Cíl: Cílem výzkumu bylo popsat zkušenosti hospitalizovaných dětí (adolescentů), doprovodu hospitalizovaných dětí předškolního a mladšího školního věku a všeobecné sestry s etickými problémy při hospitalizaci dítěte na standardním dětském lůžkovém oddělení. Metodika: Byl proveden kvalitativní výzkum. Pro sběr dat byl použit nestandardizovaný rozhovor. Výzkumný soubor tvořilo osm informátorů, osoby doprovázející dítě při hospitalizaci (n = 5), hospitalizované děti v adolescentním věku (n = 2) a dětská sestra (n = 1). Na základě kvalitativní analýzy dat byly vytvořeny 4 kategorie (1. informovaný souhlas, 2. přítomnost blízkého člověka, 3. srozumitelné informace a 4. péče sester), které byly dále členěny do podkategorií. Výsledky: Mezi nejčastější etické problémy při hospitalizaci dítěte na standardním lůžkovém oddělení respondenti uvedli: problémy s informovaným souhlasem v neúplných rodinách, nedostatečnou informovanost o nemoci a zdraví dítěte, deficit v oblasti edukace, nedostatek komunikace se sestrou a nedostatečnou péči. Adolescenti považovali za etický problém při hospitalizaci pocit malého respektování vlastní autonomie u rozhodování o sobě sama, potřebu většího soukromí a respektu k pocitům studu. Závěr: Většině etických problémů lze předcházet zvládnutím efektivní komunikace a dostatečné informovanosti a to již před hospitalizací dítěte v ambulanci praktického lékaře pro děti a dorost, případně v odborné ambulanci., Objective: The aim of the research was to describe the experience of hospitalized children (adolescents), accompaniment of hospitalized children preschool and younger school age and general nurses with ethical problems during the hospitalization of children on the standard children's ward. Methods: Qualitative research was conducted. For data collection was used nonstandardized interview. The research group consisted of eight informants, persons accompanying the child during hospitalization (n = 5), hospitalized children in adolescence age (n = 2) and child nurse (n = 1). Based on qualitative data analysis were created four categories (1. informed consent, 2. the presence of a loved one, 3. understandable information 4. care of nurses ), which were further divided into subcategories. Results: The most common ethical problems in the child's hospitalization on the standard ward respondents were stated: problems with informed consent in single-parent families, lack of information about the child's disease and child's health, the deficit in the area of education, lack of communication with the nurse and lack of care. Adolescents considered as the ethical problem during hospitalization the feeling of little respect for autonomy in deciding of themselves, the need for greater privacy and respect for the feelings of shame. Conclusion: Majority of ethical problems can be prevented by mastering effective communication and sufficient awareness, and this can be achieved before the hospitalization of child in the ambulance of GP for children and adolescents, or in specialist clinic., Žaneta Beránková, and Literatura
Cieľ: Cieľom štúdie bolo analyzovať faktory ovplyvňujúce dentálnu hygienu, medzi ktoré patrí konzumácia sladkostí a sladených nápojov u školskej mládeže z hľadiska veku a pohlavia a zároveň analyzovať výskyt dentálnej hygieny počas dňa. Metodika: Súbor respondentov tvorilo 1187 žiakov (620 dievčat a 567 chlapcov) vo veku 11, 13 a 15 rokov v oblasti stredného Slovenska. Zber údajov prebiehal v období máj 2010 až jún 2010. Na zber údajov bol použitý štandardizovaný dotazník vytvorený pre potreby HBSC štúdie (Health Behaviour in School – aged Children). Výsledky boli vyhodnotené Chí-kvadrát testom. Výsledky: Sladkosti a kolu v jednotlivých vekových kategóriách konzumuje každý deň alebo viac krát za deň približne tretina respondentov. Výskyt umývania zubov viac ako raz denne je približne rovnaký vo všetkých vekových kategóriách od 53% u 11 ročných chlapcov do 64% u 15 ročných dievčat. Vo všetkých vekových kategóriách dievčatá udávajú častejšie umývanie zubov v porovnaní s chlapcami. Záver: Výsledky poukazujú na zvýšenú konzumáciu sladkostí a koly počas dňa u školskej mládež stredného Slovenska, čo môže spôsobovať zvýšenú kazivosť zubov. Dentálna hygiena je v našom súbore respondentov nedostačujúca. Na základe získaných výsledkov je možné konštatovať, že potreba zvyšovania úrovne vedomostí nielen detí, ale