Cíl: Přezkoumání benefitů metody klokánkování pro nedonošené novorozence, to znamená najít dostatek důkazů pro opodstatnění používání metody klokánkování na Jednotce intenzivní péče pro novorozence. Metodika: Vyhledávání validních indikátorů pozitivního vlivu metody klokánkování prostřednictvím metody praxe založené na důkazech. Pro získání relevantních zdrojů byly využity licencované databáze a volně přístupné databáze - Medline, Bibliomedica, Nursing: Best Evidence for Nursing care, Google Scholar. Výsledky: Analýza výsledků prokázala jednoznačně pozitivní vliv metody klokánkování na nedonošené novorozence umístěné na Jednotce intenzivní péče, a to zejména v oblasti vnímání bolesti u invazivních výkonů, efektivního kojení, zvýšení váhového přírůstku, delšího a klidnějšího spánku, pozitivnější interakce matka – dítě, udržení nebo zvýšení tělesné teploty, zkrácení doby hospitalizace, růstu do délky, růstu obvodu hlavičky a aktivace centrální nervové soustavy. Závěry: Výzkumné studie (n = 34) a jeden systematický přehled ukazují pozitivní vliv metody klokánkování na nedonošeného novorozence umístěného na Jednotce intenzivní péče v oblastech biopsychosociálních. V podmínkách českého zdravotnictví chybí vypracované platné standardy pro využívání metody v praxi., Objective: To review benefits of kangaroo care method for premature babies, it means finding enough evidence to justify using the kangaroo care method in the Intensive care unit for newborns. Methods: Searching for valid indicators of a positive influence method of kangaroo care through method evidence-based practice. To obtain relevant sources were used licensed databases and freely available databases - Medline, Bibliomedica, Nursing: Best Evidence for Nursing care, Google Scholar. Results: Analysis of results showed a clearly positive impact of the kangaroo care methods on premature infants placed in intensive care, especially in the perception of pain in invasive procedures, effective breastfeeding, increased weight gain, longer and quieter sleep, more positive interactions of mother - child, maintaining or increasing body temperature, reduced hospitalization, growth in length, head circumference growth and activation of the CNS. Conclusions: The research studies (n = 34) and one systematic review have shown a positive influence of the kangaroo care methods on premature newborns placed in intensive care in the areas of bio-psycho-social. Cieľ: Cieľom štúdie bolo posúdiť bolesť, funkčný stav, úzkosť, depresiu, sociálne aktivity a sociálnu oporu u seniorov s reumatoidnou artritídou (RA) a ich vzájomné vzťahy, posúdiť možné rozdiely vzhľadom na pohlavie, rodinný stav, vek, trvanie ochorenia a komorbiditu. Metódy: U 60 seniorov s RA bola posudzovaná intenzita bolesti vizuálnou analógovou škálou (VAS), funkčná disabilita Stanfordským dotazníkom hodnotiacim zdravie indexom disability (HAQ-DI), úzkosť Beckovým inventárom úzkosti (BAI), depresia Zungovým sebahodnotiacim dotazníkom depresie (SDS) a sociálna interakcia škálou sociálnych aktivít (SA) a sociálnej opory (SO) nástroja Škály merania vplyvu chorôb kĺbov 2 (AIMS2). Výsledky: U seniorov s RA bolo zistené pociťovanie priemerne silnej bolesti, stredná až ťažká funkčná disabilita, prežívanie stredne silnej úzkosti a depresie, obmedzovanie v spoločenských aktivitách. Vo vnímaní sociálnej opory bolo zistené najlepšie skóre. Medzi sledovanými premennými boli Pearsonovou koreláciou zistené väčšinou signifikantné (P ≤ 0,05) kladné slabé až silné vzťahy. Medzi pohlaviami neboli zaznamenané rozdiely. Horšie skóre (P ≤ 0,05) bolo zistené v oblasti prežívania úzkosti a vo vnímanej sociálnej opore u osamelo žijúcich seniorov oproti seniorom žijúcim v manželskom zväzku. Štatisticky významný pozitívny vzťah (P ≤ 0,05) sa potvrdil medzi vekom a prežívaním úzkosti, medzi trvaním ochorenia a depresiou, medzi komorbiditou a funkčným stavom a depresiou. Záver: Ako naznačujú výsledky, prioritou v starostlivosti o túto skupinu seniorov by nemalo byť iba zmierňovanie bolesti a zlepšovanie/udržiavanie funkčných schopností, ale aj redukcia úzkosti a depresie, a to o to viac, čím sú seniori starší, čím u nich trvá ochorenie dlhšie, s komorbiditou a tiež u osamelo žijúcich., Aim: The aim of this study was to assess pain, functional status, anxiety, depression, social activity, and social support and their interactions in older adults with rheumatoid arthritis (RA), and to assess possible differences by gender, marital status, age, duration of disease, and co-morbidity. Methods: Pain intensity was assessed by the Visual Analogue Scale (VAS), functional disability by the Stanford Health Assessment Questionnaire – Disability Index (HAQ-DI), anxiety by the Beck Anxiety Inventory (BAI), depression by the Zung Self-rating Depression Scale (SDS), social activity by the