Zlatým standardem léčby karcinomu endometria je chirurgická léčba. Adjuvantní radioterapie ve formě brachyterapie či zevní radioterapie je indikována u středně a vysoce rizikového karcinomu, kde snižuje zejména výskyt lokoregionální recidivy. U vysoce rizikového karcinomu jsou však pacientky ohroženy i generalizací onemocnění, což je důvodem pro zvažování zařazení adjuvantní chemoterapie do léčebného schématu. Chemoterapie může být použita současně s radioterapií jako konkomitantní chemoradioterapie nebo sekvenčně po skončení radioterapie. Článek popisuje výsledky prací, které použily adjuvantní chemoterapii. Výsledky nejsou zatím zcela jednoznačné, na závěry velkých studií stále čekáme. Přesto u vysoce rizikového karcinomu endometria již dnes v klinické praxi zvažujeme použití chemoterapie., Surgical treatment is the gold standard for treating endometrial carcinoma. Adjuvant radiotherapy in the form of brachytherapy or external radiotherapy is indicated in moderate and high-risk carcinoma where it particularly reduces the rate of locoregional recurrence. In high-risk carcinoma, however, patients are also at risk of disease generalization, which is the reason for considering the inclusion of adjuvant chemotherapy in the treatment regimen. Chemotherapy can be used concomitantly with radiotherapy as concomitant chemoradiotherapy or sequentially after radiotherapy termination. The article describes the results of papers that have utilized adjuvant chemotherapy. The results have not been entirely unequivocal so far, and the conclusions of large studies are yet to be published. Aktivace imunitního systému je velmi důležitým krokem v léčebné strategii metastazujícího melanomu a větší pochopení funkce kontrolních bodů imunity vedlo k vývoji řady checkpoint inhibitorů. Ipilimumab, monoklonální protilátka proti CTLA-4 byl první lék, který prokázal zlepšení přežití u pacientů s metastazujícím melanomem. Medián celkového přežití (OS) u pacientů léčených ipilimumabem byl, při porovnání s pacienty léčenými gp100 vakcínou, delší (10,9 proti 6,4 měsíce) a také pacienti léčení ipilimumabem a DTIC měli, proti pacientům léčeným samotným DTIC zlepšené OS (11,2 proti 9,1 měsíce). Většina nežádoucích účinků vyvolaných léčbou ipilimumabem jsou imunitně zprostředkované a mezi nejčastější patří gastrointestinální a kožní toxicita, endokrinopatie a hepatotoxicita. Koncem roku 2014 byly schváleny FDA pembrolizumab a nivolumab, nové monoklonální protilátky proti dalšímu významnému kontrolnímu bodu imunity PD-1 receptoru. Vstupujeme do nové éry v léčbě melanomu, ale nemůžeme říci, že je problematika léčby melanomu vyřešena. Je nutný další výzkum v oblasti biomarkerů a kombinované léčby, včetně optimálního dávkování i doby terapie., Activation of the immune system is very important step in the treatment strategy of metastatic melanoma and greater understanding of function of the immune checkpoints led to the development of several immune checkpoint inhibitors. Ipilimumab, an anti-CTLA-4 monoclonal antibody, was the first agent to demonstrate a survival benefit in patients with metastatic cutaneous melanoma. The median OS rate for patients treated with ipilimumab was, compared with patients receiving a gp100 vaccine longer (10.9 versus 6.4 months), and also patients treated with ipilimumab and DTIC compared with those treated with DTIC alone had improved OS (median OS 11.2 versus 9.1 months.) The majority of ipilimumab-related adverse events are immune related, and the most common are gastrointenstinal and skin toxicity, endocrinopathy and hepatotoxicity. New monoclonal antibody against other important immunity checkpoint PD-1 receptor, pembrolizumab and nivolumab, got FDA approval at the end of 2014. We are approaching the new era of the treatment of melanoma but we cannot say, that we resolved the problem. Úvod do problematiky: Karcinom ovarií je onemocnění s vysokou mortalitou i navzdory radikální operaci a zavedení chemoterapeutických režimů s platinovými