Zdůvodnění: Nejvýhodnější hodnota snížení systolického krevního tlaku vedoucí ke snížení kardiovaskulární morbidity u pacientů bez diabetu není dosud známa. Metodika: Autoři studie SPRINT randomizovali 9 361 osob se systolickým krevním tlakem ≥ 130 mm Hg a se zvýšeným kardiovaskulárním rizikem, ale bez diabetu, na skupinu s cílovým systolickým krevním tlakem < 120 mm Hg (intenzivní léčba) a na skupinu s cílovým systolickým tlakem < 140 mm Hg (standardní léčba). Primárním kombinovaným cílem byly infarkt myokardu, jiné akutní koronární syndromy, cévní mozková příhoda, srdeční selhání, nebo úmrtí z kardiovaskulárních příčin. Výsledky: Po jednom roce studie byl průměrný systolický krevní tlak 121,4 mm Hg v intenzivně léčené skupině a 136,2 mm Hg ve standardně léčené skupině. Intervence byla přerušena brzy po dosažení mediánu sledování 3,26 let pro významně nižší výskyt primárního kombinovaného cíle v intenzivně léčené skupině oproti standardně léčené skupině (1,65 % ročně vs 2,19 % ročně; HR 0,75; 95% CI 0,64–0,89; p < 0,001). Celková mortalita byla též významně nižší v intenzivně léčené skupině (HR 0,73; 95% CI 0,60–0,90; p = 0,003). Výskyt závažných nežádoucích účinků – hypotenze, synkopy, abnormality elektrolytů a poškození nebo selhání ledvin, ale nikoliv pádů se zraněním – byl vyšší v intenzivně léčené skupině oproti standardně léčené skupině. Závěry: Snaha o snížení krevního tlaku u pacientů s vysokým rizikem kardiovaskulární příhody, ale bez diabetu, na hodnoty < 120 mm Hg vedla v porovnání se snížením systolického krevního tlaku < 140 mm Hg k nižšímu výskytu fatálních a nefatálních větších kardiovaskulárních příhod i celkové mortality, i když některé nežádoucí účinky byly častější ve skupině pacientů intenzivně léčených. Methods: We randomly assigned 9 361 persons with a systolic blood pressure of 130 mm Hg or higher and an increased cardiovascular risk, but without diabetes, to a systolic blood-pressure target of less than 120 mm Hg (intensive treatment) or a target of less than 140 mm Hg (standard treatment). The primary composite outcome was myocardial infarction, other acute coronary syndromes, stroke, heart failure, or death from cardiovascular causes. Results: At 1 year, the mean systolic blood pressure was 121.4 mm Hg in the intensive treatment group and 136.2 mm Hg in the standard-treatment group. The intervention was stopped early after a median follow-up of 3.26 years owing to a significantly lower rate of the primary composite outcome in the intensive-treatment group than in the standard-treatment group (1.65 % per year vs 2.19 % per year; hazard ratio with intensive treatment, 0.75; 95% confidence interval (CI) 0.64 to 0.89; p < 0.001). All causes mortality was also significantly lower in the intensive-treatment group (hazard ratio, 0.73; 95% CI 0.60 to 0.90; p = 0.003). Rates of serious adverse events of hypotension, syncope, electrolyte abnormalities, and acute kidney injury or failure, but not of injurious falls, were higher in the intensive-treatment group than in the standard-treatment group. Conclusions: Among patients at high risk for cardiovascular events but without diabetes, targeting a systolic blood pressure of less than 120 mm Hg as compared with less than 140 mm Hg, resulted in lower rates of fatal and nonfatal major cardiovascular events and death from any cause, although significantly higher rates of some adverse events were observed in the intensive-treatment group. Úvod: Sympatická kožní odpověď (SSR) je jednoduchý a široce dostupný test sudomotorických funkcí. Jedná se o polysynaptický reflex s variabilní aferentací a eferentní částí, zprostředkovanou tenkými nemyelinizovanými vlákny typu C. Cílem práce bylo zhodnocení validity SSR v diagnostice neuropatie s postižením tenkých senzitivních nervových vláken. Metodika: SSR byla vyšetřena na horních a dolních končetinách u 69 pacientů s bolestivou periferní neuropatií. U 33 pacientů této skupiny šlo o izolovanou neuropatii tenkých nervových vláken (NTV). Třicet šest pacientů mělo kombinované postižení vláken tenkých a silných. Nálezy byly srovnány se souborem 89 zdravých kontrol. Hodnocena byla výbavnost odpovědí elektrickým proudem a prudkým inspiriem a jejich