aj rodičov o zásadách správneho stravovania a o zásadách dentálnej hygieny je neodmysliteľnou súčasťou výchovy k zdraviu a preventívnych opatrení v školskom veku., Aim: The aim of the study was to analyse the factors affecting dental hygiene such as consumption of sweets and sweet soft drinks in school-aged pupils in terms of their age and gender and at the same time to analyse the occurrence of dental hygiene during the day. Methods: The set of respondents consisted of 1187 pupils (620 girls and 567 boys) at the age of 11, 13 and 15 years in the area of central Slovakia. The data collection was conducted in the period of May 2010 to June 2010. For data collection was used standardized questionnaire for HBSC studies (Health Behaviour in School–aged Children). The results were analysed by Chi-square test. Results: Approximately one third of respondents in each category consumes sweets and Coca-cola every day or several times a day. The occurrence of cleaning the teeth more than once a day was equally proved in all age groups ranging from 53% in 11 years old boys to 64% in 15 years old girls. The girls of all age groups indicate more frequent cleaning of teeth in comparison with boys. Conclusion: The results show increased consumption of sweets and Cocacola during the day in the school youth from central Slovakia, which may cause an increase in tooth decay. Dental hygiene in our set of respondents is insufficient. The aim of the study was to establish the frequency of hypovitaminosis D in children with type 1 diabetes mellitus (T1D), its influence on biochemical and densitometric parameters and the relation to diabetic nephropathy. 58 children with T1D at the age 9–19 years were enrolled to the study. Vitamin D concentration less than 30 ng/ml was considered as insufficient. 37 children (63.79%) had vitamin D level under 30 ng/ml, from these 19 subjects (32.7%) had vitamin D level under 20 ng/ml and 2 subjects (3.44%) under 10 ng/ml. Children with vitamin D deficiency had significantly lower magnesium concentration and lower Z score of lumbar spine (−1.34 ± 1.24 vs. −0.30 ± 1.21, p = 0.01) compared to diabetics with sufficient vitamin D concentration. No significant difference was found in parameters calcium, phosphorus or glycosylated hemoglobin. Patients with diabetic nephropathy (n = 18) showed no significant difference in vitamin D, glycosylated hemoglobin or Z score of lumbar spine compared to the patients without nephropathy (n = 40). Subjects with nephropathy had significantly longer diabetes duration, significantly higher cholesterol and triacylglycerol concentration. Cieľ: Cieľom výskumu bolo u detí s bronchiálnou astmou zistiť vplyv ochorenia a stupňa kontroly astmy na kvalitu života dieťaťa a kvalitu života jeho rodičov. Metodika: Na zber empirických údajov sme použili štandardizované dotazníky na kvalitu života – Paediatric Asthma Quality of Life Questionnaire (PAQLQ) a Paediatric Asthma Caregiver´s Quality of Life Questionnaire (PACQLQ) a na hodnotenie kontroly astmy – Asthma Control Questionnaire (ACQ). Výsledky boli analyzované pomocou metód deskriptívnej a induktívnej štatistiky. Výsledky: Vo vzorke detí (n = 72) sme zistili, že 27 (37,5 %) detí malo astmu pod kontrolou a 28 detí (38,9 %) bolo v pásme nekontrolovanej astmy. Celkové skóre ACQ (1,24 ±1,07) potvrdilo stredný stupeň kontroly astmy. Celkové skóre dotazníka PAQLQ u detí bolo 5,34 ±1,16, čo indikuje, že deti s astmou vyjadrujú dobrú kvalitu života. U detí ochorenie viac vplývalo na aktivity ako na emocionálne prežívanie a výskyt symptómov. Celkové skóre dotazníka PACQLQ u rodičov bolo 4,53 ±1,07, u rodičov nebol zaznamenaný rozdiel v doménach emocionálne prežívanie a aktivity. Výskum potvrdil, že stupeň kontroly astmy štatisticky významne ovplyvňuje kvalitu života dieťaťa aj jeho rodičov, pričom čím je vyššia kvalita života dieťaťa, tým je vyššia kvalita života rodičov. Záver: Cieľom komplexného manažmentu liečby je dosiahnuť čo najoptimálnejší stupeň kontroly astmy, a tým vytvoriť predpoklady k vyššej kvalite života dieťaťa a jeho rodičov. Využívaním posudzovacích nástrojov na hodnotenie kvality života je možné identifikovať závažný dopad choroby na každodenný život detí aj rodičov., Aim: The aim of the research was to find out the impact of the disease and the degree of asthma control on quality of life in children with bronchial asthma and