The purpose of this study was to investigate the content neurospecific markers protein S-100 and neuroenolaza in blood serum and tear fluid of patients with ocular ischemic syndrome. Material and methods. We observed 43 patients aged 57 to 79 years, mean age 67.3 ± 2.7 years. Control group consisted of 11 volunteers without ophthalmic symptoms. The main group consisted of 32 patients with OIS. The neurospecific proteins S100 and NSE were investigated in blood serum and tear fluid. Results. The study found that in patients of the control group the content of protein were within the normal range: S -100 in the tear fluid – 0,0662 ± 0,00335 mkg/l, in the blood serum 0,0508 ± 0,00241 mkg/l. In patients of the main group the indicators of protein in the tear fluid were elevated in all patients - 3,12 ± 0,246 mkg/l ( p<0.005). The normal evels in blood serum of marker S-100 was in 30 patients - 0,0589 ± 0,00303 mkg/l, while, in 2 patients protein S-100 were raised and averaged 0,2175±0,00725 mkg/l. It was found that in patients of the control group content of protein NSE in the tear fluid and blood serum were within normal values - 15,86 ± 0,148 Ng/ml, 15,60 ± 0,202 Ng/ml respectively. In the main group the amount of protein NSE tended to increase in the tear fluid in 23 patients and averaged 33,012 ± 3,2626 Ng/ml (p<0.005), a significant decrease the quantity of protein was observed in 9 patients, which amounted to 5,166 ± 0,8301 Ng/ml. At normal levels in the blood serum protein NSE detected in 30 patients and averaged 14,48 ± 0,263 Ng/ml, whereas, in 2 patients there was a significant increase of content of protein NSE and was 27,47 ± 3,068 Ng/ml. Conclusions. In a review of the book " Medical law and ethics " by Jonathan Herring, I made it my goal to familiarize the reader with the issues of medical law and ethics, which author discuss in his work . The book should be a valuable source of knowledge for both, experts and laymen, because the author dealt with many topics from different angles, that raise a number of legal and ethical issues (the issue of euthanasia , research on humans , cloning , sterilization , etc. .). Cíl: Výzkum se zaměřuje na zjištění míry syndromu vyhoření, motivačních faktorů dle Herzbergovy dvoufaktorové teorie a jejich vzájemného vztahu u všeobecných sester pracujících ve Fakultní nemocnici Olomouc. Cílem výzkumu bylo poznání osobních priorit faktorů pracovního prostředí všeobecných sester a stupně jejich saturace zaměstnavatelem. Vzájemnou komparací zjištěného pak přehledně ukázat největší divergence mezi přáními a realitou všeobecných sester v pracovním prostředí a jejich souvislostí se syndromem vyhoření. Metody: Pro účely výzkumu byl zvolen kvantitativní přístup. Ke sběru dat byly použity dva dotazníky: standardizovaný dotazník MBI (Maslach Burnout Inventory) a nestandardizovaný dotazník Motivačních faktorů vytvořený na principech Herzbergovy dvoufaktorové motivační teorie a po diskuzích s manažery nemocnic. Výsledky: Z výsledků výzkumu vyplynulo, že existuje statisticky významný vztah mezi mírou syndromu vyhoření a mírou neuspokojení faktorů pracovního prostředí dle Herzbergovy teorie. Nepotvrdil se předpoklad o existenci signifi kantního negativního vztahu mezi mírou syndromu vyhoření a spokojeností s fi nanční odměnou. Na žebříčku preferencí mají nejvyšší pracovní hodnotu faktory mzda, péče o pacienty a jistota pracovního místa. Mezi nejvíce divergující faktory patřila mzda a jistota pracovního místa. Výsledky potvrzují protektivní vliv saturace faktorů pracovního prostředí dle Herzbergovy teorie na výskyt syndromu vyhoření. Závěr: Výzkum prokázal význam saturace faktorů pracovního prostředí jako protektivního faktoru ve vztahu k syndromu vyhoření. Potvrdilo se, že se zvyšující se mírou saturace se snižuje míra vyhoření u všeobecných sester pracujících ve Fakultní nemocnici Olomouc., Aim: Th e research deals with the rate of burnout syndrome, motivation factors of Herzberg’s two-factor motivation theory and their mutual relation in general nurses working in the University Hospital Olomouc. Th e aim was to identify personal priorities among hygiene factors with general nurses and the degree of their saturation by the employer. Comparison of the fi ndings clearly shows the biggest divergence between wishes and reality of the working environment of general nurses and the relation to burnout syndrome. Methods: A quantitative approach was chosen for the research. To collect data, two questionnaires were used: a standardised MBI (Maslach Burnout inventory) and a non-standardised Motivation factors questionnaire, which was on the principles of Herzberg’s motivation theory aft er several discussions with hospital management. Results: Th e result of the survey research revealed an existing, statistically signifi cant relation between the burnout