reprodukovatelnost, amplituda a latence. Výsledky: Hodnocení latencí ani amplitud odpovědí neprokázalo jasný přínos pro diagnostiku postižení tenkých nervových vláken. Nejspolehlivější typ SSR abnormity je tedy nevýbavnost odpovědí. Postižení tenkých autonomních vláken však bylo v našem souboru pomocí SSR prokazatelné pouze u malé části pacientů se senzitivní neuropatií – senzitivita dosáhla necelých 10 % u pacientů s čistou NTV a 33 % u pacientů s kombinovaným postižením vláken tenkých a silných (u nichž je současně pravděpodobnější těžší stupeň postižení vláken tenkých). Závěr: Při zohlednění nízké senzitivity není SSR optimální metoda využitelná jako jediný diagnostický test neuropatie tenkých vláken., Introduction: Sympathetic skin response (SSR) is a simple and widely available test of sudomotor functions. Afferent part of this polysynaptic reflex is variable, while the efferent direction of the reflex arch is via thin unmyelinated C fibres. No reliable study of the SSR diagnostic validity in patients with sensory small fiber neuropathy (SFN) has so far been published. In clinical practice, however, we repeatedly meet patients, who have been diagnosed with SFN based solely on abnormal SSR. The aim of the study was to evaluate diagnostic validity of SSR in patients with sensory small fibre neuropathy. Methods: SSR was recorded from palms and soles of 69 patients with painful sensory neuropathy (33 of them with pure SFN and 36 with mixed small and large nerve fibre dysfunction) using electrical stimulation and inspiratory gasp stimuli. Small nerve fiber involvement was confirmed by reduced intraepidermal nerve fiber densities in skin biopsy samples of all cases. The results were compared with those of 89 healthy controls. The outcome for health controls were also used to establish age-stratified normative data. We assessed electric stimuli and sudden inspiration response recall end their reproducibility, amplitude and latency. Results: Both the SSR latencies and amplitudes showed very low diagnostic validity in small fibre neuropathy patients. The absence of SSR response represented the most reliable abnormality. However, using this parameter, dysfunction of small autonomic nerve fibres has only been found in a small part of our sensory neuropathy patients: sensitivity did not exceed 10% in pure SFN patients and 33% in those with mixed small and large nerve fibre dysfunction (where more pronounced small sensory nerve fibre involvement has previously been found). Conclusion: Considering its low sensitivity, SSR should not be used as the only test to confirm sensory small fibre neuropathy. Ačkoliv konvenční techniky zobrazení magnetickou rezonancí mají dominantní postavení při vyšetření centrálního nervového systému, přesto tyto techniky nedokážou dát informaci o funkčních vlastnostech mozkové tkáně. Kromě konvenčních MR technik však existují i MR metody umožňující některé z těchto vlastností zobrazit. Mezi ně můžeme zařadit funkční magnetickou rezonanci, difuzně vážené zobrazování, resp. zobrazení difuzního tenzoru a morfometrickou analýzu. Diabetes mellitus a zejména diabetes mellitus 2. typu představuje jeden z nejvýznamnějších rizikových faktorů kardiovaskulárních onemocnění. Z hlediska ovlivnění KV rizika má enormní význam nejen pozitivní ovlivnění glykemie, ale také léčba diabetické dyslipidemie a arteriální hypertenze. Předložená práce poskytuje analýzu terapie antihypertenzivy a hypolipidemiky u všech diabetiků registrovaných u VZP v období let 2010–2013. V uvedeném období bylo s diagnózou diabetes mellitus léčeno 866 570 pojištěnců VZP, přičemž většinu tvořili diabetici nezávislí na inzulinu. Přibližně polovina pacientů byla dispenzarizována diabetology a polovina lékaři jiných specializací. Z antihypertenziv byli pacienti nejčastěji léčeni léky ovlivňujícími systém renin-angiotenzin, dále pak betablokátory a diuretiky. Preskripce hypolipidemické léčby, zejména preskripce statinů, v souladu s platnými doporučeními, pozvolna narůstá, nicméně i nadále zůstává nedostatečná (v roce 2013 zůstalo bez hypolipidemické léčby 43,6 % diabetiků z ambulancí diabetologů a 51,3 % diabetiků z ambulancí praktických lékařů!). V doplňující analýze dat za rok 