Arthritis Impact Measurement Scales 2 (AIMS2) with scale of Social Activity (SA), and social support (SO) by AIMS2 with scale Support from family and friend in 60 elderly with RA. Results: There were found average strong pain, moderate to severe functional disability, moderate anxiety and moderate-to-marked depression, and limitation in social activity in elderly with RA. The best score was found in scale of social support. Significant (p ≤ 0.05) mild to severe relations were confirmed between explored objectives by Pearson’s correlations. Between genders no significant differences were recorded. Worse score (p ≤ 0.05) was detected in anxiety and social support in elderly living alone compared to married elderly. Between age and anxiety, duration of disease and depression, co-morbidity and functional disability, and depression were confirmed significant (p ≤ 0.05) positive relationships. Comparative assessment of clinical efficiency of omeprazole and rabeprazole in treating gastroesophageal reflux disease (GERD). 65 patients with a verified diagnosis GERD were examined. Comparative clinical investigations of using of omeprazole and rabeprazole have revealed effectiveness of both drugs in the therapy of GERD. In order to study cardiac pathology in different subtypes of ischemic stroke we examined 165 stroke patients. The 1st group included 90 (54.5%) patients with hemodynamic stroke, the 2nd one - 75 (45.5%) patients with cardioembolic stroke. Control group consisted of 45 individuals without cerebrovascular diseases. Cardiac pathologies with the prevalence of various types of ischemic heart disease were observed in all groups. The parameters of left ventricular stroke volume and ejection fraction were lower in patients with hemodynamic stroke than in other groups, but were within normal limits. Potential sources of cardiogenic embolism were found in all groups: in the 1st group at 74.4%, in the 2nd one at 100%. Background and objective: Investigation of the effects of MnTnHex-2-PyP on some markers of inflammation and lipid peroxidation in an asthma mice model. Methods: The experiment was carried out on 24 female mice C57Bl/6, divided into four groups: group 1, controls; group 2, injected with ovalbumin (OVA); group 3, treated with MnTnHex-2-PyP and group 4, treated with OVA and MnTnHex-2-PyP. The animals from groups 1 and 3 were injected i.p. on days 0 and 14 with a 100 μl phosphate-buffered saline (PBS), and those from groups 2 and 4 were injected with a 100 μl ovalbumin solution, containing 20 μg OVA. On days 24, 25 and 26 the mice from groups 1 and 2 were inhaled with PBS for 30 min, and those from groups 2 and 4 were given a 1% ovalbumin solution. One hour before inhalation, and 12 hours later the animals from groups 1 and 2 were injected i.p. with 100 μl PBS, and those from groups 3 and 4 received a 100 μl MnTnHex-2-Pyp solution in PBS сontaining 0.05mg/kg. Results: Ovalbumin alone (group 2) increased the total cell number, total protein content, the levels of IL-4, IL-5 and 8-isoprostane in bronchoalveolar lavage. Elevations were observed in IgE level in serum, and the malone dialdehyde (MDA) content in the lung homogenate. These markers were decreased significantly in group 4 as compared to the OVA group. Cíl: Cílem studie bylo zjistit, jak rodinní pečovatelé pečující o blízkou osobu v České republice hodnotí definující charakteristiky ošetřovatelské diagnózy Přetížení pečovatele. Metodika: Na klinickou validizaci ošetřovatelské diagnózy Přetížení pečovatele byl použit „Na pacienta zaměřený Fehringův model klinické diagnostické validity“. Výzkum u rodinných pečovatelů probíhal v měsících srpen a září 2010 v Agentuře domácí péče Victoria na území města Šumperk. Do výzkumného souboru bylo vybráno 50 rodinných pečovatelů, kteří pečovali o své blízké v domácím prostředí déle než 2 roky. Souhlas se zařazením do výzkumu vyjádřilo 23 rodinných pečovatelů, co je 46 %. Výsledky: Rodinní příslušníci za hlavní definující charakteristiky Přetížení pečovatele označili celkem 8 charakteristik: obavy z možné institucionalizace péče příjemce; obavy, jak vyřešit péči o člena rodiny, když nebudu schopen tuto péči vykonávat; obavy o zdraví příjemce péče v budoucnosti; obavy související se schopností pečovatele poskytovat péči v budoucnosti; konflikt v rodině; nejistota ze změny vztahu s příjemcem péče; smutek ze změny vztahu s příjemcem péče; starost o členy rodiny. Za vedlejší bylo označeno 10 definujících charakteristik: obtíže v dokončování požadovaných úkolů, obtíže se sledováním příjemce péče v průběhu nemoci, obtíže ve vykonávání požadovaných úkolů, vysoký krevní tlak, kardiovaskulární onemocnění, somatizace, frustrace, depresivní nálada, porušený spánek, stres. Závěr: Pro skupinu vybraných pečovatelů nebyly všechny definující charakteristiky uvedeny v