syndrome rate and the degree of dissatisfaction with hygiene factors according to Herzberg’s theory. Th e assumption about the existence of a signifi cant negative relation between burnout syndrome rate and salary was not confi rmed. Factors of salary care for the patient and job security occupy the top levels on the preference list. Th e most diverging factors include salary and job security. Th e results confi rm the protective eff ect the saturation of Herzberg’s hygiene factors has on the burnout syndrome rate. Conclusion: Th e survey research confi rmed the importance of saturating hygiene factors as a protective factor against burnout syndrome. Při zjištění tumoru v pánvi se nalézáme před problémem stanovení jak jeho origa, tak i biologického chování. Možnost predikovat biologické chování pánevních tumorů již před léčbou je klíčem ke zvolení individuálního přístupu, který zatíží pacientku úměrně k maximálnímu možnému léčebnému efektu. V článku se dále zabýváme pouze gynekologickými nádory. Hlavní prognostické a prediktivní pooperační faktory dobře známé a rutinně používané jsou histologický typ nádoru, stádium onemocnění, stupeň diferenciace, přítomnost lymfangioinvaze, postižení lymfatických uzlin a pooperační nádorové reziduum. Při předoperační predikci biologického chování gynekologických pánevních tumorů vycházíme z anamnézy, klinických ukazatelů - věku a přidružených chorob nemocné (performance status), dále ze stanovení markerů z krve, grafických vyšetření (zejména sonografie) a charakteristik nádoru stanovených na základě histologického, imunohistochemického či molekulárně-genetického vyšetření tkáně získané z excize, laparoskopie či tru-cut biopsie. Mezi prediktivně-prognostické biomarkery, které mohou pomoci předpovídat úspěšnost naší léčby a je možné je využít v triage pacientek před léčbou, je možné zařadit například markery z periferní krve CA125, HE4, ROMA index, přítomnost BRCA 1/2 mutace, předoperační trombocytózu, neutrophil-to-lymphocyte ratio, a dále tkáňové markery, jako je mutace p53, Her2/Neu pozitivitu, přítomnost ER/PR, pozitivitu L1CAM, dále panel 9 proteinových markerů PROVAR, intraepitelální tumor-infiltrující lymfocyty, stanovení proliferační aktivity pomocí proteinu Ki67 (MIB1), genové expresní profily, proteiny rezistence atd. Tyto biomarkery budou v následujícím textu probrány. Pacientka, daný tumor a zvolený léčebný přístup tvoří vždy individualitu, jakési jedinečné spojení, jehož chování a z toho plynoucí QoL (Quality of life - kvalita života), DFI (Disease free interval - období bez nemoci) a OS (Overall survival - celkové přežití) je obtížné predikovat. Proto žádné z dosud používaných vyšetření ani žádný v současnosti známý marker nedokáže lékaři ani pacientce přesně odpovědět na tyto základní existenciální otázky, které si pacientka a její nejbližší pokládají nejčastěji., When we detect a tumor in the pelvis we find ourselves faced with the problem of determining both the origin, and biological behavior. The possibility to predict biological behavior of pelvic tumors already before treatment is the key to individual patient management with least patient burden proportional to potentially attainable goal. In the article below we deal only with the gynecological tumors. The main postoperative prognostic and predictive factors such as staging, grading, histological type of tumor, lymph node involvement (as included in staging) and postoperative tumor residue are well known and routinely used. However, if we focus on the possibility of preoperative prediction of tumor biological behavior, it can be based on several factors. These include clinical indicators such as patient age at time of diagnosis, patient performance status, blood “markers” assessment, imaging studies results (particularly ultrasound) and processing of biopsy samples obtained by excision from the tumor during laparoscopy or tru-cut biopsy, etc. Other predictive biomarkers that can generally predict success of our therapy include, e.g. CA125, HE4, ROMA index, presence of the BRCA 1/2 or p53 mutation, Her2/neu, ER/PR or L1CAM positivity, PROVAR (a panel of 9 protein markers), intraepithelial tumor-infiltrating lym - phocytes, preoperative thrombocytosis, neutrophil-to-lymphocyte ratio, Ki67 positivity rate (MIB1), gene expression profiles, protein of resistance, etc. These biomarkers will be discussed below. Since the patient, her tumor and the therapeutic approach always form a unique combination, the resulting QoL (Quality of life), DFI (Disease free interval) and OS (Overall survival) are difficult to predict. Cíl: Analýzou literárních pramenů zjistit možnosti standardizace specializované ošetřovatelské péče k dekanylaci sheathu (vodiče) z arteria radialis, femoralis. Východiska práce: Procesuální standardy ošetřovatelských postupů při výkonech a jejich významnost