2014 jsme zjistili, že v tomto roce bylo léčeno z celkového počtu osob s farmakologickou léčbou diabetu nějakým hypolipidemikem 57,3 % osob. Nedostatečné využití zaznamenává rovněž kombinační hypolipidemická léčba, dosud zastoupená zejména kombinací statinu s fibrátem. Nicméně v následujících letech očekáváme na základě pozitivních výsledků subanalýzy studie IMPROVE-IT nárůst kombinační hypolipidemické terapie, resp. kombinační léčby statiny v kombinaci s ezetimibem. To influence cardiovascular risk there is enormous important not only positive influence of glycemia, but also the treatment of diabetic dyslipidemia and hypertension. The present work provides an analysis of lipid-lowering and antihypertensive therapy for all diabetics registered with General Health Insurance Company in the period 2010–2013. In this time 866 570 patients with diabetes mellitus registered with General Health Insurance Company were treated, the majority of them were diabetics independent on insulin. Approximately half of the patients were observed by dialectologists and half of them by the doctors of other specialization. Out of antihypertensive medication, patients were most often treated by drugs that affect the renin-angiotensin system, as well as beta-blockers and diuretics. Prescription of the lipid-lowering therapy, especially prescription of statins, in accordance with the guidelines, is increasing, but remains insufficient (at 2013 43.6% diabetics treated by dialectologists and 51.3% diabetics treated by GP´s didn´t have lipid lowering therapy). Inadequate use of combination lipid-lowering therapy was recorded too, still represented mainly by combination of statin and fibrate, but in coming years we expect (based on the positive results of the subanalysis IMPROVE-IT study), an increase of combination therapy statin and ezetimibe. Cíl práce: Zhodnotit bezpečnost a účinnost léčby bazálním inzulinem glargin v režimu bazál/bolus v podmínkách běžné klinické praxe v České republice. Soubor a metodika: Projekt LINDA byl neintervenční, multicentrický (n = 255), národní observační projekt, který probíhal v podmínkách běžné klinické praxe. Do sledování bylo zařazeno celkem 4 998 pacientů s diabetes mellitus 1. a 2. typu (DM1T, DM2T) s převážně inzulinovou terapií (99,7 %), po následném zahájení léčby inzulinem glargin v režimu bazál/bolus. Pacienti byli sledováni po dobu 6 měsíců od zahájení této terapie. Primárním cílem projektu bylo sledování četnosti výskytu závažných hypoglykemických příhod při léčbě bazálním inzulinovým analogem glargin (Lantus?) v podmínkách běžné klinické praxe. Sekundárními sledovanými parametry byla změna glykovaného hemoglobinu (HbA1c), glykemie nalačno (FPG), změna tělesné hmotnosti, změna dávek inzulinu, změna počtu hypoglykemických příhod ve srovnání s předchozí léčbou a sledování četnosti nežádoucích účinků. Výsledky: Závažné hypoglykemie byly pozorovány během léčby inzulinem glargin u 0,8 % pacientů. Při srovnání výskytu hypoglykemií s předchozí terapií bylo prokázáno klinicky a statisticky významné snížení jejich četnosti. Procento pacientů s hypoglykemickou příhodou (17,6 %), závažnou (0,8 %) a závažnou noční hypoglykemií (0,3 %) bylo za poslední měsíc léčby s inzulinem glargin v režimu bazál/bolus konzistentně menší ve srovnání s posledním měsícem léčby před zahájením této terapie (42,5 %; 17,6 %; 13,8 % pacientů). U pacientů s DM1T došlo k poklesu všech hypoglykemií z hodnoty 37,80 ? 15,95 příhod/pacient/rok na 8,76 ? 4,38 příhod/pacient/rok (p < 0,001), závažných hypoglykemií z hodnoty 5,64 ? 3,27 příhod/pacient/rok na 0,0396 ? 0,012 příhod/pacient/rok (p < 0,001) a závažných nočních hypoglykemií z hodnoty 3,84 ? 2,04 příhod/pacient/rok na 0,0096 ? 0,003 příhod/pacient/rok (p < 0,001). U pacientů s DM2T poklesla četnost všech hypoglykemií z hodnoty 12,48 ? 7,57 příhod/pacient/rok na 1,68 ? 0,78 příhod/pacient/rok (p < 0,001), závažných hypoglykemií z hodnoty 2,04 ? 0,94 příhod/pacient/rok na 0,0132 ? 0,005 příhod/pacient/rok (p < 0,001) a závažných nočních hypoglykemií z hodnoty 1,32 ? 0,77 příhod/pacient/rok na 0,0048 ? 0,0008 příhod/pacient/rok (p < 0,001). Bylo prokázáno statisticky významné zlepšení metabolické kompenzace při užití inzulinu glargin. U DM1T došlo ke snížení hladiny glykovaného hemoglobinu (HbA1c) z hodnoty 7,74 ? 