deriváty. Více než u 60 % pacientek vznikne recidiva onemocnění (především intraperitoneálně) a většina z nich zemře během 5 let. Cíl: Cílem retrospektivní studie bylo zhodnotit ozáření abdominopelvické dutiny u rizikové skupiny pacientek (stadium III), které následovalo po léčbě chirurgické a 6 sériích chemoterapie (CHT). Soubor: Na ONO Nemocnice Č. Budějovice, a.s. v letech 1985–1999 bylo léčeno celkem 211 pacientek s maligním tumorem ovarií, retrospektivní vyhodnocení bylo možné provést u 206 z nich. Z toho bylo 106 stadia III (51,4 %). Z celého souboru (206) byla provedena hysterektomie s oboustrannou adnexetomií (HE+AE) ve 37,4 %, a s omentektomií (HE+AE+OE) pouze ve 26,7 %. CHT byla podána v 86,9 %, radioterapie (RT) byla aplikovaná v 66 %, z toho v 80,2 % byla ozářena abdominopelvická dutina. Metody statistického zpracování: Statistické hodnocení přežití bylo provedeno podle Kaplan-Meiera. Výsledky: Při hodnocení všech tří tehdy radikálních léčebných modalit u stadia III (n = 106) bylo 5 a 10leté přežití 57,5% a 46,3%, oproti ostatním kombinacím s přežitím 35,9 % a 28,9 %. Toxicitu léčby jsme nehodnotili, výsledky by nebyly validní. Závěr: abdominopelvické ozáření u rizikových pacientek zlepšilo přežití., Introduction: Ovarian carcinoma is high mortality disease despite of radical surgery and introduction of chemotherapeutical regimes with platinum derivates. Recurrence is more then 60 % (first of all intraperitoneal) and majority of patients dies within 5 years. Objective: The aim of retrospective study was to evaluate whole abdomino-pelvic radiotherapy for stadium III patients followed after surgery and 6 series of chemotherapy (CHT). Study group: Group of 211 ovarian cancer patients were treated in Department of oncology of Hospital Ceske Budejovice between 1985 and 1999, retrospective evaluation was made for 206 of them. 106 of them was with stadium III (51,4 %). Total hysterectomy and bilateral adnexectomy (HE+AE) was in 37,4 % and total hysterectomy and bilateral adnexectomy and omentectomy (HE+AE+OE) only in 26,7 % (n = 206). CHT was administered in 86,9 %, radiotherapy (RT) in 66 % (whole abdominal radiotherapy in 80,2 %). Methods: Kaplan-Meier method was used for survival evaluation. Results: Overall survival rates at 5 resp. 10 years for stadium III were 57,5 % resp. 46,3 % for all three at that time radical treatment combinations versus 35,9 % resp. 28,9 % to the rest. Toxicity was not evaluated because of nonsignificant results. Tento článek se zabývá karcinomem prostaty v jeho nejčastější podobě, přináší stručný přehled jeho patofyziologie a diagnostiky. Uvádí také jednotlivé strategie hormonoterapie, možnosti chemoterapie, poukazuje na významnou roli androgenních receptorů při léčbě hormonálně dependentního i rezistentního onemocnění, v neposlední řadě zohledňující kvalitu života. Pro přehlednost jsou zde uvedené i nové nadějné preparáty, jejichž využití je zatím omezené ve fázi klinického zkoumání., This article focuses on prostate cancer in its most common form and presents a brief overview of its pathophysiology and diagnostics. The individual strategies of hormonal therapy and chemotherapy are outlined. It highlights the important role of androgen receptors in hormone dependent and resistant cancer treatment, taking into account the quality of life of the patient. Onemocnění z depozice lehkých řetězců (light chain deposition disease – LCDD) patří mezi vzácná systémová onemocnění způsobená monoklonální proliferací terminálně diferencovaných B-lymfocytů s produkcí volných lehkých řetězců imunoglobulinu a jejich ukládáním především v ledvinách či v dalších tkáních. Náplní sdělení je poukázat na úskalí diagnostiky této klinické jednotky a na příkladu série 4 nemocných s LCDD předložit důkazy o vysoké účinnosti léčby s bortezomibem z pohledu hematologické i orgánové terapeutické odezvy. Z našeho hodnocení vyplývá, že léčba bortezomibem je u LCDD velmi rychlá a účinná, bývá však při intenzivním dávkovacím schématu