pro výsledkové hodnocení kvality, se významně uplatňují při odborných výkonech prováděných sestrou. Postup při dekanylaci sheathu z arteriálního povodí po srdeční katetrizaci může ovlivnit fi nální výsledek katetrizace. Standardizace dekanylace sheathu je významný fenomén k redukci komplikací a měřítkem kvality výkonu jako celku. Metodika: Rešerše odborných článků databází PubMed, Medline, Cochrane, MedscapeNurses za období 1998–2010. K vyhledávání byl použit Booleovský operátor-or/and se zadáním klíčových slov. Dohledáno bylo 59 odkazů, dále jsme pracovaly se 16 odkazy, které vyhovovaly záměrům práce. Výsledky: Z dohledaných odkazů vyplývá, že ošetřovatelské standardy k dekanylaci sheathu často vychází ze závěrů ošetřovatelského výzkumu (EBP), zaměřují se na redukci krvácení z místa vpichu již od počátků provádění srdečních katetrizací. Standardy se zaměřují na způsob a délku prováděné komprese na arteria femoralis, tak arteria radialis. Dle odkazů je význam kladen na eliminaci komplikací (hematom, krvácení), jednak zavedením mechanických kompresních pomůcek, ale zejména vyškoleným ošetřovatelským personálem ve způsobech komprese, typech a správnosti použití kompresních zařízení. V odkazech je kladen důraz na časnou mobilizaci pacientů po katetrizaci, toto je možné za dodržení podmínek stanoveným ošetřovatelským standardem k dekanylaci sheathu. Závěry: Ošetřovatelský standard je důležitou součástí výkonu přispívající k redukci komplikací a zlepšení komfortu pacienta, má důležitou platformu zejména v zahraničí., Aim: Main aim of this report was to determine the possibilities of standardisation specialized nursing care for decannulation of the sheath from arteria radialis, femoralis by using analysis of literary sources. Introduction: Processual standards nursing procedures at operation and their importance for quality evaluation are signifi cantly enforced by nurse. Th e procedure of the decannulation of sheath from arterial basin can infl uence the fi nal eff ect of catheterization. Th e sheath standardisation is an important phenomenon contributing to reduction of complications and the quality of the operation as well. Methods: Th e methodology is based on research of professional articles from PubMed, Medline, Cochrane, MedscapeNurses databases for a period 1998–2010. It was used a Boolean operator (or/and), with entering the key words. It was found 59 references by using key words,then we worked with 16 references, which were relevant for the purpose of our report. Results: Th e references point to the fact that nursing standards for decannulation of the sheath are oft en based on conlusions of the nursing research (EBP). Th ey focus on a reduction of bleeding at the injection site from the beginning of the heart catheterization. Th e standards are focusing on manners and the the length of the compression on arteria femoralis, arteria radialis. Cieľ: Ochorenie detskej mozgovej obrny je možné označiť ako neurovývojové neprogresívne postihnutie mozgu, ktoré spôsobuje narušenie motorického vývoja dieťaťa. Veľmi často zasahuje do viacerých oblastí vývoja – kombinuje sa s mentálnym postihnutím, zmyslovými chybami a ovplyvňuje rozvoj komunikačných zručností. Cieľom štúdie bolo vytvoriť novú teóriu s použitím metaparadigmy ošetrovateľstva pomocou metódy „zakotvenej teórie“, ktorá by dokazovala opodstatnenosť a súvzťažnosť ošetrovateľskej starostlivosti so všetkými oblasťami života (fyzickej, psychickej, sociálnej, legislatívnej) u takto postihnutých detí. Metódy: V štúdii sme vypracovali komplexné kazuistiky troch prípadov. Na získavanie informácií sme zvolili metódu pološtandardizovaného rozhovoru a metódu štúdia zdravotnej dokumentácie oslovených osôb. Komplexné kazuistiky sme analyzovali metódou „zakotvenej teórie“. Výsledky: Informácie, ktoré sme získali z kazuistík nám poskytli cenné vyjadrenia, na základe ktorých sme vytvorili novú teóriu s použitím metaparadigmy ošetrovateľstva podľa metódy „zakotvenej teórie“, ktorá poukazuje na kauzalitu vzťahu ošetrovateľskej starostlivosti so všetkými oblasťami života (fyzickej, psychickej, sociálnej, legislatívnej) u detí so spastickou formou detskej mozgovej obrny. Zároveň poukazuje aj na potrebu multidisplinárnej spolupráce odborníkov z oblasti neurológie, fyzioterapie, psychológie, logopédie, špeciálnej pedagogiky i sociálnej oblasti v rámci interakcie s pacientom. Záver: Naša teória dokazuje, že kvalitná ošetrovateľská starostlivosť, komplexný multidisciplinárny prístup odborníkov z oblasti neurológie, fyzioterapie, psychológie, špeciálnej pedagogiky ako aj sociálnej starostlivosti môže zlepšiť podmienky života dieťaťa s mozgovou obrnou v celom holistickom rozsahu., Aim: Disease of cerebral palsy can be described as neurodevelopmental disability non-progressive brain, which causes disruption of motor development children.