1,71 % na 6,43 ? 1,39 % (∆ -1,31 ? 0,32 %; p < 0,001) a u DM2T z hodnoty 8,13 ? 1,56 % na 6,72 ? 1,40 % (∆ -1,41 ? 0,28 %; p < 0,001). Statisticky významně (p < 0,001) se zvýšil počet pacientů s hodnotami HbA1c < 5,4 %. Statisticky významný byl také pokles glykemie nalačno (FBG) a hodnot 6bodového glykemického profilu, a to u obou typů diabetu (p < 0,001). Změna terapie a následná léčba inzulinem glargin byla lékaři a pacienty vnímána pozitivně. Závěr: Za podmínek běžné klinické praxe zahájení léčby inzulinem glargin v režimu bazál/bolus u pacientů s předchozí inzulinovou terapií vedlo ke snížení četnosti výskytu hypoglykemických příhod včetně závažných a závažných nočních hypoglykemií a ke zlepšení metabolické kompenzace pacientů s diabetem (snížení glykovaného hemoglobinu, glykemie nalačno a hodnot 6bodového glykemického profilu). Byla zaznamenána větší spokojenost se stávající léčbou ze strany pacientů a lékařů. Patients and methods: The LINDA project was a non-interventional, multicenter (n = 255), national, observational project. A total of 4,998 patients with Type 1 and 2 diabetes mellitus (T1DM, T2DM) with predominantly insulin therapy (99,7 %), after switch on insulin glargine at basal-bolus regimen, were enrolled in this project. The patients were followed up for 6 months after initiation of the therapy with insulin glargine. The primary objective of the project was to investigate the incidence of severe hypoglycemic episodes during the treatment with basal insulin analogue glargine (Lantus?) in a common clinical practice setting. The secondary endpoints were changes in glycosylated hemoglobin (HbA1c) levels, fasting plasma glucose (FPG), body weight, insulin dose, change of number of hypoglycemic episodes in comparison the previous therapy and the frequency of adverse effects. Results: Severe hypoglycaemia were observed during treatment with insulin glargine at 0.8 % patients. When comparing the incidence of hypoglycemia with the previous therapy, we demonstrated a clinically and statistically significant reduction in their frequencies. The percentage of patients with hypoglycemic episodes (17.6 %), severe hypoglycemia (0.8 %) and severe nocturnal hypoglycemia (0.3 %) over the last month of treatment with insulin glargine using the basal-bolus regimen was consistently lower compared to the last month of treatment before initiation of this therapy (42.5 %, 17.6 %, and 13.8 % of the patients, respectively). In patients with T1DM, the incidence of hypoglycemia decreased from 37.80 ? 15.95 episodes/patient/year to 8.76 ? 4.38 episodes/patient/year (p < 0.001) for all hypoglycemic episodes; from 5.64 ? 3.27 episodes/patient/year to 0.0396 ? 0.012 episodes/patient/year (p < 0.001) for severe hypoglycemia; and from 3.84 ? 2.04 episodes/patient/year to 0.0096 ? 0.003 episodes/patient/year (p < 0.001) for severe nocturnal hypoglycemia. In patients with T2DM, the incidence of hypoglycemia decreased from 12.48 ? 7.57 episodes/patient/year to 1.68 ? 0.78 episodes/patient/year (p < 0.001) for all hypoglycemic episodes; from 2.04 ? 0.94 episodes/patient/year to 0.0132 ? 0.005 episodes/patient/year (p < 0.001) for severe hypoglycemia; and from 1.32 ? 0.77 episodes/patient/year to 0.0048 ? 0.0008 episodes/patient/year (p < 0.001) for severe nocturnal hypoglycemia. A statistically significant improvement in the metabolic control was demonstrated when using insulin glargine. The glycated hemoglobin (HbA1c) decreased from 7.74 ? 1.71 % to 6.43 ? 1.39 % (∆ -1.31 ? 0.32 %, p < 0.001) in patients with T1DM, and from 8.13 ? 1.56 % to 6.72 ? 1.40 % (∆ -1.41 ? 