The aim of our study was to determine the effect of selected cytostatics on a human ovarian cancer cell line A2780 as a model system for ovarian cancer treatment. This cell line is considered cisplatin-sensitive. Panel of tested cytostatics included cisplatin, paclitaxel, carboplatin, gemcitabine, topotecan and etoposide. These cytostatics have a different mechanism of action. To evaluate cytotoxic potential of the tested compounds, the methods measuring various toxicological endpoints were employed including morphological studies, MTT assay, dynamic monitoring of cell proliferation with xCELLigence, cell cycle analysis, caspase 3 activity and expression of proteins involved in cell cycle regulation and cell death. Úvod: Incidence akutního krvácení do horní části gastrointestinálního traktu (GIT) se udává 85–108/100 000 obyvatel za rok, přičemž nonvariceální krvácení odpovídá za 80–90 %. Antiagregační a antikoagulační terapie patří mezi významné rizikové faktory pro krvácení do horní části GIT. Cíl práce: Zjistit výskyt krvácení do horní části GIT u běžné populace pacientů v podmínkách krajské nemocnice, srovnání podílu antiagregancií, antikoagulancií a dalších rizikových léků na dané problematice. Soubor a metodika: Retrospektivní analýza pacientů, kteří v roce 2013 (leden–červen) podstoupili za hospitalizace gastroskopii pro akutní krvácení do horní části GIT nebo pro anémii (Hb < 100 g/l) s verifikovaným zdrojem ztrát v horní části GIT. Výsledky: Soubor tvořilo 111 pacientů s průměrným věkem 69 ? 15 let, mužů bylo 60 %. Nonvariceální krvácení tvořilo 90 %. Žádný z pacientů s variceálním krvácením (10 % pacientů) neužíval antiagregační či antikoagulační terapii. Pacientů s nonvariceálním krvácením do horní části GIT bylo 100 s průměrným věkem 70 ? 15 let, 61 % mužů. Příznaky akutního krvácení (hemateméza, meléna) mělo 73 % těchto pacientů. Nejčastější příčinou krvácení byla vředová choroba gastroduodena, a to v 54 % případů. Ze všech pacientů s nonvariceálním krvácením do GIT užívalo antiagregancia 32 %, antikoagulancia 19 %, dalších 10 % pacientů užívalo nesteroidní antirevmatika, selektivní inhibitory zpětného vychytávání serotoninu nebo kortikoidy. 30denní mortalita pacientů s nonvariceálním krvácením byla 11 % a roční mortalita 23 %. Nebyl zjištěn významný rozdíl v mortalitě, nutnosti aplikace krevních transfuzí nebo chirurgické intervence u pacientů na antitrombotické terapii a bez ní. 25 % (8 pacientů) s terapií kyselinou acetylsalicylovou (ASA) nebylo k této léčbě indikováno dle guidelines. Závěr: V souboru pacientů ošetřených gastroskopií pro symptomatické nonvariceální krvácení do GIT nebo anémií (Hb < 100 g/l) je signifikantně větší podíl léčených antiagregancii než antikoagulancii. To může reflektovat větší zastoupení této léčby v populaci, ale také její vyšší riziko pro vznik GIT krvácení. V souvislosti s tím vyznívá varovně fakt, že stále existuje nezanedbatelné procento pacientů, kteří užívají kyselinu acetylsalicylovou i přesto, že nesplňují indikaci k této terapii., Background: The incidence of acute upper gastrointestinal bleeding is about 85–108/100,000 inhabitants per year, nonvariceal bleeding accounts for 80–90 %. Antiplatelet and anticoagulation treatment are the significant risk factors for upper gastrointestinal bleeding. Objectives: To evaluate the occurrence of upper gastrointestinal bleeding in the general community of patients in a county hospital. And to compare the role played by antiplatelet and anticoagulation drugs and other risk medication. Design and methods: Retrospective analysis of patients over 18 years of age who underwent endoscopy for acute upper gastrointestinal bleeding or anaemia (haemoglobin < 100 g/l) with proved source of blood losses in upper gastrointestinal tract during a hospital stay in 2013 (from January to June). Results: We included 111 patients of average age 69 ? 15 years, men 60 %. Nonvariceal bleeding accounted for 90 % of the cases. None of the patients with variceal bleeding (10 % of patients) took antiplatelet or anticoagulation therapy. There were 100 patients with nonvariceal bleeding of average age 70 ? 15, 61 % men. With the symptoms of acute bleeding (hematemesis, melena) presented in 73 % of patients. The most frequent cause of bleeding was gastric and duodenal ulcer (54 %). 32 % of patients with nonvariceal bleeding had antiplatelets, 19 % anticoagulants and 10 % used nonsteroidal anti-inflammatory drugs, selective serotonin reuptake inhibitors or corticosteroids. 