Comparative assessment of clinical efficiency of omeprazole and rabeprazole in treating gastroesophageal reflux disease (GERD). 65 patients with a verified diagnosis GERD were examined. Comparative clinical investigations of using of omeprazole and rabeprazole have revealed effectiveness of both drugs in the therapy of GERD. In order to study cardiac pathology in different subtypes of ischemic stroke we examined 165 stroke patients. The 1st group included 90 (54.5%) patients with hemodynamic stroke, the 2nd one - 75 (45.5%) patients with cardioembolic stroke. Control group consisted of 45 individuals without cerebrovascular diseases. Cardiac pathologies with the prevalence of various types of ischemic heart disease were observed in all groups. The parameters of left ventricular stroke volume and ejection fraction were lower in patients with hemodynamic stroke than in other groups, but were within normal limits. Potential sources of cardiogenic embolism were found in all groups: in the 1st group at 74.4%, in the 2nd one at 100%. Cíl: Cílem studie bylo zjistit, jak rodinní pečovatelé pečující o blízkou osobu v České republice hodnotí definující charakteristiky ošetřovatelské diagnózy Přetížení pečovatele. Metodika: Na klinickou validizaci ošetřovatelské diagnózy Přetížení pečovatele byl použit „Na pacienta zaměřený Fehringův model klinické diagnostické validity“. Výzkum u rodinných pečovatelů probíhal v měsících srpen a září 2010 v Agentuře domácí péče Victoria na území města Šumperk. Do výzkumného souboru bylo vybráno 50 rodinných pečovatelů, kteří pečovali o své blízké v domácím prostředí déle než 2 roky. Souhlas se zařazením do výzkumu vyjádřilo 23 rodinných pečovatelů, co je 46 %. Výsledky: Rodinní příslušníci za hlavní definující charakteristiky Přetížení pečovatele označili celkem 8 charakteristik: obavy z možné institucionalizace péče příjemce; obavy, jak vyřešit péči o člena rodiny, když nebudu schopen tuto péči vykonávat; obavy o zdraví příjemce péče v budoucnosti; obavy související se schopností pečovatele poskytovat péči v budoucnosti; konflikt v rodině; nejistota ze změny vztahu s příjemcem péče; smutek ze změny vztahu s příjemcem péče; starost o členy rodiny. Za vedlejší bylo označeno 10 definujících charakteristik: obtíže v dokončování požadovaných úkolů, obtíže se sledováním příjemce péče v průběhu nemoci, obtíže ve vykonávání požadovaných úkolů, vysoký krevní tlak, kardiovaskulární onemocnění, somatizace, frustrace, depresivní nálada, porušený spánek, stres. Závěr: Pro skupinu vybraných pečovatelů nebyly všechny definující charakteristiky uvedeny v klasifikačním systému NANDA International významné., Aim: The aim of the study was to find out how the family caregivers caring for their close relatives in the Czech Republic evaluate defining characteristics of the nursing diagnosis Caregiver Role Strain. Methods: Fehring´s patient – focused clinical diagnostic validity model was used to clinical validation of the nursing diagnosis Caregiver Role Strain. The research was ongoing during the months August and September 2010 at Home Care Agency Victoria in region of city Šumperk. Fifty family caregivers, who take care for their family members more than 2 years, were chosen to the research sample. Twenty three caregivers (46 %) agreed with their including to the research. Results: Caregivers classified in total the following eight defining characteristics as major ones: apprehension about possible institutionalization of care receiver, apprehension about care receiver´s care if caregiver unable to provide care, apprehension about the future regarding care receiver´s health, apprehension about the future regarding caregiver´s ability to provide care, family conflict, uncertainty regarding changes relationship with care receiver, grief regarding changed relationship with care receiver, concerns about family members. Caregivers classified ten defining characteristics as minor ones: difficulty completing required tasks, difficulty watching care receiver go through the illness, difficulty performing required tasks, hypertension, cardiovascular disease, somatization, frustration, feeling depressed, disturbed sleep, stress. Cíl: Cílem výzkumu bylo zjistit kvalitu života seniorů s demencí pohledem rodinných příslušníků. Dále pak porovnat vnímání kvality života seniorů s demencí rodinnými příslušníky v závislosti na prostředí, ve kterém senior žije. Metodika: Byl proveden