klasifikačním systému NANDA International významné., Aim: The aim of the study was to find out how the family caregivers caring for their close relatives in the Czech Republic evaluate defining characteristics of the nursing diagnosis Caregiver Role Strain. Methods: Fehring´s patient – focused clinical diagnostic validity model was used to clinical validation of the nursing diagnosis Caregiver Role Strain. The research was ongoing during the months August and September 2010 at Home Care Agency Victoria in region of city Šumperk. Fifty family caregivers, who take care for their family members more than 2 years, were chosen to the research sample. Twenty three caregivers (46 %) agreed with their including to the research. Results: Caregivers classified in total the following eight defining characteristics as major ones: apprehension about possible institutionalization of care receiver, apprehension about care receiver´s care if caregiver unable to provide care, apprehension about the future regarding care receiver´s health, apprehension about the future regarding caregiver´s ability to provide care, family conflict, uncertainty regarding changes relationship with care receiver, grief regarding changed relationship with care receiver, concerns about family members. Caregivers classified ten defining characteristics as minor ones: difficulty completing required tasks, difficulty watching care receiver go through the illness, difficulty performing required tasks, hypertension, cardiovascular disease, somatization, frustration, feeling depressed, disturbed sleep, stress. Cíl: Cílem výzkumu bylo zjistit kvalitu života seniorů s demencí pohledem rodinných příslušníků. Dále pak porovnat vnímání kvality života seniorů s demencí rodinnými příslušníky v závislosti na prostředí, ve kterém senior žije. Metodika: Byl proveden kvalitativní výzkum, metodou sběru dat byl nestandardizovaný rozhovor. Výzkumu se zúčastnilo 9 rodinných příslušníků pacientů s demencí. Na základě analýzy dat byly stanoveny 4 kategorie: 1. změny v psychické oblasti, 2. změny kognitivních funkcí, 3. změny v oblasti aktivit denního života a instrumentálních všedních činností a 4. celkové vnímání kvality života. Výsledky: Z pohledu rodinných příslušníků nejvíce zhoršuje kvalitu života seniorů s demencí ztráta paměti, která je vyřazuje z běžného života, dále ztráta schopnosti orientace v prostoru a čase a ztráta schopnosti vykonávat činnosti běžného denního života. Kvalitu života snižují také smyslové poruchy, které pacienty uvádějí do ještě větší sociální izolace a mohou v nich podporovat nárůst paranoidity. Rodinní příslušníci hodnotili kvalitu života svých blízkých v domácím prostředí výrazně lépe v porovnání s kvalitou života pacientů s demencí umístěných v domově pro seniory. V domácím prostředí měli senioři více podnětů, byli více aktivizováni a vtahováni do rodinného a společenského života. Po nástupu do institucionální péče došlo k zhoršení příznaků onemocnění a postupnému vyřazení nemocných z aktivizačních programů. Závěr: Důležitou součástí zvýšení kvality života pacientů s demencí je podpora rodinných pečovatelů, aby nemocní mohli zůstat co nejdéle ve svém přirozeném prostředí., Objective: The aim of the research was to determine the quality of life of seniors with dementia from the perspective of family members. Furthermore, to compare the perceptions of quality of life of seniors with dementia by family members depending on the environment in which the senior lives. Methodology: A qualitative research was performed, for the method of data collection was used non-standardized interview. Research was attended by nine family members of patients with dementia. Based on analysis of data there were determined four categories: 1. changes in the psychological field, 2. changes in cognitive function, 3. changes in the area of activities of daily living and instrumental activities of everyday life and 4. overall perception of quality of life. Results: From the perspective of family members most deteriorating quality of life of seniors with dementia is the memory loss, which excludes them from normal life, as well as loss of ability to orientation in space and time, and loss of ability to perform normal activities of daily life. The sensory disturbance also reduces the quality of life, which leads patients into a greater social isolation and this may promote the growth of the paranoia. Family members rated the quality of life of their loved ones at home as significantly better in comparison with the quality of life of patients with dementia placed in the home for the elderly. In a domestic environment have seniors more stimulations, are more activated, and are more drawn into the family and social life. After coming into institutional care occurs worsening of symptoms and the gradual removal