BACKGROUND: Radio frequency (RF) and chemical peels have been used for nonablative skin rejuvenation. Both of these cause collagen remodeling in the dermis and neo-collagen formation resulting in facial rejuvenation. There is limited literature on the evaluation of collagen remodeling by objective methods. OBJECTIVE: To compare the benefits of monopolar radiofrequency and glycolic acid peels in facial rejuvenation with regards to histopathology and Ultrabiomicroscopic sonography (UBM). METHODOLOGY: In this study, forty patients with mild to moderate photoaging received four treatments with 3 weeks interval of monopolar radiofrequency on one side of face and glycolic acid peels in increasing concentrations (NeostrataR) on the other side. Pre and post treatment, 2 mm biopsies were taken from both preauricular areas and Ultrasonography using a 35 MHz probe was done from outer canthus of eye and nasolabial folds from both sides of face. A blinded assessment was done to measure the increase in the grenz zone and dermal thickness. RESULTS: In 35/40 patients there was a significant increase in the grenz zone on histopathology and decrease in subepidermal low-echogenic band (SLEB) on UBM of the nasolabial folds on both sides of the face (p < 0.05). Introduction: Obstruction of the appendiceal lumen is the primary cause of appendicitis. The most common causes of luminal obstruction are fecaliths and lymphoid follicle hyperplasia. Additionally, bacterial infections or enteric and systemic viral diseases can cause a reaction of the lymphoid follicle. Case presentation: An 11-year-old boy with active phase of chickenpox presented on our Pediatric surgery emergency department under the impression of acute appendicitis. An appendectomy was performed on the same day. An inflamed and edematous retrocecal appendix was removed during surgery. Histological investigation of the appendix revealed transmural acute inflammation, with diffuse proliferation of inflammatory cells, with characteristic intranuclear inclusion surrounded by a clear halo. The PCR analysis of peripheral blood and appendix tissue specimen revealed positive VZV DNA. Hemangiom je nejčastější primární benigní nádor páteře typu hamartomu, který nikdy nemalignizuje. V drtivé většině případů se jedná o asymptomatické solitární léze hrudní páteře jakožto náhodné nálezy radiografických vyšetření. Pouze v 1 % případů se hemangiom obratle projeví klinicky a agresivita této benigní léze pak spočívá v expanzivním chování nádorové tkáně či v důsledku patologické fraktury obratle. Klinickými příznaky potom jsou především dorzalgie, v pokročilých případech pak známky kořenové nebo míšní komprese. Radiodiagnostika je poměrně snadná, protože hemangiom obratle vykazuje zcela typické grafické příznaky, pro potvrzení diagnózy agresivního hemangiomu obratle jsou stanovena grafická kritéria. Spektrum léčebných modalit je velmi široké od konzervativní léčby přes hodně využívanou vertebroplastiku po radikální chirurgickou léčbu. Komplementárně jsou často využívány embolizační výkony, alkoholová sklerotizace a lokální radioterapie. Agresivní hemangiom obratle je poměrně velmi vzácná diagnóza. Proto zatím neexistují velké soubory či klinické studie, podle kterých by bylo možné stanovit optimální terapeutický postup., Hemangioma is the most common primary benign hamartoma type spine tumor. Malignant degeneration has never been reported. In the majority of cases, this is an asymptomatic solitary lesion of the thoracic spine found incidentally during a radiographic examination. Only 1% of cases manifests clinically and this benign lesion is considered aggressive because of the expansive nature of the tumor and because it may cause pathological fracture of the vertebrae. Clinical symptoms then include dorsalgia and either root or spinal neurological symptoms. Radiological diagnosis is relatively easy because vertebral hemangioma is associated with quite typical graphic signs; graphic criteria to confirm the diagnosis of aggressive vertebral hemangioma have been clearly defined. The range of treatment options is very wide, from conservative treatment through frequently used vertebroplasty to radical surgical treatment. Embolization procedures, alcohol sclerotherapy and local radiotherapy are often used as complementary procedures. Aggressive vertebral hemangioma is a relatively rare diagnosis, so far there are no large cohorts or clinical studies from which it would be possible to determine an optimal therapeutic approach. Akromegalie patří mezi vzácná a závažná onemocnění, základem její léčby je kombinace terapie chirurgické, radiační a medikamentózní. V práci se popisuje případ akromegalie diagnostikované před 10 lety ve stadiu, v němž byla možná již jen parciální resekce hypofyzárního tumoru. Přesto se podařilo dosáhnout dlouhodobé stabilizace stavu kombinací opakovaného chirurgického zákroku, radioterapie lineárním urychlovačem i Leksellovým gama nožem a medikamentózní léčby somatostatinovým analogem oktreotidem a blokátorem receptorů pro růstový hormon pegvisomantem. Dávka oktreotidu i pegvisomantu byla opakovaně měněna podle výsledků vyšetření hladiny IGF1, v současnosti je podáváno somatostatinové analogon Sandostatin LAR 30 mg jednou za 3 týdny a blokátor receptorů pro růstový hormon Somavert 20 mg denně. Přes přítomnost závažné choroby nemocný již po 10 let vede spokojený život. Náklady na finančně náročnou léčbu jsou z etického hlediska plně odůvodněné., Acromegaly is a rare and serious disease. A successful and rational therapy of acromegaly ought to combine surgery, radiotherapy and pharmacotherapy. The submitted article presents a case of acromegaly that was only diagnosed at the stage when total pituitary adenoma removal was impossible. Even so, the long-term stabilisation of the disease was reached by way of repeated surgery through transfenoidal and transcranial approach, by linear accelerator radiation therapy and Leksell Gamma Knife radiotherapy and by pharmacotherapy with somatostatin analogon octreotide and growth hormone receptor antagonist pegvisomant. The octreotide and pegvisomant dosage has been repeatedly changed according to IGF1 levels. The contemporary somatostatin analogon Sandostatin LAR 30 mg is given once every 3 weeks and the growth hormone receptor antagonist Somavert 20 mg is applied daily. Despite this serious disease, the patient has already been living contentedly for 10 years. Úvod: Alveolární echinokokóza je vzácné a závažné zoonotické parazitární onemocnění. Kazuistika: Autoři demonstrují případ mladého nemocného s postižením jater, bránice a plic touto formou infekce. Diagnóza echinokokové infekce byla stanovena na základě anamnézy, klinické symptomatologie v kombinaci s USG, CT, MRI a sérologickými metodami. Byla provedena radikální bloková resekce 7. jaterního segmentu, dolního plicního laloku vpravo a bránice. Definitivní stanovení diagnózy alveolární echinokokózy bylo provedeno histopatologicky a pomocí PCR metody ze vzorku resekované tkáně. Závěr: Nemocný je 8 měsíců po operaci bez potíž, trvale dispenzarizován s long-life terapií albendazolem., Introduction: Alveolar echinococcosis is a life-threatening zoonotic parasitic disease. Its incidence is rare. In some cases, the correct and timely diagnosis can be difficult. Case report: The authors present the case of a young patient with liver, diaphragm and lung involvement. The suspicion of echinococcus infection was made on the basis of medical history, clinical symptoms, and a combination of ultrasonography, computed tomography, magnetic resonance imaging tests and serological methods. The patient underwent multimodal treatment with albendazole and en-bloc resection of the liver, lung and diaphragm. The definitive diagnosis of alveolar echinococcosis was determined from samples of the resected tissues using histopathology and polymerase chain reaction methods. The patient has been followed regularly and is on life-long treatment with albendazole. Úvod: Cílem práce je zhodnocení našich zkušeností a výsledků s použitím hybridních rekonstrukcí v terapii multietážového postižení tepenného systému dolních končetin a vyhodnocení vlivu indikace a typu hybridní rekonstrukce na jejich průchodnost. Metoda: Retrospektivní analýza souboru pacientů s prospektivním sběrem dat. Na základě typu hybridní rekonstrukce rozdělení pacientů do 5 skupin. Skupina pacientů „1“ podstoupila transluminální angioplastiku (TA) ? implantaci stentu do pánevního řečiště v kombinaci s plastikou tepen v třísle (35 pacientů; 27,6 %). Skupina pacientů „2“ podstoupila TA ? implantaci stentu do pánevního řečiště v kombinaci s infrainguinálním bypassem (15 pacientů; 11,8 %). Skupina pacientů „3“ podstoupila plastiku tepen v třísle v kombinaci s TA výtokového traktu končetiny: AFS ? AP ? bércové tepny (52 pacientů; 40,9 %). Skupina pacientů „4“ podstoupila provedení infrainguinálního bypassu v kombinaci s TA distálně od místa distální anastomózy rekonstrukce (3 pacienti; 2,4 %). Skupina pacientů „5“ představuje heterogenní skupinu hybridních rekonstrukcí, které nespadaly do některé ze 4 výše uvedených skupin (22 pacientů; 17,3 %). Na základě indikace k výkonu rozdělení souboru do 3 skupin: skupina „ALI“ pacienti s akutní končetinovou ischemií, skupina „CLI“ pacienti s kritickou končetinovou ischemií, skupina „II“ pacienti s klaudikačními obtížemi. Analýza průchodností provedena s použitím Kaplan-Meier life tables. Porovnání průchodnosti mezi skupinami provedeno pomocí log-rank testu. Statistická analýza provedena s použitím softwaru IBM SPSS Statistics 21.0 . Výsledky: Soubor prezentuje 127 pacientů, kteří podstoupili 127 hybridních intervencí. Technický a klinický úspěch hybridní rekonstrukce byl 96,9 % a 98,4 %. 