0.28 %, p < 0.001) in patients with T2DM. A statistically significant (p < 0.001) increase in the number of patients with HbA1c < 5.4 % was further demonstrated. The decrease in fasting blood glucose (FBG) and 6-point blood sugar profile was also statistically significant in both types of diabetes (p < 0.001). Changes in therapy and subsequent treatment with insulin glargine were perceived positively by both physicians and patients. Conclusion: In the common clinical practice setting, the initiation of treatment with insulin glargine using the basal-bolus regime in patients with previous insulin therapy resulted in a reduction in the incidence of hypoglycemic events, including severe hypoglycemia and severe nocturnal hypoglycemia, and improved metabolic control in patients with diabetes (reduced glycated hemoglobin, fasting glucose values and 6-point blood glucose profile). Greater satisfaction with the current treatment was reported by both patients and physicians. Figurální (neboli neverbální) fluence je schopnost exekutivních funkcí, která nám poskytuje informace o divergentním myšlení, rozdělené pozornosti, plánování a mentální flexibilitě. Zhoršený výkon v testech zaměřených na figurální fluenci nacházíme u pacientů s neurologickým i psychiatrickým postižením. Pětitečkový test (Five Point Test, 5TT) je jeden z neuropsychologických testů, jenž slouží ke zhodnocení figurální fluence. Úkolem probanda je vytvořit co nejvíce obrazců v časovém limitu. Cílem této studie bylo vytvořit normy k 5TT pro českou dospělou populaci. Předkládáme normativní data pro dospělé ve věku od 20 do 85 let (n = 503). Hodnotili jsme počet správných odpovědí a počet perseverací. Počet správných odpovědí je ovlivněn věkem a vzděláním (r = –0,3; resp. 0,4; p < 0,0001), proto jsou normy rozděleny na pásma po 10 letech a dále podle ukončeného vzdělání. Počet perseverací s těmito proměnnými souvisí jen slabě (rs = 0,1; resp. –0,1; p < 0,05). Pohlaví nemá vliv na počet správných odpovědí ani perseverací (t = 0,09; p > 0,9 pro oba skóry)., Figural (or nonverbal) fluency is the ability of executive functions to provide information about divergent reasoning, divided attention, planning and mental flexibility. Impairments of figural fluency have been found in individuals with various neurological or psychiatric diseases. Five Point Test (5TT) is a neuropsychological test that assesses figural fluency. A participant is asked to generate as many unique designs as possible in a certain time limit. The aim of this study was to create Czech population norms for the Five Point Test. Normative data for adult population aged between 20 and 85 years (n = 503) are presented. We assessed the number of correct answers and the number of perseverations. The number of correct answers is influenced by age and education (r = –0.3 and 0.4, respectively, p < 0.0001); for this reason the norms are stratified into ten age ranges and also according to completed education. The number of perseverations correlates with these variables only weakly (rs = 0.1 and –0.1, respectively, p < 0.05). Gender has no impact neither on the number of correct answers nor on perseverative responses (t-test, p > 0.9 for both scores). Cíl: Cílem našeho článku je předložit normativní data pro fonemickou verbální fluenci (VF), pro hlásky K, P, S, i sémantickou VF, pro kategorie zvířata a zelenina. Úvod: Testy verbální fluence jsou jedna z nejrozšířenějších metod hodnocení kognitivní výkonnosti. V českém prostředí dosud neexistuje kvalitní normativní studie testů VF se zaměřením na populaci starších osob, u kterých se tato metoda používá nejčastěji. Metodika: Testy VF jsme administrovali v rámci neuropsychologické baterie 540 zdravým osobám (292 žen, 248 mužů) nad 59 let. V části fonemické VF jsme použili hlásky K, P, S a celkový skór všech hlásek (K + P + S) jako analogii anglického originálu F, A, S v Controlled Oral Word Association Test (COWAT/<span class=""></span>FAS). V rámci sémantické VF jsme hodnotili výkonnost ve dvou kategoriích, zvířata a zelenina. Výsledky: Věk má statisticky významný (p < 0,001) slabý až středně silný vliv na výkon ve VF, reprezentovaný součtem skórů K + P + S (r = –<span class=""></span>0,236), zvířata (r = –<span class=""></span>0,359) a zelenina (r = –<span class=""></span>0,264). Rovněž počet let vzdělání ovlivňuje slabě až středně silně výkon ve VF. Statisticky významně (p < 0,001) pouze ve skórech K + P + S (r = 0,297) a zvířata (r = 0,357). Mezi věkem a skórem zelenina vztah není signifikantní (r = 0,028; p = 0,523). Ve skóru zelenina existuje signifikantní (p < 0,001) rozdíl mezi muži a ženami. Dále předkládáme normativní data pro věkové skupiny 60–<span class=""></span>75 let, 70–<span