doprovázena řadou nežádoucích účinků. Z nich je nejzávažnější periferní neuropatie, ohrožující i případné setrvání nemocného v léčbě. Méně intenzivní režimy (tzv. bortezomib weekly) představují alternativu s nižším výskytem nežádoucích účinků. Vysokodávkovaná chemoterapie s podporou autologní transplantace krvetvorných buněk je u vhodných kandidátů doporučeným a relativně bezpečným postupem. Orgánová léčebná odezva se významně opožďuje za hematologickou odezvou a zůstává otázkou, zdali je postižení tkání depozity lehkých řetězců plně reverzibilní. Klíčová slova: autologní transplantace krvetvorných buněk – bortezomib – hematologická léčebná odezva – onemocnění z depozice lehkých řetězců – orgánová léčebná odezva, Light chain deposition disease (LCDD) is a rare systemic condition caused by monoclonal proliferation of terminally differentiated B-lymphocytes with production of free light chains and their deposition in kidneys or other organs. The aim of our study is to show the pitfalls of the diagnostics, and to demonstrate the effect of bortezomib-based therapy on a series of 4 patients with LCDD, from the point of hematological and organ therapeutic response. We include that bortezomib based treatment provides rapid and effective hematological response. It is, however, often accompanied by adverse events, especially within intensive treatment schedules. The most serious adverse effects includes peripheral neuropathy, which might be dose or treatment-limiting. Less intensive regimens („bortezomib weekly“) suggest an alternative with expectation of lower incidence of adverse effects. Autologous stem cell transplantation is a recommended and relatively safe approach in convenient candidates. Organ response is significantly delayed after hematological response, and organ damage by light chain deposits might not be fully reversible. Gastrointestinální stromální nádory jsou nejčastější mezenchymální nádory gastrointestinálního traktu. Onkogenní aktivující mutace receptorových tyrozinkináz KIT (75–80 %) a PDGFRA (10 %) mají zásadní význam v molekulární patogenezi GISTu. Kompletní chirurgická resekce je standardem v léčbě lokalizovaných GISTů, avšak u třetiny pacientů dochází k recidivě do 2 let po resekci. Novější léčebné strategie a zavedení cílené léčby přináší signifikantně delší přežití pacientů s GISTem. Imatinib, inhibitor receptorových tyrozinkináz, a jeho zavedení v léčbě GISTu je nejvýznamnějším krokem a má význam v léčbě 1. linie neresekabilních nebo pokročilých GISTů, rovněž v léčbě adjuvantní po resekci GISTu s vysokým rizikem rekurence. Sunitinib je indikován v léčbě druhé linie. Regorafenib byl nedávno schválen pro léčbu třetí linie pacientů s progresí nebo intolerancí léčby imatinibem a sunitinibem. Včasná identifikace skupiny pacientů rezistentních na terapii, pacientů s významnou toxicitou léčby, je důležitým cílem multidisciplinární personalizované léčby pacientů s GISTem., Gastrointestinal stromal tumors (GISTs) are the most common mesenchymal tumors of gastrointestinal tract. Oncogenic activating mutations in the receptor tyrosine kinases, KIT (75–80 %) and platelet- derived growth factor receptor alpha (PDGFRA, 10 %) play a crucial role in the molecular pathogenesis of GIST. Complete surgical resection is the standard treatment for localized GISTs, however, recurrence is observed in approximately a third of patients 2 years following resection. Better treatment strategies and targeted therapies have improved survival rates for patients with GIST significantly. Imatinib, the receptor tyrosine kinase inhibitor, has revolutionized the therapeutic landscape for GIST patients and remains the mainstay first-line clinical option for both unresectable and advanced GISTs, as well as for adjuvant treatment after resection of high risk GISTs. Sunitinib is indicated as second-line therapy. Regorafenib has been approved recently as third-line treatment of patients who progressed on or are intolerant to prior imatinib and sunitinib.