30denní perioperační mortalita byla 3,1 %. Výsledky primární (PP), primární asistované (PAP) a sekundární (SP) průchodnosti ve 2 letech byly nejlepší u pacientů s hybridním výkonem typu „1“ a „3“, tedy kombinací plastiky tepen v třísle a transluminální angioplastiky vtokového nebo výtokového traktu končetiny. Výsledky skupiny „1“ v pořadí PP, PAP a SP ve 2 letech: 80,6 %, 84,3 % a 84,3 %. Výsledky skupiny „3“ ve 2 letech ve stejném pořadí: 81,1 %, 82,8 % a 86,3 %. O poznání horších výsledků bylo dosaženo u pacientů ze skupiny „2“ a „5“. Výsledky PP, PAP a SP ve 2 letech u pacientů ze skupiny „2“ byly: 39,4 %, 59,1 % a 59,1 %. Výsledky průchodnosti pacientů z heterogenní skupiny „5“ byly ve 2 letech: 30,4 %, 49,2 % a 70,7 %. Při zohlednění vlivu indikace na průchodnost rekonstrukce ve 2 letech byly nejlepší výsledky primární asistované a sekundární průchodnosti dosaženy u pacientů ze skupiny „CLI“: 81,6 % a 86,2 %. Jenom primární průchodnost ve 2 letech byla nejlepší u pacientů s klaudikacemi (skupina „II“), a to 71,1 %. Pacienti indikováni k hybridnímu výkonu pro akutní končetinovou ischemií (ALI) dosahovali nejhorších výsledků z uvedených 3 skupin, a to PP, PAP a SP ve 2 letech: 33,7 %, 46,2 % a 45,5 %. Amputation-free survival ve 2 letech měl nejlepší hodnotu pro pacienty ze skupiny „CLI“, a to 89,1 % („II“ 81,9 %; „ALI“ 61,4 %). Overall-survival a amputation-free survival ve 2 letech za celý soubor dosahovaly hodnot 93,2 % a 82,3 %. Závěr: Hybridní revaskularizace je kombinovaný výkon, který umožňuje řešení multietážového postižení tepenného systému dolních končetin v rámci jednoho sezení. Je zatížen nízkým rizikem periprocedurální mortality a morbidity pro pacienta. Lze jím dosáhnout dobrých výsledků průchodnosti tepenného systému a záchrany končetiny bez ohledu na typ chronické ischemie (klaudikace, kritická končetinová ischemie). Výsledky u pacientů s akutní končetinovou ischemií jsou inferiorní. Jako výkon s nejlepšími výsledky nám vychází kombinace plastiky tepen v třísle spojená s transluminální angioplastikou vtokového nebo výtokového traktu končetiny., Introduction: The goal of this study was to evaluate our experience and results of single staged hybrid procedures for revascularization of lower extremities affected by multilevel arterial occlusive disease. We analysed the effect of the indication and type of reconstruction on patency. Method: Patients were retrospectively reviewed. Data collection was conducted prospectively. Patients were divided into 5 groups based on the type of hybrid reconstruction. Group “1” included patients who underwent transluminal angioplasty (TA) ? stenting of iliac arteries with endarterectomy (EA) and patch arterioplasty of the femoral bifurcation (35 patients; 27.6 %). Group “2” included patients who underwent TA ? stenting of iliac arteries with infrainguinal bypass (15 patients; 11.8 %). Group “3” consisted of patients who underwent TA ? stenting of outflow lower extremity arteries: superficial femoral artery ? popliteal artery ? crural arteries in combination with EA and patch arterioplasty of the femoral bifurcation (52 patients; 40.9 %). Group “4” represented patients who underwent infrainguinal bypass surgery in combination with TA distal to the site of open reconstruction (3 patients; 2.4%). Group “5” represented a heterogenous population of patients who underwent a hybrid reconstruction which did not belong to any of the 4 previously mentioned groups (22 patients; 17.3 %). The patients were divided into 3 groups based on the indication criteria of the intervention: Patients with acute limb ischemia (ALI) were put into group “ALI”. Patients with critical limb ischemia (CLI) were put into group “CLI”. Patients with claudications were put into group “II”. Patency analyses were performed using Kaplan-Meier life tables. Differences in patency rates between the different groups of patients were determined using the log-rank test. Statistical analysis was performed using the software IBM SPSS Statistics 21.0. Results: The study included 127 patients who underwent 127 hybrid arterial procedures. Technical and clinical success rates were 96.9% and 98.4%. 