A hybrid approach to elephant trunk technique for treatment of thoracic aortic aneurysms combines a conventional surgical and endovascular therapy. Compared to surgery alone, there is a presumption that mortality and morbidity is reduced. We present a case report of a 42-year-old man with a giant aneurysm of the entire thoracic aorta, significant aortic and tricuspid regurgitation and ventricular septum defect. Hereditární hemoragická teleangiektázie neboli morbus Rendu‑Osler‑Weber je fibrovaskulární dysplazie s autozomálně dominantní dědičností charakteristická přítomností teleangiektázií a arteriovenózních malformací kůže, sliznic a vnitřních orgánů. Plicní arteriovenózní malformace jako zdroj septických embolů mohou zapříčinit vznik mozkového abscesu, který tak bývá prvním a často jediným projevem heriditární hemoragické teleangiektázie. V práci jsou prezentovány tři kazuistiky pacientů trpících touto chorobou, kde prvním a závažným příznakem byl vzniklý mozkový absces. Protože se jedná o relativně vzácné onemocnění, nebývá vždy zvažováno v rámci diferenciální diagnostiky a tím pádem ani diagnostikováno. U každého pacienta s mozkovým abscesem je nezbytné na toto onemocnění pomýšlet a v případě podezření je nutné jej řádně vyšetřit. Při verifikované plicní arteriovenózní malformaci je třeba ji ošetřit a zabránit tak vzniku recidivujících mozkových abscesů. Péče o tyto pacienty je komplexní a multioborová., Hereditary hemorrhagic telangiectasia or Rendu‑Osler‑Weber disease is an autosomal‑dominant disorder that results in fibrovascular dysplasia and is characterized by telangiectases and arteriovenous malformation of the skin, mucosa and viscera. Pulmonary arteriovenous malformations as a source of septic emboli may give rise to brain abscess that so often is the first and only clinical manifestation of hereditary hemorrhagic telangiectasia. Three cases of patients suffering from this disease with brain abscess as the first and serious symptom are presented. Since this is a relatively rare disease, it is not always considered in the differential diagnosis and thus not diagnosed. This disease has to be considered in patients with brain abscess and, in case of a suspicion, it must be properly examined. When pulmonary arteriovenous malformation is verified, it has to be treated in order to prevent recurrent brain abscesses. Care of these patients should be comprehensive and multidisciplinary. Cíl práce: Prezentujeme kazuistiku pacienta, u kterého došlo k rozvoji oboustranného seminomu na podkladě intratubulární germinální neoplazie – neklasifikovatelný typ (IGCNU – intratubular germ cell neoplasia unclassified type). Kazuistika: Pacient s anamnézou seminomu levého varlete klinického stadia I a IGCNU po radikální orchiektomii a adjuvantní radioterapii byl odeslán k urologickému vyšetření pro suspektní tumor solitárního pravého varlete dle sonografického vyšetření. Indikovali jsme 3biopsii varlete, která prokázala přítomnost IGCNU. Následovala radikální orchiektomie, kde byl histologicky verifikován klasický seminom. Závěr: Intratubulární germinální neoplazie představuje vysoké riziko rozvoje invazivního germinálního tumoru varlete. Léčba zahrnuje radioterapii či chemoterapii cisplatinou. Alternativou je pečlivá dispenzarizace a pravidelné sonografické kontroly, zejména u pacientů se solitárním varletem, kteří chtějí zachovat fertilitu a endokrinní funkci. Biopsie kontralaterálního varlete u pacientů s germinálním tumorem k vyloučení IGCNU by měla být provedena v případě kryptorchizmu, poruchy spermiogeneze a atrofií varlete s objemem pod 12 ml, vhodná je rovněž u pacientů s mikrolitiázou varlete a předchozím výskytem IGCNU., Objective: We are presenting a case report of young man who developed an asynchronous bilateral seminoma based on an intratubular germ cell neoplasia of unclassified type (IGCNU). Case report: A patient, with history of a stage I seminoma of the left testis, based on an IGCNU of the left testis, after a radical orchidectomy and adjuvant radiotherapy, was referred to the urology clinic with a suspected tumor of the right testis. A testicular biopsy was made and the histological examination showed an IGCNU. A radical orchidectomy was performed and a