperovaný pre tenkočrevný ileózny stav a výraznú produkciu z entero-kutánnej fistuly. Napriek intenzívnej parenterálnej hyperalimentácii sa stav pacienta nezlepšil, metabolický rozvrat sa stal pacientovi osudovým. Patofyziológia mechanizmu formovania kostného tkaniva v mäkkých tkanivách nie je známa. The authors present an unusual case of mesenteric ossification formation in a 30 year-old man after surgery due to status ileosus on the basis of intestinal incarceration. After being released into home treatment the patient had to be hospitalized again and reoperated. During the next six weeks, the patient was reoperated three more times due to intestinal status ileosus and significant leaking of the entero-cutaneous fistula. Despite intensive parenteral hyperalimentation the patient‘s condition did not improve and metabolic breakdown became fatal for the patient. Pathophysiology of the mechanism of bone tissue formation in soft tissues is unknown. Intraluminální (mechanický) ileus tenkého střeva může být způsoben také fytobezoárem. V tomto případě se jednalo o fytobe-zoár, který byl zapřený v rozšířeném Meckelově divertiklu. Nález na axiálních CT skenech byl poměrně málo zřetelný, neboť Meckelův divertikl splýval s ostatními distendovanými kličkami ilea. Až provedení kvalitních MPR rekonstrukcí příčinu obtíží zřetelně objasnilo., Intraluminal (mechanical) ileus of small intestine can be caused by phytobezoar. In this case there was phytobezoar clogged up in widened Meckel diverticulum. The finding was in axial CT scans rather less obvious, because Meckel diverticulum merged with other distended loops of ileum. Poúrazový hydrocefalus způsobený poruchami cirkulace mozkomíšního moku často komplikuje klinický průběh a proces léčby po kraniocerebrálním poranění. Hydrocefalus po poranění míchy je popisován pouze výjimečně. Sdělení popisuje dvě kazuistiky nemocných s kompletní lézí krční míchy, u kterých v poúrazovém období došlo k rozvoji hydrocefalu. Rozbor obou případů a přehled literárních dat potvrzují rozhodující úlohu extraspinálních faktorů pro rozvoj hydrocefalu po míšním poranění. I když k rozvoji hydrocefalu po míšním poranění dochází výjimečně, tato možnost by měla být zvážena při pozdním zhoršení klinického stavu nemocného po poranění krční míchy, zvláště při přítomnosti anatomických anomálií mokového prostoru a poúrazovém subarachnoidálním krvácení., Posttraumatic hydrocephalus caused by cerebrospinal fluid circulation disturbances frequently complicates the clinical course and treatment after craniocerebral injury. Hydrocephalus complicating spinal cord injury is only exceptionally reported. The paper presents two cases of complete cervical spinal cord injury with subsequent development of hydrocephalus. The analysis of both cases and literature data confirmed the dominant role of non-spinal factors in the development of hydrocephalus after spinal cord injury. Despite the exceptional occurrence of hydrocephalus after spinal cord injury, this diagnosis should be considered in cases of delayed deterioration of a patient with cervical spinal cord injury, particularly if cerebrospinal fluid space abnormalities and posttraumatic subarachnoid haemorrhage are present., and J.
