The significance of ABO blood system groups antigens in development of some malignant tumors is already established. The alteration of hormonal homeostasis must also be taken into account. Hence the aim of the investigation was to study ABO and Rh blood system antigens and hormonal status among reproductive age women with benign and malignant breast tumors. Methods: The determination of hormones was made by the enzymatic analysis method (ELAIZA), provided by proper ELAIZA kits. For the study of ABO and RhHr system antigens, internationally recognized immunoserology methods were used. Results: High index of the breast gland tumors were revealed in patients with A(II) phenotypic group, according to the ABO system. The frequency distribution of O(I) phenotypic group was low among women with breast tumors. Among D, C, E, c and e antigens of the Rh system, the frequency of D and E antigens were increased in benign and malignant breast tumors patients. The study of hormonal balance revealed thyroid gland hypofunction and increased level of estradiol on the background of increased testosterone and decreased progesterone levels. Such hormonal imbalance and excess production of estradiol creates conditions for malignant tumor formation in reproductive age women. Conclusion: The highest frequency of breast cancer in reproductive age was revealed in A(II) group patients. Cíl: Cílem výzkumu bylo popsat zkušenosti hospitalizovaných dětí (adolescentů), doprovodu hospitalizovaných dětí předškolního a mladšího školního věku a všeobecné sestry s etickými problémy při hospitalizaci dítěte na standardním dětském lůžkovém oddělení. Metodika: Byl proveden kvalitativní výzkum. Pro sběr dat byl použit nestandardizovaný rozhovor. Výzkumný soubor tvořilo osm informátorů, osoby doprovázející dítě při hospitalizaci (n = 5), hospitalizované děti v adolescentním věku (n = 2) a dětská sestra (n = 1). Na základě kvalitativní analýzy dat byly vytvořeny 4 kategorie (1. informovaný souhlas, 2. přítomnost blízkého člověka, 3. srozumitelné informace a 4. péče sester), které byly dále členěny do podkategorií. Výsledky: Mezi nejčastější etické problémy při hospitalizaci dítěte na standardním lůžkovém oddělení respondenti uvedli: problémy s informovaným souhlasem v neúplných rodinách, nedostatečnou informovanost o nemoci a zdraví dítěte, deficit v oblasti edukace, nedostatek komunikace se sestrou a nedostatečnou péči. Adolescenti považovali za etický problém při hospitalizaci pocit malého respektování vlastní autonomie u rozhodování o sobě sama, potřebu většího soukromí a respektu k pocitům studu. Závěr: Většině etických problémů lze předcházet zvládnutím efektivní komunikace a dostatečné informovanosti a to již před hospitalizací dítěte v ambulanci praktického lékaře pro děti a dorost, případně v odborné ambulanci., Objective: The aim of the research was to describe the experience of hospitalized children (adolescents), accompaniment of hospitalized children preschool and younger school age and general nurses with ethical problems during the hospitalization of children on the standard children's ward. Methods: Qualitative research was conducted. For data collection was used nonstandardized interview. The research group consisted of eight informants, persons accompanying the child during hospitalization (n = 5), hospitalized children in adolescence age (n = 2) and child nurse (n = 1). Based on qualitative data analysis were created four categories (1. informed consent, 2. the presence of a loved one, 3. understandable information 4. care of nurses ), which were further divided into subcategories. Results: The most common ethical problems in the child's hospitalization on the standard ward respondents were stated: problems with informed consent in single-parent families, lack of information about the child's disease and child's health, the deficit in the area of education, lack of communication with the nurse and lack of care. Adolescents considered as the ethical problem during hospitalization the feeling of little respect for autonomy in deciding of themselves, the need for greater privacy and respect for the feelings of shame. Východiskem článku (s názvem „Etika v lékařském výzkumu za účasti lidských subjektů z pohledu lékařského a filozofického“) je poukázat na vybrané aspekty, které se napříč vztahují k etice v lékařském výzkumu. Jsou to: Hippokratova přísaha jako deontologický kodex, definice lege artis, bioetika a strukturování vztahu lékaře a pacienta. Zdravotnická (lékařská) etika je považována za normativní etiku. Její normativnost vychází z pevných zásad a instrukcí, které jsou nerozlučně spojeny s aplikací vnitřní mravnosti v oblasti medicíny. Normativní etika se neustále snaží nalézat vhodné odpovědi na otázky – Co je dobré? Co je zlé? Jaké jsou mravní ideály a hodnoty všeho správného a humánního? Jaké normy jsou úzce spojeny s pocity povinnosti, odpovědnosti, správnosti, nesprávnosti, náklonnosti, úcty, lítosti…? Obecně se dá říci, že celá etika (nejenom zdravotnická/lékařská etika) stojí a padá na zásadě, že je potřeba ve vztahu k druhému člověku konat jen to, co je dobré (optimální, žádoucí), a zdržet se toho, co je zlé (neoptimální, nežádoucí). Etika lékaře, ale i dalších zdravotnických pracovníků, vychází ze vztahu „Já“ a ten „druhý.