Cieľ: Výskum bol zameraný na identifikáciu drţania diéty u adolescentných respondentov a niektorých príčin jej drţania vo vzťahu k pohlaviu a veku. Metodika: Na získanie empirických dát boli pouţité vybrané poloţky sebaposudzovacieho meracieho nástroja s názvom Mapovanie stravovacích návykov medzi adolescentmi (The Project EAT Survey, Neumark- Sztainer et al.). Výskumnú vzorku tvorilo 387 adolescentných respondentov, toho času študentov prvých a štvrtých ročníkov stredných škôl. Výsledky: Podľa výsledkov výskumu ţeny častejšie drţia diéty (redukčné alebo pre jej udrţanie) či uţ v minulom roku (p = 0,002) aj v terajšej dobe (p = 0,000). Sú nespokojné so svojou hmotnosťou (p = 0,000) a postavou (p = 0,009). Respondenti vo veku 18 - 20 rokov uviedli, ţe si ich členovia rodiny doberali alebo si robili posmech z ich hmotnosti (p = 0,000). Vyššie percento ţien zaţilo negatívne hodnotenie ich hmotnosti zo strany členov rodiny (27,5 %) a vrstovníkov (33,5 %). Záver: Sestra v klinickej praxi môţe byť nápomocná pri identifikovaní drţania diét u adolescentov a jej príčin z hľadiska prevencie dopadu drţania diét na telesné a duševné zdravie adolescenta a pri realizácii uţ cielených intervencií, napr. edukácie v rámci takto obsahovo zameranej prevencie., Aim: The research was aimed at identifying dieting among adolescent respondents and some of the reasons for its keeping in relation to sex and age. Methods: For obtaining empirical data selected items of self-assessing measuring tool called Mapping dietary habits among adolescents were used (The Project EAT Survey, Neumark-Sztainer et al.). The research sample consisted of 387 adolescent respondents, previously high school students (of the first and fourth study year). Results: According to results of research women more frequently keep a diet (in order to lose weight or keep it the same) in last year (p = 0,002) as well as at present-day period (p = 0,000). They are not satisfied with their weight (p = 0,000) and figure (p = 0,009). Respondents aged 18-20 years reported that their family members have teased or made fun of their weight (p = 0,000). A higher percentage of women had experience a negative evaluation of their weight on the part of the family members (27,5 %) and peers (33,5 %). Conclusion: A nurse in clinical practice can be helpful in identifying dieting among adolescents and its causes in order to prevent the impact of diets on physical and mental health of adolescent and in the implementation of already targeted interventions, e.g. education in the context of the following content targeting prevention., Michaela Miertová, and Literatura 40
Cíl: Posoudit efekt samoléčby a autodiagnózy dysmenorey u studentek univerzity v Maiduguri. Metody: 289 studentek trpících dysmenorhoeou z celkového počtu 25 000 studentek univerzity v Maiduguri ze státu Borno, Nigerie, bylo randomizováno k léčbě dysmenorey. Studentky byly vybrány na základě dotazníku, který vyplnily v lednu 2011. Výsledky: Střední věk ± směrodatná odchylka (SD) účastnic byl 22.50 ± 3.12 roku. 112 (38.8 %) užívalo analgetika a nefarmakologické prostředky k tišení bolesti, buď současně, nebo následně (p < 0.001). Nejčastějšími nemedikamentózními prostředky byly horká lázeň (15.2 %) a ohřívací dečka umístěná na podbřišek (5.5 %). Nejčastějšími léky byly nesteroidní antirevmatika (NSAID) užívané při nástupu menstruace. V 72 % byla léčba účinná, ve 4.2 % nebyla. Většina účastnic se medikovala sama a nezabývala se možnými vedlejšími účinky (p < 0.001). Většina účastnic (63 %) utratila 300 Nigerijských Naira a méně (1.9 US Dolarů a méně) za léčbu dysmenorey. Závěry: Výsledky potvrdily, že NSAID byly nejčastěji užívané medikamenty k léčbě dysmenorey, ale většinou bez znalosti potencionálních vedlejších účinků. Proto doporučujeme zapojení profesionálních zdravotníků do tohoto procesu., Objective: This study was performed to evaluate self-medication for the treatment of self-diagnosed dysmenorrhoea among a group of University of Maiduguri female students. Methods: The population of this randomized study was 289 female students suffering from dysmenorrhoea out of over 25 000 students attending a college of medicine and eight non-medical faculties of university of Maiduguri in Borno state, Nigeria. The students were interviewed using a self-administered pre-tested structured questionnaire in January, 2011. Results: The Mean ± Standard Deviation (SD) of age of the participants was 22.50 ± 3.12 years. Significant number 112 (38.8%) of participants use drugs to remedy menstrual pains followed by non-drug therapy while few combine both the drug and non-drug therapy for the pain management (p < 0.001). The most common non drug remedies employed by the participants were taking a hot bath (15.2%) and placing a heating pad