kvalitativní výzkum, metodou sběru dat byl nestandardizovaný rozhovor. Výzkumu se zúčastnilo 9 rodinných příslušníků pacientů s demencí. Na základě analýzy dat byly stanoveny 4 kategorie: 1. změny v psychické oblasti, 2. změny kognitivních funkcí, 3. změny v oblasti aktivit denního života a instrumentálních všedních činností a 4. celkové vnímání kvality života. Výsledky: Z pohledu rodinných příslušníků nejvíce zhoršuje kvalitu života seniorů s demencí ztráta paměti, která je vyřazuje z běžného života, dále ztráta schopnosti orientace v prostoru a čase a ztráta schopnosti vykonávat činnosti běžného denního života. Kvalitu života snižují také smyslové poruchy, které pacienty uvádějí do ještě větší sociální izolace a mohou v nich podporovat nárůst paranoidity. Rodinní příslušníci hodnotili kvalitu života svých blízkých v domácím prostředí výrazně lépe v porovnání s kvalitou života pacientů s demencí umístěných v domově pro seniory. V domácím prostředí měli senioři více podnětů, byli více aktivizováni a vtahováni do rodinného a společenského života. Po nástupu do institucionální péče došlo k zhoršení příznaků onemocnění a postupnému vyřazení nemocných z aktivizačních programů. Závěr: Důležitou součástí zvýšení kvality života pacientů s demencí je podpora rodinných pečovatelů, aby nemocní mohli zůstat co nejdéle ve svém přirozeném prostředí., Objective: The aim of the research was to determine the quality of life of seniors with dementia from the perspective of family members. Furthermore, to compare the perceptions of quality of life of seniors with dementia by family members depending on the environment in which the senior lives. Methodology: A qualitative research was performed, for the method of data collection was used non-standardized interview. Research was attended by nine family members of patients with dementia. Based on analysis of data there were determined four categories: 1. changes in the psychological field, 2. changes in cognitive function, 3. changes in the area of activities of daily living and instrumental activities of everyday life and 4. overall perception of quality of life. Results: From the perspective of family members most deteriorating quality of life of seniors with dementia is the memory loss, which excludes them from normal life, as well as loss of ability to orientation in space and time, and loss of ability to perform normal activities of daily life. The sensory disturbance also reduces the quality of life, which leads patients into a greater social isolation and this may promote the growth of the paranoia. Family members rated the quality of life of their loved ones at home as significantly better in comparison with the quality of life of patients with dementia placed in the home for the elderly. In a domestic environment have seniors more stimulations, are more activated, and are more drawn into the family and social life. After coming into institutional care occurs worsening of symptoms and the gradual removal of patients from activation programs. According to our research, Uzbek individuals with Parkinson’s disease have high prevalence of 0/0 polymorphisms of GSTT1 and GSTM1 genes, as well as combinations of GSTT1 (0/0)/GSTM1/(0/0) genotypes. These mutations are associated with an earlier debut of the disease, its mixed form, and rapid rate of progression. Purpose: To investigate the protective action of Ceruloplasmin (Cp) on the lysis of erythrocytes in men with prostate tumors. Material and Methods: The blood erythrocytes of the patients with benign hyperplasia of prostate (BHP) and prostate adenocarcinoma (CaP) were studied. Patients at the age 60-75 with early stages of a cancer have been investigated. The control group was consisted of apparently healthy males with the compatible age. n=15 for each group. The clinical stage of the disease was diagnosed by means of rectal, histological and echographic examination of the prostate gland. Photometric methods were applied to register lysis dynamics in order to test out the protective action of Cp. Results: The nonspecific protective function of Cp preparation on BHP and CaP erythrocytes as well as on the control group erythrocytes was revealed. CaP erythrocytes have shown more sensitivity to the lysis provocative factor than BHP and the control group erythrocytes, that was presumably attributed to the structural and functional changes of the erythrocytes developed in the presence of