class=""></span>85 let a 80–<span class=""></span>96 let. Závěr: Výsledky naší studie prokazují statisticky významný vliv věku na všechny uvedené zkoušky VF. Introduction: Verbal fluency (VF) is one of the most frequently used neuropsychological methods for the assessment of cognitive performance in clinical and experimental neuropsychology. However, representative normative data for the Czech population of older and very old adults are so far lacking. Methods: We administered VF as part of neuropsychological battery to 540 (292 women, 248 men) healthy older adults (60–<span class=""></span>96 years of age). In LF, the letters K, P, S and their total score were used as analogous to the original Controlled Oral Word Association Test (COWAT/<span class=""></span>FAS). In SF, we evaluated performance in two categories –<span class=""></span> animals and vegetables. Results: Age was significantly (p < 0.001) related to a sum of K + P + S (r = –<span class=""></span>0.236) as well as animals (r = –<span class=""></span>0.359) and vegetables (r = –<span class=""></span>0.264). However, the association was moderate. Education was also moderately related (p < 0.001) to the sum of K + P + S (r = 0.297) and animals (r = 0.357). However, we did not find a significant relationship between age and vegetables (r = 0.028; p = 0.523). Vegetables were also the only measure that showed highly significant sex differences (p < 0.001). We present normative Czech data for 60–<span class=""></span>75, 70–<span class=""></span>85 and 80–<span class=""></span>96 age groups. Conclusion: The results of our study confirm a significant moderate influence of age and education (with the exception of vegetables for the latter) on all VF measures. There were highly significant sex differences in the vegetable category. Úvod: Posouzení významu PET/CT pro iniciální staging karcinomu jícnu s důrazem na metastatické postižení uzlin a průkaz vzdálených metastáz. Dále bylo cílem práce ověření významu PET/CT vyšetření při hodnocení efektu neoadjuvantní léčby. Metody: V prospektivní studii byl sledován soubor 354 nemocných s karcinomem jícnu, kteří byli vyšetřeni na I. chirurgické klinice LF UP a FN Olomouc v letech 2006–2012. Vstupní PET/CT vyšetření bylo provedeno u 349 nemocných. Analyzovali jsme přínos vyšetření ke stanovení stagingu onemocnění a v závislosti na něm jsme stanovovali strategii léčby. Na základě vstupního PET/CT byl u 102 nemocných zjištěn různý stupeň generalizace a byli indikovaní k paliativní či symptomatické terapii. U 247 pacientů bylo zjištěno omezení nádoru na jícen ev. regionální uzliny. Po posouzení celkového stavu a dle přání pacienta byla dále u 188 nemocných indikovaná neoadjuvantní radiochemoterapie (RCHT), léčbu nedokončilo 32 nemocných. U 156 pak bylo v průměrném odstupu 8,4 týdnů od ukončené neoadjuvantní terapie provedeno kontrolní vyšetření PET/CT. Na základě tohoto vyšetření bylo kompletní vymizení známek nádoru (complete response- CR) zjištěno u 38 vyšetřených (24,4 %), regrese u 89 (57,0 %), stacionární nález u 10 (6,4 %) a progrese u 19 (12,2 %). K chirurgické léčbě bylo indikováno 97 pacientů, z toho resekci jícnu bylo možné provést u 85 a u 12 byl výkon pouze paliativní resp. šlo o exploraci. Výsledky: Vstupní PET/CT vyšetření u 349 pacientů u naprosté většiny správně posoudilo rozsah onemocnění v souhlase s histologicky ověřenou diagnózou. Falešně negativní výsledek byl jen u 5 nemocných (1,43 %). Při hodnocení efektu neoadjuvantní léčby byli pacienti rozděleni do skupin dle nálezu při kontrolním PET/CT po neoadjuvanci a bylo vyhodnocováno jejich přežívání. Byl zjištěn signifikantní rozdíl (p=0,0004) v přežívání mezi skupinami s rozdílnou reakcí na neadjuvantní léčbu (CR (n=38), regrese (n=89), stacionární nález (n=10), progrese (n=19)) bez ohledu na další léčbu po neoadjuvanci, ve prospěch pacientů s lepší reakcí na neadjuvantní léčbu. Signifikantní rozdíl v přežívání byl zaznamenán i mezi skupinou pacientů po neoadjuvanci radikálně operovaných (n=85) a neoperovaných (n=59) ve prospěch operovaných (p=0,003). Nejdelšího průměrného přežívání 38,6 měsíců (medián 29,0 měsíců) bylo dosaženo ve skupině operovaných po neoadjuvanci, kde kontrolní