“ Jedná se o vztah mezi člověkem pomáhajícím a člověkem trpícím. Lékař o nemocného člověka pečuje, snaží se mu nabídnout (pokud je to možné) co nejlepší řešení jeho situace. Velkou pozornost zasluhuje i lékařův morální závazek jednat s pacientem jako s člověkem, který se ocitl v obtížné životní situaci. Kromě toho lékaře morálně zavazuje i to, že musí plně respektovat zásady, které odpovídají povaze pacientova onemocnění. Uvědomění si nemoci bývá u pacientů dosti velkým problémem. Cílem článku je zvýraznění křehkosti a komplikovanosti vztahu mezi lékařem a pacientem. Lékař a pacient jsou hlavními subjekty, které jsou středem pozornosti zdravotnické (lékařské) etiky. Pacient jako subjekt lékařského výzkumu vždy a za všech okolností rozvrhuje sám sebe. Lékař je sice rozhodujícím činitelem v ekvivalenčním vztahu, působí na nemocného, snaží se nemocnému vysvětlit závažnost jeho onemocnění, pokouší se nemocného přimět k samovolnému plnění základních léčebných pravidel, ale je to především pacient, který primárně rozhoduje o svém zdraví. Lékaři mají jedno obrovské privilegium – a to takové, že se snaží pomáhat lidem. Pacienti by měli lékařům důvěřovat a měli by se na ně jako na odborníky obracet ve svých zdravotních nesnázích., A solution of the article (called „Ethics in the medical research with a present of human subjects from the point of view of medical and philosophical“) is to show some chosen aspects which relate crosswise to ethics in medical search. They are: Hippocratus oath as a deontological codex, a definition lege artis, bioethics and forming a structure of a relation between the doctor and the patient. Health (medical) ethics is considered for a normative ethics. Its normativing comes out from settled rules and instructions which are inseparably connected with an application of inside morality in the area of medicine. Normative ethics tries to find adequate answers for questions – What is good? What´s bad? What are moral ideals and values of everything. What is right and human? What norms are closely connected with some feelings of duty, responsibility, accurancy, incorrectness, inclination, esteem, regret…? Generaly speaking, the whole ethics (not only health/medical ethics) stands and falls down on the principle, that there is a great need in the connection to other person to do only what is good (optimal, necessary) and delay what is bad (unoptimal, unnecessary). The doctor´s ethics, but ethics of other health service assistants, comes out from a relationship „Me“ and „the other.“ It is so called relationship of a helping man and a suffearing one. The doctor takes care of an ill person, tries to ofer (if it is possible) the best solution of his/her situation. A great attention deserves the doctor´s moral obligation, to act with a patient as a man who is in a difficult life situation. Except of this, the doctor is obliged to respect principles that correspond patient´s health disease. Realization of the disease is quite large problem of the patients. The emphasis of delicacy and complexity of doctor´s and patient´s respects are the aim of the article. The doctor and the patient are the main subjects who stand in the centre of view of health (medical) ethics. The patient, as a subject of a medical search, always and at any circumstances distributes himself or herself. The doctor is admittedly determining element of his/her disease, tries to make an ill patient to self – filling of the basic cure rules, but first of all it is the patient who primarily decides about his/her health. Rachels je autor zastávající novodobou koncepci utilitaristické morálky. Ve své publikaci The End of Life velmi podrobně probírá všechny důležité momenty filosofické a etické diskuse o eutanazii a vytyčuje tak kontury území, na němž se jak zastánci jeho závěrů, tak i jejich odpůrci, musí pohybovat. Hovoří o posvátnosti lidského života, rozebírá různé formy eutanazie, kriticky hodnotí některé důležité distinkce, široce a někdy nekriticky užívané v lékařské etice: rozdíl mezi usmrcením (killing) a ponecháním zemřít (letting die), mezi řádnými a mimořádnými léčebnými prostředky, mezi aktivní a pasivní eutanazií. Důležitým prvkem Rachelsovy reflexe je i diskuse o důležitosti úmyslu (intence) v etickém vyhodnocování lidských skutků., This contribution is the second of a series of articles devoted to the ethical evaluation of euthanasia. It discusses the problems of euthanasia on the background of Rachel´s famous book The End of Life. Rachels as an author is holding a modern concept of utilitarian morality. In his publication The End of Life he discusses in detail all the important moments of philosophical and ethical debate about euthanasia. In this way he also outlines the contours of the territory for his proponents as well as their opponents for recent discussion on euthanasia and end of life decisions. He talks about the problems of conception of the sanctity of human life, discusses the various