quality of life in their parents. Methods: To collect empirical data we have used standardized questionnaires concerning quality of life – Paediatric Asthma Quality of Life Questionnaire (PAQLQ) and Paediatric Asthma Caregiver’s Quality of Life Questionnaire (PACQLQ) and for the assessment of asthma control we have used questionnaire Asthma Control Questionnaire (ACQ). The results were analysed using the methods of descriptive and inductive statistics. Results: In the sample of children studied (n = 72) we have found that 27 children (37.5%) have their disease under control and 28 children (38.9%) were in the range of uncontrolled asthma. Total ACQ score (1.24±1.07) confirmed medium degree of asthma control (partially controlled asthma). Total score of PAQLQ questionnaire in children (5.34±1.16) indicates that children with asthma represent good quality of life. Bronchial asthma in children had more significant influence on activities than on emotions and incidence of symptoms. Total score of PACQLQ questionnaire in parents was 4.53 ±1.07 and we have not registered differences between domains of emotions and activities. The research has proved that degree of asthma control has statistically significant influence on the quality of life of the child and his parents, whilst the higher the quality of life of the child, the higher the quality of life of parents. Conclusion: The aim of comprehensive management of asthma therapy is to achieve the most optimal level of asthma control and thus to establish conditions for higher quality of live of the child and his parents. Cíl: Cílem studie bylo zjistit, jaký je výskyt obezity u dětí ve školním věku a zmapovat faktory, které se na vzniku obezity u dětí podílí. Metodika: Výzkumný soubor tvořilo 228 ţáků 4. a 9. tříd navštěvujících základní školy v městě Ostrava. Průzkumné šetření probíhalo pomocí dotazníkového šetření a měření antropometrických parametrů dětí. Dotazník byl zaměřen na stravovací zvyklosti dětí a fyzickou aktivitu. Měřením se zjišťovaly parametry dětí: tělesná hmotnost, tělesná výška, obvod pasu a paţe. Poté probíhalo měření koţních řas. Šetření probíhalo v období od listopadu 2011 do dubna 2012. Výsledky: Na základě hodnot BMI bylo zjištěno, ţe se obezita vyskytuje z celkového počtu 228 dětí u 12,28 % dětí školního věku a 9,21 % dětí má nadměrnou hmotnost. Z celkového zkoumaného vzorku dětí bylo zjištěno, ţe je 16,67 % obézních chlapců a 8,33 % obézních dívek. Nadměrnou hmotnost má 12,04 % chlapců a 6,67 % dívek. Vyšší procento obézních dětí bylo zastoupeno u dětí 4. tříd. Z průzkumu vyplynulo, ţe v průměru mají vyšší zastoupení tělesného tuku ve všech věkových kategoriích dívky neţ chlapci. Šetřením bylo zjištěno, ţe děti mají nedostatek pohybové aktivity. Nejčastější činností dětí ve volném čase je sledování televize a počítačové hry. Závěr: Prevalence obezity se zvyšuje nejen u dospělých, ale stále častěji u dětské populace. Obezita se stává celosvětovým problémem, který přináší do ţivota dětí mnoho problémů nejen po stránce fyzické, ale také psychické. Dětský věk je důleţitým obdobím pro včasné zahájení boje proti obezitě. Prevence zaujímá nejdůleţitější místo v předcházení obezity u dětí a problémům, které jim přináší., Aim: To determine the prevalence of obesity among school children and to map the factors contributing to the development of childhood obesity. Methods: The research sample consisted of 228 grade 4 and 9 pupils from primary schools in the city of Ostrava, Czech Republic. The survey was conducted using a questionnaire and anthropometric measurements. The questionnaire was concerned with children’s eating habits and physical activity. The following parameters were measured: body weight, body height, waist and arm circumference. Skinfold thickness was also assessed. The study was carried out between November 2011 and April 2012. Results: Based on BMI values, obesity was found in 12.28% of the 228 school children. A total of 9.21% of children were overweight. In the sample, there were 16.67% of obese boys and 8.33% of obese girls. Moreover, 12.04% of boys and 6.67% of girls suffered from overweight. The percentage of obese children was higher in the grade 4 group. The survey showed that, on average, girls had a higher proportion of body fat than boys in all age categories. The research also revealed a lack of physical activity in school children. The