seminoma was found. Conclusion: IGCNU is a precursor lesion of germ cell testicular tumors. Treatment of the IGCNU includes radiotherapy or chemotherapy with cisplatin. Surveillance remains an option for those with a solitary testicle who wish to preserve fertility and endocrine function. A contralateral testicular biopsy, in patients with a history of germ cell tumors, should be made in cases of cryptorchidism, poor spermatogenesis, and testicular atrophy with a testicular volume of less than 12 ml. Unilateral otosclerosis combined with avascular necrosis of stapes crura is a rare entity. It should be considered in a case of high grade otosclerosis. Symptoms are the same as in patients who suffer from common otosclerosis. Patients complain on progressive hearing loss and tinnitus. The diagnosis is made clinically by conventional audiologic evaluation and radiologically by x-ray mastoid Schuller's view and CT scan. HRCT scan makes visible all parts of ossicular chain and gives surgeon some information about ossicular chain damage. Castlemanova choroba je vzácnou nosologickou jednotkou, která byla v literatuře poprvé popsána v roce 1954 Castlemanem a Lownovou. Jde o onemocnění primárně postihující lymfatické uzliny, u kterého se rozlišují čtyři histomorfologické podtypy a dva klinické obrazy (uni a multicentrický). Nejčastěji se klinicky projeví jako lymfadenopatie mediastina, krku či retroperitonea. Příznaky nemoci jsou výrazně heterogenní a záleží na daném histologickém podtypu. Taktéž prognóza se liší podle typu nemoci, kdy může jít jak o zcela benigní nálezy (zejména forma unicentrická), tak o potenciálně maligní formu, při které nemoc progreduje až do stadia maligní lymfoproliferace (multicentrická forma). Autoři prezentují případ 29letého muže s klinicky manifestním (ikterus, poruchy střevní pasáže) objemným tumorem retroperitonea lokalizovaným subhepatálně, který byl úspěšně radikálně exstirpován. Pooperační průběh byl nekomplikovaný. Definitivní histopatologická diagnóza potvrdila hyalinně-vaskulární typ Castlemanovy nemoci. Vzhledem k tomu, že se jednalo o multicentrickou formu onemocnění, bude následovat adjuvantní onkologická léčba. Klíčová slova: Castlemanova choroba − tumor retroperitonea, Castleman disease is rare diagnosis. It was first mentioned in literature in 1954 by doctor Castleman and Lown. It is rare sickness which primary affects lymfatic nodes. There are four histo-morfologic subtypes and two clinic manifestations (uni and mulicentric). Comon clinical picture is simple lymfadenopathy of neck, medistinal a retroperitonal lymfatic nodes. Signs of this disease are heterogenic and depend on histological subtype. Also prognosis depends on type of illness. It should take place as benign diagnosis (unicentric form) but also can be potencionally malignant form which shlould exacerbate to malignant lymfoproliferation (multicentric form). Authors present the case of 29 years old men with clinical manifestation (jundice, intermitent bowel obstruction) of lagre retroperitoneal tumor in subhepatal localization. The treatment was radical surgical extirpation, surgery was without complications. Definitive histological diagnosis was confirmed as hyaline-vascular type of Castleman disease. Cíl práce: Upozornit na možnosti laparoskopického chirurgického řešení náhlé příhody břišní v graviditě. Typ práce: Kazuistika. Vlastní pozorování: Formou kazuistiky prezentujeme případ těhotné ve 23. týdnu gravidity s náhlou příhodou břišní neznámé etiologie a její laparoskopické řešení. Závěr: Laparoskopie v rukou zkušeného operatéra se jeví jako velmi bezpečná metoda v případech nejasné bolesti podbříšku, a to i v graviditě v závislosti na velikosti dělohy, tedy stáří gravidity. V tomto případě byla příčinou torze fibromu tuby, která byla řešena salpingektomií., The aim of the study: Draw attention to possibilities of LPSC surgical solutions of acute abdominal evens in pregnancy. Type of paper: Case study. Own observation: We are presenting a case of a pregnant woman at 23 weeks who suffered an acute abdominal event of unknown etiology and its laparoscopic solution. Conclusion: Laparoscopy in the hands of an experienced surgeon appears as a very safe method in cases of lower