forms of euthanasia, critically evaluates some important distinctions between terminology in medical ethics: the difference between killing (killing) and allowing to die (letting die), between ordinary and extraordinary means of treatment, the active and passive euthanasia. Pojednává o preferenčním utilitarismu Petera Singera a ukazuje, jakým způsobem se v rámci tohoto etického systému euthanasie ukazuje jako eticky oprávněná volba. Článek je rozdělený do dvou hlavních částí. V první části jsou předloženy základní principy utilitarismu a uvádí se důležité rozlišení mezi lidskými živočichy a osobami (založené na schopnosti vyšších kognitivních funkcí). Druhá hlavní část článku se podrobně věnuje vyjasnění základních forem euthanasie (vyžádaná, nevyžádaná, nedobrovolná) a argumentuje, že mezi pasivní a aktivní euthanasií neexistuje žádný morálně významný rozdíl. Článek končí konstatováním, že aktivní euthanasie je morálně lepší volbou než pasivní euthanasie., This contribution is the first of a series of articles devoted to the ethical evaluation of euthanasia. It discusses the preference utilitarianism of Peter Singer and shows that euthanasia can be a morally permissible choice. The article is divided into two main parts. In the first part the basic tenets of utilitarianism are set forth, the important distinction between human animals and persons is made (based on the capacity for higher cognitive functions). Hodnocení a sledování průběhu veřejně politických opatření (včetně politiky podpory zdraví) je, zejména v rámci diskurzu na poli veřejné politiky jako vědní disciplíny, chápáno jako nedílná součást tvorby a realizace veřejné politiky, která by měla přispívat k naplňování demokratických principů ve veřejné správě. Vlastní evaluační praxe, a tedy i konečný smysl hodnocení, se však může značně lišit od odborných akademických metodologických doporučení a deklarací v rámci strategických veřejně politických dokumentů. Cílem předkládaného článku je na základě systematické rešerše vytvořit teoretický rámec pro exploraci a porozumění evaluační praxi v politice podpory zdraví na národní úrovni (včetně podmínek, které na ni mohou působit) a představit zjištění některých zahraničních autorů, kteří zkoumali jednotlivé případy evaluační praxe., Public policy (including health promotion policy) evaluation and monitoring is considered, especially in the discourse on the ground of public policy as an academic discipline, to be an integral part of public policy creation and implementation, which should contribute to promotion of democratic principles in public administration. The particular evaluation practice and hence the very meaning of evaluation, may differ greatly from academic methodological recommendations and declarations included in strategic public policy documents. Motivation Until recently it was considered that 65 years is cutoff for defining patients as elderly, but newer reports indicate that this age limit shift to 70 years of age. Elderly patients with advanced non small cell lung cancer, associated comorbidities and poor performance status represent a specific population and a challenge for use of chemotherapy. Primary aim was to evaluate the impact of mono therapy with oral etoposide on overall survival in elderly patients (≥ 70 years of age) with advanced non small cell lung cancer and poor performance status (PS) ≥ 2 (clinical stage IIIb and IV ), and as well to evaluate tolerability of this therapy. Secondary aim was to evaluate response rate. Methods Retrospectively, medical records of 79 female and male patients with advanced non small cell lung cancer and poor performance status treated with oral etoposide (2x25 mg 20 days/10 days pause) in period from 2007 till 2010 were checked for relevant data. Data regarding demographics, performance status, overall survival, response rates and drug toxicity were collected. For statistical analysis we used Pearson chi-square test, T-test, Kaplan-Meier product limited method and Cox regression. Results Median overall survival (OS) was 31 weeks, in patients with PS 2 overall survival was 34 weeks, and in group with PS 3 was only 24 weeks. Partial response was registered in 20.2% of patients, stable disease in 41.85 % and disease progression in 38% of patients. Treatment was well tolerated, febrile neutropenia and toxic deaths were not registered. Toxic effects didn't have statistically significant influence on OS. Cílem mé práce je zamyslet se nad existencialismem a postmoderním přístupem z pohledu vzájemných podobností i rozdílů. Existenciální analýza stojí podle mého názoru mezi modernou a postmodernou, protože na jedné straně vychází z poznávání objektivního světa (moderna), ale na druhou stranu k ní přidává požadavek subjektivního prožívání (postmoderna). Došel jsem k následujícím podobným předpokladům existenciálního a postmoderního přístupu: obrat k subjektivnímu prožívání, nereduktivní přístup k životní zkušenosti, schopnost svobodné volby, princip osobní zodpovědnosti, zdůrazňování vnitřních zdrojů klienta a orientace na budoucnost, širší kontext mezilidských vztahů či dané kultury, neexistence