most frequent leisure activities were watching TV or playing computer games. Conclusion: The prevalence of obesity is increasing not only in the adult population but also in children. Obesity is becoming a global issue that causes many problems in the life of children, both physical and mental. Childhood is the right time to start a fight against obesity. Cíl: Práce se zabývá porovnáním posturální stability u dvou vybraných souborů probandů, a to u dětí s obezitou a u atletů. Metody: U obou skupin bylo provedeno posturografi cké vyšetření pomocí dvou testů – Limity stability v náklonu tzv. Limits of Stability (LOS) a Senzorický test – Sensory Organisation Test (SOT). Na základě takto získaných výsledků byly porovnány a statisticky vyhodnoceny rozdíly mezi jednotlivými soubory. Výsledky: Práce prokázala významný vliv obezity na individuální posturální stabilitu jedince. Prokázali jsme statisticky signifi kantní rozdíly jak v reakčním čase (RT) a maximální exkurzi při testu LOS, tak i ve volbě pohybových strategií pro udržení posturální stability při testu SOT. Závěry: U dětí s obezitou je, v rámci fyzioterapie, vhodné zaměřit pohybovou aktivitu na zlepšení celkové postury a posturální stability a to např. využitím funkčního tréninku., Aim: The paper deals with the comparison of postural stability in two selected groups of probands: children with obesity and athletes. Methods: Posturographic examination was carried out in both groups through two tests – Limits of Stability (LOS) and Sensory Organization Test (SOT). Variations between individual groups were compared and statistically evaluated based on the acquired results. Results: Obesity as well as overall posture has a signifi cant impact on postural stability. We demonstrated statistically signifi cant variations in reaction time (RT) and maximum excursion in LOS test, and in SOT test in the choice of movement strategies for maintaining postural stability. Cieľ: Cieľom prieskumu bolo zistiť prevalenciu a rizikové faktory hypertenzie bieleho plášťa (WCH) u detí. Metodika: Formou retrospektívnej štúdie sme analyzovali zdravotné záznamy 140 detí (36 dievčat, 104 chlapcov) s vysokým krvným tlakom nameraným v kardiologickej ambulancii, u ktorých bolo vykonané 24-hodinové ambulantné monitorovanie krvného tlaku (ABPM) a určený typ hypertenzie. Hypertenzia bola diagnostikovaná, keď bol systolický a/alebo diastolický krvný tlak meraný auskultačnou technikou ≥ 95. percentil pre pohlavie, vek a výšku. WCH bola definovaná ako priemerná hodnota krvného tlaku v ambulancii nachádzajúca sa v percentilovom pásme hypertenzie a hodnota tlaku krvi počas 24-hodinového monitorovania ambulantného tlaku nachádzajúca sa v pásme normálnych hodnôt krvného tlaku. Výsledky: V skupine 140 detí bola potvrdená prítomnosť WCH u 51 detí (36,4 %) – 13 dievčat a 38 chlapcov. Z celkového súboru sa WCH vyskytovala u 36,1 % zaradených dievčat a 36,5 % zaradených chlapcov. Systolický TK sa v kardiologickej ambulancii nachádzal nad 95. percentilom, diastolický TK pod 90. percentilom. Väčšina pacientov s WCH bola vo veku 15 – 19 rokov (80,4 %) a v pásme od 3. do 90. percentilu BMI (72,5 %). U pacientov prevládala negatívna rodinná anamnéza hypertenzie (54,9 %) a zriedkavé návštevy kardiologickej ambulancie (2-4x). Krvný tlak v ambulancii merala deťom výlučne sestra. Záver: Zistenie prevalencie WCH a jej rizikových faktorov je u detí významné, pretože prevažuje názor, že v detskom veku je WCH predispozíciou k trvalej hypertenzii. Na základe zistení rizikových faktorov, môžeme vykonávať prevenciu WCH., Aim: The aim of this study was to find out the prevalence and risk factors of white coat hypertension (WCH) in children. Methods: We realized the retrospective study and analyzed records of 140 children (36 girls, 104 boys) with high blood pressure measured in cardiology outpatients' clinic, to whom a 24-hour ambulatory blood pressure monitoring (ABPM) was made and the type of hypertension was specified. Hypertension was diagnosed when systolic and/or diastolic blood pressure measurements with auscultatory technique were ≥ 95th percentile for sex, age and height. White coat hypertension (WCH) has been defined as mean blood pressure (BP) readings in outpatients' clinic being located in the hypertensive range while 24-hour ambulatory blood pressure monitoring is in the normal range. Results: In the group of 140 