of patients from activation programs. Cíl: Cílem práce bylo zjistit a následně porovnat kvalitu života žen s přirozeným otěhotněním s kvalitou života žen po umělém oplodnění. Kvalita života byla u těchto dvou skupin žen hodnocena během těhotenství dvakrát, a to ve 20. – 28. týdnu těhotenství a před termínem porodu. Metodika: Ke sběru empirických dat byl použit standardizovaný dotazník Světové zdravotnické organizace WHOQOL – BREF. Výzkumný vzorek tvořilo 160 těhotných žen, z nichž bylo 80 žen po spontánním otěhotnění a 80 žen po umělém oplodnění. Výzkum byl realizován ve Fakultní nemocnici v Ostravě, Gyncentru v Ostravě a Gyncentru v Opavě. Výsledky: Těhotné ženy po spontánním i po umělém oplodnění hodnotily kvalitu života v těhotenství jako dobrou při prvním (20. – 28. týden těhotenství) i při druhém hodnocení (před termínem porodu). Spokojenost se zdravím hodnotily oba soubory žen při prvním i druhém hodnocení v průměru jako dobrou. Největší rozdíl v hodnocení byl v první doméně posuzující fyzické zdraví. Ve 20. – 28. týdnu těhotenství hodnotily ženy po spontánním otěhotnění své fyzické zdraví lépe než před termínem porodu (p < 0,001). Naproti tomu u žen po umělém oplodnění nedošlo k tak významnému rozdílu mezi prvním a druhým hodnocením. Při hodnocení dalších tří domén těhotnými ženami ať už po spontánním nebo umělém oplodnění nedošlo k výrazným rozdílům v hodnocení. Závěr: Kvalita života přímo nezávisí na způsobu otěhotnění, ale je velmi individuální, stejně tak i výskyt nepříjemných pocitů provázejících těhotenství., Objective: The aim of the work was to find out and consequently compare the quality of life of women who got pregnant naturally with those who underwent artificial insemination. Quality of life was evaluated twice during the pregnancy, between the 20th and 28th week and just before actual childbirth. Methods: A standardized questionnaire WHOQOL – BREF was used for empirical data collection. The sample was made up of 160 pregnant women divided into two groups: 80 women who had got pregnant naturally and 80 women who underwent artificial insemination. The research was realized at the Faculty hospital Ostrava, Gyncentre Ostrava and Gyncentre Opava. Results: Both groups confirmed quality of life as good during the first and the second evaluation and satisfaction with one's health was on average good as well. The biggest difference was related to physical health. Between the 20th and 28th week of pregnancy women who got pregnant naturally determined their physical health as better than before actual childbirth (p < 0,001). However, not so for women who were artificially inseminated, where differences were not so significant. In the other areas of the study no significant differences were found between either groups. Cíl: Cílem sledování bylo zhodnotit efektivitu zavedených nefarmakologických aktivit na podporu a zachování kognitivních funkcí na ženském gerontopsychiatrickém oddělení psychiatrické léčebny v Kroměříži a zhodnotit spokojenost hospitalizovaných klientek s těmito aktivitami. Metodika: Na začátku a ke konci hospitalizace bylo použito škály Mini-Mental State Examination (MMSE) a testu kreslení hodin (CDT) k orientačnímu zhodnocení kognitivních funkcí a na závěr hospitalizace bylo použito řízeného rozhovoru. Výsledky: Ke konci hospitalizace nebylo prokázáno statisticky signifikantního zlepšení ve stavu kognitivních funkcí (krátké období pozorování je nejpravděpodobnějším důvodem). Klientky však hodnotí psychoterapeutické aktivity k ovlivnění kognitivních funkcí kladně. Závěr: Nefarmakologické postupy mají významné místo v léčbě demence a doplňují farmakologické i podpůrné léčebné postupy., Aim: Aim of the study was to evaluate the effectiveness of non-pharmacological approaches influencing cognitive functions that are used in routine practice in inpatient women psycho-geriatric ward in Mental hospital in Kromeriz. Furthermore the study tried to evaluate satisfaction with these activities in hospitalised clients. Methods: At the beginning and at the end of hospitalisation was used range of Mini-Mental State Examination (MMSE) and Clock Drawing Test (CDT) to make an indicative assessment of cognitive functions, and at the end of hospitalization was used a controlled interview. Results: At the end of hospitalisation there was no significant evidence of improving cognitive functions (short period of research is the most probable reason). Nevertheless, psychotherapeutic activities were evaluated by clients positively. According to our research, Uzbek individuals with Parkinson’s disease have high prevalence of 0/0 polymorphisms of GSTT1 and GSTM1 genes, as well as combinations of GSTT1 (0/0)/GSTM1/(0/0) genotypes. These mutations are associated with an earlier debut of the disease, its mixed form, and rapid rate of progression.