jediné pravdy, důraz na plynutí času, obrat k jazyku a přizpůsobení se jazyku klienta. Naproti tomu se oba směry vyznačují rozdíly v teoriích poznání, v různém zaměření na smysl a cíl života, v jiném pojetí emocí a z toho vycházejícího uchopování nepříjemných prožitků. Rozdílný pohled na některá témata vnímám jako podněcující, protože člověku skýtá příležitost přemýšlet o věcech novým, obohacujícím způsobem., The purpose of my work is to think of the existentialism and the postmodern approach from the view of their mutual similarities and differences. In my opinion, the existential analysis stands between the modernism and the postmodernism, because on the one hand it is based on the cognition of the objective world (the modernism) and on the other hand, there is also the requirement of subjective experience there (the postmodernism). I came to the following similar premises of the existential and postmodern approach: the return to subjective experience, non-reductive attitude to the life experience, the ability of free choice, the principle of personal responsibility, the accent on inner sources of the client and the focus on the future, broader context of the interpersonal relationships or the given culture, the absence of the only truth, the accent on the passage of time, the return to the language and the accommodation to the language of the client. On the contrary, both of the directions differ in the theories of cognition, in different focus on the meaning and goal of life, in different conception of emotions and consequent grasping of unpleasant experiences. Backround: Worldwide, approximately 1 billion people, from which 200250 million are women, smoke. Based on the Estonian Medical Birth Register data, in 2010, 7,5% of pregnant women were smokers, and in 2012, 7% of pregnant women were smoking during their pregnancy. Thus, smoking rates among pregnant women fell only 0,5%. The objective of the research was to find out the views of pregnant women who were smoking during pregnancy of the effect of smoking on the health of them and their babies, and experiences in quitting smoking, changing indicators, e.g. smoking and socioeconomical status (initial exposure to smoking, employment, longterm partnerships etc.) in 20092013. Methods: This research is a phenomenological study, which was being carried out from October 2009 until January 2013. Data collection methods were semistructured interviews with 45 pregnant women smoking during their pregnancy and being registered for antenatal care in three health care institutions of Republic of Estonia. An inductive approach for qualitative analysis was used. Results: Most women smoking during pregnancy started smoking in their teens, their parents smoked, they were under 30 years old housewives or unemployed. On an average of 1120 cigarettes were consumed each day, the pregnancy of the research group did not influence frequency and tobacco intake. The participants in the research group reported that tobacco consumption helped them relax, gave them a chance to spend time in a good company, and it was not regarded as a bad habit but a social addiction, being caused by availability of tobacco products. Although most of them had an opinion about harmful effects of smoking, e.g. a cough, asthma and rapid fatigue, on the health of them and less on their expected babies, they continued smoking. Conclusions: The smoking and socioeconomical status of women smoking during pregnancy in 20092013 has not changed, health awareness of them is low and has not improved. Support groups with close relatives are needed to motivate cessation. In the first place, prevention needs a good example, purposeful cooperation, which starts in daily life context at homes, in educational institutions and in the community. Limiting the availability of tobacco products, postponing and preventing an initial exposure to smoking are thought to be of great importance. Introduction: Oral Lichen planus (OLP) is chronic inflammatory oral mucosal disease of unknown etiology. Basement membrane damage and T‑cell migration in OLP may be mediated by matrix metalloproteinases (MMPs). We examined the expression of matrix metalloproteinase 9 to support this hypothesis. Materials and methods: The study population consisted of 71 patients with OLP and 10 control patients with oral fibromas. Indirect immunohistochemistry was used for detection of MMP 9 expression (polyclonal rabbit anti‑human MMP antibody). Results: In all cases of OLP, the MMP‑9 expression was seen mainly in the area of lymphocytic inflammatory infiltrate in the lamina propria including lymphocytes within the overlying epithelium. In addition, it was observed in the epithelial keratinocytes, particularly in the stratum basale and stratum spinosum with occasional positivity in the superficial layer. Fibroblasts and endothelium of small vessels in the lamina propria showed MMP9 expression as well. In all cases of oral mucosal fibromas, the MMP‑9 expression was seen only in fibroblasts and in endothelium of small vessels with occasional positivity within the overlying epithelium.