on abdomen (5.5%). However, the most common drugs used by the participants were Non-Steroidal Anti-Inflammatory Drugs (NSAIDs) which were usually taken orally at the onset of menstruation. Among the participants that used drug, 72% found it effective while 4.2% reported ineffectiveness. Significant majority of the participants usually self-prescribed their drugs, and were ignorant of the common side effects of these drugs (p < 0.001). Most participants (63%) spent 300 Nigerian Naira and below (1.9048 US Dollar and below) on drugs for dysmenorrhoea every month. Conclusion: The results confirmed that NSAIDs were the most common drugs used for dysmenorrhoea treatment, but frequently used without knowledge of potential side effects due to self-medication. Cíl: Hlavním cílem výzkumného šetření bylo zjistit, které faktory ovlivňují pouţívání ošetřovatelských diagnóz v klinické praxi. Dalším cílem bylo zjistit rozdíly v hodnocení faktorů ovlivňujících pouţívání ošetřovatelských diagnóz v praxi podle věku, vzdělání a typu pracoviště. Metodika: Pro získání dat byl sestaven nestandardizovaný dotazník zaměřený na zjištění faktorů ovlivňujících pouţívání ošetřovatelských diagnóz. Výzkumný soubor tvořilo 253 registrovaných všeobecných sester z 9 zdravotnických zařízení v České republice. Sestry hodnotili na Likertově škále od 1 do 5, jak jednotlivé faktory ovlivňují pouţívání ošetřovatelské diagnostiky v praxi. Výzkumné šetření probíhalo od června 2011 do listopadu 2011. Výsledky: Podle našeho výzkumného souboru nejvíce ovlivňují pouţívání ošetřovatelských diagnóz v praxi následující faktory: počet pacientů na 1 sestru, administrativní zátěţ, syndrom vyhoření, nedostatek času, rozdíly v teorii a praxi a nedostatek personálu. Statisticky významné rozdíly v hodnocení faktorů podle věku byly zjištěny v hodnocení 2 faktorů: rozdíly v teorii a praxi, nedostatek motivace. Podle dosaţeného vzdělání byly statisticky významné rozdíly zjištěny v hodnocení 4 faktorů: nejednotnost ošetřovatelských diagnóz, terminologie diagnóz, rozdíly v teorii a praxi, rutinní práce. V závislosti na typu pracoviště byly statisticky významné rozdíly nalezeny v hodnocení 5 faktorů: terminologie diagnóz, rozdíly v teorii a praxi, nedostatek praktických zkušeností s ošetřovatelskou diagnostikou, rutinní práce, syndrom vyhoření. Závěr: Rozeznání faktorů ovlivňujících pouţívání ošetřovatelských diagnóz v praxi a zlepšení institucionální podpory můţe ulehčit implementaci ošetřovatelské diagnostiky v klinické praxi, Aim: The main aim of the study was to identify which factors influenced the use of nursing diagnoses in clinical practice. Another aim was to ascertain whether age, education and type of workplace affected the relative importance accorded to these factors. Methods: A non-standardized questionnaire on factors influencing the use of nursing diagnoses was administered to a sample consisting of 243 registered general nurses from 9 healthcare facilities in the Czech Republic. Using Likert´s 5-point scale, the nurses rated the importance of several factors in influencing their use of nursing diagnoses in practice. The research was conducted between June 2011 and November 2011. Results: Based on our sample, there are several factors which influence the use of nursing diagnoses in practice. Some of them are: number of patients per nurse, administrative strain, burnout syndrome, lack of time, discrepancies between theory and practice, lack of staff. Statistically significant age-related differences in rating factors were found for the following: discrepancies between theory and practice, lack of motivation. Statistically significant differences in ratings relative to education were found for four factors: disunity of nursing diagnoses, diagnostic terminology, discrepancies between theory and practice, workplace routine. Based on type of workplace, statistically significant differences in ratings were found for five factors: diagnostic terminology, discrepancies between theory and practice, lack of practical experience in nursing diagnostics, workplace routine, burnout syndrome. Cieľ: Cieľom práce bolo validizovať defi nujúce charakteristiky ošetrovateľskej diagnózy Defi cit vedomostí vybraným súborom slovenských sestier – expertov, teda zistiť, ktoré defi nujúce charakteristiky sú sestrami považované za hlavné a ktoré za vedľajšie. Metódy: Na hodnotenie defi nujúcich charakteristík ošetrovateľskej diagnózy Defi cit vedomostí bol zvolený Fehringov model validity diagnostického obsahu. Výsledky: Za hlavné defi nujúce charakteristiky boli označené: „slovné vyjadrenie problému“ a „chybné, nepresné dodržiavanie inštrukcií“. Za vedľajšie defi nujúce charakteristiky boli označené: „chybné, nepresné výsledky testu“ a „neprimerané správanie“. Charakteristika „prehnané správanie“ dosiahla vážené skóre menej ako 0,5. Záver: Nie všetky defi nujúce charakteristiky ošetrovateľskej diagnózy Defi cit vedomostí uvedené v klasifi kačnom systéme NANDA-International sú pre vybraný súbor sestier rovnako významné pre jej stanovenie., Aim: The aim of the study was to validate the nursing diagnosis Deficient Knowledge by a chosen sample of Slovak nurses – experts, i. e. to find out which defining characteristics are considered major and minor. Methods: The Fehring’s Diagnostic Content Validity Model was used to evaluate the defi ning characteristics of nursing diagnosis Deficient Knowledge. Results: Th e following defi ning characteristics were marked as major: “verbalization of the problem”, “inaccurate compliance with instructions”. The following characteristics were marked as minor: “poor and inaccurate performance in a test” and “inappropriate behaviour”. The defining characteristic “exaggerated behaviour” had the weighted ratio less than 0.5. The aim of the study was to establish the frequency of hypovitaminosis D in children with type 1 diabetes mellitus (T1D), its influence on biochemical and densitometric parameters and the relation to diabetic nephropathy. 58 children with T1D at the age 9–19 years were enrolled to the study. Vitamin D concentration less than 30 ng/ml was considered as insufficient. 37 children (63.79%) had vitamin D level under 30 ng/ml, from these 19 subjects (32.7%) had vitamin D level under 20 ng/ml and 2 subjects (3.44%) under 10 ng/ml. Children with vitamin D deficiency had significantly lower magnesium concentration and lower Z score of lumbar spine (−1.34 ± 1.24 vs. −0.30 ± 1.21, p = 0.01) compared to diabetics with sufficient vitamin D concentration. No significant difference was found in parameters calcium, phosphorus or glycosylated hemoglobin. Patients with diabetic nephropathy (n = 18) showed no significant difference in vitamin D, glycosylated hemoglobin or Z score of lumbar spine compared to the patients without nephropathy (n = 40). Subjects with nephropathy had significantly longer diabetes duration, significantly higher cholesterol and triacylglycerol concentration. This research paper presents the results of ovarian reserve estimation for 125 women with the Polycystic Ovary Syndrome (PCOS) who have undergone various methods of surgical treatment resection of the ovaries, thermokauterisation and drilling by laser (HoYag). Ovarian reserve was estimated according to the amount of antral follicles, level of follitropin and Müllerian inhibiting substance (MIS), also named antiMüllerian hormone (AMH). Blood flow in ovarian tissue was also examined after various methods of surgical treatment. Cíl: Cílem šetření bylo zjistit, jak studenti ošetřovatelství hodnotí svoji klinickou praxi, spolupráci s mentorem, jaké aspekty ovlivňují jejich spokojenost s praxí a jak se hodnocení praxe liší na jednotlivých odděleních. Metodika: Výzkumný soubor zahrnoval 59 studentů druhého a třetího ročníku oboru všeobecná sestra. Dotazníkové šetření bylo realizováno standardizovaným nástrojem pro hodnocení klinické výukové prostředí a supervize CLES (Saarikoski, 2002) po absolvované klinické praxi studentů (2008). Výsledky: Byly potvrzeny rozdíly v hodnocení supervize a klinického výukového prostředí vzhledem k typu oddělení, kdy interní oddělení byla studenty hodnocena kladněji. Studenti hodnotili nejvyššími známkami dimenzi ošetřovatelská péče na oddělení, nejhůře pak vztah mezi mentorem a studentem. Současně byly potvrzeny pozitivní korelace mezi celkovou spokojeností studentů a jejich hodnocením. Statisticky významné vztahy byly identifikovány mezi zátěží sester na oddělení a hodnocením studentů – s vyšší fyzickou zátěží sester byla supervize a klinické výukové pracoviště hodnoceno kladněji. Závěr: Výsledky šetření ukazují na význam supervize a kvalitního klinického výukového prostředí pro ošetřovatelské praxe. Praktická výuka tvoří významnou část vzdělávacího programu sester, a proto je nezbytné sledovat zkušenosti a reflexe studentů., Objective: The aim of this study was to find out how nursing students evaluate their clinical practice, co-operation with the mentor, what aspects affects their satisfaction with the practice and how the evaluation of practice varies from wards. Methods: The sample comprised of 59 nursing students of the second and third year of study. The survey was realized by standardized Clinical Learning Environment and Supervision instrument CLES (Saarikoski, 2002) after the completion of students clinical practice (2008). Results: Differences were confirmed in the evaluation of the supervision and clinical learning environment depending on the type of wards, when the internal wards were evaluated by students more favourably. Students