PET/CT ukázalo CR. Nebyl však prokázán signifikantní rozdíl (p=0,587) ve výsledcích mezi skupinami operovaných s rozdílnou reakcí na neadjuvantní léčbu (regrese, stacionární nález). Počet případů v jednotlivých hodnocených skupinách není zatím natolik velký, abychom získané výsledky mohli považovat za jednoznačně průkazné a ve sledování a zařazování dalších nemocných do studie budeme dále pokračovat. Závěr: V práci je dokumentován význam a přínos PET/CT v iniciálním stagingu karcinomu jícnu, zejména v průkazu metastatického onemocnění, a to jak v postižení uzlin, tak v odhalení vzdálených metastáz. PET/CT má velký význam pro stanovení léčebné strategie. Dále byl ověřen význam PET/CT vyšetření při hodnocení efektu neadjuvantní léčby., Introduction: To evaluate the significance of PET/CT for the initial staging of esophageal cancer with emphasis on metastatic lymph node affection and detection of distant metastases. Furthermore, the aim of the work was to analyze the significance of PET/CT examination when evaluating the effect of neoadjuvant therapy. Methods: A set of 354 patients with esophageal cancer treated at the 1st Department of Surgery, University Hospital Olomouc and Medical Faculty at Palacky University in Olomouc between the years 2006−2012 were analyzed in a prospective study. The initial PET/CT examination was performed in 349 patients. We analyzed the benefit of this examination in regard to disease staging and based on the result, therapeutic strategy was determined. The initial PET/CT showed varying degrees of disease generalization in 102 patients, these patients were indicated for palliative or symptomatic therapy. In 247 patients, the disease was limited only to the esophagus and /or regional lymph nodes. After considering the patient’s overall condition and taking into account the wishes of the patient, 188 patients were indicated for neoadjuvant chemoradiotherapy (CRT); 32 patients did not complete this treatment. In 156 patients a follow-up PET/CT scan was performed after an average of 8.4 weeks following completion of neoadjuvant therapy. Based on this examination, a complete response- CR, was observed in 38 patients (24.4%), regression of the tumor in 89 (57.0%), stationary findings were seen in 10 (6.4%), and progression in 19 (12.2%). Ninety-seven patients were indicated for surgical resection; however, esophagectomy was only possible in 85 patients, in the remaining 12 patients only an explorative laparotomy was performed due to disease progression. Results: The initial PET/CT examination performed in 349 patients correctly described the extent of the disease in accordance with the histologically confirmed diagnosis in virtually all patients. A false positive result was seen in only 5 patients (1.43%). When evaluating the effect of neoadjuvant therapy, the patients were divided into groups based on the findings of the follow-up PET/CT after neoadjuvant therapy and their overall survival was evaluated. A significant difference (p=0.0004) in survival was observed between the groups based on the different reactions to neoadjuvant therapy (CR (n=38), regression (n=89), stationary findings (n=10), progression (n=19)) without taking into account the following treatment the patient received after neoadjuvant therapy. Patients who had a better response to neoadjuvant therapy had better survival results. There was also a significant difference in survival between the group of patients who completed neoadjuvant therapy and underwent radical surgical resection (n=85) versus those patients who completed neoadjuvant therapy but did not undergo subsequent surgery (n=59). The operated group had a significantly higher overall survival (p=0.003). The longest mean survival, 38.6 months (median 29.0 months), was achieved by the group of patients who completed neoadjuvant therapy, showed a complete response on the follow-up PET/CT, and underwent surgical resection. However, a significant difference was not observed (p=0.587) between the groups who underwent surgical resection and whose follow-up PET/CT results differed (regression or stationary findings). To date, the number of cases in the individual groups is not great enough to consider