30-day perioperative mortality rate was 3.1%. The primary (PP), assisted-primary (APP) and secondary patency (SP) results at 2 years were the best amongst the patients from groups “1” and “3”. These patients underwent patch arterioplasty ? EA of the femoral bifurcation in combination with transluminal angioplasty of inflow or outflow arteries of the lower extremity. The resulting PP, APP and SP rates at 2 years in group “1” were 80.6%, 84.3% and 84.3%. Patency rates in group “3“ in the same order at 2 years were 81.1 %, 82.8% and 86.3%. Significantly lower patency rates were achieved in patients from groups “2“ and “5“. The resulting PP, APP and SP rates in group “2” at 2 years were 39.4%, 59.1% and 59.1%; in group “5“ at 2 years they were 30.4%, 49.2% and 70.7%. Taking into account the effect of the indication on patency rates, we found that patients from group “CLI” had the best APP and SP rates at 2 years: 81.6% and 86.2%. Only the PP rate at 2 years was the best in group “II” (patients with claudications): 71.1%. Patients who underwent their operations because of acute limb ischemia (group “ALI”) achieved the worst results among these 3 groups. Their PP, APP and SP rates at 2 years were 33.7%, 46.2% and 45.5%. Patients from the group „CLI“ had the best amputation-free survival at 2 years: 89.1% amongst the 3 groups subdivided according to the indication for the intervention (“II“ 81.9%; “ALI“ 61.4%). Overall survival and amputation-free survival at 2 years were 93.2% and 82.3%, respectively, for the whole studied population. Conclusion: The hybrid procedure is an intervention which combines endovascular and open reconstruction performed at the same time. Hybrid procedures provide effective therapy for patients with multilevel lower extremity arterial disease. They have low complication rates, periprocedural morbidity and mortality. Using these procedures, we are able to achieve good limb salvage rates and patency rates irrespective of the form of chronic limb ischemia (claudications, critical limb ischemia). Results for patients with acute limb ischemia are inferior. The combination of patch arterioplasty ? endarterectomy of the femoral bifurcation with transluminal angioplasty of inflow or outflow limb arteries is our hybrid procedure of choice with the most favourable results., and E. Oligodendrogliomy patří k vzácným, avšak nejlépe prozkoumaným nádorům v neuroonkologii. Již dlouho je známa jejich větší senzitivita na radioterapii a chemoterapii ve srovnání s ostatními gliomy. Specifickým nálezem je častá přítomnost chromozomální kodelece 1p/19q. Teprve výsledky dlouhodobého sledování nemocných ve dvou zásadních klinických studiích fáze III – RTOG 9402 a EORTC 26951 prokázaly příznivý účinek kombinované onkologické léčby radioterapie a chemoterapie v kombinaci prokarbazin, lomustin‑CCNU a vinkristin (PCV) u nemocných s anaplastickým oligodendrogliomem a anaplastickým oligoastrocytomem s 1p/19q kodelecí. Přítomnost kodelece 1p/19q je důležitým diagnostickým, pozitivním prognostickým a silným prediktivním biomarkerem oligodendrogliomů. Diskutuje se o dalších molekulárně genetických charakteristikách oligodendrogliomů – mutaci metabolického enzymu Izocitrát dehydrogenázy 1 a 2 (IDH 1/2), metylaci promotoru genu pro O‑6–metylguanin‑metyltransferázu (MGMT), hypermetylačním stavu ostrůvků cytozin‑guanin nádorového genomu (G‑CIMP) a možnosti léčby těchto nádorů. Uvedeny jsou aktuální poznatky optimálního managementu anaplastických oligodendrogliomů respektující zásady personalizované medicíny., Oligodendrogliomas are uncommon but extensively investigated tumours in neurooncology. Their superior sensitivity to radiotherapy and chemotherapy compared to other gliomas has long been known. Chromosomal codeletion 1p/19q is frequent in this tumour type. A long-term follow up of two landmark phase III trials – RTOG 9402 and EORTC 26951 has shown a favourable effect of combined radiotherapy and chemotherapy - procarbazine, lomustine (CCNU), vincristine – in patients with anaplastic oligodendrogliomas and anaplastic oligoastrocytomas carrying the codeletion 1p/19q. This codeletion serves as an important diagnostic, positive prognostic and strong predictive biomarker. The role of the other molecular biomarkers (isocitrate dehydrogenase – IDH1, IDH2 mutations, methylation of the MGMT promoter, glioma cytosine - guanine islands methylator phenotype – G-CIMP) in oligodendroglial tumours is also discussed. Plicní aspergilóza patří mezi zánětlivá onemocnění způsobená houbami rodu Aspergillus. V závislosti na stavu imunitního systému a plicního parenchymu nemocného existují invazivní, semi-invazivní a neinvazivní formy onemocnění. V diagnostice tohoto onemocnění hrají hlavní úlohu prostý rentgenový snímek hrudníku a HRCT plic.