rated the dimension of nursing care at the word the highest marks, the lowest was rated the relationship between mentors and students. At the same time were confirmed positive correlations between students’ overall satisfaction and their evaluation. Statistically significant relationship were identified between the nurses´ workload in the wards and students´ assessment – with the higher workload of nurses was the supervision and clinical learning environment evaluated more favourably. Conclusion: The results of survey show the importance of clinical supervision and quality of clinical learning environment for nursing practice. The article presents results of a study of the relationship 'locus of control health and subjective wellbeing. In a heterogeneous sample (N = 239) in the stage of youth (1930 years) shall be established: locus of control determines 11.56% of the changes in the assessment of health, changes in health contribute to 12.82% of the variation in assessment of happiness and 6.10% in the measurement of life satisfaction. Young people with an internal locus of control are healthier, happier and more satisfied with their lives than those with an external locus of control. In a study of cause effect relationship is established that internal locus of control determines a healthier lifestyle. Inadequate health care, more severe risk factors (stress, unhealthy diet, inadequate sleep, smoking) among young people with an external locus of control, outline prospects for greater vulnerability of this group of the population of the so called "Diseases of modern society." Cieľ: Cieľom práce bolo validovať definujúce charakteristiky ošetrovateľskej diagnózy Deficit objemu telových tekutín vybraným súborom slovenských sestier – expertov, teda zistiť, ktoré definujúce charakteristiky sú sestrami povaţované za hlavné a ktoré za vedľajšie. Metodika: Na hodnotenie definujúcich charakteristík ošetrovateľskej diagnózy Deficit objemu telových tekutín bol zvolený Fehringov model validity diagnostického obsahu (DCV model). Definujúce charakteristiky hodnotili sestry, ktoré získali minimálne 4 body podľa modifikovaných Fehringových kritérií. Súbor tvorilo 116 slovenských sestier – expertov. Definujúce charakteristiky boli sestrami hodnotené na Likertovej škále od 1 do 5 (1 – ţiadna významnosť, 2 – malá významnosť, 3 – stredná významnosť, 4 – veľká významnosť, 5 – najväčšia významnosť). Zber údajov prebiehal v období máj aţ október 2010. Výsledky: Za hlavné definujúce charakteristiky bolo súborom slovenských sestier – expertov zo 17 charakteristík označených 6 charakteristík: zníţený turgor koţe, zníţená diuréza, suché sliznice, zníţené napätie jazyka, zníţená venózna náplň, zníţený krvný tlak. Za vedľajšie definujúce charakteristiky boli označené nasledujúce charakteristiky: zvýšená merná hmotnosť moču, suchá pokoţka, zvýšený hematokrit, slabosť, smäd, oslabený pulz, zníţený objem pulzu (nitkovitý pulz), zvýšená pulzová frekvencia, zmeny v psychickom stave, zvýšená telesná teplota. Charakteristika „náhla strata hmotnosti (s výnimkou tretieho priestoru)“ dosiahla váţené skóre menej ako 0,5. Záver: Súbor slovenských sestier za významné znaky pre určenie ošetrovateľskej diagnózy Deficit objemu telových tekutín povaţuje 6 znakov., Aim: The aim of this study is to validate the defining characteristics of a nursing diagnosis of Deficient Fluid Volume by considering a selected sample of Slovak nurse – experts. In other words, to ascertain which defining characteristics are rated by these nurses as major or minor. Methods: Fehring’s Diagnostic Content Validation model (DCV model) was used for validation of this nursing diagnosis. Slovak nurses who met the modified criteria (4 points) recommended by Fehring comprised the sample of experts - a total of 116 Slovak nurses – experts. Defining characteristics were rated by the nurses using Likert’s 5-point scale (1 – not at all characteristic, 2 – very little characteristic, 3 – somewhat characteristic, 4 – considerably characteristic, 5 – very characteristic). Data collection occurred between May and October 2010. Results: Out of 17 defining characteristics of a nursing diagnosis of Deficient Fluid Volume, the Slovak nurses’ sample classified six cues as major: decreased skin turgor, decreased urine output, dry mucous membranes, decreased tongue turgor, decreased venous filling, and decreased blood pressure. The following cues were classified as minor defining characteristics: increased urine concentration, dry skin, elevated hematocrit, weakness, thirst, decreased pulse volume and pressure, increased pulse rate, changes in mental status, increased body temperature. The weighted score of the defining characteristic „sudden weight loss (except in the third space) was less than 0.5.