30-days mortality of patients with nonvariceal bleeding was 11 %, annual mortality was 23 %. There was no significant difference in mortality, blood transfusion requirements or surgical intervention between the patients with antithrombotic agents and without them. 25 % of patients (8 patients) using acetylsalicylic acid did not fulfil the indication for this treatment. Conclusion: Among the patients examined by endoscopy for symptomatic nonvariceal bleeding and/or anaemia (haemoglobin < 100 g/l) significantly higher portions of patients are taking antiplatelet rather than anticoagulation therapy. This may be caused by greater use of these drugs in the population, but on the other hand it may reflect an association with greater risk of gastrointestinal bleeding. Invazivní metody léčby chronické formy ischemické choroby srdeční byly po dlouhou dobu považovány za kauzální terapii, která bezpochyby snižuje morbiditu a mortalitu nemocných. Na druhé straně životní styl a farmakologická terapie byla nahlížena jako spíše doplňující, ale nikoliv rozhodující léčba. V posledních letech je možno vidět změnu v pohledu na ischemickou chorobu srdeční. Je to mnohem difuznější proces v koronráním řečišti a není to tedy jen otázka kritických stenóz, ale stejně nemocného ohrožují i další úseky věnčitých tepen, kde se hovoří o vulnerabilních placích. Překvapením pro experty byly závěry řady studií potvrzujících neschopnost invazivch metod prodloužit život nemocným s chronickou formou ICHS. Na druhé straně přibývají důkazy o významném vlivu úpravy životního stylu a rozrůstajících se možnostech farmakolgoické terapie s pozitivním vlivem na morbiditu a mortalitu. Článek prezentuje současné názory na tyto otázky, které jsou nyní živě diskutovány., Invasive methods for treatment of coronary artery disease in chronic form were, for a long time, believed to be causal treatment that definitely diminish patient morbidity and mortality. By contrast, lifestyle and pharmacological therapy were seen as valuable but not lifesaving. In the last few years a change can be seen in the view on coronary artery disease. It is a much more diffusive and complicated process in the coronary tree. It is not merely a question of the critical stenotic areas; the new idea of vulnerable plaque, which presents the same danger of complication as critically stenotic areas, is also important. A great surprise for experts was the result of various studies confirming the inability of invasive procedures to prolong life in patients with chronic coronary disease. On the other hand, there has been more evidence that lifestyle changes and expanding pharmacological possibilities can have a significant positive effect on morbidity and mortality. Cieľ: Úlohou tejto prehľadovej práce o diagnóze a manažmente artériovej hypertenzie je zamerať sa na dokumenty a štúdie o hypertenzii publikované v ostatných rokoch s cieľom určiť ich príspevok k rozšíreniu našich vedomostí o artériovej hypertenzii v klinickej praxi. Diskusia: Artériovú hypertenziu nedefinujú príznaky a znaky, ale čísla hodnôt krvného tlaku. Vzniku trombózy v arteriálním nebo venózním cévním systému předchází složitá souhra faktorů vnějšího prostředí a faktorů genetických. Rozsáhlé genomové asociační studie se ukazují jako velice přínosné v identifikaci lokusů spojených s kardiovaskulárními onemocněními a souvisejícími rizikovými faktory. Ještě zbývá mnoho práce v identifikaci určujících genů a jejich kauzálních variant, stejně jako v definici mechanizmů, které ovlivňují vývoj a progresi onemocnění. Z výsledků studií lokusů spojených s dříve identifikovanými onemocněními se očekává zlepšení našich znalostí o patofyziologii kardiovaskulárních onemocnění a možnost identifikovat nové cíle pro léčbu. V oblasti aterotrombózy a trombofilie jsou významné výsledky z asociačních studií zaměřených na oblast ischemické choroby srdeční, ischemické cévní mozkové příhody a žilního tromboembolizmu. The genomewide association approach has proved successful in identifying loci associated with cardiovascular disease and related risk factors. However, much work remains to identifyning the culprit genes and causal variants as well as the mechanisms whereby they influence disease development and progression. In-depth studies of previously identified disease-associated loci are expected to improve our understanding of the pathophysiology of cardiovascular disease and identify novel targets for treatment. In the field of atherothrombosis and thrombophilia are significant results from association studies focused on the area of coronary artery disease, ischemic stroke, venous thromboembolism. INTRODUCTION: The issue of resistance to antiplatelet therapy has raised many questions in the area of neurovascular diseases. The first objective of this work was to determine the prevalence of aspirin resistance in neurovascular patients with clinical non-responsiveness to aspirin treatment and a high-risk of atherothrombotic complications using two interpretable and independent methods (aggregation and PFA 100). The second objective was to find the correlation between both assays and to evaluate the results in groups at risk for various cerebrovascular diseases. MATERIAL AND METHODS: Laboratory tests of aspirin resistance were performed in 79 patients with clinical non-responsiveness to aspirin treatment suffering from neurovascular diseases. Patients were divided into the two groups: expected low risk for aspirin resistance due to the first manifestation of a neurovascular disease (n = 34) and expected high risk due to the second clinical manifestation of a neurovascular disease (n = 45). RESULTS: The prevalence of aspirin resistance in both groups combined as determined by the PFA-100 and CPG techniques were 50.6% and 17.7%, respectively. No correlation was found between the two techniques. CONCLUSIONS: No significant prevalence of aspirin resistance was demonstrated by either method despite the heterogeneous pathophysiological mechanisms. Cíl: Převod Testu paměti pro záměry (Memory for Intentions (Screening) Test; MIST, 2010) do české verze zahrnoval standardizaci pomůcek, převod administrace a skórování, validační studii na české populaci, aby MIST bylo možné použít na české populaci. Úvod: Konstrukt prospektivní paměti (PP), tj. schopnosti si zapamatovat a realizovat záměry, je nezbytný pro uchování soběstačnosti, vykonávání aktivit denního života. V posledních letech hraje čím dál významnější roli pro diagnostiku poruch paměti. Standardizovaná a validovaná měřítka PP však v české diagnostice chybí. Metodika: Český převod MIST jsme administrovali 30 zdravým osobám. Výsledky: Celkový skór PP v testu MIST koreloval na střední úrovni s měřítky z neuropsychologické baterie zejména s retencí v paměti, mentální flexibilitou, odolností k interferenci a premorbidní inteligenční úrovní (všechna rho = 0,37–0,42, všechna p < 0,05). Reliabilita testu MIST ve smyslu vnitřní konzistence byla u základních osmi subškál nedostatečná (α = 0,50) i ve smyslu reliability obou polovin testu (split‑half reliabilita = 0,56), avšak reliabilita šesti subškál tříděných dle typu (oddálení, vodítka a modality odpovědi) byla vysoká (α = 0,88; split‑half = 0,95). Závěr: Výsledky validační studie naznačují, že český převod MIST je srovnatelný s verzí originální. Studie zpřístupňuje standardní vyšetření PP a umožňuje měření PP na klinických populacích. We included standardized testing material, translation of administration and scoring, and assessment of normative data for the MIST in the Czech population. Introduction: Prospective memory (PM), i.e., the ability to remember and implement intentions after a delay, is essential as a subsystem of episodic memory for the maintenance of independence and execution of activities of daily living. PM assessment thus plays an important role in the diagnosis of episodic memory disorders. However, there are currently no standardized and validated PM tools in Czech language. Methods: The Czech version of MIST was administered to 30 healthy persons. Results: The MIST Summary score correlated at a medium level with a range of neuropsychological measures including memory retention, mental flexibility, and resistance to interference (all rho = 0.37–0.42; all p < 0.05). The reliability of MIST in terms of internal consistency was insufficient when analyzing the eight individual MIST trials (α = 0.50), as was split‑half reliability (split‑half reliability = 0.56). In contrast, there was a high degree of reliability between six subscales classified by type (delay, cue and mode of response; α = 0.88, split‑half = 0.95). Conclusion: The reliability and validity of the Czech version of MIST is comparable to the original English version. Psoriasis patients are often susceptible to cardiovascular diseases (CVD), including atherosclerosis. Traditional markers (biochemical and inflammatory) and diagnostic tools could detect occlusive but not subclinical atherosclerosis. Carotid intima-media thickness (CIMT), has recently been recognised as a non invasive diagnostic tool for identification of premature atherosclerosis. Therefore we evaluated 80 psoriasis patients and 80 age sex matched healthy controls for serum leptin levels and apolipoprotein B/apolipoprotein A-I ratio (apoB/apoA-I ratio) in relation with CIMT of carotid artery. Carotid intima-media thickness and carotid plaques were simultaneously measured by carotid sonography. Serum concentration of leptin and apolipoprotein were measured using enzyme-linked immuno sorbent assay (ELISA) and nephelometry respectively. Raised CIMT correlated to age of onset of the disease, serum leptin and apoB/apoA-I ratio in psoriasis patients. Taking into account, values that were above the 75 percentile of the three markers (leptin, apoB/apoA-I ratio and CIMT) the odds ratio was 4.26 (2.06-8.80 CI). Leptin and apoB/apoA-I ratio showed significant cumulative association with CIMT. Results of predictive analysis supports measurement of CIMT along with estimation of serum leptin and apoB/apoA-I ratio for prediction of premature atherosclerosis in psoriasis patients. and K. Brachial Plexus is formed by the union of the anterior rami of cervical 5, 6, 7, 8 and thoracic 1 nerves. These nerves unite and divide to form the key nerves innervating the upper limb. Variations in the course of these nerves are clinically important to anesthetists, neurologists and orthopedicians. We report bilateral variations in the arterial and neural structures in the upper limb of a 65 year old cadaver. The muscles of the arm on one side were innervated by the median nerve with absence ofmusculocutaneous. While on the other side the musculocutaneous nerve contributed to the formation of the median nerve. There was a presence of high bifurcation of brachial artery on both sides. Knowledge of such variations in the innervations of muscles and the arterial supply of the limbs are important to remember before performing any reconstructive procedures or interventions on the limb. and V.