malignant tumor., N. Mediterranean spotted fever (MSF) in Bulgaria shows increasing severity, especially in patients over sixty years. As T lymphocytes are the primary effector cells in MSF and are altered by the ageing process we aimed to specify the changes in their numbers in elderly patients. We performed a clinicoepidemiological study, haematological and biochemical analysis of 132 patients of 60+ and 30 patients between 19 and 57 years with MSF. We investigated the cell immunity of 20 patients of 60+ using immunofluocytometry. The control group consisted of 10 younger patients and 10 healthy individuals of 60+. MSF showed more unfavourable course in the elderly.We found T-lymphocytes depletion in all patients with MSF. Activated T lymphocytes were increased in elderly patients with MSF. We found significant differences between the number of activated T lymphocytes in elderly patients and healthy elderly persons. The observed T-lymphocytes depletion in MSF corresponded to the disease severity. Cieľ: V práci sme sa zamerali na vedomosti pacientov s astmou v oblastiach self-manažmentu tohto ochorenia a najmä na vplyv faktoru dĺžky trvania tohto ochorenia na úroveň vedomostí o self-manažmente. Zároveň sme zisťovali vzťah medzi úrovňou vedomostí o self-manažmente ochorenia a kontrolou astmy. Metodika: V tejto prierezovej štúdii bolo do súboru zaradených 297 respondentov. Na testovanie kontroly astmy a úrovne vedomostí pacientov sme použili valídne nástroje a to Dotazník vedomostí o self-manažmente astmy pre dospelých (Measuring asthma self-management knowledge in adults) a Test kontroly astmy (Astma Control Test ACTTM). Na štatistické spracovanie dát boli využité: Pearsonov Chi-kvadrat test nezávislosti a Spearmanov korelačný koeficient. Výsledky: Až 220 z 297 respondentov (74,07 %) preukázalo nedostatočné vedomosti, a to konkrétne v doméne Vedomosti o liečbe astmy. V oblasti Manažment bronchiálnej astmy malo nedostatočné vedomosti 191 respondentov (64,31 %). Početnosť správnych odpovedí zároveň naznačuje, že úroveň vedomostí v doménach self-manažmentu astmy, významných pre kontrolu ochorenia, bola vyššia v skupine pacientov diagnostikovaných v priemere jeden rok ako v skupine, ktorá trpí ochorením dlhšie (6-10 rokov), i keď rozdiel nie je štatisticky významný. Záver: Nedostatky vo vedomostiach astmatikov, vyššia frekvencia chýb v oblastiach self-manažmentu, vrátane self-monitoringu a horšia kontrola astmy pri ochorení diagnostikovanom viac ako 6 rokov naznačujú, že je potrebné v klinickej praxi venovať čas a pozornosť reedukácii, opakovanému posúdeniu vedomostí a praktických zručností (aplikácia metód self-manažmentu, inhalačnej techniky) v skupine už skôr diagnostikovaných astmatikov., Aim: This project is aimed to find out the patient knowledge with asthma in the area of self-management of this disease and particularly the influence of the length of the disease on the knowledge level about self-management. At the same time we tried to find out the relation between the knowledge level of the asthma self-management and control of asthma disease. Methods: The total number of 297 adult asthma patients were included in the cross sectional study. The valid instruments Asthma Control Test and Measuring asthma self management knowledge in adults were used for testing asthma control and the degree of patient knowledge in self management of asthma. The Chi-squared test and the Spearman´s rank correlation coefficient were used for statistical processing of data. Results: A total of 220 out of 279 respondents (74%) showed poor knowledge of selfmanagement of asthma, particularly in “Knowledge of asthma treatment” domain and 191 patients in “Management of bronchial asthma” domain (64%). Patients who had been diagnosed one year ago had better level of knowledge in selfmanagement of asthma than those who had been treated for asthma longer than six years (6-10 years). This difference was not significant, but it was related to those domains which has significant influence on asthma control (domains 2-5). Conclusion: Poor knowledge of asthma patients, higher frequency of errors in self management and worse control of asthma in patients with asthma longer than six years imply the need for better and repeated education, check of patient knowledge and skills for self-management, use of peak flow meter included in this group of patients., Michaela Dingová, Petronela Osacká, and Literatura