children the presence of WCH was confirmed in 51 children (36,4 %) - 13 girls and 38 boys. WCH occurred in 36,1 % of girls and 36,5 % of boys in the total sample of respondents. Systolic BP in cardiology outpatients' clinic reached the value above the 95th percentile, diastolic BP below the 90th percentile. The most patients with WCH were 15 – 19 years old (80,4 %) and in 3rd - 90th percentile of BMI (72,5 %). Patients prevailed negative family history of hypertension (54,9 %) and fewer visit (2-4 times) of cardiology outpatients' clinic. Blood pressure of children was measured exclusively by a nurse in this outpatients' clinic. Conclusion: Determination of the prevalence of WCH and its risk factors in children is important because of prevailing opinion that WCH in children is predisposition in permanent hypertension. The purpose of study was to analyze clinical and genetic polymorphism of Duchenne/Becker progressive muscular dystrophies among patients with neuromuscular diseases in Uzbekistan. 106 male patients with progressive pseudohypertrophic forms of muscular dystrophy were retrospectively and prospectively analyzed in the period from 2004 till 2014: 93 patients with Duchenne PMD aged from 3 years to 18 years and 13 patients with Becker PMD aged from 10 years to 25 years, who had been examined in the medico-genetic consulting department of the Republican Center “Mother and Child Screening” of Tashkent city. Many aspects of surgical treatment of intestinal malrotation in children remain to be debatable. In the opinion of the majority of the specialists, surgical treatment is required after the diagnosis taking into account serious complications of intestinal malrotation. Purpose. The purpose of this research was to conduct an analysis of surgical tactics and operative treatment method for isolated and associated intestinal malrotations in children. Material and methods. We observed 123 children at the age of one day to 15 years with malrotation during the period of 2002 to 2013. Results. We presented the data from observing 123 children at the age of one day to 15 years with various clinical-anatomic forms of intestinal malrotation over from 2002 to 2013. In 62 patients (50.4%), the evidences of the high intestinal obstruction were prevalent, while 61 (49.6%) showed signs of low intestinal obstruction. 116 patients (94,3%) were given operative intervention: radical – 95(81,9%) and palliative – 21 (18,1%). In 56 % of the cases, various simultaneous surgeries were required. There are proposed differential approaches in relation to anatomic form of malrotation and possibility of the fixation of large intestine in the physiological position. Conclusion. The results obtained from the operative treatment are presented. The acute destructive pneumonias (ADP) occupy up to 80% of the total number of pneumonias. They require constant improvement of treatment strategy. Nowadays the use of surfactants is a part of most treatment protocols. The aim was to study the features of the solid phase bronchoalveolar lavage in children with the ADPs in the dynamics of complex treatment with exogenous surfactant. Material and methods: We examined 39 patients of contaminated surgery. We identified 2 groups of patients. The patients of first group (n=27) had pulmonary pleural form of ADP, the second group (n=12) had pulmonary form of ADP. All patients got classical treatment and the earlier draining of pleural cavity. We used as an antiseptic reamberin 1.5% by 10 ml/kg and endobronchially injected exogenous surfactant Bl in dose12 mg/kg body weight a day, 6 mg/kg every 12 hours. All the children were made a bronchoscopy to obtain BAL to study the crystallization properties. The solid phase of BAL was studied by method of cuneal dehydration. Results: All facies before treatment were divided into two groups according to classification of facies of biological fluids. Only the facies of the second and the third types were detected there. It was revealed that the sizes of the zones of the facies were different in the comparison groups before treatment and after. And the level of crystalline structures and amorphous aggregates were different in the groups with different degrees of inflammation. Conclusion: So, we can assume that the change in surfactant system is characterized by changes in the morphological structure of solids phases of BAL.