abdominal pain of unclear origin also in pregnancy, depending on the size of the uterus, i.e. the stage of pregnancy. Prezentujeme případ mladé ženy, jež prodělala v dospělém věku operaci vrozené cysty pravého hepatiku, kterou vzhledem k těsnému kontaktu s velkými cévami nebylo možné kompletně resekovat. Byla provedena parciální resekce a cysto-jejuno-anastomóza (CJA) kdrenáži žluči. Pacientka byla 4 roky bez obtíží. Posléze se dostavily bolesti břicha, subikterus a laboratorní známky cholestázy Kvůli těmto potížím a opakovaným cholangoitidám prodělala řadu vyšetření a intervencí, které měly zajistit ozřejmění příčiny obstrukce a obnovení odtoku žluči. Použité zobrazovací metody neprokázaly jednoznačnou příčinu obtíží, proto bylo přistoupeno k operační revizi, při níž byl peroperační ultrasonografií prokázán tumor v oblasti CJA. Nález byl vzhledem k lokalizaci inoperabilní. Pacientka měla následně zjištěnu diseminaci malignity v játrech i v uzlinách a zemřela 5 měsíců od zjištění maligní diagnózy na hepatorenální selhání. Tento poměrně raritní případ dokládá zvýšené riziko maligního zvratu vrozených malformací žlučových cest především v případě jejich diagnózy a léčby v dospělém věku. V dostupné literatuře existuje jen málo dokumentovaných případů metachronního karcinomu po resekci žlučových cest., We present a case of a young woman, who underwent surgery for congenital bile duct cyst in adulthood. Due to location of the cysts it was impossible to completely remove it. The cyst was partially resected and it was performed cyst-jejunal anastomosis (CJA) to drain of bile. After operation the patient was 4 years without any problems. After this interval appeared pain in the right upper quadrant and subicterus with laboratory evidence of cholestasis. The patient suffered from recurrent cholangitis due to which a number of diagnostic tests and interventions were performed. Used imaging metods did not demonstrate an unequivocal cause of problems, therefore subsequently surgical revision was performed and the in-traoperative ultrasonography demonstrated tumor in anastomosis. The finding was inoperable due to inappropriate location. In the following period liver and lymphnode metastases appeared and patient died five months after diagnosis of malignancy. This relatively rare case illustrates an increased risk of malignant transformation of congenital malformations of the biliary tract. Denosumab is a human monoclonal antibody representing a novel therapy of osteoporosis. Contrary to always other antiosteoporotic drugs, it is not contraindicated in advanced chronic kidney disease, as its pharmacokinetic does not differ from patients with normal kidney function. However, published case reports in chronic kidney disease (CKD) patients stopped the therapy after single dose because of hypocalcemia. Diastematomyelie je vzácná vrozená vada patřící mezi spinální dysrafizmy a obvykle je asociována se syndromem míšního ukotvení. Diastematomyelie je tvořena dvěma polovinami rozštěpené míchy s vlastními durálními vaky, které jsou rozděleny osteokartilaginózním septem. Tato vada bývá diagnostikována především v dětském věku. U dospělých je vzácná a dominujícím příznakem je bolest. Autoři prezentují kazuistiku ženy ve věku 44 let s diastematomyelií, která si stěžovala na chronické bolesti zad a progredující slabost dolních končetin. U pacientky jsme provedli laminektomii, deliberaci míchy a plastiku durálního vaku. Po operaci pacientka zůstala bez neurologického deficitu a udávala výraznou úlevu od bolestí., Diastematomyelia is a rare congenital defect, a type of spinal dysraphisms, often associated with the tethered cord syndrome. Diastematomyelia is formed by the two halves of the split spinal cord that each has own dural sac separated by a rigid or fibrotic septum. This defect is mainly diagnosed in children. Pain is the dominant symptom in adult patients. The authors present a case of a woman aged 44 years with diastematomyelia who complained of chronic back pain and progressive weakness in her legs. In this patient, laminectomy was performed, the spinal cord released and the dural sac restored. After the surgery, the patient had no neurological deficits and reported significant pain relief.