the obtained results conclusive, and we will continue to include more patients into the study and continue with the analysis. Conclusion: The work documents the significance and benefit of PET/CT in the initial staging of esophageal cancer, especially in detecting metastatic disease- positive lymph nodes as well as distant metastases. Současné studie naznačují možnou důležitou úlohu melatoninu v Huntingtonově nemoci (HN) a jeho možné terapeutické využití při léčbě této nemoci. HN je dědičné neurodegenerativní onemocnění, které doprovází snižování hladiny melatoninu s postupem onemocnění. U normálních (nenádorových) buněk působí melatonin antiapoptoticky díky svým antioxidačním vlastnostem a schopnosti zabránit aktivaci proteinu p53. Dále melatonin zvyšuje expresi BDNF (brain derived neurotrophic factor) a dalších neuroprotektivních faktorů. Cílem této studie bylo stanovit netoxickou dávku melatoninu pro primární kožní fibroblasty izolované z transgenních miniprasat pro N‑koncovou část lidského mutovaného huntingtinu (TgHD) a popsat efekt tohoto ošetření na tyto buňky vystavené genotoxickému stresu. Buňky byly kultivovány v médiu obohaceném různými dávkami melatoninu. Analýzou proliferačních křivek získaných mikroskopováním živých buněk v pravidelných časových intervalech jsme stanovili efekt různých koncentrací melatoninu.Ukázali jsme, že vyšší dávky melatoninu jsou pro primární prasečí buňky toxické. Je zajímavé, že TgHD buňky byly oproti kontrolním buňkám více citlivé k tímto dávkám melatoninu. Stanovili jsme efektivní dávku melatoninu a současně jsme ukázali její efekt na proliferaci u buněk vystavených genotoxickému stresu. Redakční rada potvrzuje, že rukopis práce splnil ICMJE kritéria pro publikace zasílané do biomedicínských časopisů., According to the recent studies, melatonin might play an important role in Huntington’s disease (HD) and act as a novel therapeutic approach in the treatment of the disease. HD, the inherited neurodegenerative disorder, is accompanied by gradual melatonin reduction as it progresses. Melatonin in normal cells (non‑tumor) has the anti‑apoptotic ability due to its antioxidant property and its ability to prevent the activation of p53. Furthermore, melatonin increases the expression of BDNF (brain derived neurotrophic factor) and other neuroprotective factors. The aim of this study was to evaluate the nontoxic dose of melatonin for primary skin fibroblasts isolated from minipigs transgenic for the N‑terminal part of human mutated huntingtin (TgHD), and the effect of melatonin treatment to these cells exposed to genotoxic stress. Cells were cultured in medium supplemented with different doses of melatonin. Using time lapse microscopy, we estimated the effect of decreasing melatonin concentrations by analyzing the proliferation curves. We show that higher doses of melatonin are toxic for primary porcine fibroblasts. Interestingly, TgHD cells were more sensitive to these doses of melatonin treatment than wild type cells. Introduction: We studied influence of mud-bath on bone status in male Wistar rats with subchronic arthritis. Methods: Arthritis was induced by 2 subplantar injections of Freund’s adjuvans with heat-killed Streptoccocus pyogenes into paw. Groups: intact (int) on chippings; (con) arthritis on chippings; (san38) arthritis on hot sand; (mu38) arthritis on hot mud; (mu21) arthritis on mild mud. Bone mineral density (BMD, g/cm2) was measured by dual energy X-ray absorptiometry and femurs were tested biomechanically. Bone markers osteocalcin (OC), PINP and CTX were analysed in bone. Results: BMD of right femur decreased vs. left in san38 (p = 0.030) and mu38 (p = 0.047). Fracture load of right/left femur (N) decreased in experimental groups, significantly in san38 (p = 0.05). Fracture threshold of neck decreased in right vs. left in experimental groups, but significantly in san38 (p = 0.05). OC decreased in mu38 vs. con (1.84 ± 0.14/2.62 ± 0.23). PINP decreased in int vs. san38 (p = 0.005) and mu21 (p < 0.001). CTX decreased in int vs. mu38 (p = 0.006) and mu21 (p = 0.005). Conclusion: The hot bath appears indifferent in relation to osteoporosis, while cold mud-bath shows good effect on bone metabolism.
